1.Analyze the Eighth Item of Pulse Syndrome Complex and Treatment of Miscellaneous Gynecological Diseases from Jinkui Yaolve

Objective]To explore and analyze its original implication of the eighth article in Chapter 22 from Jinkui Yaolve. [Methods]Through learning the Jinkui Yaolve closely and referring to the explanations of masters of generations,base on the original text to analyze and elaborate the etiology, pathogenesis, syndromes and therapeutic principles with regard to miscellaneous gynecological diseases.[Results] The eighth item is the general program of miscellaneous gynecological diseases,its principal pathogenic factors include deficiency, stale chills and stagnation of Qi.Moreover,the deficiency plays a leading role in the pathogenesis.Through exploring deeply,I hold that the pulmonary abscess is actually the pulmonary asthenia in the second sentence.When the pathogenic factors attack the body,the triple energizers manifest different symptoms and signs.There is no difference between male and female on the conditions which is confined to the upper warmer and the middle warmer.Such symptoms as pulmonary asthenia,chill colic,hypochondriac pain and abdominal pain,aching at the Guanyuan aera below the navel, asthenic fever,scaly dry skin etc.Their special dissertation distributes in other chapters of Jinkui Yaolve.Yet,the symptoms in the lower warmer which are only seen in the female,owing to deficiency-cold,blood-stasis and stagnation.[Conclusion] In history, Jinkui Yaolve firstly puts forward pulse syndrome complex and treatment of miscellaneous gynecological diseases,the core ideas on miscellaneous gynecological diseases of Zhang Zhongjing also have reference significance for the present.

This article shows a new design of telerehabilitation system for balance function assessment and training in our laboratory. The system is based on C/S network architecture, and realizes the telecommunication through socket network communication technology. It implements the teletransmission of training data and assessment report of sit-down and stand-up, online communication between doctors and patients, and doctors'management of patient information. This system realizes remote evaluation and telerehabilitation of patients, and brings great convenience for the patients.

4.Development of an approach for simultaneous determination ofmulti-mycotoxins in fresh fruits

Objective:
To develop an approach for simultaneous detection of multi-mycotoxins in fresh fruits, so as to provide technical supports for mycotoxins surveillance in fresh fruits.
Methods:
Fresh fruits were collected from markets and homogenized. Then, 2 g of fresh fruits were added with 10 mL of 0.1% formic acid ( 99∶1, v/v ) in acetonitrile and wortexed for 10 min. Following extraction with 1 g of sodium chloride and 4 g of anhydrous sodium sulfate, samples were centrifuged and 5 mL of the supernatant was cleaned up with 25 mg C18. Following centrifugation, the supernatant was dried under nitrogen. The residue was dissolved in 300 μL of methanol-acetonitrile mixture solution ( 1∶1, v/v ), and mixed evenly in 700 μL of the distilled water. Samples were then eluted in gradient series of 0.1% formic acid and 5 mmol ammonium formate and methanol-acetonitrile mixture solution ( 1∶1, v/v ). The 15 mycotoxins were determined using liquid chromatography-tandem mass spectrometry ( LC-MS/MS ) with electrospray ion source (ESI+/ESI-) under multiple reaction monitoring. In addition, a matrix-matched standard curve was employed for quantitative analysis.
Results:
There was a good linear relationship for 15 mycotoxins at concentrations of 0.25 to 10 ng/mL ( R2>0.992 ), the LC-MS/MS method showed the detection limits of 0.1-1.0 μg/kg, the spiked recovery rates of 71.68%-117.50%, and the relative standard deviations ( RSDs ) of 0.01%-13.60%. The detection rate of mycotoxins was 27.09% in 203 fresh fruits sold in markets.
Conclusions
The optimized LC-MS/MS method can be used for simultaneous determination of multi-mycotoxins in fresh fruits.
5.Study on the Percutaneous Properties of Man-PEI25k Nanocomplex under the Treatment of Microneedles

OBJECTIVE:To study the percutaneous properties of Man-PEI25k nanocomplex under the treatment of microneedles. METHODS:Using fluorescent dye water-soluble carboxyl CdSe/ZnS quantum dot (QD) as model drug, Man-PEI25k/QD and Man-PEI25k/QD nanocomplex with different grafting rates(1∶3,1∶6)were formed through electrostatic adherence with PEI25k and Man-PEI25k. The distribution of QD in the active epidermal layer and dermis of skin were observed by confocal microscopy after the treatment of microneedles,using free QD as control. The accumulative retention amounts of QD in the active epidermal layer and dermis of skin were determined by fluorescence spectrophotometer after PEI25k/QD and Man-PEI25k/QD nanocomplex treated with mi-croneedles. RESULTS:The amounts of Man-PEI25k/QD nanocomplex in active epidermal layer and dermis were significantly higher than that of PEI25k/QD nanocomplex under the treatment of microneedles in vivo;the amounts of Man-PEI25k/QD (1∶6) nanocom-plex in active epidermal layer and dermis of skin were significantly higher than that of Man-PEI25k/QD(1∶3)nanocomplex. In in vi-tro transdermal diffusion experiments,microneedles could increase the retention amounts of nanocomplex in active epidermal layer and dermis of skin significantly. The retention amounts of Man-PEI25k/QD nanocomplex in active epidermal layer were increased by 2 times of that of PEI25k/QD under the treatment of microneedles after 48 h;at the same time,in the dermis that was increased by 1.5 times,with statistical significance (P<0.01). CONCLUSIONS:Microneedles can improve the percutaneous properties of Man-PEI25k nanocomplex in active epidermal layer.
6.Determination of ibotenic acid and muscimol in wild mushroom by dansyl chloride derivatization-liquid chromatography-tandem mass spectrometry

Objective :
To develop an analytical method of ibotenic acid (IBA) and muscimol (MUS) in wild mushroom by dansyl chloride (DNSCl) derivatization-liquid chromatography-tandem mass spectrometry (LC-MS/MS), and to provide technical support for etiological identification of mushroom poisoning events.
Methods :
The sample was extracted with hydrochloric acid solution, derived by bimolecular DNSCl, diluted and inorganic salts precipitated with acetonitrile. The extract was separated by a waters XBridgeTM BEH C18 column and measured by LC-MS/MS.
Results :
The limits of detection for IBA and MUS in wild mushroom were 0.15 mg/kg and 0.1 mg/kg, respectively. Good linear relationship was obtained for IBA and MUS at the range of 0.5-250 mg/kg with the correlation coefficient of 0.997 and 0.999, respectively. The average recoveries at three spiking levels were 84.5%-102.0% with relative standard deviations (RSDs, n=6) of 4.7%-8.6% for IBA. The average recoveries were 88.6%-95.4% with RSDs (n=6) of 4.9%-7.5% for MUS.
Conclusion
The optimized sample extraction and bimolecular DNSCl derivatization conditions can achieve rapid and accurate analysis of IBA and MUS in wild mushroom poisoning sample.
7.Modeling gray-scale and contrast-enhanced ultrasound in the diagnosis of thyroid lesions

Objective To investigate the discriminant analysis model of gray-scale ultrasound (GSUS),contrast-enhanced ultrasound (CEUS) and the combination of them in the differential diagnosis of benign and malignant thyroid lesions and the diagnostic values.Methods Ultrasound images of 211 thyroid lesions confirmed by pathology were synthetically reviewed by scoring 5 GSUS indicators including shape (X1),orientation (X2),interior echogenicity (X3),halo sign (X4),and microcalcification (X5),as well as 6 CEUS indicators including relative arrival time of microbubhles in the periphery (X6) and interior (X7),peak periphery (X8) and interior (X9)echogenicity,peripheral ring-enhancement (X10),homogeneity of enhancement (X11).The diagnostic models with their values of GSUS,CEUS and the combination of them were explored by discriminant analysis.Results The discriminant analysis function of GSUS in the diagnosis of thyroid benign and malignant lesions was g1 (X) =0.715 X1+0.276X2 + 1.028X3 +1.197X4 +0.923X5-2.202 with the diagnostic value 86.3%,the discriminant analysis function of CEUS was g2(X) =-0.392X6 +0.541X7-0.117X8 +0.562X9 + 1.173X10 +2.200X11-1.956 with the diagnostic value 89.1%,and the discriminant analysis function of the combination of GSUS and CEUS was g3 (X) =0.418X1 + 0.173X2 + 0.626X3 + 0.558X4 + 0.183X5-0.476X6 + 0.474X7-0.071X8 + 0.399X9 + 0.985X10 +1.639X11-2.530 with the diagnostic value 91.0%.Conclusions GSUS and CEUS were valuable in the differential diagnosis of benign and malignant thyroid lesions,and the combination of GSUS and CEUS was most valuable.
8.Effects of age factors on pharmacokinetics of ropivacaine during combined lumbar plexus-sciatic nerve block

Objective To evaluate the effects of age factors on the pharmacokinetics of ropivacaine during combined lumbar plexus-sciatic nerve block in patients.Methods Twenty patients of both sexes,aged ≥ 18 yr,weighing 50-75 kg,of American Society of Anesthesiology physical status Ⅰ or Ⅱ,scheduled for elective lower extremity surgery,were divided into 2 groups (n=10 each) according to age:young and middle-aged group (18-59 yr) and elderly group (≥ 60 yr).Combined lumbar plexus-sciatic never block was performed using an ultrasonic instrument and a nerve stimulator.Lumbar plexus block was performed with 0.5% ropivacaine 30 ml.Sciatic never block was performed with 2% lidocaine 10 ml plus 0.75% ropivacaine 10 ml.Before administration and at 5,10,15,20,30,45,60,120,180 and 360 min after administration,blood samples were collected from the radial artery for determination of the blood concentration of ropivacaine by high-performance liquid chromatography.The area under the concentrationtime curve,maximum concentration (Cmax),time to C terminal elimination half-life and clearance were calculated.Results Compared with young and middle-aged group,the blood concentration of ropivacaine was significantly decreased at 5-45 min after administration,the Cmax was significantly decreased,terminal elimination half-life was significantly prolonged (P<0.05),and no significant change was found in area under the concentration-time curve,the time to C and clearance in elderly group (P> 0.05).Conclusion Age factors can affect the pharmacokinetics of ropivacaine during combined lumbar plexus-sciatic nerve block,and both absorption and metabolism of ropivacaine are slower in elderly patients than in young and middle-aged patients.
9.Correlation between cervical lesion development and histone acetylation modification that regulates RAR-β2 expression

Objective:To investigate the relationship between cervical lesion development and histone acetylation that regulates RAR-β2 expression. Methods:Immunohistochemistry and Western blot analysis were performed to detect AcH3, RAR-β2, and involu-crin expression in normal cervical tissues as well as in tissues with cervical intraepithelial neoplasia (CIN)Ⅱ-Ⅲand squamous cell cer-vical carcinoma. The relationship among histone acetylation level, RAR-β2 expression, and cervical lesion severity were analyzed. Re-sults:AcH3, RAR-β2, and involucrin expression were reduced or absent with the progression of cervical lesions;significant differences were noted between the groups (P<0.05). Histone acetylation level and RAR-β2 expression were positively correlated (r=0.797, P<0.05). AcH3 and RAR-β2 expression, which were both associated with the cervical lesions, were negatively correlated [r=-0.547(AcH3), r=-0.585(RAR-β2), P<0.05]. Conclusion:Histone acetylation modification is associated with the regulation of RAR-β2 expression. This pro-cess is also likely to participate in the carcinogenesis of cervical carcinoma.
10.A preliminary clinical study on the assessment of liver fibrosis by elastography point quantification technique with multivariate regression analysis

Objective To preliminarily evaluate the feasibility of elastography point quantification (ElastPQ) technology in the determination of liver stiffness as well as its impact factors.Methods Amount to 235 patients with liver neoplasms underwent liver stiffness measurement in the right lobe of liver using an ultrasound scanner (iU Elite,Philips).ElastPQ values were obtained and compared with the liver fibrosis stage (S),the grade of necroinflammatory activity (G) and steatosis assessed histologically as well as gender and age.The factors related to ElastPQ values were explored by stepwise regression in multiple linear regression analysis and the regression equation was established.Results In the multiple linear regression model of ElastPQ values,liver fibrosis and necroinflammatory activity were associated with ElastPQ (P < 0.05) while other factors including age,gender and hepatic steatosis had no effect on ElastPQ statistically (P >0.05).The equation of linear regression was ElastPQ =1.205S + 1.075G + 4.537.Conclusions ElastPQ technique is a reliably noninvasive tool in the liver stiffness measurement.Liver fibrosis and necroinflammatory activity are the main factors affecting liver stiffness measured by ElastPQ.
