1.Effects of fosinopril and metoprolol on cardiac remodeling and plasma levels of neurohormones in heart failure patients

Objective To explore the effects of fosinopril and metoprolol on cardiac remodeling and the plasma levels of angiotensin II(AngⅡ),endothelin-1(ET-1) and nitric oxide(NO) in patients with congestive heart failure(CHF).Methods A total of 80 patients with CHF were divided into 3 groups,fosinopril(20 mg/d),metoprolol(from 6.25 mg,twice a day to 100 mg,twice a day or the highest tolerant dose) and combination groups(as in single use),and all were treated for 12 months.Electrocardiography including ejection fraction(EF)and left ventricular end diastolic diameter(LVEDD)was carried out,and plasma levels of the 3 neurohormones were detected with conventional methods at admission and 12 months after the treatment.(Results Plasma) levels of AngⅡ,ET-1 and NO were decreased significantly in all groups.No significant difference was seen between fosinopril and metoprolol groups,but the plasma levels was decreased remarkably(P
2.Relationship between cardiac resynchronization therapy and change of left atrial volume index

Objective To prospectively assess the association between cardiac resynchronization therapy (CRT )and left atrial volume .Methods Fourteen patients with chronic heart failure ,sinus rhythm and left bundle branch block were enrolled in this study and prepared for CRT implantation .Clinical and echocardiographic evaluations were performed before CRT implantation and 6 months later .Followings were parameters of the left atrial and left ventricular volume:left ventricular end-systolic volume index (LVESVi)and end-diastolic volume index(LVEDVi) ,maximal LAV index(iLAVmax) ,etc ,LAV index before atrial systole(iL-AVpre)and minimal LAV(iLAVmin) .CRT responders were defined as those whose reduction rates were more than 10% in iL-VESV in 6-month follow-up .Results Responders(71 .4% )and non-responders(28 .6% )had similar baseline ,clinical characteristics and pre-implantation LV volumes .However ,iLAVmax in the responders was remarkably lower than that in non-responders .CRT therapy significantly reduced LVESVi(P<0 .01) ,LVEDVi(P<0 .001) ,iLAVmax(P=0 .004) ,iLAVpre(P=0 .003)and iLAVpost (P<0 .01) ,but when compared with the non-responders ,iLAVmax was much lower in responders .Correlation analysis showed that there was a significant correlation of LVEDVi with iLAVmax ,iLAVpost(P<0 .05) .Conclusion CRT therapy can significant-ly improve iLAVpre and iLAVmax may be a predictor of LV reverse remodeling .
3.The exploration of medical microbiology teaching in seven-year MD program——from case-based discussion to theme discussion of PBL teaching

In order to complete the seven-year microbiology teaching better,we used the theme discussion of PBL teaching in our teaching activities,i.e.by setting a certain scenario and giving some background information before the students'group discussion. The practice proved that the theme discussion of PBL teaching method in theoretical teaching and learning changed the traditional case-based discussion chiefly by teachers; as a result,the relative immobilization of the teaching mode became more flexible. These methods of teaching and learning not only stimulated the students'initiative and enthusiasm,but also helped them to think critically. Our assessment of PBL teaching and learning showed that the themes discussion of PBL in medical microbiology achieved better results.

6.Progress in Research of the Role of Hepatitis C Virus Protein in Cell Signal Transduction Pathway

Abnormal cell signal transduction is associated with the occurrence and development of human diseases. Some virus pathogenicity and infection mechanism are due to virus antigen protein acting on the host cell signal transduction pathway, leading to host cell signal transduction disorder. Hepatitis C virus (HCV) is a major pathogen of chronic hepatitis C, which causes cirrhosis and hepatocellular carcinoma. But the pathogenesis of HCV and persistent infection mechanism remain far from clear. HCV pathogenesis may be related to the HCV protein expression interfering host cell signal transduction pathways. The studies of hepatitis C virus proteins acting on host cell signal transduction pathways, not only help to clarify the impact of its pathogenic mechanisms, but also benefit to new drug design and development for new treatment methods. This article summarizes the recent progress in research on the effect of hepatitis C virus protein in cell signal transduction pathways in the past few years.
7.Nursing of patients with rheumatic valvular heart disease with atrial fibrillation treated with CoxⅢ labyrinth and simultaneous valve replacement operation

Objective To summarize the effect and experience of nursing of patients with rheumatic valvular heart disease with atrial fibrillation treated with Cox Ⅲ labyrinth and simultaneous valve replace-ment operation. Methods A retrospective analysis was carried out in the nursing of 58 patients with rheumatic valvular heart disease with atrial fibrillation treated with Cox Ⅲ labyrinth and simultaneous valve replacement operation in our hospital from April 2006 to July 2008. Results One patient died of periop-erative renal failure,one died of low cardiac output,one died of infectious endoearditis.Tbe mortality rate reached 5.2%.Other patients were discharged from hospital after completely cured.The survived patients maintained sinus rate during hospitalization days after operation and short-term follow-up after being dis-charged.Among 43 patients who completed follow-up of 6 months after operation,3 patients (7.0%) damon-strated atrial fibrillation,while others still kept sinus rate. Conclusions Attention should be paid to psy-chological nursing,respiratory tract preparation,cardiac function monitoring and nursing of anticoagulation treatment for patients treated with Cox Ⅲ labyrinth and simultaneous valve replacement operation.

Objective To establish a method for determination of chrysophanol,emodin and rhein in Huangzhi capsules.Method Chrysophanol,emodin and rhein in Huangzhi capsules was determined by TLC-scanning with a mixture of n-hexane-ethylacetate-formic acid(30∶10∶0.5) as the developing system.The detection wavelength was 435 nm and 610 nm.Results The calibraction curve of chrysophanol,emodin and rhein was linear in the range of 0.014 65~0.073 25,0.013 35~0.066 75,0.012~0.072 ?g respectively.Conclusion The method was simple,accurate with good repeatability,and suitable for the content determination of chrysophanol,emodin and rhein in Huangzhi capsules.
9.A study of the efficacy and safety of antithrombotic therapy in aging patients with atrial fibrillation and coronary disease

Objective To investigate the efficacy and safety of different strategy of antithrombotic therapy in elderly patients with atrial fibrillation (AF) and coronary disease.Methods 224 inpatients and outpatients with AF and coronary disease whose mean follow-up time was 2 years,were divided into warfarin group,warfarin combined with antiplatelet drug(aspirin or clopidogrel) group,antiplatelet drug(aspirin or clopidogrel) group and control group.The thromboembolic event rate and adverse events incidence were observed in different groups.Results The risk of thromboembolic events,particular emerging risk of cerebral infarction in the control group(12.50 %) was significantly higher than other groups (P< 0.05).Thrombotic event rates in warfarin + single joint group(2.78%) was lower than other groups(warfarin group 4.29 %,single joint group 5.81%),but there was no significant statistical difference.The overall risk of bleeding in the control group (3.13 %) was significantly lower than other antithrombotic group (P<0.05).The incidence of skin bruising in single joint group was lower than other antithrombotic groups,the overall bleeding risk,especially gasfrointestinal bleeding in warfarin+ single joint group was significantly higher than other groups (P< 0.05).Conclusion Elderly patients with coronary heart disease and atrial fibrillation need antithrombotic therapy to prevent thrombotic events.Using warfarin alone or warfarin combined antiplatelet drug does not lead to serious bleeding events,but generally bleeding risk should be attracted more attention.
10.Preparation and characterization of nanoparticles in-loaded one-way release-controlled chitosan membrane

Objective To prepare and characterize nanoparticles in-loaded one-way release-controlled chitosan membrane,and to explore the release-controlled rule of the film in vitro.Methods The chitosan nanoparticles were prepared by inverse crosslinking-emulsion method.The one-way release-controlled membrane was prepared by a casting method.Transmission electron microscope (TEM) was used to evaluate the morphological properties and particle size analyzer was used to analyze particle size distribution.The morphology of the membrane was inspected through scanning electron microscope (SEM).MTT assay was applied to determine the biological safety of chitosan nanoparticles.The distribution of the nanoparticles was observed by fluorescence microscope.The in vitro release studies were adopted to evaluate the release-controlled rule.Results The four kinds of nanoparticles had spherical shapes and uniform particle size.The size of the hyaluronic acid-coated chitosan nanoparticle was (255.40±39.10) nm.Hyaluronic acid-coated chitosan nanoparticles showed the best property of sustained release and biocompatibility.The membrane had a loose inner layer and a dense outer layer,and the distribution of the nanoparticles was uniform in the inner layer of the membrane.The release of protein from membrane was unidirectional and the membrane displayed good controlled release property.Conclusions The nanoparticles in-loaded one-way release-controlled chitosan membrane presents good one-way sustained release performance.It is potentially useful in delivery system of growth factors.
