1.Clinical observation on the association among carotid atherosclerosis, homocysteine and anterior ischmic optic neuropathy

AlM: To investigate the relationship among carotid atherosclerosis, homocysteine ( Hcys ) and anterior ischmic optic neuropathy( AlON) .METHODS: Sixty-five AlON patients enrolled in AlON group and 70 non - AlON controls enrolled in control group. All the participants in 2 groups were examined by carotid artery color doppler flow imaging to evaluate the incidence of carotid atherosclerosis. Plasma levels of Hcys, vitamin B12 and folate were measured by automatic biochemical analyzer. The incidence of carotid atherosclerosis, plasma levels of Hcys, vitamin B12 and folate were compared statistically between AlON and control group.RESULTS: Fifty - six AlON patients ( 86%) exhibited carotid atherosclerosis, which was found in 43 control subjects ( 61%) . Fifty - nine AlON patients ( 91%) exhibited increased plasma levels of Hcys compared with that in 38 control subjects ( 54%) . Lowered vitamin B12 was found in 37 patients with AlON ( 57%) and in 43 control subjects ( 61%) . Lowered folate was found in 54 AlON patients ( 83%) and in 32 control subjects ( 46%) . The incidence of atherosclerosis in carotid artery ( P =0. 001), mean Hcys level (P=0. 0005) and lowered folate ( P = 0. 0006 ) were different statistically between AlON group and control group. Mean vitamin B12 level was indifferent between AlON group and control group ( P=0. 0722). Logistic regression analysis showed that carotid atherosclerosis was an a risk factor for AlON (OR=1. 312, 95%CI:0. 927~1. 772), whereas elevated Hcys level (OR=2. 378, 95% CI: 1. 479 ~ 3. 821 ) was also shown to be significant risk factor for AlON. CONCLUSlON: Carotid atherosclerosis and elevated Hcys were related with AlON. Reducing above-mentioned risk factors related to thrombus may be crucial to the prevention and cure of AlON.
2.Application of Color Doppler Ultrasonography by Bed Side in the Early Diagnosis of Hypoxic-Ischemic Encephalopathy in Full Term Neonates

Objective To explore the value of color doppler ultrasonography by bed side in the early diagnosis of HIE in full term neonates.Methods The changes of cerebral parenchymal and cerebral arterial blood stream parameter on 35 cases of neonates clinically diagnosed HIE of mild and moderate degree and 40 cases of normal newborns on the 24,48 and 72 hours after birth were observed by color doppler ultrasonography by bed side.Results 1.The cerebral parenchyma was even echo in normal newborns,but it was maldistributed and reinforced in mild asphyxia neonates and it was more serious in moderate degree.The echo of cerebral parenchyma in mild degree was near normal in 48 hours after birth,while the echo of cerebral parenchyma in moderate degree was still maldistributed and reinforced in 48 and 72 hours after birth.2.There was obvious changes in the cerebral arterial blood stream parameter and hemodynamics of the asphyxia newborns compared with normals.The systolic peak velocity(Vs)and end diastolic velocity(Vd)of the cerebral arteries in mild and moderate degree were obviously lower than that of control group in 24,48 hours after birth(Pa0.05).3.Resistance index(RI)of the cerebral arteries in mild and moderate degree were higher than that of control group in 24,48 hours after birth(Pa0.05).Conclusion Color doppler ultrasonography by bed side is a convenient,noninvasive method for diagnosing HIE.
3.Nitric Oxide Regulated Expression of ?-Aminobutyric Acid B Recepto r Subunits during Recurrent Febrile Seizures

Objective To explore the effect of nitric oxide (NO) on ?-aminobutyric acid B receptor (GABA_BR) subunits during recurrent febrile seizures (FS).Methods Twenty-four Sprague-Dawley rats aged 21 days were randomly divided into 4 groups: control group (37.0 ℃ water,n=8), FS group (45.2 ℃ water,n=8), FS + SNP group (45.2 ℃ water,n=8), FS+L-NMMA group (45.2 ℃ water,n=8). FS rats were induced 10 times in a warm-water bath, once every 2 days. The plasma level of NO was detected by the spectrophotometer. The expressions of GABA_BR subunit mRNA and c-fos gene were examined by in situ hybridization. The expressions of GABA_BR subunit and Fos protein were observed by immunohistochemistry. Results The plasma level of NO increased in FS + SNP group while decreased in FS+L-NMMA group compared with that in FS group. The expressions of GABA_BR_2 were down-regulated in FS+SNP group, while GABA_BR_1 hardly changed compared with those in FS group. In FS+L-NMMA group, both the expression of GABA_BR_2 and GABA_BR_1 up regulated compared with those in FS group. The expressions of c-fos gene and Fos protein were significantly enhanced after recurrent FS. SNP elevated the expressions of c-fos gene and Fos protein, while L-NMMA down regulated the expressions of them.Conclusion NO may play a regulatory role through modulating GABA_BR function in the pathogenesis of recurrent FS.

5.Effects of high performance work system on hospital employees' work engagement

Objective To investigate perception of high performance work system (HPWS) and work engagement for hospital employees in Hangzhou,take work climate for concerning employees as a meditating factor to analyze how HPWS influences work engagement by setting structural equation model.Methods Using cluster random sampling to select 983 hospital employees for questionnaire survey,data was analyzed by statistical description,one-way ANOVA and SEM.Results Hospital employees' average work engagement score was 21.62 ± 4.37.Both HPWS and work climate for concerning employees had overall effects on work engagement for 0.63 and 0.44 respectively.Meanwhile,HPWS made a direct effect for 0.24 and indirect effect for 0.39 mediated by work climate for concerning employees.Conclusion Work engagement for hospital employees could be further enhanced and carrying out HPWS and work climate for concerning employees shall help hospital employees feel the organization put great importance on them and work even harder in return.
6.Studies on anti-endotoxin activity of F022 from Radix Isatidis.

OBJECTIVETo study the anti-endotoxin activity and mechanism of F022 from Radix Isatidis.
METHODThe production of TNF-alpha and IL-6 of murine peritoneal macrophages stimulated by LPS was measured by ELISA. The temperature in rabbits was tested after i.v. administration of LPS. The lethality of BCG-primed mice was induced by LPS.
RESULTIf F022 was added to macrophages culture simultaneously with LPS or 1 h before addition of LPS, production of TNF-alpha and IL-6 by macrophages was remarkably inhibited in vitro. F022 inhibited the fever induced by LPS in rabbits and protected BCG-primed mice from LPS induced lethality if given before administration of LPS.
CONCLUSIONThe anti-endotoxin effect of F022 may inhibit LPS binding to its receptor, and it may be a LPS receptor antagonist.

7.NMDAR expression in the cochlear nucleus and hearing damage in neonatal rats with hyperbilirubinemia.

OBJECTIVETo study the role of N-methyl-D-aspartate-receptor (NMDAR) expression in the development of hearing damage in neonatal rats with hyperbilirubinemia.
METHODSSixty seven-day-old Sprague-Dawley rats were randomly injected with bilirubin of 100 microg/g (low-dose treatment group) or 200 microg/g (high-dose treatment group) or normal saline (control group). Auditory brainstem response (ABR) was examined. The concentrations of bilirubin in blood and brain were measured. NMDAR expression in the cochlear nucleus slices was examined by immunohistochemistry assay.
RESULTSABR reflecting threshold obviously increased, and I, II and III wave latency as well as I-II, II-III and I-III interval were more prolonged in the two bilirubin treatment groups when compared with the control group. The NMDAR expression in the cochlear nucleuse in the two bilirubin treatment groups was obviously lower than that in the control group. The NMDAR expression in the cochlear nucleuse was negatively correlated with the brain bilirubin content and the ABR reflecting threshold in the two bilirubin treatment groups.
CONCLUSIONSAn increased NMDAR activity may play an important role in hearing damage following hyperbilirubinemia.

8.Gamma-aminobutyric acid B receptor regulates the expression of hydrogen sulfide/cystathionine-beta-synthase system in recurrent febrile seizures.

OBJECTIVEFebrile seizure (FS) is one of the most common seizure types in children. Our previous studies have demonstrated that both gamma-aminobutyric acid B receptor (GABABR) and hydrogen sulfide (H2S) are involved in the pathogenesis of FS. This study was designed to explore the effect of GABABR on H2S/cystathionine-beta-synthase (CBS) system in recurrent FS.
METHODSSixty-four Sprague-Dawley rats aged 21 days were randomly assigned into four groups: Control (37 degrees C water bath exposure), FS, FS+baclofen (GABABR excitomotor), and FS+phaclofen (GABABR inhibitor) groups (n=16 each). FS was induced by warm water bath exposure (45.2 degrees C, once every 2 days, 10 times in total. The plasma level of H2S was detected by the spectrophotometer. The expression of CBS mRNA was examined by in situ hybridization. The expressions of CBS protein was observed by immunohistochemistry.
RESULTSThe plasma level of H2S increased in the FS+baclofen group (427.45 +/- 15.91 micromol/L) but decreased in the FS+phaclofen group (189.72 +/- 21.53 micromol/L) compared with that in the FS group (362.14 +/- 19.71 micromol/L). The expressions of CBS mRNA and protein were up-regulated in the FS+baclofen group but were down-regulated in the FS+phaclofen group compared with those in the FS group.
CONCLUSIONSGABABR modulated the expression of H2S/CBS system in recurrent FS.

9.Febrile seizure, but not hyperthermia alone, induces the expression of heme oxygenase-1 in rat cortex.

BACKGROUNDFebrile seizure (FS) is the most common seizure disorders. Approximately one third of children with a febrile seizure have recurrent events. The mechanism of FS remains unclear. Heme oxygenase-1 (HO-1) is a member of the heat shock proteins family and can be induced in the brain by various stresses, including hyperthemia and seizure. This study aimed at investigating the changes of HO-1 in the cortex of rats after recurrent FS.
METHODSFS in rats was induced ten times, once every 2 days. In a bath of warm water, developing rats were randomly divided into two groups: control group (n = 16) and warm water-treated group (n = 50). The latter group was subdivided into hyperthermia group (n = 19) and FS group (n = 23). The expression and content of HO-1 mRNA in cortex were observed using in situ hybridization and quantitative reverse transcription-polymerase chain reaction (RT-PCR). The content of HO-1 protein in cortex was measured using Western blotting.
RESULTSHO-1 mRNA expression of cortex neurons in FS group was markedly increased in comparison with those in hyperthermia and control groups (P = 0.00), however, there was no statistic difference between hyperthermia group and control group (P = 0.16). The relative amount of HO-1 mRNA in cortex in FS group was increased by 53.13% and 96% in comparison with those in hyperthermia group and control group respectively (P = 0.00), but there was no obvious difference between the later two groups (P = 0.051). Western blotting analysis showed that the HO-1 protein content in cortex in FS group was increased by 198% and 246% in comparison with those in hyperthermia group and control group respectively (P = 0.00). There was no obvious difference in HO-1 protein content between the later two groups (P = 0.09).
CONCLUSIONSRecurrent FS in rats can cause the increase of HO-1 mRNA and protein in cortex which may be involved in the mechanism of FS. The short-time recurrent hyperthermia can not induce the increase of HO-1 mRNA and protein.

10.Research of quantitative management evaluation system for clinical nursing work

Objective To explore a practical,measurable difficulty coefficient table of clinical nursing operation techniques,so as to perfect the evaluation system of clinical nursing work quantitative management.Methods Totals of 90 clinical nursing specialists from five three level of first-class comprehensive hospitals and one three level of first-class specialized subject hospital were investigates by questionaire of "the evaluation index system for assessing the difficulty of clinical nursing operation techniques" on 27 basic clinical nursing operation projects and 12 college clinical nursing operation projects.Results Eighty-two questionnaires were taken back,excluding invalid questionnaire eight copies,with 91.1% of effective recovery rate.The difficulty coefficient of 39 clinical nursing operation techniques ranged between 0.67 and 1.98.Rating on the 27 items of basic nursing projects by nursing experts or backbone from different hospitals had no significant different scores [(1.21 ± 0.22) vs (1.22 ±0.19) ;t =0.495,P >0.05],while significant difference was found in 12 items of special nursing projects [(3.4±0.38) vs (2.9±0.31);t=5.169,P<0.05].Conclusions "The evaluation index system for assessing the difficulty of clinical nursing operation techniques" can evaluate the difficulty of clinical nursing operation techniques systematically and comprehensively.The difficulty coefficient table of 27 items of basic clinical nursing operation techniques can be extended in different nature hospitals.It can be referred to explore the difficulty of college clinical nursing operation techniques.
