1.Diurnal variation, moon phase and seasonal variation in onset of stroke

ObjectiveTo determine the diurnal variation,moon phase,and seasonal variation in the onset of stroke. MethodsThe time parameters,including the time,the moon phase and the season of the stroke occurrence for 905 in-patients from 1989-2001 were investigated.ResultsThere was a significant difference (D=0.155,P<0.01) for circadian rhythm in the onset of 321 cases with cerebral infarction,and the incidence frequency at dawn was much higher than that at other time,but the difference of the moon phase and seasonal rhythm in the onset of cerebral infarction were not significant(P>0.05).There were not diurnal variation,moon phase and seasonal rhythms in the onset for cerebral haemorrhage(P>0.05).ConclusionsThe cerebral infarction occur more frequently at dawn time.
2.Correlation between blood pressure variability and prethrombotic state indexes in patients with chronic heart failure

Objective:To explore the correlation between blood pressure and prethrombotic state indexes in patients with chronic heart failure (CHF) .Methods:A total of 96 cases with NYHA classⅡselected from inpatients of our department from Apr 2010 to Feb 2013 were enrolled into CHF group ,another 90 patients with normal cardiac function were regarded as normal control group .Daytime ,nighttime and 24h blood pressure and related indexes of prethrombotic state were detec-ted in both groups .Correlation between blood pressure variability and prethrombotic state indexes were analyzed .Results:Compared with normal control group ,there were significant rise in nighttime diastolic blood pressure [nDBP ,(80.53 ± 7.73) mmHg vs .(84.74 ± 8.74) mmHg] ,daytime systolic blood pressure standard deviation [dSSD ,(11.87 ± 4.98) mm-Hg vs .(14.97 ± 5.12) mmHg] ,night time SSD [nSSD (11.62 ± 4.83) mmHg vs .(15.05 ± 5.42) mmHg] and 24hSSD [24hSSD ,(12.98 ± 3.22) mmHg vs .(15.75 ± 5.08) mmHg] , P<0.01;and significant increase in plasma levels of von Willebrand factor [vWF ,(75.64 ± 6.83) IU/dl vs .(90.75 ± 7.32) IU/dl] ,D-dimer [ (0.31 ± 0.03) g/L vs .(0.73 ± 0.13) g/L] and plasminogen activator inhibitor-1 [PAI-1 ,(22.33 ± 1.14) μg/L vs .(27.46 ± 1.15) μg/L] in CHF group ,P<0.01 all .In CHF patients ,24hSSD were positively correlated with vWF and granule membrane protein (GMP-140) levels (r=0.403 ,0.452 ,P<0.05 both) ,24hDSD and nDSD were positively correlated with GMP-140 level (r=0.421 ,0.413 ,P<0.05 both) ,there were no significant correlation among other blood pressure variability indexes and PTS indexes , P>0. 05 all .Conclusion:In CHF patients ,blood pressure variability and prethrombotic state indexes levels significantly rise ,the both are significant positive correlation .
3.Application of different detection methods in the diagnosis of rheumatoid arthritis

Objective To compare the autoantibody detection and enzyme linked immunosorbent assay of protein chip in the diagnosis of rheumatoid arthritis (RA).Methods 81 cases of RA were given autoantibodies protein chip and enzyme linked immunosorbent assay(ELISA),the project included rheumatoid factor(RF)and anti cyclic citrullinated peptide antibody(Anti -CCP),anti nuclear factor(APF),anti keratin antibody(AKA)and anti-nuclear antibody(ANA),recorded the test results,time and cost,the income data were statistically analyzed.Results Patients with RA autoantibody protein chip for detection of CCP,AKA,RF,APF,ANA positive rates were 93.83%, 92.59%,90.12%,95.06%,92.59%,ELISA for the detection of CCP,AKA,RF,APF and ANA positive rates were 95.06%,91.36%,92.59%,91.36%,93.83%,two detection methods had no significant differences(χ2 =1.908, 1.345,1.764,2.246,1.102,all P >0.05);autoantibody detection protein chip required time and cost were respec-tively (1.33 ±0.19)h,(151.24 ±29.83)yuan,which were significantly less than the detection of ELISA (6.81 ± 1.24)h,(899.67 ±121.35)yuan,the differences were statistically significant(t =7.989,8.235,all P <0.05 ). Conclusion The detection of antibody protein chip for RA diagnosis with high accuracy,short time,less cost,help the doctor to diagnose the disease quickly obtain test results.
4.Prokaryotic expression and polyclonal antibody preparation of the glutathione S-transferase and midkine fusion protein

ObjectiveTo express and purify glutathione S-transferase(GST) and midkine(MK) fusion protein and prepare the ployclonsl antibody against MK.MethodsThe coding sequence of MK gene was amplified by RT-PCR from human gastric cancer tissue and inserted into pGEX-1λT vector.The recombinant vector was transformed into E.coli.BL21 ( DE3 ) to express the fusion protein GST-MK via IPTG induction.The expressed fusion protein was purified by GST affinity system and used to immunize rabbits for preparing the ployclonal antibody against GST-MK.ResultsThe expression vector pGEX-MK was constructed successfully.The expressed fusion protein was obtained by GST affinity system.The titer of the polyclonal rabbits antiserum was 1 ∶ 64 000 by indirect ELISA,Western blotting analysis showed that the antibody specifically bind to MK.ConclusionThe expressed protein and prepared polyclonal antibody provided useful reagents for MK further reaserch.
5.Study on the correlation between plasm homocysteine with carotid intima-media thickness in patients with type 2 diabetes

Objective To investigate the correlation between the plasm level of homocysteine(Hcy) and carotid intimal-medial thickness(IMT) in type 2 diabetes(T2DM).Methods 54 patients with preliminary diagnosis of T2DM were divided into two groups according to IMT level:normal IMT group and IMT increased group.30 healthy people were chosen as control.Hcy were detected in all patients and healthy people.Results Hcy levels in T2DM was (24.77 ± 10.17) μmol/L,which was higher than that in healthy people [(10.95 ± 2.60) μmol/L] (P < 0.05).Hcy levels in IMT increased group was (19.80 ±4.97)μmol/L,which was higher than that in normal IMT group [(15.00 ±4.83) μmol/L] (P < 0.05).The positive correlation between Hcy and IMT in T2DM was shown (r =0.6384,P < 0.05).Conclusion High Hcy was an independent risk factor of atherosclerosis in patients with type 2 diabetes.
6.Characteristics and treatment of scirrhous gastric carcinoma

Scirrhous gastric carcinoma is a special type of advanced gastric cancer accompanied by extensive interstitial fibrosis and diffuse cellular infiltration. Scirrhous gastric carcinomas are commonly classified as Borrmann 4 gastric carcinomas. Poorly differ-entiated adenocarcinoma and gastric signet ring cell carcinoma are observed during histologic typing. Thickening of the stomach and stenosis are characteristic of scirrhous gastric carcinomas. The prognosis of this type of gastric cancer is poor because of the likelihood of peritoneal metastasis and lymph node metastasis and the limited cure rates of simple surgery. However, operation should be conduct-ed when scirrhous gastric carcinomas are associated with ileus and urinary obstruction induced by peritoneal metastasis. R0 resection and postsurgical chemotherapy with S-1 are necessary for patients who are able to undergo recoverable operations. Preoperative S-1/CDDP combination chemotherapy is the preferred therapy. Moreover, chemotherapy is the best choice for patients who are unable to un-dergo recoverable surgery. Intraperitoneal chemotherapy is practical for controlling the behavior of scirrhous gastric carcinomas, partic-ularly peritoneal metastasis. Moreover, the efficiency of molecular-targeted therapy using bevacizumab and gene therapy through adeno-virus gene therapy still requires further study.
7.Surgical treatment of stage Ⅳ gastric cancer

Stage Ⅳ gastric cancer is defined as gastric cancer with non-curative factors,and the long-term survival rate of patients with stage Ⅳ gastric cancer is around 10%.Chemotherapy has been used for the main therapy of the patients with unresectable stage Ⅳ gastric cancer.Palliative resection of the primary tumor has been recommended only for presence of obstruction,bleeding and other surgical emergency.Debulking operation was irrelevant to the outcome that has been considered by research results.However,the patients with stage Ⅳ gastric cancer of only one non-curative factors (No.16a2,b1 lymph node enlargement;degree of peritoneal metastasis as P0 and positive peritoneal cytology;a small number of liver metastases)have good long-term survival when these patients have achieved R0 or R1 resection.In this regard,comprehensive treatment including surgical resection is the main treatment policy for the patients with stage Ⅳ gastric cancer.In recent years,some clinical studies on improvement of prognosis through surgical resection after conversion therapy have received extensive attention.
8.Analysis of 60 Gastrointestinal Tumor Cases Diagnosed by B Mode Ultrasonography

This paper reports on the real-tim ultrasonographic diagnosis of gastrointestinal tumors in 60 patients ; and the ultrasonograms of gastrointestinal tumors and their classification are analysed. In this report, the diagnostic value of ultrasound in gastrointestinal tumors has been deduced. We consider that pseudorenal signs are the ultrasonography of gastrointestinal system and most of them are gastrointestinal tumors . Our observations suggest that the sonography of pseudorenal sign may be used as an anatomic feature of abdominal masses.
9.Effect of QS Alex 755 nm Laser on the Expression of p16INK4a on the Melanoma Cell Lines

Objective To investigate the effect of QS Alex 755 nm laser on the p16INK4a expression of melanoma cell lines. Methods Cultured melanoma cell lines, HTB 66, Sk-mel-24, G361 were irradiated with different doses of QS Alex 755 nm laser (0.85 ~ 2 J/cm2) and the irradiated cells were harvested 24 h after irradiation. The expression of p16INK4a protein and mRNA levels were detected by flowcytometer and RT-PCR respectively. Results Compared with the nonirradiated cells the level of p16INK4a protein in HTB 66 cell line increased significantly after laser irradiation, and no change in p16INK4a-negative cell line (Sk-mel-24) and less laser-sensitive cell line (G361). Loss of p16 functional activity leading to disruption of the cell cycle arrest was observed in cell line HTB 66. RT-PCR analysis indicated that p16INK4a mRNA increased in a dose-dependent manner in HTB 66. The p16INK4a mRNA expression was not changed in p16INK4a protein negative Sk-mel-24 and G361 cell lines. Conclusions QS Alex 755 nm laser increases the expression of p16INK4a protein in HTB 66 cell line, which may imply that survival cells after laser therapy may undergo DNA damage. Hence, the laser therapy should not be recommended to treat congenital nevi among those with high risk of aberrant p16.
