2.Changes of carotid, lipidemia and fibrinogen in patients with type 2 diabetes mellitus complicated with cerebral infarction

Objective To investigate the changes of carotid, lipidemia and fibrinogen in type 2 diabetes mellitus patients complicated with cerebral infarction (DCI). Methods 58 cases of DCI (DCI group) were enrolled in this study, and 36 cases of cerebral infarction without diabetes mellitus served as control group. The location, number and nature of carotid atherosclerotic plaque and inner diameter of carotid artery were assessed by Doppler ultrasonography in all the patients, as well as blood lipid, blood glucose and fibrinogen were evaluated. Objective To observe the effects of hot compress on oxaliplatin-induced neurotoxicity.Methods One hundred and twenty patients undergoing chemotherapy with oxaliplatin enrolled in this study were randomly assigned to the control group and expenment group.The control group was given routine health education and the experiment group was treated with hot compress.We compared the incidence of nerotoxocity after 2 and 4 treatment courses.Resulsts The incidence of neurotoxiticity was 75.0%in the experiment group and 91.7%in the control group after 2 treatment courses(P<0.01).The incidences were 86.7%in the experiment group and 98.3%in the control group after 4 treatment courses(P<0.001).Conclusion Hot compress can effectively reduce the incidence of neurotoxicity induced by chemotherapy with oxaliplatin.

Gastric cancer incidence is one of the most common malignancies in our country and is the second most common in the worldwide,clinic doctors always emphasize early diagnosis and treatment of gastric cancer patients,in order to reduce the mortality,however,most patients' condition often have been in the late fall and these patients were badly in efficacy.Looking for a new diagnosis way is a medical prddem,with molecular biology advance and gene chips was improved,it is possible for the early screening of gastric cancer.This assay aims to briefly analyse the role of gene chips in the research progress of early gastric cancer.
5.Fat metabolism following traumatic stress

Lipids metabolism can change after traumas such as operation or injury.The process is com-prised of accelerated hydrolysis of triglyceride in adipose tissue,accelerated absorption of fatty acids by extra-he-patic tissue,improved lipid oxidation rate,and augmented re-esterification of fatty acids in liver and adipose tis-sue.Mechanisms of lipid mobilization include sympathetic stimulation,insulin resistance,and inflammation.How-ever,lipid absorption by liver and intestine is inhibited after trauma.Through post-trauma nutritional support,glu-cose administration improves re-esterification of fatty acids and inhibits lipid oxidation without affecting lipolysis.Fat emulsion infusion inhibits both absorption and secretion of triglyceride in liver.Therefore,clinicians should a-void over-alimentation of glucose and fat to prevent lipid overload,particularly in liver. Intervention of post-trau-matic lipid metabolism improves the outcomes:administration of long-chain triglycerides can prevent exhaustion of essential fatty acids,and administration of cholesterol can improve absorption and oxidation of fat emulsion.
6.Clinical Observation on the Treatment of Rheumatoid Arthritisin Early Stage with TongLuoShu Liquid

Objective To investigate the clinical efficacy of TongluoShu Liquid in treating rheumatoid arthritis at early stage. Methods 100 cases were randomly recruited into a control group and a treatment group with 50 cases in each group. The treatment group was given TongluoShu liquid, 150ml / time, twice a day; and the control group was given Diclofenac Sodium Sustained Release Tablets (Deegan), one granule for each time, twice a day, and Methotrexate 7.5mg / time, once per week. Results In the treatment group, symptoms, signs and biochemical indicators were significantly improved, and the swelling of the soft tissue around the joints in stage Ⅰ and Ⅱ were also reduced. Besides, osteoporosis was controlled and the progress of destruction to the cartilage of stage Ⅲ was detained, though there was no statistical difference between the two groups after the treatment. Conclusion ShuTongluo liquid is a safe and effective medicine with less adverse reactions in treating rheumatoid arthritis.
7.Intermedial to Long Term Surgical Management in Aortic Valve Insufficiency After Ventricular Septal Defect Repair

Objective:To explore the intermedial to long term surgical management in aortic valve insufficiency(AI)after ventricular septal defect(VSD)repair.Methods:A total of 22 consecutive patients with AI after VSD repair in our hospital from January 1996 to December 2007 underwent surgical treatment were enrolled in this study. Valve perforation was mainly pathological changes. Aortic valve replacement was performed in 13 patients and aortic valvuloplasty was done in 9 patients.Results:One patient died after the operation.Post operative X-ray and echocardiogram revealed that both cardio-thoracic ratio(0.57±0.07 vs. 0.52±0.04 P<0.05)and left ventricular end diastolic diameter(54.5±10.2 mm vs. 46.7±5.8mm P<0.05)decreased markedly. 3 patients suffered from mild aortic regurgitation after valvuloplasty.Conclusion:The iatrogenic injury of aortic valve during VSD repair could be corrected satisfactorily by valve replacement or valvuloplasty.
9.Covered retrievable metal stent in the treatment of refractory benign esophageal stricture

Objective To evaluate the efficacy and safety of the covered retrievable metal stent in the treatment of refractory benign esophageal stricture. Methods Six patients with refractory benign esophageal stricture who failed in endoscopic dilation were selected. There were 3 cases of stricture after caustic chemical ingestion, 2 cases of anastomotic stricture after surgery, and 1 case of re-stricture after metal stenting. The shape and diameter of the covered stent were individually designed. Stent was placed across the esophageal stricture. Symptoms after stenting, mucosal hyperplasia at the ends of stent and symptoms after retrieval of stent were followed up. Results Stent placement was performed successfully in all patients. After the stent placement, dysphagia was resolved, and all patients could consume soft food. Three to six months after stenting, all the stents were removed successfully by endoscopy. No mucosal hyperplasia and re-stricture occurred. In 4 cases, after stent had been removed, followed up for 2 to 12 months, symptoms of dysphagia were resolved persistently, and no further treatment was necessary. In the other 2 cases, the stent migrated, and dysphagia recurred within 1 month after removal of the stent. Besides one case of retrostemal pain after stenting, no other complication was noted. Conclusion Individually designed covered retrievable metal stent is a safe and effective way to treat refractory benign esophageal stricture.
