1.Application of comprehensive nursing intervention in patients with gastroesophageal reflux

Objective To investigate the effect of comprehensive nursing intervention on patients with gastroesophageal reflux disease(GERD).Methods 68 patients with GERD from January 201 2 to January 201 5 in the 1 st Affiliated Hospital of Bengbu Medical College were randomly divided into observation group (n =33)and control group(n =35).The symptom scores and life quality scores of the patients in the two groups were compared. Results 4 weeks after discharging,the symptom scores of reflux,heartburn,chest pain,belch in the observation group were (2.5 ±1 .0)points,(2.8 ±1 .2)points,(2.8 ±1 .3)points,(2.1 ±1 .2)points respectively,which in the control group were (4.8 ±1 .6)points,(4.4 ±2.1 )points,(3.8 ±1 .2)points,(3.4 ±1 .4)points respectively,the differ-ences were statistically significant(t =7.34,4.87,4.32,4.71 ,all P <0.05).1 2 weeks after discharging,the symptom scores of reflux,heartburn,chest pain,belch in the observation group were (1 .8 ±0.8)points,(2.2 ±1 .0)points, (1 .5 ±0.4)points,(1 .5 ±0.6)points respectively,which in the control group were (4.7 ±1 .4)points,(4.5 ± 1 .5)points,(3.0 ±0.6)points,(3.2 ±1 .6)points respectively,the differences were statistically significant(t =1 0.98,7.43,6.1 2,6.02,all P <0.05).Meanwhile,the symptom scores of the observation group 1 2 weeks after discharging were lower than those of the observation group 4 weeks after discharging (t =2.96,1 .86,7.55,4.62,all P <0.05).The life quality scores of the observation group were significantly higher than the control group,the differ-ences were statistically significant(all P <0.05).Conclusion Comprehensive nursing intervention can significantly improve symptoms and quality of life in patients with GERD.With the comprehensive intervention of patients under-taking,patients can benefit more.

As a most effective monomer composition from bark of Pacific Yew,paclitaxel and its derivatives are used in clinical practice as broad spectrum anticancer drugs.Since its discovery in the 1970 s,many researches had been carried out,mainly focusing on the modification,structure-activity relationship and pharmacological activity.The great successes pressed ahead the development of a series of taxol-like drugs,including taxol,docetaxel,cabazitaxel,larotaxel.Nowadays,studies of taxol are still the hotpots,which concentrated on the new source such as cultivation of tissue,fungus culture and new dosage forms.As the representative of drugs research from natural source,taxol is worth to be summarized of its history and ongoing development for looking forward to bring new innovation mentality in new drugs.
3.Silencing the PIK3CA Gene Enhances the Sensitivity of Childhood Leukemia Cells to Chemotherapy Drugs by Suppressing the Phosphorylation of Akt

PURPOSE: This study aimed to investigate the effects of PIK3CA on the sensitivity of acute B lymphocytic leukemia cells (Nalm-6 cells) to chemotherapy drugs. MATERIALS AND METHODS: Children's normal B lymphocytes and Nalm-6 cells were cultured. Nalm-6 cells were transfected with PIK3CA siRNA (siPIK3CA group) or its negative control (PIK3CA-Control group). Normal Nalm-6 cells were named Mock group. Nalm-6 cells transfected by PIK3CA siRNA were treated with Akt inhibitor (siPIK3CA+Akti-1/2 group). mRNA and protein expression was detected by qRT-PCR and Western blot. Proliferation and sensitivity to chemotherapeutic drugs was detected by MTT assay. Cell cycle and apoptosis was explored by low cytometry. Transwell assay was performed to test invasion. RESULTS: PIK3CA mRNA (p=0.008) and protein (p=0.006) expression was higher in Nalm-6 cells than that in normal B lymphocytes. Compared with the Mock group and PIK3CA-Control group, Nalm-6 cells of the siPIK3CA group had lower OD495 values (all p < 0.05) and invasion cell numbers (p=0.03 and p=0.025), as well as a higher proportion of G0/G1 phase cells (p=0.020 and p=0.022), percentage of apoptosis (p=0.016 and p=0.022), and inhibition rate (all p < 0.05). pAkt expression in the siPIK3CA group (p=0.026 and p=0.031) and siPIK3CA+Akti-1/2 group (p=0.019 and p=0.023) was lower than that in the Mock group. CONCLUSION: PIK3CA silencing inhibited Nalm-6 cell proliferation and invasion, and promoted their apoptosis and sensitivity to chemotherapeutic drugs, potentially through regulation of the PI3K/AKT signaling pathway.

4.Construction and validation of a risk prediction model for unplanned readmission of patients undergoing cardiac resynchronization therapy

Objective:To construct a risk prediction model for unplanned readmission of patients undergoing cardiac resynchronization therapy (CRT) and verify the performance of the model.Methods:Using convenience sampling, patients who underwent CRT at the Department of Cardiovascular of the First Affiliated Hospital of Zhengzhou University from July 2017 to July 2020 were selected as the modeling group ( n=279) and the internal validation group ( n=120). CRT patients admitted to the Department of Cardiovascular of the First Affiliated Hospital of Zhengzhou University from August 2021 to August 2022 due to the same or related diseases were selected as the external validation group ( n=86). Multivariate Logistic regression was used to explore the influencing factors of unplanned readmission of CRT patients and establish the prediction model. The fitting effect and discrimination of the model were evaluated through the Hosmer-Lemeshow test and receiver operating characteristic (ROC) curve. The nomogram was established based on R-4.1.2 and Rstudio software. Results:The multivariate Logistic regression analysis showed that creatinine, left atrial diameter, pulmonary artery systolic pressure, New York Heart Association (NYHA) classification, and body mass index (BMI) were risk factors for unplanned readmission in CRT patients, with statistically significant differences ( P<0.05). The prediction model formula was: P=1/{1+exp[- (0.792×creatinine+1.408×left atrial inner diameter+0.887×pulmonary artery systolic pressure+0.769×NYHA classification-0.970×BMI-2.266) ]}. The area under the ROC curve was 0.874, the maximum value of the Jordan index was 0.636, the optimal threshold was 0.256, the sensitivity was 0.826, and the specificity was 0.810. The accuracy of internal validation and external validation was 90.00% and 90.70%, respectively. Conclusions:The constructed prediction model for unplanned readmission of CRT patients has good predictive performance, and the visualized nomogram improves the practical performance of the model. It helps medical and nursing staff identify high-risk groups of unplanned readmission of CRT patients in the early stage and provides a basis for formulating nursing strategies for different risk groups.
