3.Relationship between heart rate recovery and variabillity after treadmill exercise test

Objective To observe the relationship between short-term heart rate variability(HRV) and heart rate recovery (HRR) after exercise.Methods 495 patients (273 men),whose age ranged 19-85 years,were submitted to treadmill exercise tests and short-term HRV evaluations.The standard deviation of the normal-to-normal interval (SDNN),the square root of the mean squared differences of successive normal-to-normal intervals (RMSSD),the number of interval differences of successive normal-to-normal intervals greater than 50 ms (NN50 count),the proportion derived by dividing NN50 count by the total number of normal-to-normal intervals (pNN50) and frequency(lowfrequency power,high-frequency power,total power) domains were observed.Results Among 495 patients,106 patients(68 men) were elderly(age≥65 years).Male gender and hypertension were significantly higher in elderly patients.The young patients had higher HRR after exercise.HRR at 4min (54 ± 13) vs (60 ± 12) beats/min,P =0.003 was the most significant predictor for positive exercise test result.In the young group,both time domain measures (SDNN:correlation coefficient 0.34,P < 0.001 ; RMSSD:correlation coefficient 0.37,P < 0.001) and frequency domain measures (LF:correlation coefficient 0.21,P < 0.001 ; HF:correlation coefficient 0.13,P =0.01 ; total power:correlation coefficient 0.22,P < 0.001) were significantly associated with HRR at 4min.Conclusion HRR at 4min was significantly associated with short-term HRV of time and frequency domains in young individuals,but not elderly ones,who received treadmill exercise test.
4.The clinical diagnosis of the prostate cancer by transrectal ultrasonography

Purpose:To evaluate the use of transrectal ultrasonography in diagnosis of prostate cancer.Methods:A high-frequency probe was used to examine 56 cases of prostate cancer transrectally and the sonographic features were analyzd. Results:On Trus image lesions were located in peripheral zone in 48(76.8%), transition zone in 8 (14.3%),and central zone in 5 (8.9%). 45 cases (80.4%) of the prostate cancer were hypoechoic,4(7.1%) were hyperechoic, 5(8.9%) had mixed echogenicity and 2 (3.6%)showed isoechoic appearance. Indirect signs represented prostatic asymmetry,intermittent capsular and dilation or disappear of the seminal vesicles.Conclusions:TRUS is imperative in the diagnosis and staging of prostate cancer.
5.Research actuality of chemosensitivity testing in chemotherapy

This paper explains the research status of chemotherapy sensitivity test in three aspects of history of development, specific methods and clinical application. Chemotherapy sensitivity test is an important way to achieve individual treatment of cancer, after more than 60 years, there are two major categories(in vivo method and in vitro method) of more than 10 kinds of methods. The basic steps of sensitivity test inelude culture of primary tumor cells, chemotherapy drugs mixed, the reaction mixture, observing the results of detection and analysis of indicators. This paper focuses on basic principles, main steps and characteristics of six methods, such as the renal capsule of nude mice model of human cancer, the difference between staining cell, tetrazolium salt colorimetric, adenosine triphosphate based bioluminescence tumor chemosensitivity assay, collagen gel embedded culture method and targeting molecule sensitivity assay. Through the results of several clinical trials, it can be seen that chemotherapy under the guidance of drug sensitivity test significantly improved more than experience chemotherapy in efficient rate, median progression-free survival time, survival time, clinical complete remission rate, pathological complete remission rate, etc.

We first study the photosensitization to bacteria and fungi with a definite structure, watersoluble porphyrin. We found that the porphyrin had strong sterilization on the Staphylococcus au- reus and Candida albicans. When the porphyrin concentrations increased, its sterilization strengthened. The sterilization was related to the photoradiating time. When photoradiating time was one hour, its sterilizing ability was the greatest. Its sterilizing ability was similar to the penicillin's.

9.Kidney transplantation in rats:A modified technique and analysis

BACKGROUND: Rats model of kidney transplantation are common used in the research of organ transplantation and kidney associated diseases. However,the existed modeling methods have various problems in transplanting time or transplantation effect. OBJECTIVE: To establish a lateral kidney transplantation model with simplity,safty,and high success rate. DESIGN,TIME AND SETTING: The analysis experiment of animal model was performed at the Department of Urology,Fudan University between March and June,2008. MATERIALS: 60 Wistar rats and 60 Sprague Dawley rats were selected as donors and recipients,respectively. METHODS: The left kidney graft was transplanted heterotopically into the left infrarenal position,to perform donor abdominal aorta and left renal vein end-to-side anastomoses to the recipient abdominal aorta and inferior vena cava. The ureter of kidney allograft was inserted into bladder traced by 5-0 needle. Bilateral native kidneys nephrectomy of the recipient was performed immediately. The operation time and survival rate were measured. MAIN OUTCOME MEASURES: Transplanting time,warm ischemia time,cold ischemia time,arteriovenous anastomosis and operation time,graft failure rate and reason,survival rates. RESULTS: The operation cost 100-140 minutes. The warm ischemia time was about 8-10 seconds,while the cold ischemia time was 40-60 minutes. Operation achievement ratio is 85%. Reasons of failures were analysed as follows: stenosis of the anastomotic stoma and thrombosis (6.67%),hemorrhage of the anastomotic stoma (5%),infection (1.67%),and unknown reason (1.67%). CONCLUSION: The establish method with advantages of convenience and reliability,which can notably shortened time of warm ischemia of the graft and urinary tract reconstruction and decreased complications.
10.Recent progresses in neurosurgery

Objective To investigate the present status and developments of Chinese military neurosurgery, make an objective analysis about the achievements and deficiencies, and propose a developing trend for the field of Chinese military neurosurgery. Methods The scientific literatures of neurosurgery published domestically and abroad in recent 5 years were partially reviewed. The advances and achievements in basic researches and clinical works of neurosurgery acquired during the period of the 11th Five Year Plan were analyzed. Results During the period of the 11th Five Year Plan, Chinese military neurosurgery was still in the lead position in traditional areas such as traumatic brain injury. In the meantime, great achievements were obtained in the researches of brain and spinal cord tumors, endovascular treatment and functional neurosurgery, and 47 items of Army Science and Technology Advancement Prizes or Military Medical Achievement Awards were acquired. A great progress was made in personnel training. However, unbalanced development in neurosurgery field still existed within military departments, between military and civil departments and between different regions, so an enhanced technical interchange and cooperation is needed. Conclusions On the coming period, the research work of military neurosurgery should focuse on basic and clinical research of gene therapy to keep in step with neurosurgical trends in the world. On the other hand, attention should be paid to technological innovation with military features such as professional and general treatment of traumatic brain injury and researches on the development of protective equipments.
