Objective To investigate the neurotoxicity of intrathecal (IT) doxepin in rats.Methods Twenty-four adult male Wistar rats weighing 250-300 g in which intrathecal micro-catheter was successfully implanted without complications were randomly divided into 4 groups ( n =6 each):control group ( group N ) and 3 doxepin groups receiving IT doxepin 40,20 and 10 mmol/L 0.2 μl/g respectively (groups D40,D20,D10 ).In N group normal saline was injected IT instead of doxepin.The animals were killed at 6 h after IT administration.The lumbar segment of the spinal cord was removed for microscopic examination and determination of myelin basic protein (MBP) content in the spinal cord using a double-antibody sandwich (ELISA).Results The severity of neuronal damage and decrease in MBP content in the spinal cord induced by IT doxepin were dose-dependent.Conclusion The neurotoxicity induced by IT doxepin is dose-dependent.

Traditional operation for esophageal cancer includes esophagectomy and digestive tract reconstruction,which is associated with more postoperative complications and high mortalities due to severe trauma.Endoscopic surgery(including thoracoscope,mediastinoscope,and laparoscope) for radical resection of esophageal cancer has mild operative trauma,less bleeding and mild postoperative pain,resulting in less postoperative complications and low mortalities.This article analyzes the current status of endoscopic surgery in radical operation of esophageal cancer and the existing problems,providing reference for the development of minimally invasive surgery of esophageal cancer.

Objective: To determine the expression of Fas ligand (FasL) in the patients with esophageal carcinoma and its clinic significance. Methods: The expression of FasL was measured by immunohistochemical technique in 30 patients with esophageal cancer. The numbers of tumor infiltrationg (TIL) were accounted by immunohistochemically stained CD45. Results: 76.6% of specimen expressed FasL. The difference of FasL expression was significant between each TNM stage. There was a negative correlation between the number of TIL and the expression of FasL. Conclusion: The expression of FasL in esophageal carcinoma may correlate with its invasion and metastasis.

The non-invasive positive-pressure ventilation(NPPV) play an important role in treating patient with upper airway obstructive diseases.The use of NPPV in children has significantly increased in recent years internationally.Nevertheless,the paediatric NPPV support in China was less reported.This review aimed to clarify the indications,efficacy and technical methods in NPPV,and therefore searched papers including clinical studies,reviews and guidelines focusing on children with upper airway obstructive diseases who were treated on NPPV.The review concluded that NPPV is a treatment choice for patient who has upper airway obstruction and can not be solved by surgery.The NPPV avoids the impairment to the body caused by recurrent hypoxemia and arousals,decreases the rate of intubation and improves the quality of life.NPPV need to be preformed in a professional children's medical center.Education and training for the parent is the key point of improving treatment compliance.

Chest wall resection and reconstruction remains a severe challenge for reconstructive surgeons,which often leads to conservative treatment regimens in clinical practice,consequently resulting in poor outcomes(high morbidity and mortality).In recent 20 years,advances in muscle flap surgery and availability of chest reconstructive prosthesis have encouraged the surgeons to take an active attitude toward chest wall resection;many "unresectable" lesions now have a chance to be resected and cured.This article reviews the problems concerning the principles for chest wall resection,reconstruction,prosthesis selection,etc.in chest wall reconstruction.

FOXP3,a member of the forkhead/winged-helix family,exerts its immunosupression effect by regulating the development,differentiation and maturation of CD4+ CD25+ regulatory T cells(Treg).Treg plays an important role in immune escape mechanism in oncology research.This article reviews the expression of FOXP3 in various types of tumors and its role in tumor immunology and immunotherapy.

Respiratory support is the treatment of respiratory diseases.Currently, the commonly used methods of respiratory support include nasal catheter oxygen or mask oxygen, noninvasive and invasive ventilation, etc.High flow nasal cannula oxygen (HFNC) is a new kind of therapy, which was commonly used in preterm infants as the instead of continuous positive airway pressure (CPAP). However, in recent years, HFNC is more commonly used in children′s intensive care unit and general wards.In this paper, the efficacy and safety of HFNC in pediatric are reviewed.

BACKGROUND: Preparing the tracheal prosthesis by biomaterials is crucial for studying the implant of long tracheal defect.The biocompatibility and biodegradability of collagen/hydroxyapatite (Col/HAp) and poly(lactic-co-glycolic acid) (PLGA) in vivo, as a artificial material for tracheal prosthesis, need to be observed.OBJECTIVE: To explore in vivo biodegradability of Col/HAp and PLGA of tracheal prosthesis in rats.DESIGN: Randomized controlling observation.SETTING: Department of Thoracic and Cardiovascular Surgery, Clinical College of Yangzhou University; Department of Thoracic and Cardiovascular Surgery, Changzheng Hospital of the Second Military Medical University of Chinese PLA.MATERIALS: The experiment was carried out in the National Experimental Animal Center of the Second Military Medical University of Chinese PLA from March 2003 to December 2004. Sixteen SD rats of either gender were offered by this center, aged three months and weighed 150-170 g. Col/HAp and PLGA (the copolymer of polylactic acid and polyglycolic acid according to the percent of 90:10) were self-made.METHODS: ①Empirical process: Rats were fed adaptively for 1 week and anesthetized. Then undermining dissection was performed along with musculi dorsal surface toward spine bilaterally, so as to form two capsular gaps, which were implanted with Col/HAp sponge and PLGA fiber mesh cloth respectively, in a size of 10 mm ×10 mm. ②Empirical evaluation: The postoperative activity, incision healing and rejection of rats were observed; 2, 4, 6, 8 weeks postoperatively, the Col/HAp sponge and PLGA mat that were embedded subcutaneously as well as surrounding tissues were determined using scanning electron microscope or transmission electron microscope.MAIN OUTCOME MEASURES: In vivo biodegradability of two materials implanted in rats at different time points.RESULTS: ①All the animals could carry out normal activity, respiration and diet; no incision infection, fluidity, necrosis or sinus tract appeared; there was no edema in the skins of implanted area, neither hypersensitiveness nor toxicity was found; the implant materials had no rejection. ②The features of the implanted materials varied throughout the implantation. During the follow-up, the infiltration of inflammatory cells was found in the interface between the implants and neighboring host tissues in the early stage, phagocytes and fibroblastic cells were also observed later. As the process went on, the materials were biodegraded gradually, encapsulated by phagocytes, and replaced by newly generated fibrous tissues. No remarkable harmful influences of the composite materials on the neighboring host tissues such as apomorphosis, necrosis, hyperplasia and foreign body reaction were observed grossly and microscopically.CONCLUSION: All the implants show a good biocompatibility and biodegradability. Both Col/HAp and PLGA go through a gradual process of biodegradability, biological absorption and replacement by host tissues ultimately in vivo, which suggest that these two kinds of composite biomaterials will be used safely in developing tracheal prosthesis.

Objective To prepare the composite of Polycaprolactone(PCL) and Bone powder(BP) for bone repairing,and evaluate the biocompatibility of the material.Methods The materials were prepared in college of chemical engineering and materials science,Shanghai Jiaotong University,and the animal tests were fulfilled in Animal Experimental Center of the Second Military Medical University of PLA from June 2010 to March 2011.The PCL and BP were blended together to get the copolymer by haake ( Rheocord 900 ) torque rheometer.Then the extracts of the material in DMEM for 24 hours were obtained.Firstly biological safety detection of the materials was performed:①cytotoxicity test:the extracts were mixed with DMEM to seed rabbit osteoblasts for 24 hours.The cell proliferation was detected by MTT method.The cell proliferation rate and toxicity grate were also calculated ( cytotoxicity of grade 0 or 1 was qualified).②hemolysis test:the extracts were mixed with fresh rabbit blood,hemolysis rate were calculated ( hemolysis mte less than 5％ was qualified).③pyrogenic test:the extracts were injected into the rabbit ear vein with the dose of 10 ml/kg.The anus temperature was measured per hour for three times.The elevator degree was calculated by subtracting the natural temperature from the highest one.Secondly,cellular adhesion test of the material composite was performed.The composite was casted on the cover glasses.The samples were put in 6-well tissue culture plates,totally 6 × 104 osteoblasts were seeded each well and cultured for 3 days.The cells were observed at 24 and 72 hours of culture,and photography was performed using inverted phase contrast microscope.④general toxioity test:the extracts were injected into mouse vein with the dose of 50 ml/kg.The toxic symptoms were observed within 72 hours.Results The novel copolymer of PCL/BP was prepared.①Biological safety:the cell viability of the composite was 87.8％ and cytotoxicity was grade 1.The hemolysis rate was 2.4％,less than 5％.The maximal variety of body temperature in rabbits were 0.4℃,which accorded with the national standard ( ＜0.6℃ ).The general toxicity test showed that there were no adverse effects in the animals,such as death,convulsion,paralysis,respiratory depression,diarrhea and decreasing weight.②Cell adhesion:adhesive cells manifold obviously and assembled 24 hours later,inosculated gradually 3 days later,and the cells were in good fettle.Conclusion The composite of PCL/BP accords with cytotoxicity demand of the biological materials,presenting no toxicity,pyrogenicity or hemolysis.It possesses good biocompatibility and cellular adhesion.
