2.Vitamin E-stabilized ultrahigh molecular weight polyethylene in joint replacement

BACKGROUND:As the latest generation of ultrahigh molecular weight polyethylene, vitamin E-stabilized ultrahigh molecular weight polyethylene has been widely put into use of various kinds of joint replacement. However, wear debris and osteolysis caused by that are stil the vital factor influencing the clinical curative effect. OBJECTIVE:To discuss and summarize the material characteristics and application progress of vitamin E-stabilized ultrahigh molecular weight polyethylene, and to seek the theoretical basis which can guide us to launch vitamin E-stabilized ultrahigh molecular weight polyethylene clinical y. METHODS:The first author retrieved the CNKI and Medline databases by computer. The keywords were“ultra-high molecular weight polyethylene, vitamin E-stabilized, joint replacement, application”in English. RESULTS AND CONCLUSION:Total y 92 articles were retrieved. According to the inclusion and exclusion criteria, 46 articles were included in result analysis. The results show that vitamin E-stabilized ultrahigh molecular weight polyethylene can not only enhance the physicochemical property of prosthesis, but also can integrate with free radicals to increase antioxidant rate of prothesis in order to prolong the long-term curative effect of prosthesis. However, it is stil lack of vast of clinical cases and fol ow-up as evidence. In case of that, long-term clinical efficacy remains to be assessed.
3.Reconstruction of the posterior cruciate ligament with ligament advanced reinforcement system artificial ligament transplantation under knee arthroscopy

AIM: To observe the functional recovery of knee joint in patients with posterior cruciate ligament (PCL) under knee arthroscopy with ligament advanced reinforcement system (LARS) artificial ligament transplantation. METHODS: From January 2005 to May 2006, six patients with PCL injury treated by reconstruction of PCL with LARS artificial ligament transplantation were selected from the Department of Orthopedics, Huadu Hospital Affiliated to Southern Medical University. All subjects agreed with the operation before the surgery and all reconstruction surgeries were of second period. ① The tibial tunnel was established firstly. ② Then was the femoral tunnel. ③ Under knee arthroscopy, the "Y"-shape double cords of LARS artificial ligament were cut through ante-out and meta-in bone tunnels of femur, and fixed at 90o and 30o flexion respectively. Patients were required to motor 3 days postoperative, and patients actively exercised their quadriceps muscle of thigh at one week later. Regular follow-ups were conducted postoperatively, and functional evaluation of the knee joints was performed by according to Lysholm standards, including 8 aspects: Limping, supporting, interlocking, unstable, painful, swell, upstairs and downstairs and squatting with a total score of 100 points. The higher the score was, the better the functional recovery of knee joint. RESULTS: All of the 6 patients were available at follow-up, and no one withdrew from the study. The follow-up was conducted at the 7th, 7th, 10th, 10.4th, 12.8th, and 14th months with an average duration of 10.2 months. After operation, unstable symptoms of knee joint in all subjects disappeared with negative sag sign and posterior drawer. 4.A study about posture changing to ameliorate dysuria for patients with femoral artery puncture and catheterization

ObjectiveTo explore whether the posture changing can ameliorate the dysuria for patients with femoral artery puncture and catheterization, and observe the influence of posture changing on blooding in the point of puncture. Methods Changing patients′ posture under the order of nurses, and then observed the amelioration condition of dysuria. Results After using intervention method, the rate of urinary stasis decreased from 31.67% to 4.17%, the rate of using urethral catheterization also decreased from 8.33% to 1.67%. Conclusion Posture changing was an effective method to ameliorate the dysuria for patients with femoral artery puncture and catheterization.
5.Application of Orem self-care theory to the patients after renal transplantation

ObjectiveTo observe the effect of Orem self-care theory on the patients after renal transplantation.Methods102 patients after renal transplantation into were randomly divided into the observation group(52 cases),which received Orem self-care,and comparison group(50 cases),which received general care.Self-care ability,rate of infection and mental status of the 2 groups patients were observed and recorded.ResultsFor patients of the observation group,self-care ability of was obviously better(P<0.01),the rate of infection was lower(P<0.05),and mental status was improved(P<0.01) compared with patients of the observation group.ConclusionThe application of Orem self-care on patients after renal transplantation can promote self-care ability and decrease post-operation complications.
6.Assessment on left ventricular function of pulmonary heart disease patients using strain rate imaging analysis

Objective To investigate the value of left ventricular function in patients with chronic pulmonary heart disease(CPHD) using strain rate imaging (SRI).Methods This study included 64 patients with CPHD (30 patients with compensatory function and 34 with decompensatory function) and 30 normal control.Peak systolic strain rate (SRs),peak early diastolic strain rate (SRe) and peak late diastolic strain rate (SRa)of left ventricular basal and middle segments were measured,and then mean peak strain rate (mSR) was calculated.The above digital parameters were analyzed compared with the conventional echocardiographic indices.Results The indices (mSRs,mSRe,mSRe/mSRa)in CPHD were reduced and mSRa increased as compared with those in controls (all P=0.000).And the above indices showed the same change when decompensatory group was compared with compensatory group (P=0.000,0.038,0.015,0.001).Negative correlation was noted between LVEF and mSRs in patients with CPHD (r=0.75,0.82;P=0.000).Conclusions LVEF is negatively related with mSRs in CPHD patients.mSRs can reflect the status of left ventricular function.SRI is a more sensitive tool in quantitative evaluation of left function of CPHD.
8.Efficacy and Influence on Serum IL-4 and IFN-γLevels of Xiyanping Injection in Acute Bronchitis Chil-dren

Objective:To study the efficacy and influence on serum interleukin 4 (IL-4) and interferon-γ(IFN-γ) levels of Xiy-anping injection in acute bronchitis children .Methods:Totally 74 cases of acute bronchitis children were divided into the experimental group and the control group with 37 cases in each .The control group was treated with the conventional treatment , while the experimen-tal group was given Xiyanping injection additionally .Before the treatment and in 7 days after the treatment , the level changes of IL-4 and IFN-γ, improvement time of clinical symptoms , clinical curative effect and adverse reactions of the two groups were compared .Re-sults:After 7 d treatment , two groups of children with the level of serum IL-4 levels were lower than the previous treatment , the IFN-γlevels were higher than previous treatment (P<0.05); And experimental groups with serum IL-4, IFN-γlevels were significantly better than control group (P<0.05).Experimental group’s lung sound, cough and expectoration, antifebrile time significantly were shorter than the control group(P<0.05), the clinical total effective rate was significantly higher than the control group (P<0.05). The incidence of adverse drug reactions in two groups had no statistical significance (P)0.05).Conclusion:Xiyanping injection com-bined with the conventional treatment can obviously increase serum levels of IL -4 and IFN-γin acute bronchitis children , improve the clinical symptoms of patients , and shorten the disease course with few adverse reactions and promising clinical curative effect .
9.Expression of adiponectin and adiponectin receptor 2 in the livers of patients with hepatitis B virus-related liver failure

Objective To explore the expressions of adiponectin and adiponectin receptor 2 (AdipoR2) in the liver of patients with hepatitis B virus (HBV)-related liver failure and their clinical implications in the pathogenesis of liver failure.Methods The healthy controls (HC),patients with chronic hepatitis B (CHB) and patients with HBV-related liver failure (HBV-LF) were enrolled in the study.Each group had 20 participants.The liver tissues from 11 CHB patients who were diagnosed by liver biopsy,6 patients with HBV-LF and 6 liver donors during liver transplantation were collected.Histological specimens were observed by hematoxylin-eosin staining and Masson trichrome staining under microscope.The mRNA and protein expressions of adiponectin and AdipoR2 in the liver were measured by semi-quantitative reverse transcription and polymerase chain reaction (SqRT-PCR) and immunohistochemical staining,respectively.The level of serum adiponectin was detected by enzymelinked immunsorbent assay.Serum biochemical parameters and HBV DNA levels were also measured.The pairwise comparison between groups was done by Student-Newman-Keuls and mann whitney U test.The relationship between two variables was analyzed using Spearman correlation.Results The level of serum adiponectin in HBV-LF group [(0.86 ± 0.15) ng/mL] was higher than those in HC [(0.59±0.15) ng/mL] and CHB groups [(0.62±0.13) ng/mL,Z=3.963,Z=3.774,both P<0.01)],but showing no difference between CHB and HC groups (P>0.05).The expressions of adiponectin and AdipoR2 mRNA in the liver were significantly higher in HBV-LF group (0.251 ±0.028 and 0.223 ± 0.021,respectively) than those in HC (0.091 ± 0.018 and 0.072 ± 0.020,respectively) and CHB (0.121±0.019 and 0.097±0.017,respectively) groups (q=18.428,17.069,19.807,18.673,respectively; all P<0.01).Also,the expressions of adiponectin and AdipoR2 mRNA in CHB group were significantly higher than those in HC group (q=3.895,3.860,both P<0.05).The expressions of adiponectin and AdipoR2 proteins in the liver were significantly higher in HBV-LF group (8.482±0.772 and 7.654±0.272,respectively) than those in HC (4.045± 0.815 and 2.804±0.623,respectively) and CHB (5.545±0.613 and 4.775±0.458,respectively) groups (q=15.327,11.542; Z=2.802,3.266; respectively; all P<0.01).Also,the expressions of adiponectin,AdipoR2 proteins in CHB group were significantly higher than those in HC group (q=5.894,Z=3.266,both P<0.01).In HBV-LF group,serum adiponectin levels as well as the expressions of adiponectin mRNA and protein in the liver were negatively correlated with serum albumin (r=-0.815,-0.886,-0.943; P<0.01 or P<0.05),but positively correlated with serum alanine aminotransferase (r=0.701,0.886,0.943; P<0.01 or P<0.05).The expression of AdipoR2 mRNA in the liver was negatively correlated with serum albumin (r=-0.943,P<0.01),but positively correlated with serum aspartate aminotransferase (r=0.829,P<0.05).Conclusions The expressions of adiponectin and AdipoR2 are up-regulated during HBV infection,especially in patients with HBV-LF,which might reflect the degree of necroinflammation in the liver.
