Chinese Journal of Emergency Medicine 2010;19(8):811-816
Objective A meta-analysis of clinical trials of prehospital thrombolysis versus in-hospital thrombolysis or PCI for acute myocardia infarction (AMI). Method We collected the literature on prehospital thrombolysis for AMI published at home and abroad from January 1989 to April 2009. Recanalization rate of infarct-related arter, complication incidence and mortality ( < 30 d) were regarded as result indicators. Software RevMan 4.2.8 was used to process meta-analysis. Results There were only 27 literatures selected. Prehospital thrombolysis was associated with higher recanalization rate of infarct-related arter[OR 2.67,95%CI(1.86 ~ 3.85);P <0.01] and lower mortality ( < 30 d ) [OR 0.71,95%CI(0.64 ~ 0.78); P < 0.01] than in-hospital thrombolysis. There was no significant difference in complications between them. Compared with PCI, prehospital thrombolysis was associated with higher complication incidence [OR 2.91,95% CI ( 1.26 ~ 3.80); P = O.005]. There was no significant statistical difference of mortality ( <30 d ),but the sensitivity analysis was not good. Conchusions The effect and prognosis of prehospital thrombolytic are superior to that of in-hospital thrombolysis, and the same with PCI,but prehospital thrombolytic has higher incidences of complications, and the sensitivity is not good.

China Pharmacy 2015;(23):3270-3272
OBJECTIVE:To investigate the clinical efficacy of integrated treatment of traditional Chinese medicine and west-ern medicine for subacute thyroiditis. METHODS:98 patients with subacute thyroiditis were randomly divided into control group and observation group by random number table with 49 patients in each group. Control group was given conventional oral treat-ment of prednisone tablets, and observation group was additionally treated with Chinese patent medicine(Shuanghuanglian oral so-lution+Antiviral oral solution+Tripterygium glycosides tablet)on the basis of control group. A treatment course lasted for 4 weeks, and both groups received 2 courses of treatment. Therapeutic efficacy, symptom and sign improvement time, total treatment course, recurrence rate and ADR were observed and compared between 2 groups. RESULTS:the total effective rate of observation group was 100%,significantly better than the control group 79.59%,and there were significant differences(P<0.01). The defer-vescence time, thyroid pain recovery time, thyroid swelling fadeaway and total treatment course of observation group was shorter than those of control group, with statistical significance(P<0.01). Following up 2 months after drug withdrawal, the recurrence rate of observation group was 6.12%,which was significantly lower than that of control group(22.45%),with statistical signifi-cance (P<0.05). Small number of ADR occurred in 2 groups. CONCLUSIONS:The integrated treatment of traditional Chinese medicine and western medicine for subacute thyroiditis can control the symptom rapidly, reduce the possibility of palindromia and reduce ADR.

China Pharmacy 2015;(29):4107-4109
OBJECTIVE:To evaluate the clinical efficacy and safety of triple therapy in the treatment of type 2 diabetes melli-tus(T2DM),and to open up more effective way to treat T2DM. METHODS:76 patients with T2DM were randomly divided into control group and observation group with 38 cases in each group. The control group received conventional therapy,i.e. oral hypo-glycemic agents Glipizide controlled-release tablet 5 mg,qd+Metformin tablets 5 mg,tid. The observation group was treated with triple therapy,i.e. berberine hydrochloride 0.5 g,tid, for 3 months,on the basis of control group. The biochemical index were compared between 2 groups Before and after treatment, including fasting blood glucose (FBG),postprandial 2 h blood glucose (2 h FPG),glycosylated hemoglobin(HbA1c),insulin(FINS),total cholesterol(TC)and triglyceride(TG),high/low density li-poprotein cholesterol (HDL-C/LDL-C),etc.,to evaluate its safety. RESULTS:After treatment,FBG,2 h FPG,HbA1c,FINS, TC and LDL-C of 2 groups and TG of observation group were all decreased significantly, while the level of HDL-C was increased significantly, with statistical significance(P<0.05);FBG,2h FPG,HbA1c,FINS,TC and LDL-C of observation group were im-proved more significantly than control group, the difference was statistically significant(P<0.05). ADR was found in 4 patients of observation group, and the symptoms were relieved after symptomatic treatment. CONCLUSIONS:In the treatment of T2DM,tri-ple therapy can down-regulate blood lipid and blood sugar stably, have definite therapeutic efficacy and little side effect.

International Journal of Pediatrics 2010;37(1):29-32
After primary EB virus(EBV) infection there are expansions of EBV-specific CD8~+/CD4~+ CTLs responses against lytic peptides and latent peptides in order to control EBV infection. Clinical manifestations of EBV infection are diverse. Its pathogenesis is not clear. The surveillance of EBV specific-immunity plays an important role to control viral replication. There are differences between the CTL responses against lytic peptides and latent peptides. And the migration and activation of EBV specific CTLs with different cell surface molecules play an important role in EBV infection .

Objective To explore whether the neural stem cells(NSCs) can act directly as a gene target cell which can be infected by the recombinant retrovirus and express the products of exogenous genes after infection. Methods The NSCs were cultured with supernatant containing the recombinant retroviruses with the genes of NGF or GDNF for two days. After screened with G418,the infected NSC were expanded at the present of bFGF in culture. The PC12 cells and the neurons of ventral midbrain of rat were cultured by the medium from the infected NSC,which were called as GDNF\|containing conditioned medium NGF or GDNF\|containing conditioned medium the morphological changes of the dopamine neurons of the ventral midbrain and expression of exogenous genes of the infected NSCs were detected by immunohistochemistry staining. Results It was estimated that about fifty percent of NSCs via retrovirus\|mediated NGF or GDNF gene transduction were G418\|resistant.These infected NSCs began to differentiate.Long and radical processes reached out from the sphere of proliferation and the cells migrated towards outside along the processes. The NSC infected with gene of NGF showed an astroid\|shape with larger body and processes. The NSC infected with gene of GDNF showed a shuttle\|shape with a smaller body and long processes. The PC12 cells increased in the NGF\|containing conditioned medium and stretched out long neurites. The dopamine neuron of the ventral midbrain which were immunoreactive for TH also showed a larger body and longer processes in the GDNF\|containing conditioned medium. Most of G418\|resistant NSCs were immunoreactive for NGF or GDNF. Conclusion NSC can act directly as a gene target cell which not only be infected by the recombinant retrovirus, but also express and secrete the products of exogenous genes.

AIM To study the effects of Aβ25~35 and Apo E4 on neuronal intracellular free Ca2+([Ca2+]i). METHODS Hippocampal and cortical neurons suspension of newborn(0~3 days) SD rats was produced. After incubated with fura-2/AM,the neurons suspension was divided into four groups: control, Aβ25~35, Apo E4, Aβ25~35+Apo E4. Each groups [Ca2+]i was measured using a RF-5000 dual wavelength spectrofluorometer after incubated with double distilled water, Aβ25~35, Apo E4, Aβ25~35+Apo E4 for 3 min, respectively. The neurons outocorrelation function(ACF) of the scattering light intersity was analyzed by the microscope quasi-elastic light scattering(MQLS) technique The frequency shift line width by ACF. The Γ can sympolize the cell menbrane flilidity. RESULTS Both Aβ25~35 and Apo E4 could significantly enhance hippocampal and cortical neurons rest [Ca2+]i, furthermore, the effect of 5 μmol*L-1 Aβ25~35 was higher than the effect of 1 μmol*L-1 Aβ25~35 (P<0.05), and they also amplified KCl-induced rise in [Ca2+]i in hippocampal and cortical neurons(P<0.05). The interaction of Aβ25~35 and Apo E4 could also significantly enhance hippocampal and cortical neurons rest [Ca2+]i andamplified KCl-induced rise in [Ca2+]i in hippocampal and cortical neurons(P<0.05), but they had no synergic or additive effect. The frequency shift line widith Γ of both hippocampal and cortical neurons were decreased by both Aβ25-35 and ApoE4. CONCLUSION Aβ25~35 and Apo E4 could enhance neuronal intracellular free Ca2+, amd decrease meirpma; ,e,brame f;iodotu. But their interaction had no synergic or additive effect. It suggested that the amplified effect of Aβ25~35 and Apo E4 on neuronal [Ca2+]i and membane fluidity may be relative to their neurotoxity.
