After an analysis of the urgent need of systematic medical information service in village and township hospitals and doctors and the advantageous conditions for medical academic libraries to provide information service for them, its imple-mentation strategies were proposed and how to promote the construction of libraries and the development of medical cause in villages and townships by providing information service in countryside were pointed out.

Setting Chinese as preparatory courses is an effective way to harmonize Chinese language teaching and medical teaching.By this way,some low-level students can be disqualified from further study,thus both the level of Chinese teaching and medical professional teaching can be improved.In the process of the eductional system reform from 5 years to 6 years,building Chinese training based preparatory course,will consummate the oversea medical undergrade education system.

Objective To observe the clinical effects of surgical treatment of retinal detachment(RD) caused by macular hole(MH) in high myopia. Methods The clinical materials of 149 eyes of 149 high myopia patients with RD caused by MH were reviewed. The cases were divided into complete posterior vitreous detachment (PVD) group and incomplete PVD group. The anatomic successful rate of operative treatment was evaluated according to the applications of vitrectomy surgery and non vitrectomy surgery respectively in each group. The visual acuity changes after the operations were also observed. Results The anatomic successful rates were as follow: 77.9% in total cases with vitrectomy surgery and 25.9% with non vitrectomy surgery ( P 0.05). Conclusions The scleral buckling combined with vitrectomy, gas intraocular tamponade and postoperative photocoagulation is an effective and optimal procedure for RD caused by MH in high myopia.

The incidence of gastric cancer is in the fourth place of all cancers in worldwide ,and the fatal-ity rate is in the second place .Standard chemotherapy regimens play an important role in prolonging gastric cancer survival rate.However,most of gastric cancers are in advanced stage when diagnose is finally made .The current medical practice can only meet the limited treatment options .In the last several decades ,study of the molecular mechanisms of gastric cancer gets constant new progress .By inhibiting the human epidermal growth factor receptor 2(Her-2)for advanced gastric cancer,it provides a new treatment for patients.Trastuzumab is a kind of Her -2 monoclonal antibody .It is the first kind of targeted therapy drug for the treatment of metastatic gastric cancer .A combination of trastuzumab with cisplatin and 5-fluorouracil based chemotherapy benefits gastric cancer overall survival.Overexpression of Her -2 provides evidence for the targeted therapy .So we reviewed Her -2 excessive expression of gastric carcinoma in recent years and represented by bead sheet resistance to targeted therapy .In this paper ,research of overpression of Her -2 and targeted threrapy in gastric cancer are summarized .

Objective To observe the clinical efficacy of enhanced external counterpulsation (ECP) in the treatment of non-proliferative diabetic retinopathy (NPDR).Methods It was a case-control study.83 patients (166 eyes) of NPDR were randomly divided into ECP treatment group (ECP group) and conventional therapy group (drug group) by date of visit odd and even number,each with 42 patients of 84 eyes and 41 patients of 82 eyes respectively.The blood glucose,glycated hemoglobin (HbAlc),low-density lipoprotein (LDL-C),corrected visual acuity,stages of diabetic retinopathy (DR) and hemodynamic index such as peak systolic velocity (PSV),diastolic velocity (EDV),resistance index (RI) of ophthalmic artery (OA) and central retinal artery (CRA) were not statistically different between these two groups (P＞ 0.05).All patients were regularly educated for lifestyle management and diabetes mellitus.The following parameters were controlled by drugs including blood glucose＜8.0 mmol/L,HbA1c＜7.5 ％,blood pressure ＜140/90 mmHg (1 mmHg=0.133 kPa) and LDL-C＜3.1 mmol/L.The ECP group received both ECP and medical treatments.The ECP parameters were gasbag pressure 0.35-0.45 kg/cm2,40 min/each time,1 time/d.35 days of ECP was one course,one course each year for 3 years.The treatment effectiveness was determined at the end of follow-up after 3 years of treatment.The blood glucose,HbA1c,blood pressure,LDL-C,visual acuity,fundus changes and hemodynamic index of OA and CRA before and after treatment were comparatively analyzed.Improved or stable visual acuity and fundus condition was judged as effective treatment.Results At latest follow up,the differences of blood glucose,HbA1c,blood pressure and LDL-C were not statistically significant (P＞0.05).Compared to drug group,the PSV and EDV of OA and CRA were increased,but RI of OA and CRA was decreased in ECP group (P＜0.05).The vision effective ratio was 91.67％ and 30.49％ in ECP group and drug group respectively with a significant different (x2 =65.56,P＜0.05).The fundus effective ratio was 92.86％ and 48.78％ in ECP group and drug group respectively with a significant different (x2 =43.38,P＜ 0.05).During follow-up,1 eye (1.19％) progressed to proliferative DR (PDR) in ECP group,while 6 eyes (7.32％) progressed to PDR in drug group.The difference of PDR incidence rate between two groups was significant (x2 =3.87,P＜ 0.05).During the ECP treatment,there were 2 patients with small blisters in the skin of lower limb and cured finally.There were no other treatment-related complications.Conclusion ECP in the treatment for NPDR can improve PSV and EDV,reduce RI of OA and CRA;improve vision and fundus state,reduce the incidence of PDR.

BACKGROUND: DNA enzyme targeting early growth response factor 1 (Egr-1) mRNA (ED5) can inhibit expression of downstream target genes by specificaly inhibiting expression of early growth response factor 1. OBJECTIVE:To investigate the effects of ED5 on the expression of plasma tissue factor after vascular baloon injury in rats and the mechanism of inhibiting neointimal hyperplasia. METHODS: Intimal injury models of the left carotid artery were made in rats. Then, ED5, MgCl2 and FuGene6 were injected into the injured vascular segment. RESULTS AND CONCLUSION:At days 3, 7, 14, 21, the expression levels of Egr-1 and tissue factor in plasma were significantly down-regulated in the ED5 transfection group compared with the MgCl2 and FuGene6 groups (P < 0.01); and neointimal hyperplasia was significantly inhibited by ED5 at days 7, 14 and 21 after modeling (P < 0.01). ED5 may inhibit neointimal hyperplasia folowing baloon injury of rat common carotid artery through down-regulation of tissue factors.

This study aimed to review the research progress on the contribution of microRNA (miRNA) to the chemotherapy re-sistance of breast cancer. With“miRNA,”“breast cancer,”and“chemotherapy resistance”as key words, the literature was searched in Pubmed and CNKI databases between 01-2000 and 10-10-2012. We described the characteristics and mechanisms of breast cancer che-motherapy resistance and focused on the contribution of miRNAs in the chemotherapy resistance of breast cancer. MiRNA participate in chemotherapy resistance of breast cancer through different ways. This article summarizes different miRNAs that participate in che-motherapy resistance and identifies their specific pathways in drug resistance. The potential clinical utilization of the serum miRNA as a tumor marker is also discussed. Further research on the effect and mechanism of serum microRNA in chemotherapy resistance will bring a new chapter in the targeted therapy of breast cancer.

The pathogenesis of diabetic retinopathy is complicated.The vast network of multiple factors including unifying mechanism,inflammatory reaction,neuron degeneration and metabolic memory of glucose,and the four established pathogenic molecular pathways are hotspots of mechanism research for diabetic retinopathy.Nevertheless,these researches may be only one corner of the iceberg of DR mechanism,and we still face enormous challenges in DR mechanism research.Collaboration with multiple disciplines to study the relationship between DR and diabetes and other systemic diseases,search novel therapy targets may increase the result in an unexpected windfall for DR basic research.

Objective To analyze the clinical features and results of array-comparative genomic hybridization (array CGH, aCGH) in a Chinese girl with Phelan-McDermid syndrome. Methods The clinical symptoms of a child with Phelan-Mc-Dermid syndrome were retrospectively analyzed. Routine G-banding was performed to analyze the karyotype, and the aCGH was used to analyze subchromosomal abnormalities. Results The routine karyotype analysis showed a normal female karyotype without abnomalities in chromosome number and structure. aCGH analysis finely mapped the deletion of Chr22q13.2-qter. Phel-an-McDermid syndrome was diagnosed for this case. Conclusions Phelan-McDermid syndrome can be diagnosed by the typi-cal and detailed clinical features in combination with the laboratory examinations of subchromosomal abnormalities. aCGH is one of the most valuable methods to analyze subchromosomal abnormalities and to diagnose Phelan-McDermid syndrome.
