1.Changes of urinary albumin and N-acetyl-beta-glucosaminidase in high altitude polycythemia at different altitudes and its mechanism

AIM: To study the changes of excretive amount of albumin(ALB) , N-acetyl-beta-D-glucosaminidase(NAG) in patients with high altitude polycythemia(HAPC) at different altitude and its mechanism. METHODS: Urinary ALB, NAG, activity of red blood cell superoxide dismutase(RBC-SOD), malonyldialdehyde(MDA) and erythrocyte filtration index(EFI) of 231 healthy subjects and 86 patients with HAPC at different altitude were detected. RESULTS:Excretive amount of urinary ALB, NAG, and plasma MDA, EFI were higher,and RBC-SOD was lower in patients with HAPC than those in healthy subjects, the changes were significantly obvious, with increasing altitude. Urinary ALB and NAG were negatively correlates to EFI, and no correlated to the content of MDA and RBC-SOD. CONCLUSIONS: EFI exertes an independent influence on urinary ALB and NAG. The decrease in erythrocyte deformability caused by the increase of peroxidation of lipids might participate in the pathogenesis and progress of the increase of excretive amount of ALB and NAG in HAPC.
2.The Current Situations of Domestic and Oversea Health Econometrics Analysis and the Application Prospect of Health Area in China

Objective: To plan the future for the establishment and development of health econometrics in China. Methods: To analyze the main current situation of the research on domestic and oversea health econometrics, and to look into the prospective future of its application field of public health in China. Results: Overseas studies on health economics have been relatively mature and extended to generate health econometrics as the independent discipline. However, the research on health economics in China is lagging, and on health econometrics even vacant. Conclusion: The results from foreign studies have revealed that this new discipline plays a unique and significant role in medical health research. Health econometrics occupies broad application prospect in the field of public health in China, which should be paid more attention and support.
3. Chemical constituents from active fraction in roots and rhizomes of Cynanchum paniculatum with reversal activity of multidrug resistance

Objective: To investigate the chemical consituents from the active fraction in the roots and rhizomes of Cynanchum paniculatum with reversal activity of multidrug resistance. Methods: The active fraction was evaluated for reversing activities toward three human drug-resistance cell lines to clininally common used anticancer chemotherapeutic drugs. The compounds were isolated and purified by chromatography on ODS and preparative HPLC. Their structures were elucidated on the basis of chemical and spectroscopic methods, including MS, 1D, and 2D NMR spectral techniques. Results: The active fraction exhibited the significant effects in sensitization of human drug-resistance on gastric carcinoma cell line SGC-7901/VCR and human drug-resistance on colonic carcinoma cell line HCT-8/VCR to fluorouracil (5-FU). Nine compounds were isolated and identified as hancogenin B 3-O-β-D- oleandropyranoside (1), 3β,14-dihydroxy-14β-pregn-5-en-20-one (2), neocynapanogenin F (3), glaucogenin A (4), glaucogenin C (5), neocynapanogenin F 3-O-β-D-oleandropyranoside (6), glaucogenin C 3-O-β-D-thevetoside (7), glaucogenin A 3-O-β-D- oleandropyranoside (8), and 20-hydroxypregna-4,6-dien-3-one (9). Conclusion: Compound 1 is a new steroidal saponin named paniculatumoside D. C21 Steroids isolated from the active fraction in the roots and rhizomes of C. paniculatum have the potential value as multidrug resistance reversing agents.
4.The study on constructing tissue-engineered bone by seeding rat marrow stromal stem cells onto three-dimensional porous nano-hydroxylapatite

Objective:To investigate the feasibility of using marrow stromal cells (MSC) as seed cells and three-dimensional porous nano-hydroxylapatite as scaffolds for constructing tissue-engineered bone.Methods:MSC from rat were cultured and induced to differentiate into osteoblasts in vitro. And then these induced cells were identified and seeded onto three-dimensional porous nano-hydroxylapatite scaffolds, cultured for 15 days in vitro. Scanning electron microscopy was used to evaluate the growth of these induced cells on three-dimensional porous nano-hydroxylapatite scaffolds. Results: The activity of alkaline phosphatase (ALP) and the secretion of osteocalcin of MSC from rat appeared and were increased gradually along the culture. The cells seeded on three-dimensional porous nano-hydroxylapatite scaffolds could adhere and proliferate well, and come into being many tiny calcium nodules and collagenous fibers. Conclusion:The data demonstrated that the new developed culture method is conducive to MSC’s differentiating and proliferating into osteoblasts that have a fine activity and the three-dimensional porous nano-hydroxylapatite may be considered as a suitable scaffold for the seeded cells. Using marrow stromal cells as seed cells and three-dimensional porous nano-hydroxylapatite as scaffolds is advantageous for constructing tissue-engineered bone.
5.Morphology of elderly people's periodontal ligament cells attached to the three-dimensional porous nano-hydroxylapatite

Objective:Because of the lower proliferation ability of periodontal ligament cells of elderly people,how to choose suitable scaffold materials for them becomes very important.The aim of this study was to observe the morphological manifestation of the periodontal ligament cells of elderly people seeded onto the three-dimensional porous nano-hydroxylapatite scaffolds.Methods: Periodontal ligament cells from elderly people were seeded onto the three-dimensional porous nano-hydroxylapatite scaffolds,cultured for 15 days in vitro,and observed for the morphological changes by scanning electron microscopy in comparison with those of young people.Results:Fifteen days after seeded on the three-dimensional porous nano-hydroxylapatite scaffolds,the periodontal ligament cells of the elderly people exhibited sufficient adhesive behavior,proliferated well and produced tiny calcium nodules,but the number of the nodules was smaller than that in the young people.Conclusion: The periodontal ligament cells of elderly people grow well on three-dimensional porous nano-hydroxylapatite scaffolds,and can be considered as a suitable scaffold for periodontal tissue engineering in elderly people.
6.Impact of overweight and obesity on flow mediated dilation in adult

Objective To explore the impact of overweight and obesity on flow mediated dilation (FMD) in adult people.Methods Intact clinical data of 183 healthy people, aged 30~65 in our hosipital were analyzed.According to BMI, all people were divided into obesity group (48 cases), overweight group(75 cases) and normal group (60 cases).FMD was detected by E tracing of ultrasonic.Weight, WC, BFP, baPWV, BP,FPG,CHO,HDL-C,LDL-C,TG and UC were observed at the same time.FMD of three groups were compared and the risk factors of FMD were analyzed.Results The level of FMD in the obesity group and overweight group was significantly higher than that in the normal group[(5.63±2.76)%,(6.45±2.54)% vs.(8.31±3.04)%](F=13.77,P<0.001);there was no statistically significant difference between the obesity group and overweight group (t=1.68,P>0.05).Age(β=0.078,95%CI: 1.004,1.164),BMI(β=0.465,95%CI 1.148,2.208),LDL-C(β=2.294,95%CI: 9.918,1.612) and UC(β=0.007,95%CI 0.986,1.000) were independent risk factors of decreased FMD(P<0.05).Conclusion Age, BMI, LDL-C and UC are independent risk factors of decreased FMD.

8.Effects of intermedin preconditioning on cyclin expression after renal ischemia-reperfusion in rats

Objective To observe the effect of intermedin (IMD) preconditioning on cyclin D1, cyclin E and CDKs expression, and explore its role in.promoting kidney tissue regeneration after renal ischemiareperfusion injury. Methods One hundred and forty-four healthy male Wistar rats were randomly divided into four groups: sham operation (S) group, ischemia-reperfusion injury (IR) group, empty plasmid (EP) group and IMD group. In the IR group, after the right kidney was excised, the aorta abdominalis and left renal artery were bluntly dissected in EP and IMD group, empty plasmid and IMD plasmid were transfected into the left kidney using ultrasound-micro-bubble (SonoVue) mediated system, respectively. One week later, renal IRI model was made by clasping the left renal artery for 45 min. After 1, 2, 3, 4, 7 and 14 day of reperfusion, the kidney in each group was collected to detect the expression of cyclin D1, cyclin E, CDK4 and CDK2 by western blot analysis or enzyme-linked immunosorbent assay (ELISA). Results Compared with S group, the expression of cyclin D1, cyclin E, CDK4 and CDK2 was significantly up-regulated in day 1, 2,3, 4, 7 and 14 in IR group. And the above index increased gradually after reperfusion, and reached the peak at day 7 (F=54.92, 69.60, 61.28, 77.38, P<0.05). While in IMD group, these indexes reached the peak at day 1, then progressively declined, and could not be detected at day 14 (compared with the IR group, F=54.92, 69.60, 61.28, 77.38, P<0.05). Conclusion IMD preconditioning can up-regulate the expression of cyclin D1, cyclin E, CDK2 and CDK4 in the early phase of renal ischemia-reperfusion injury that may accelerate repair of renal tissue, at least by part, by enhancinge cell proliferation.
9.Clinical application of radial endoscopic ultrasonography on preoperative T and N staging for advanced gastric cancer

Objective To assess the value of endoscopic ultrasonography (EUS) and abdominal CT scan on preoperative T and N staging of advanced gastric cancer.Methods A total of 188 patients with advanced gastric cancer received EUS and CT scan to evaluate the T and N staging and lymphatic metastasis before surgical operation.The postoperative pathologic results acted as gold standard for comparison of the two methods.The results consistent with pathologic results were considered as correct otherwise were incorrect.The accuracy of EUS and CT and the each consistency of two methods with pathology were analyzed.Results For T staging, the accuracy of EUS and CT was 87.2%(164/188), 76.6%(144/188), respectively, and Kappa value of EUS and CT was 0.726 and 0.509, respectively.There was a statistical difference between the two methods(χ2=7.181,P=0.007).For lymph node metastasis, the accuracy of EUS and CT was 72.9%(137/188) and 79.8%(150/188), respectively, and Kappa value of EUS and CT was 0.397 and 0.487, respectively.No statistical difference was found between them(χ2=0.963,P=0.326).The consistency test between the two methods for T and N staging and lymph node metastasis revealed that the Kappa value was 0.507 and 0.649, respectively.The accuracy of EUS on T staging was 93.0%, 93.3%, 96.8% and 91.5% in cardiac cancer, gastric fundus cancer, gastric corpus cancer, and gastric antrum cancer, respectively.Corresponding Kappa value in the 4 different positions were 0.843,0.881,0.940 and 0.710, respectively according to consistency tests.The accuracy of gastric antrum cancer with pylorus invasion was 63.2% and the consistency test Kappa value was 0.340.Conclusion EUS is an accurate method for T staging and lymphatic metastasis of advanced gastric cancer, but there is a lower accuracy for T staging of gastric antrum cancer with pylorus invasion.
