1.Clinical Observation of Electroacupuncture plus Needle-cupping for Lumbar Intervertebral Disc Herniation

Objective To observe the clinical efficacy of electroacupuncture plus needle-cupping in treating lumbar intervertebral disc herniation (LIDH). Method Eighty patients with LIDH were divided into an observation group and a control group by random number table, 40 in each group. The observation group was intervened by electroacupuncture plus needle-cupping, while the control group was by electroacupuncture plus cupping. The clinical efficacy, Modified Japanese Orthopaedic Association Scores (M-JOA) and Visual Analogue Scale (VAS) were observed and comparatively analyzed. Result The total effective rate was 95.0% in the observation group, significantly different from that of the control group (P<0.05); the M-JOA scores increased significantly (P<0.05) and VAS scores dropped significantly (P<0.05) in both groups, and the changes of the scores were markedly different between the two groups (P<0.05), indicating that the improvement in the observation group was more significant. Conclusion Electroacupuncture plus needle-cupping can produce a significant therapeutic efficacy in treating LIDH.
2.Clinical features of immunocompetent patients with Penicilliosis marneffei

Objective To discuss the clinical features of immunocompetent patients with Penicilliosis marneffei.Methods To analyze and summarize the clinical data of 4 immunocompetent patients with Penicilliosis marneffei.Results Clinical manifestation of patients centered on intermittent slight or mild fever,lymphadenectasis,skin eruptions,osteolytic destruction,anaemia,and leukocytosis.The cultural Penicillium marneffei in specimen such as blood,pus showed a higher positive rate.Amphotericin B and itraconazole was available in treatment.Conclusion Immunocompetent people can be infected by Penicillium marneffei,and they appear nonspecific in clinical manifestation after infection.A timely and effective antifungal therapy can improve the prognosis of patients.
3. Application prospect of oncolytic virus detection of circulating tumor cell in precision medicine of bladder cancer

Morbidity and mortality rates of bladder cancer (BC) are still rising with a poor prognosis. Therefore, better biomarkers are urgently needed to advance the accurate diagnosis and treatment of BC. The limitations of the different detection techniques of circulating tumor cell (CTC) cause that the CTC as biomarkers of point of view of diagnosis and treatment of BC is not yet clear. This review first compares and analyzes the current CTC detection technology methods, and then reviews the five aspects of screening, diagnosis, staging, curative effect monitoring, prognostic evaluation, and personalized treatment of patients with malignant tumors with oncolytic virus and CTC. The application of oncolytic virus detection CTC in BC was evaluated. The results suggest that oncolytic virus with fluorescent protein combined with fluorescence microscopy and flow cytometer are used to detect CTC. This method can be only used to detect live CTCs, instead of dead CTCs. The CTC count is more accurate, efficient with high sensitivity and specificity. It can also perform phenotypic analysis of CTC and single-cell sequencing. It can be used for screening, diagnosis, and guidance of targeted therapy for bladder cancer, assessing efficacy and judging prognosis which has a very broad clinical application prospect for advancing the accurate diagnosis and treatment of BC.
4.Evaluation of diffusion weighted imaging after contrast in characterizing breast tumors

Objective To evaluate the effect of intravenous administration of gadolinium-DTPA on diffusion weighted imaging (DWI)of breast lesions and apparent diffusion coefficient(ADC)values,and to evaluate the diagnostic performance of post-contrast ADC for differentiating benign from malignant lesions in breast.Methods Thirty-eight histopathologically-confirmed breast lesions were analyzed.We examined the signal to noise ratio(SNR)of normal breast tissue and lesions and evaluated the contrast to noise ratio(CNR)of each lesion.We also assessed the relation between pre-and post-contrast ADC values.Results SNRs and CNRs of DWI before and after contrast were not significantly different.The mean post-contrast ADC values for malignant lesions significantly decreased after contrast injected (mean of -1 1.6%),while ADC values in the benign lesions were not significantly different before and after contrast.Conclusion Post-contrast ADC values may be a better indicator of potential breast tumors than pre-contrast ADC values.
5.Initial experience of using improved motion sensitized driven equilibrium prepared balanced steady-state free precession

Objective To assess the application value of NCE-MRA using iMSDE prepared bSSFP sequence in lower limb in patients with diabetes. Methods This prospective study included a total of 35 patients with type II diabetes who underwent CE-MRA on the 1.5T MR scanner after the NCE-MRA. The obtained MIP images were independently rated by two radiologist with a four score table and using CE-MRA as a reference standard to evaluate the diagnostic accuracy of NCE-MRA for the narrowed arteries. The difference of the percent age of diagnostic arterial segments between NCE-MRA and CE-MRA on diabetic patients was evaluated by the χ2 test. Results Compared with CE-MRA, the diagnostic-value arterial segment ratio of pelvic arteries on NCE-MRA is decreased significantly. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of two readers in NCE-MRA were 95%/87%, 96%/95%, 57%/72%, 99%/99%, and 96%/95%(k=0.76), respectively. Conclusions The NCE-MRA using iMSDE prepared bSSFP sequence is capable of depicting vascu-lar lesions for the lower extremities in diabetic patients with the advantages of contrast agent free, short scan times and good diagnostic value in the thigh and calves.
6.Comparison of breath-hold and respiratory-triggered proton MR spectroscopy in quantification of liver fat content

Objective To compare the consistency and correlation of multiple breath-hold (BH) with respiratory-triggered (RT) 1H-MRS for quantification of hepatic lipid content. Methods Sixty subjects were underwent RT 1H-MRS of the liver (Couinaud segment VII) and BH 1H-MRS at 1.5 Tesla Magnetic Resonace Imaging (MRI). The peak areas of water and methylene obtained on RT and BH 1H-MRS were recorded respectively and the liver fat fraction was calculated. Pearson correlation coefficient , Bland-Altman 95% limit of agreement, and concordance correlation coefficient were calculated. Results Mean liver fat fraction measured in RT and BH 1H-MRS were (8.6 ± 8.7)% and (9.4 ± 9.3)% respectively. There was a strong correlation between RT and BH 1H-MRS(r = 0.973, P < 0.000 1, concordance correlation coefficient = 0.95). With the Bland-Altman method, 91.7% data points were within the 95% limits of agreement. Conclusion RT and BH 1H-MRS are alternative tools for intrahepatic lipid quantification. These two methods have a strong correlation and perfect consistency.
7.Establishment and in vivo imaging observation of a nude mouse model of type 2 diabetes mellitus and pancreatic cancer

ObjectiveTo establish a nude mouse model of type 2 diabetes mellitus (T2DM) and pancreatic cancer that allows dynamic observation of tumor formation process and facilitates in vivo research. MethodsAt first, human pancreatic cancer PANC-1 cells were transfected with lentiviral vector GV260 to construct the pancreatic cancer cell line PANC-1-Luc with stable expression of firefly luciferase. Then, 36 specific pathogen-free nude mice were randomly divided into control group with 12 mice and model group with 24 mice (nude mice with T2DM and pancreatic cancer). The mice in the control group were fed with breeding diet and were then given ectopic subcutaneous implantation of PANC-1-Luc cells, and those in the model group were first given high-fat diet and intraperitoneal injection of 1% STZ, followed by ectopic subcutaneous implantation of PANC-1-Luc cells. The fluorescence in vivo imaging system and the manual measurement method were used for simultaneous and dynamic monitoring of the growth of pancreatic cancer in nude mice in the two groups, and the tumor growth curve was plotted to investigate the correlation between fluorescence value and tumor volume. Subcutaneous tumors and pancreatic islets were observed under a microscope to verify whether the model was successfully established, and immunohistochemistry was used to measure the expression of Ki-67 in tumor tissue to investigate the influence of hyperglycemia on the growth of pancreatic cancer in nude mice. The independent-samples t test was used for comparison of normally distributed continuous data between groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between groups. ResultsThe optimal virus titer was determined as 5×107 TU/mL for the stable transfection of lentiviral vector in PANC-1 cells, and the optimal concentration selected with puromycin was 20 μg/mL, with an optimal selection time of 9 days. The fluorescence value of PANC-1-Luc cells was linearly and positively correlated with the number of cells, with the linear equation of y=42.56x-42 504 (r=0.977, P=0.004). The blood glucose value of T2DM nude mice was 23.05 (19.25 — 26.40) mmol/L, with a blood glucose level of >11.1 mmol/L in each nude mouse, and there was a significant difference in blood glucose value between the T2DM nude mice and the control nude [6.15 (5.20 — 7.30) mmol/L] (Z=-8.45, P<0.001). Compared with the control group, the model group had reductions in the number and volume of pancreatic islets, with irregular shapes and unclear boundaries, and pathological examination confirmed that the xenograft tumor was pancreatic cancer tissue, which showed that the model was established successfully. In the model group, there was a linear positive correlation between subcutaneous tumor size and fluorescence values, with the linear equation of y=232 348 691x-8 258 608 (r=0.911, P=0.031). The model group had a significantly higher positive rate of Ki-67 than the control group (50.333%± 7.808% vs 15.917%±4.055%, t=13.55, P<0.001), suggesting rapid tumor proliferation in the model group. ConclusionThe T2DM nude mouse model of pancreatic cancer established in this study can simulate the pathological process of the development and progression of pancreatic cancer in the context of T2DM and dynamically observe the influence of hyperglycemia on the growth of pancreatic cancer cells in vivo, thereby providing a new experimental vector for the in vivo study of the development and progression of pancreatic cancer in the context of T2DM.
8.Open, multicenter, phase Ⅳ clinical trial of Shenbei Guchang capsules in treatment of diarrhea type irritable bowel syndrome.

To evaluate the safety and effectiveness of Shenbei Guchang capsules in treatment of diarrhea type irritable bowel syndrome (yang deficiency of spleen and kidney) under widely used conditions, an open, multicenter, controlled, phase Ⅳ clinical trial was conducted in the drug clinical trial centers of 16 domestic hospitals. 2 123 patients from June 10, 2011 to November 29, 2012 were enrolled in the trial. Drug clinical trial was approved by Sichuan Academy of Medical Sciences, Sichuan Provincial People's Hospital Ethics Committee before implementation. Before the start of trial, subjects were selected according to the research scheme and inclusion criteria, then they would step into the 14 d study after signing Informed Consent Form. All subjects were treated according to the research scheme, evaluated the conditions and filled in CFR sheet, to provide the evaluation data and information on safety and efficacy of Shenbei Guchang capsules. Shenbei Guchang capsules were used to treat diarrhea type irritable bowel syndrome in widely used conditions (2 123 cases), and 2 029 cases of them entered FAS set, cure+markedly effective in 1 921 cases, with a comprehensive curative effect rate of 94.68%; 2 010 cases of them entered PPS set, cure+markedly effective in 1 906 cases, with a comprehensive curative effect rate of 94.83%. The primary symptoms of IBS were abdominal pain and diarrhea. After treatment, both abdominal pain and diarrhea were improved, with significant differences (P<0.000 1). There were significant differences in traditional Chinese medicine symptom scores on both post-treatment day 7 and day 14 as compared with the conditions before treatment (P<0.000 1). 35 cases of adverse events occurred during the trial with an incidence of 1.65%, including 12 cases of drug-related adverse events (adverse reaction) with an incidence of 0.57%, mainly manifested as nausea, abdominal distension and dry mouth, most of which would be spontaneously relieved without any measures. No serious adverse events occurred. The commercially available Shenbei Guchang capsules are proved safe and effective for the treatment of diarrhea type irritable bowel syndrome (yang deficiency of spleen and kidney) under widely used conditions (2 123 cases), and can be continued for clinical promotion and application.
