2.Challenges and strategies of antiviral therapy for chronic hepatitis B-how to achieve the maximization of clinical cure ?
Journal of Clinical Hepatology 2017;33(8):1415-1418
Hepatitis B virus infection is still a major public health issue in the world, and antiviral therapy is the key therapeutic regimen to delay disease progression and improve outcome in patients with chronic hepatitis B (CHB).Various international guidelines recommend nucleos(t) ide analogues (NAs) and long-acting interferon as the first-line antiviral therapy.However,long-term administration of NAs has the disadvantages of long course of treatment, low HBeAg seroconversion rate, extremely low HBsAg clearance or seroconversion rate, low safety, and drug resistance. Therefore,it is an important issue to increase the seroconversion rates of HBeAg and HBsAg in treatment-experienced patients and realize clinical cure in the treatment of CHB.In recent years, many global randomized clinical trials including OSST,Switch, and ARES have shown that a combination of NAs and PEG-IFN or sequential therapy with NAs and PEG-IFN can increase the seroconversion rates of HBeAg and HBsAg in CHB patients and realize clinical cure, which provides a new direction for NAs in the treatment of CHB patients.

Objective To observe the safety and effect of magnetotherapy on rabbits with cerebral hemorrhage. Methods Fifty rabbits was used to establish the model of cerebral hemorrhage by perforating the mid artery of brain. CT and dissection were used to observe the hemorrhage. Forty five rabbits were divided into control group, 0.4T magnetic group and 0.8T magnetic group. Observation of the hemorrhage before magnetotherapy and the hemorrhagic absorption after magnetotherapy was conducted with CT scan. Results All of the 50 rabbits manifested such symptoms as weakness, low tension, and low or absent reflex on the contralateral hind limb of the operation side. Cerebral hemorrhage was proved by CT scan and dissection. There was no difference in hemorrhage between magnetotherapy and control group before magnetotherapy. After magnetotherapy, there was significant difference between the magnetotherapy and the control groups, with regard to the hemorrhagic absorption, but no difference between the 0.4T and 0.8T groups was observed. Conclusion Magnetotherapy could improve the intracranial hemorrhagic absorption in rabbit without resulting in secondary hemorrhage.

China Tropical Medicine 2023;23(6):672-
Abstract: Objective To report the clinical characteristics and genetic test results of two children with neurofibromatosis type 1 (NF1), and to provide reference for the comprehensive diagnosis-treatment and follow-up plans of NF1 patients based on the existing diagnosis and treatment progress of NF1. Methods Two children with NF1 admitted to the Department of Children's Medicine, Haikou people's Hospital in May and June 2022 were selected to analyze the clinical data of their clinical manifestations, laboratory examination, genetic test results, diagnosis and treatment and follow-up retrospectively. Results Two children had typical clinical manifestations, such as café-au-lait spots, axillary freckles, intraocular iris hamartoma. Venous blood was collected from case 1 and his parents for NF gene test, and a new mutation of c.4084C>T in the NF1 gene was found, and their parents did not have the pathogenic gene; the venous blood of the children in case 2 was tested for whole-exome gene analysis, and a heterozygous nonsense variant c.910C>T:p.R304 on the NF1 gene was found, , which was verified by Sanger sequencing to be inherited from his mother, his mother has café-au-lait spots and brain glioma, and has undergone surgery to remove the brain glioma, but has not undergone chemoradiotherapy or targeted therapy. No neurological malignancies were detected in either of the two children at follow-up until July 2022. Conclusions The clinical manifestations of NF1 are relatively typical, genetic testing is conducive to determine its classification, and regular follow-up review can help to detect and treat malignant tumors early, thus improving the patient's quality of life.
7.Effect of Early Rehabilitation Nursing on Secondary Disturbance in Hemiplegics after Stroke
Chinese Journal of Rehabilitation Theory and Practice 2006;12(9):819-820
ObjectiveTo study the effect of early rehabilitation nursing on secondary disturbance in hemiplegics after stroke.Methods140 patients were randomly divided into two groups , rehabilitation nursing group (70 cases) and control group (70 cases). Patients in the rehabilitation nursing group were given clinical treatment and regularly convalescent nursing, while those in control group were given clinical treatment and unguided self-training. Evaluation was performed in pre-treatment and 12 weeks post-treatment respectively.ResultsSecondary disturbance rate in the rehabilitation nursing group was significant lower than that in the control group (P<0.01). The motor scores and ADL scores measured by Barthel Index in both groups raised after 12 weeks treatment, but the scores in the early rehabilitation nursing group were obviously superior to those in the control group (P<0.01).ConclusionEarly rehabilitation nursing for hemiplegics afte sroke may prevent or lessen the secondary disturbance, as well as obviously improve motor function of the limbs and ADL.

Objective To investigate the application of quality control cycle activity in reducing thedisquali-fication rate of bowel preparation for colonoscopy outpatients. Methods Set up a group of quality control circle, and select the issues that reducing the disqualification rate of bowel preparation for colonoscopy outpatients as the activity theme.According to the activity steps of quality control circle, the activity was carried out.The causes of disqualifica-tion rate of bowel preparation were analyzed and the countermeasures to improve and implement were implemented. Also, the disqualification rates of bowel preparation before and after quality control circle were compared. Results The disqualification rate of bowel preparation was reduced from 39.09% to 14.03% after quality control circle activi-ty.The difference was statistically significant(χ2 =53.65,P <0.01).Target success rate was 104.42%,and the pro-gress rate was 64.11%.Conclusion The quality control circle activity can effectively reduce the disqualification rate of bowel preparation for colonoscopy outpatients, thereby improve the quality of diagnosis and treatment, and reduce the economic burden of patients. After clear improvement of focus, the leading causes of disqualification bowel prepa-ration are obtained through characteristic diagram (figure 2)analysis.Also,true causes are validated based on the three principles. The four true causes final defined include short contact time nurses and patients, large liquid dosage, none nursing intervention outside hospital, and chronic constipation without intervention.
9.Intervention effect of Dachengqi Granule on apoptosis of small intestine smooth muscle cells in rats with multiple organ dysfunction syndrome.
Chinese Journal of Integrated Traditional and Western Medicine 2014;34(5):587-591
OBJECTIVETo observe the intervention of Dachengqi Granule (DG) on the apoptosis of small intestine smooth muscle cells (SMCs) in rats with multiple organ dysfunction syndrome (MODS) and its mechanisms.
METHODSHealthy 100 adult Wistar rats were randomly divided into the control group (n =20), the MODS model group (n =40), and the DG group (n =40).E. coli suspension was peritoneally injected to rats in the model group and the DG group to establish bacterial peritonitis induced MODS model. DG at 1 mL/100 g was administered by gastrogavage to rats of the DG group, twice daily for 3 successive days. Twenty-four hours after modeling, the proximal segment of intestine was taken and stained by using terminal-deoxynucleotidyl transferase mediated nick end labeling (TUNEL) and immunohistochemistry. Changes of apoptosis quantity of SMCs and the expression of Bcl-2 associated X protein (Bax), B cell lymphoma/leukemia-2 (Bcl-2) and cytochrome c protein (Cyt c) in mitochondrial apoptotic signaling pathway were observed.
RESULTSCompared with the control group, the apoptosis quantity of SMCs and the expression of Bax and Cyt c protein significantly increased, and the expression of Bcl-2 protein significantly decreased in the MODS model group (P <0.01). Compared with the MODS model group, the apoptosis quantity of SMCs and the expression of Bax and Cyt c proteins significantly decreased, and the expression of Bcl-2 protein significantly increased in the DG group (allP <0.01).
CONCLUSIONDG could inhibit apoptosis of SMCs through suppressing activation of mitochondrial apoptotic signaling pathway in intestinal SMCs, thus promoting the recovery of the gastrointestinal motility function in rats with MODS.
