1.Risk Factors for Relapse in Patients with Inflammatory Bowel Disease:Analysis of 479 Clinical Cases

Background:Inflammatory bowel disease(IBD)is a chronic,non-specific,relapsing inflammatory bowel disorder. Prediction of prognosis and relapse is of great importance for disease control. Aims:To analyze the characteristics and related factors for relapse in IBD patients. Methods:Four hundred and seventy-nine hospitalized patients diagnosed as ulcerative colitis(UC,n = 301)or Crohn’s disease( CD,n = 178)from Jan. 2004 to Jun. 2014 were enrolled and divided into relapse group and non-relapse group. The clinical characteristics were collected and univariate and multivariate analysis were performed to identify the influencing factors for relapse. Results:The relapse rates for UC and CD were 52. 49% and 77. 53% ,respectively. In stepwise Logistic regression analysis,younger onset age( < 40 years old,OR =0. 825,95% CI:0. 777-0. 876,P = 0. 000),longer disease duration(OR = 1. 186,95% CI:1. 089-1. 291,P = 0. 000) and extensive colitis(OR = 16. 993,95% CI:2. 670-108. 170,P = 0. 003)were independent risk factors for relapse of UC,while younger onset age( < 40 years old,OR = 0. 944,95% CI:0. 915-0. 974,P = 0. 000)and poor medication adherence(OR = 31. 074,95% CI:10. 456-92. 343,P = 0. 000)were independent risk factors for relapse of CD. Conclusions:Patients at high risk for relapse of IBD can be identified by referring to the related risk factors. Early identification and intervention,persistence for maintenance therapy and improving medication adherence by health education may reduce the clinical relapse of IBD.

4. Effect of glaze technology on adhesive and mechanical properties of Yttria-stabilized tetragonal zirconia polycrystal ceramic

BACKGROUND: Any surface treatments should Improve the bond strength of yttria-stabilized tetragonal zlrconia polycrystal ceramics without damaging the original strength. Currently, there is no Information about the influence of glaze technology on the bond strength of yttria-stabilized tetragonal zlrconia polycrystal ceramics, and its Influence on the material strength Is unclear. OBJECTIVE: To evaluate the effect of glaze technology on the mechanical behavior of yttria-stabilized tetragonal zlrconia polycrystal ceramics and on the bond strength with resin cements. METHODS: Yttria-stabilized tetragonal zlrconia polycrystal ceramics specimens were prepared and randomly divided into four groups: group A: the surface was not treated; group B: sandblasting with 110 urn Al203 particles; group C: glaze + hydrofluoric acid etching; group D: glaze + hydrofluoric acid etching + sllanlzation. The surface morphology, surface roughness, phase transformation, elemental composition, shear bond strength and flexural strength of zlrconia specimens after surface treatments were tested. RESULTS AND CONCLUSION: (1) The surface roughness of treatment groups increased obviously, whose descending order was group C (0.62±0.01) um > group D (0.55±0.02) um > group B (0.11±0.02) um > group A (0.05±0.01) urn (p < 0.05). (2) The surface of specimens in the group B contained 2.2% monoclinic zirconia, while the content of other groups was zero. (3) In addition to elements of Zr and O, there was 6.49% Al element in the group B specimens, and 18.67% and 25.78% Si element In the groups C and D specimens, respectively. (4) The shear bond strength in the groups A, B, C and D was (3.11±0.40), (4.23±0.45), (6.62±0.60) and (10.46±0.83) MPa, respectively (P < 0.05). (5) The three-point flexural strength in the groups A, B, C and D was (961.07±75.53), (1 234.73±114.09), (1 024.28±120.51), and (1 036.09±80.10) MPa, respectively, among which the multiple comparison of groups A, C and D showed no significant difference (P > 0.05). (6) The effect of glaze technology on improving the flexural strength of yttria-stabilized tetragonal zirconia polycrystal ceramics is not as obvious as that of sandblasting, but the bond strength between tetragonal zirconia polycrystal ceramics and resin cements can be significantly Improved with hydrofluoric acid etching and silanization after glaze.
5.Determination of tetrodotoxin in salted pufferfish by liquid chromatography-tandem mass spectrometry

Objective:
To develop a liquid chromatography-tandem mass spectrometry (LC-MS/MS) method with cation exchange-based solid phase extraction (SPE) for determination of tetrodotoxin (TTX) in salted pufferfish.
Methods:
Evenly crushed salted pufferfish samples were subjected to ultrasound-assisted extraction with 0.5% acetic acid/50% methanol/water. The extract was cleaned with cation exchange-based SPE cartridge and eluted with 0.3% hydrochloric acid and 50% acetonitrile/water. The eluent was neutralized with ammonia and separated with a Waters XBridgeTM BEH Amide column (150 mm×3.0 mm, 1.7 μm), and determined using LC-MS/MS in a multiple reaction monitoring mode.
Results:
The matrix effects of TTX were 85.7%-92.4%, and the matrix suppression effect was under effective control following clean-up procedures using the optimized SPE method. The TTX showed a good linear relationship at the range of 2.0 to 4 000 μg/kg, with a correlation coefficient (r2) of 0.999 2. The limits of detection and quantitation for TTX in sample matrix were 1.0 μg/kg and 2.0 μg/kg, respectively. The mean spiked-recovery rates were 81.2% to 96.5% at spiked amounts of 2.0, 200 μg/kg and 2 200 μg/kg, with relative standard deviations (RSDs) of 4.3% to 7.5%. The intraday accuracy and precision of TTX were 84.4% to 95.6% and 4.9% to 5.8% in quality control samples, and the interday accuracy and precision of TTX were 86.1% to 94.9% and 5.5% to 8.5% in quality control samples. The detection of TTX was 60.5% in 38 market-sold salted pufferfish products using the established LC-MS/MS method.
Conclusion
The established LC-MS/MS method is effective for accurate quantitative determination of TTX in salted pufferfish.
6.Investigation and Analysis of the Storage Temperatures of Drugs in Our Hospital

OBJECTIVE: To investigate and analyze the storage temperatures of drugs for references of drug storage. METHODS: A classified statistical analysis was conducted on those drugs with temperature requirements in storage as instructed in drug package inserts. RESULTS: The stock drugs totaled 1 254 kinds,of which,483 (38.52%) had the item of temperature requirements in storage,158 with room temperature storage and 61 with cold storage had the item of specific temperature requirement. CONCLUSION: The storage temperature of drugs should be established based on the quality of different drugs so as to ensure the stability of drug quality. It is advisable to clarify the storage temperature in all the drug package inserts.
7.Application of self- management programme in home care of oncohematologic patients with peripherally inserted central catheters

Objective To investigate the effect of self- management programme in home care of oncohematologic patients with peripherally inserted central catheters(PICC). Methods Seventy oncohematologic patients with PICC were divided into the control group and the intervention group with 35 patients in each group by random digital table method. The control group received the routine care. The intervention group received the self- management programme. Self- care ability, anxiety, catheter- related complications and maintenance cost were observed before and on the 10th day and 30th day after intervention. Results On the 10th day and 30th day after intervention, the total score and scores of each dimension of self- care ability in the intervention group were significantly higher than those in the control group, P<0.05 or 0.01, the differences were significant. The score of anxiety in the intervention group was significantly lower than that in the control group [for the patients on the 10th day, 47.64±4.40 vs. 51.14±6.28,on the 30th day,44.36±3.71 vs. 48.64± 5.54, t=3.40, 5.18, P<0.01; for the family members of patients on the 10th day, 47.36±2.12 vs. 50.57±3.99, on the 30th day,43.57±2.04 vs. 47.57±2.79, t=4.35, 7.42, P<0.01. The incidences of catheter- related complications were not significantly different between two groups, P>0.05. Conclusions Self- management programme can effectively improve the self-care ability, relieve the anxiety and prevent the catheter- related complications in home care of oncohematologic patients with PICC.
8.Prognostic value of N-terminal B-type natriuretic peptide in patients with chronic heart failure

74 years were associated with increased cardiac events too(HR = 5.40,95%CI:2.16~11.52,P=0.0002).NYHA classification, LVEF, cTnT?cTnI and CK-MB were not independently predictive of cardiac events.Conclusions Measurement of NT-proBNP in patients with CHF can help to identify patients at higher risk for cardiac events .
9.Aminoglycosides Modifying Enzyme Genes of Acinetobacter baumannii in Burn Unit

OBJECTIVE To study the aminoglycosides modifying enzyme genes and detect resistance of the Acinetobacter baumannii in burn unit.METHODS The susceptibility of the A.baumannii strains to 11 antibiotics were tested by K-B method.The aminoglycosides modifying enzyme genes were analyzed using polymerase chain reaction(PCR).RESULTS The resistance rate to antibacterials was as follows:to TZP 61.85%,SCF 23.69%,CAZ 64.48%,FEP 63.17%,IPM 63.17%,AK 56.58%,and to CIP 73.69%.In 76 strains of A.baumannii,48 strains(63.16%) were with aac(3)-Ⅰ,39(51.32%) with aac(6′)-Ⅰ,and 46(60.53%) with ant(3″)-Ⅰ,others were negative.The total test rate of aminoglycosides modifying enzyme gene was 63.16%.CONCLUSIONS There are not only multi-drug-resistance but also 63.16% aminoglycosides modifying enzyme genes of the A.baumannii isolated from burn unit.
