1.Clinical study of Lulutong injection(路路通注射液) on improving microcirculation after burns

Objective: To observe the effects of Lulutong injection on microcirculation disturbance after burns. Methods: Eighty-eight patients with mild burn were randomly divided into two groups. From the first day after burn, the treatment group received 500 mg Lulutong injection intravenously injected in 5% glucose, once a day for 14 days as a therapeutic course. The control group was treated with 5% glucose. No large amount of solution was infused because the burned patients were mild and no shock was found. Both groups were received anti-infection treatment using 0.6 g of clindamycin. The morphology of nail-fold microcirculation , the state around the nail-fold microcirculation, hemorrheological parameters, changes in coagulation function 3, 7 and 14 days after burn and the side effects of drugs were observed. Results: Compared to the control group, in the treatment group, the arterioles dilated more markedly, the blood flow of micro-vessels became faster, the flowing score of nail-fold microcirculation decreased obviously, the state around the nail-fold microcirculation changed significantly, the total score declined faster, but no obvious changes were found in erythrocyte aggregation index, the number of leukocyte adhesion and coagulation function . There were 2 cases with dizziness. Conclusion: Lulutong injection may improve the microcirculation in patients with burn, and has little side effect.
2.The therapeutic efficacy of middle- and low-dose methylprednisolone combined with methotrexate and selenium yeast on Graves ophthalmopathy

[Summary] The efficacy of the combining therapy of methylprednisolone and methotrexate on the Graves ophthalmopathy ( GO) was observed. Whether selenium supplement is beneficial based on this combining therapy scheme was evaluated. The results showed that the combining therapy of methylprednisolone and methotrexate was effective for the patients with moderate-to-severe GO. Selenium supplement did not further improve the remission of GO.
3.Pirfenidone effects on human hypertrophic scar fibroblasts cultured in vitro

BACKGROUND:Studies have shown that cytokine inhibitor pirfenidone can inhibit biological activity of fibroblasts by regulating a variety of cytokines. It has made good progress in the research and application of anti-fibrosis of internal organs, but the effect and mechanism for hypertrophic scars and skin fibroblasts are unclear. OBJECTIVE:To investigate the effect of pirfenidone on human hypertrophic scar fibroblasts. METHODS:Human hypertrophic scar fibroblasts were cultured using tissue culture method. Passages 3-6 cel s grew wel in the logarithmic growth phase were col ected. Cel s were divided into the control group (0 g/L pirfenidone), 0.15, 0.3 and 1 g/L pirfenidone groups according to different mass concentrations. Cel s were intervened for 12, 36 and 48 hours. RESULTS AND CONCLUSION:MTT, reverse transcription-polymerase chain reaction and enzyme-linked immunosorbent assay results demonstrated that compared with the control group, cel proliferation, transforming growth factorβ1 mRNA expression, types I and III col agen secretion were decreased in the 0.15, 0.3 and 1 g/L pirfenidone groups (P<0.05), and the decrease was most significant in the 1 g/L pirfenidone group (P<0.05). At 24, 48 and 72 hours after intervention, significant differences in inhibitory rate of cel proliferation and the secretion of types I and III col agen were detected among 0.15, 0.3 and 1 g/L pirfenidone groups (P<0.05). Results confirmed that pirfenidone apparently inhibited the secretion of col agen of hypertrophic scar fibroblasts cultured in vitro, transforming growth factorβ1 expression and cel proliferation and viability.
4.Clinical results of pedicle screws with cement augmentation for treating lumbar degenerative diseases in the elderly

BACKGROUND:Elderly patients with degenerative lumbar degeneration often appear insufficient holding power of pedicle screw in spine surgery, which is prone to occur de-pinning and leads to insecure fixation. How to increase the holding power of screws has become a hot research. OBJECTIVE:To observe the early clinical effect of pedicle screws with cement augmentation for treating lumbar degenerative diseases in elderly patients. METHODS:A total of 65 old patients with lumbar degenerative diseases received a treatment between August 2012 and April 2014, and were divided into two groups according to the treatment strategy:treatment group (n=24;internal fixation of pedicle screws with cement augmentation) and control group (n=41;routine internal fixation of pedicle screws). General conditions of patients in two groups were observed and compared. Visual analog scale (VAS) and Japanese Orthopaedic Association (JOA) score system were used for evaluating the lumbar and back pain, and restoration of neurological function in lower limbs respectively. RESULTS AND CONCLUSION:Al of the patients successful y received the surgery and then were fol owed up from 3 to 20 months. The anterioposterior and lateral X-ray film revealed no loosening, loss, fracture of the screws, and no loss of intervetebral space height was found. There was no significant difference in the blood loss and hospital stay between two groups (P>0.05). JOA at postoperative 3 and 6 months, and VAS score at postoperative 3 months were significantly improved after the treatment of pedicle screws with cement augmentation, when compared to control group (P<0.05). VAS scores showed no difference at 6 months postoperatively in two groups (P>0.05). Pedicle screws with cement augmentation for treating lumbar degenerative diseases have the advantages of improving the screws holding strength, reconstructing the stability of lumbar vertebra and obtaining clinical efficacy on degenerative spine.
5.Research progress of methods for brain vigilance improvement.

Vigilance is defined as the ability to maintain attention or alertness over prolonged periods of time. Since Mid-20th century, following the increasing man-machine communication, high level of vigilance has been demanded in many areas including driving safety, medical care and therapy, aerospace and military affairs, etc. Therefore, finding quick methods to improve the level of vigilance has become a key issue in medical study. Based on physical regulation, chemical regulation and physiological regulation, the research progress has been summarized in this paper. We, furthermore, also try to predict the future trend in this academic area and develop some tentative ideas about seeking more effective and convenient ways to improve the level of brain vigilance.

6.Application of primary PCI in elderly patients with AMI

Objective To assess the feasibility,safety and efficacy of primary PCI in elderly patients with AMI.Methods Three hundred and sixty-four AMI patients were divided into ≥75 years old group (n=90) and <75 years old group (n=274).The successful operation rate,preoperative complication rate,mortality and incidence of major adverse cardiovascular events (MACE) were recorded during their hospital stay time.Results The successful operation rate was 94.4%,the postoperative TIMI 3 blood flow rate was 87.8%,and the preoperative complication rate was 10.0 % in ≥75 years old group.The mortality and incidence of MACE were significantly higher in ≥75 years old group than in<75 years old group during the hospital stay time (12.2% vs 2.9%,P =0.001;16.7 % vs 5.1%,P =0.000).Conclusion Transradial primary PCI is a safe,feasible and effective therapy for ≥75 years old AMI patients.

According to the requirement of the national assessment for achieving the infection control criteria, 42 villages (among them,25 villages belonged to the first stratum, and 17 villages belonged to the second stratum) in 14 counties from 5 provinces, including Hunnan, Hubei, Jiangxi, Anhui and Yunnan, were selected as sampling villages for the assessment.The results from the field assessment showed that 154 out of 9 067 people were found infected with Sckistosoma japonicum, with an average infection rate of 1.7% ranged from 0.31 % to 4.10% , and only Yongping Village from Weishan County and Tenglong Village from Eryuan County were not found any case. A total of 46 out of 3 323 head of cattle were infected with S. japonicum, with an average infection rate of 1.38% ranged from 0.26% to 3.79% , and no any infected individual detected in Nanling County. No outbreak occurred in those sampling villages. Therefore, it is indicated that the five sampling provinces have reached the national criteria on infection control of schistosomiasis.
8.Monitoring and analysis of lens dose to interventional radiology workers at some hospitals in Shaanxi province from 2018 to 2020

Objective:To analyze and learn about the dose to the lens of the eye of interventional radiology workers in some hospitals in shaanxi province from 2018 to 2020.Methods:From 2018 to 2020, three tertiary hospitals in shaanxi province were selected to monitor the dose to the lens of the eye of interventional radiology workers for three consecutive years. Monitoring was made for 152 person times, lasting for a monitoring period of 3 months. Analysis and comparison were carried out of dose equivalents to the interventional radiology workers in terms of different sexes, positions and departments, together with associated influencing factors during the three years.Results:The difference in equivalent dose between the three years is statistically significant ( χ2=29.15, P<0.05), and a downward trend in the average annual equivalent dose was found from 2018 to 2020. The number of interventional radiology workers who received doses to the lens of the eye mainly between MDL and 5.0 mSv accounted for 69.08%. The difference in the average annual equivalent dose in different positions and departments was statistically significant ( H=18.44, 22.55, P<0.05). The average annual equivalent dose to doctor was higher than for nurses and technicians ( Z=-3.36, -3.02, P<0.05). The average annual equivalent dose in cardiovascular medicine department was higher than in other departments, with statistically significant difference ( Z=-2.58, -3.76, -3.40, P<0.05). Logistics regression analysis showed that radiation working hours, positions, routine personal doses, workload and exposure time are the factors that affect the average annual equivalent dose. Conclusions:The annual equivalent dose to the lens of the eye of interventional radiology workers in shaanxi province meets the relevant national standards. However, the annual dose to some radiological workers has a normal high value. Therefore, it is necessary to strengthen the continued monitoring of the dose to the lens of the eye of the interventional radiology workers, and adjust the types of work for the workers based on radiation work hours, position, routine personal dose, workload and exposure time, so as to effectively reduce the dose level of the eye lens.
9.Prevalence and epidemic characteristic of overweight, obesity, and central obesity in Shanghai Pudong New Area

Objective To explore the prevalence and epidemic characteristics of overweight, obesity, and central obesity in Shanghai Pudong New Area. Methods Multi-stage stratified random sampling method was conducted in Pudong New Area in 2013. Residents over 15 years old were randomly selected from 13 communities and participated in the cross-sectional study. A structured questionnaire was used to collect the information and physical examination, biochemical assays were also conducted. SPSS 20. 0 was used to analyze the results. Results The averagebodymassindexofparticipantswas(25.04±3.79)kg/m2andtheaveragewaistcircumferencewas(82.44± 9. 51) cm. The prevalences of overweight, obesity, and central obesity were 41. 67%, 17. 70%, and 31. 32%respectively. Standardized prevalences of overweight, obesity, and central obesity were 34. 39%, 16. 96%, and 25. 66% respectively, according to 2010 national age composition of population. Statistically significant sex differences were found among the prevalence of overweight, obesity, and central obesity groups(all P<0. 05). The prevalence of central obesity in female was raised by increasing age. Residents with hypertension, hyperglycemia, dyslipidemia, and metabolic disorders had higher prevalences in overweight, obesity, and central obesity ( all P<0. 05). Conclusion The prevalences of overweight, obesity, and central obesity among the residents in Shanghai Pudong New Area were relatively high. Relevant risk factors should be explored to promote health education that may enhance people′s awareness of weight management.
10.Exploration of the relationship between the storage time of leukodepleted red blood cell and transfusion adverse reactions

【Objective】 To explore the relationship between the storage time of leukodepleted red blood cells and transfusion adverse reactions by analyzing the occurrence of transfusion adverse reactions of patients after leukodepleted red blood cells transfusion from four hospitals. 【Methods】 By using the electronic medical record management system, the collection and transfusion dates of leukodepleted red blood cells from four hospitals in Enshi Prefecture from 2018 to 2022, as well as the information on transfusion adverse reactions, were retrieved. 【Results】 From 2018 to 2022, a total of 697 61 bags of leukodepleted red blood cells were transfused in four hospitals, resulting in 166 cases of transfusion adverse reactions, among which 93 were allergic reactions, 63 were non hemolytic febrile reactions, and 10 were others, with a total incidence rate of transfusion adverse reactions at 0.24%. The average storage time of leukodepleted red blood cells with and without transfusion adverse reactions was (20.25±6.31) and (19.88±5.50) days, respectively. With a storage time of 7 days as the threshold, the incidence of transfusion adverse reactions was the lowest for a storage time of 15~21 days. The incidence of transfusion adverse reactions of leukodepleted red blood cells in two groups (with storage days ≤21 days and >21 days) was not statistically significant(P>0.05). 【Conclusion】 Allergic reactions were the main type of transfusion adverse reaction caused by leukodepleted red blood cells, and the incidence of transfusion adverse reactions decreased and then increased with the prolongation of the storage time of leukodepleted red blood cells. There was no significant difference in the incidence of transfusion adverse reactions with leukodepleted red blood cells stored for ≤ 21 days and >21 days.
