1.Effect of Aortic Valve Regurgitation on Pharmacodynamics of Cisatracurium

[Objective]The purpose of this study was to investigate the effect of severe aortic valve incompetence on the pharmacodynamics of cisatracurium.[Methods]Thirty patients were enrolled in this study:15 patients with severe aortic valve incompetence scheduled for aortic valve replacement were included in the study group(AI-group)and 15 patients without heart disease undergoing general surgical operations were allocated into the control group(C-group).Anesthesia was induced with fentanyl 5 μg/kg.propofol 1.5~2 mg/kg and cisatracurium 0.1 mg/kg and maintained by total intravenous anesthesia with propofol-remifentanil infusion.Degree of neuromuscular block was measured by train-of-four ratio using TOF-WATCH acceleragraph monitor.Onset time(from administration of cisatracurium to T1=0),total twitch suppression time,the time for spontaneous recovery of T1 to 25%and 75%.and the recovery index were recorded respectively.[Results]The onset time of cisatracurium was significantly longer in the AI group compared to the control group(5.6±0.8 min vs 3.4 ±0.4 min.P<0.001.However.there was no difference in the recovery time between the two groups.[Conclusion]We demonstrated that the onset of cisatracurium was delayed in the patients with severe aortic regurgitation in comparison to those without heart disease.
2.Analysis of hepatitis associated aplastic anemia in 5 children

Objective To explore the clinical feature, treatment and prognosis of hepatitis associated aplastic anemia (HAAA) in children. Method The clinical data of 5 children with HAAA were retrospectively analyzed. Results There were 5 patients (4 males and 1 female) with median age of 10 years (7~13 years ). The detection of hepatitis A, B, C, and E were all negative in these 5 children during their acute hepatitis period. In 2 children, parvovirus B19 antibody and EB virus were negative and the proportion of CD4+T cells and the ratio of CD4+/CD8+ were decreased, and the proportion of CD8+T cells was increased by T-lymphocyte subsets analysis. Three children were treated with anti-thymocyte globulin combined with cyclosporine, 2 of whom achieved complete remission and 1 died of pulmonary fungal infection. One child was treated with cyclosporine only and achieved partial remission. One child lost follow-up after giving up treatment. Conclusion HAAA in children can be caused by viral seronegative hepatitis, and usually has T lymphocyte immune disorders. Immunosuppressive therapy is effective. .
3.Analysis of Cis-acting Elements on Respiratory Mucin MUC5AC Expression and Regulation in Pulmonary A549 Cell Line

Objective:To study the mechanisms of neutrophil elastase (NE) induced expression of respiratory mucin MUC5AC. Methods:Using gene recombination techniques,four luciferase reporter gene plasmids containing different length of human MUC5AC gene promoter were constructed.Site-directed mutagenesis technique was used to establish mutants of Sp-l and NF-?B site in MUC5AC gene promoter; the relative luciferase activities were detected in the transfected human pulmonary A549 cells. 5.Repair and functional reconstruction for compound defects of proximal phalanx and dorsal skin at multiple digits

Objective To summarize our surgical experience in repair and functional reconstruction for compound defects of proximal phalanx and dorsal skin at multiple digits,Methods Six patients with multiple digital defects were treated in our department between June 1996 and March 2005.At the first stage,free iliac bone grafts were used to repair defects of proximal phalanges and temporary syndactyly between adjacent affected fingers was created through digital palmar skin sutures.The defects were covered with free flap transfer finally.Dorsalis pedis flaps were used in four patients,a lateral arm flap in one and a lateral thoracic flap in one respectively.At the second stage,a partial debulking procedure and division of syndaetyty followed three to six months later.Additional procedures were performed in three eases to reconstruct the digital extensor function through tendon transfer.The follow-ups ranged from six months to nine years.Results The flaps survived uneventfully in the six patients postoperatively.The dorsal aspects of reconstructed fingers demonstrated an aesthetically pleasing effect after the flap debulking procedures and division of syndactyly.Follow-up X-Ray examinations showed good lilac bone union and nearly normal structure of digital bone.The distal interphalangeal extension restored to normal in the three cases after extensor reconstruction.Conclusions Iliac bone graft to repair phalangeal defects and free flap transplantation to cover skin defects can be a good treatment for compound defects of proximal phalanx and dorsal skin at multiple digits.Secondary plastic procedure may greatly improve the appearance of a reconstructed digit,and extensor re- construction the function of distal interphalangeal extension too.

in lung tissues of silicosis model rats. Specific pathogen free adult male SD rats were randomly divided into control
group, model group, tetrandrine group and YCF group, with eight rats in each group. The rats in the model group, tetrandrine
group and YCF group were intratracheally injected with 1.00 mL of silica suspension with a mass concentration of 50.0 g/L, and
the rats in the control group were given an equal volume of 0.9% sodium chloride solution. On the 15th day after modeling, the
tetrandrine group was given tetrandrine at a dose of 27.0 mg/kg body weight, the YCF group was given YCF with a dose of 8.91 g/kg
body weight, while both the control group and model group were given 2.00 mL 0.9% sodium chloride solution. Gavage wasperformed twice a day in the morning and evening for 14 days. On day 29 of the experiment, after evaluating the tidal volume,
