1.Significance of pH changes of arterial blood in patients with acute left heart failure

Objective To investigate the pH changes of arterial blood in patients with acute left heart failure.Methods Thirty-eight patients with acute left heart failure were selected as our subjects,who all underwent diuresis,the strong heart diuresis,dilation of blood vessels,reducing the load on the heart,and making water and electrolyte balance of conventional medication,and then parallel oxygen mask,respirator mode synchronized intermittent mandatory ventilation(SIMV).If hypoxia was not significantly improved,and positive pressure ventilation with end expiratory(PEEP) treatment was conducted.Results After treatment,the level of pH,heart rate(HR),breathing rate(RR),arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2) and mean arterial pressure(MAP) were (7.4 ± 0.3,86.3 ± 6.2) times/min,(22.5 ±4.8) times/min,(84.8 ±7.3) mmHg,(43.3 ±5.5) mmHg,(81.1 ±3.2) mmHg,different from those before treatment(7.1 ±0.1),(118.3 ±8.2) times/min,(36.2 ±7.1) times/min,(47.2 ±6.6) mmHg,(48.3 ±5.2) mmHg,(96.2 ±4.5) mmHg),and the differences were significant (t =5.848,19.188,9.854,23.552,4.072,16.857 respectively; P < 0.01).With the progress of treatment,pH value and pHi value in patients showed a rising trend.Regression analysis showed a positive correlation and the regression equation was Y =-0.559 4 + 1.103 7X.Conclusion Patients with acute left heart failure show abnormal blood pH level and the corresponding change inside red blood cells.Then it causes changes in arterial blood oxygen carrying capacity of red blood cells,reduces the cell through the capillary capacity.Therefore,it can result in decrease of gas exchange levels in patients and hypoxia.Treatment should be considered imbalance the body of the improved state of acid-base.

Objective To investigate the expression of connective tissue growth factor( CTGF) and the effects of azithromycin in airway remodeling in asthmatic mice.Methods BALB/c female mice were randomly divided into 3 groups;control group(A),asthma group(B) ,and azithromycin treated group(C),with 10 mice in each group.Mice were sensitized and challenged continually with ovalbumin( OVA).The distribution and types of collagens were detected by cirius red staining.Lung hydroxyproline content was determined by acid hydrolysis.CTCF expression in mice pulmonary tissues was detected by SABC immunohistochemistry.Results After continual challenging with OVA,collagen hyperplasia was found in the airway wall and CTCF positive expression in airway epithelium.CTCF expression was closely associated with lung hydroxyproline content(r = 0.65,P <0.01).Conclusion Asthma continually challenged with OVA could result sub-epithelial fibrosis and remodeling in airway,which could be associated with the upregulation of CTCF over-expression.Azithromycin inhibited airway remodeling in asthmatic mice,possibly by inhibiting the expression of CTGF.
3.Correlative analysis of eccentricity of optical treatment zone with tear film function and corneal shape after wearing orthokeratology lenses

AIM: To explore the correlation between eccentricity in the optical treatment area and tear film function and corneal morphology after orthokeratology.
METHODS:Eighty patients(80 eyes)with myopia who were treated with orthokeratology were selected as subjects. The eccentric distance, tear film function and corneal morphology of the patient's optical treatment area were detected at different time points before and after wearing the lens. According to the median eccentric distance, the patients were divided into low eccentricity group and high eccentricity group. The differences of tear film function and corneal morphology at different time points before and after wearing were compared between the two groups. Pearson correlation coefficient was used to analyze the correlation between total eccentricity and tear film function and corneal morphological parameters at 12mo after wearing glasses.
RESULTS:Compared with before wearing lenses, the corneal surface asymmetry index and corneal surface regularity index of the two groups were significantly increased at 1wk, 3, 6 and 12mo after wearing lenses(P<0.05). Patients in the high eccentric group were more significantly elevated than in the lower eccentric group, and the differences were statistically significant(P<0.05). The time of tear film rupture, corneal curvature, basic tear secretion, and central corneal thickness were significantly lower, and the differences were statistically significant(P<0.05). Compared with the patients with lower eccentricity, the patients with high eccentricity group had more significant reduction in tear film rupture time and basic tear secretion, and the difference was statistically significant(P<0.05), however, the degree of corneal curvature and central corneal thickness decreased less(P<0.05). There was no significant difference in corneal endothelial cell density between 1wk after wearing lenses and 3mo after wearing lenses(P>0.05). The difference was statistically significant at 6mo and 12mo after wearing lenses(P<0.05). At 12mo after wearing the lens, the total eccentric distance was positively correlated with the asymmetry index of the corneal surface and the regularity index of the corneal surface(r=0.869, 0.815; P<0.05). At 12mo after wearing the lens, the total eccentric distance was negatively correlated with tear film rupture time, corneal curvature, basic tear secretion, and central corneal thickness(r=-0.865, -0.745, -0.912, -0.713; all P<0.05), and there was no significant correlation with corneal endothelial cell density(r=0.185, P=0.199).
CONCLUSION:The eccentricity of the optical treatment area occurs at the beginning of wearing lenses. And the total eccentric distance has a significant correlation with the changes of tear film function and corneal morphology after keratoplasty.
4.Effect of femtosecond laser and mechanical lamellar LASIK on the stability of corneal flap thickness

AIM: To explore the effects of femtosecond laser and mechanical lamellar blade excision laser in situ keratomileusis(LASIK)on corneal flap thickness stability and dry eye syndrome.
METHODS: Retrospective analysis of 80 patients with myopia who underwent LASIK in our hospital. According to different methods of flapping, it is divided into femtosecond laser group and mechanical laminating knife group. The flap thickness of the two groups of patients at different time points(1h, 1d, 1wk, 1mo, 3mo)were measured. The differences in corneal apex 1, 2, 3mm and corneal apex flap thickness in different directions were compared. The incidence of dry eye syndrome was compared between 1 and 3mo, 1a, 2a after surgery.
RESULTS: The apex thickness of the corneal flap of the femtosecond laser group is 99.62±4.50μm, and the thickness of the apex of the mechanical lamellar knife group is 125.25±12.81μm.The thickness of the corneal flap at different centrifugal points of the femtosecond laser flap is relatively uniform, while the thickness of the corneal flap of the mechanical lamellar knife varies greatly. The OSDI scores of the mechanical lamellar group were higher than those of the femtosecond laser group at 1, 3, 1, and 2a postoperatively(P<0.01). At 2a of follow-up, the incidence of dry eye in the femtosecond laser group was lower than that in the mechanical lamellar group(χ2=4.692, P=0.030). At 1 and 2a postoperatively, the proportion of mild dry eye in the femtosecond laser group was higher than that in the mechanical lamellar group(P=0.044, 0.001).
CONCLUSION: Compared with the mechanical lamellar scalpel, the corneal flap thickness of patients with femtosecond laser LASIK is better, more uniform and less error. And the incidence of dry eye after surgery is lower, with mild dry eyes.
5.Effects of azithromycin on airway remodeling and transforming growth factor-β1 in asthmatic mice

Objective To investigate the effects of azithromycin on airway remodeling and the expression of transforming growth factorβ1 (TGF-β1) in asthmatic mice.Methods BALB/c male mice were random divided into 3 groups:control group(A),asthma group (B),and azithromycin treated group (C),with 10 mice in each group.Bronchoalveolar lavage fluid was collected to count the number of white blood cells and eosinophilic granulocytes EOS.The morphological parameters of the bronchi were measured by computer image analysis and the pathologic changes of the bronchi and lung tissue were observed by HE staining.The expressions of TGF-β1 were detected by immunohistochemistry.Results The number of EOS in B group was significantly higher than that in control group(8.12 ±0.54 vs 0.70 ±0.40;8.12 ±0.54 vs 0.87 ±0.25,P <0.01).WAt and WAi and WAm in group C was significantly lower than that in group B (10.15 ±0.95 vs 15.36 ±0.85,4.16 ±0.32 vs 10.64 ± 1.03,3.77 ±0.15 vs 7.97 ±0.17,P <0.01)but higher than that in group A.The expression of TGF-β1 in group C was significantly lower than that in group B but higher than that in group A.TGF-β1 expression of lung tissue in C group was significantly correlated with EOS,(r =0.840,P <0.01) and WAt(r =0.735,P <0.01) and WAm (r =0.870,P <0.01).Conclusion Azithromycin inhibited airway remodeling in asthmatic mice,which might possibly be achieved through inhibiting the expression of TGF-β1.
6. The predictive value of preoperative fibrinogen level in peripheral blood combined with platelet-to-lymphocyte ratio in the prognosis of colorectal cancer

ObjectiveThe local recurrence or distant metastasis of colorectal cancer (CRC) after surgical resection still directly affects the prognosis of CRC patients.This study aims to investigate the clinical significance of preoperative peripheral blood fibrinogen (Fbg) level combined with platelet-to-lymphocyte ratio (PLR) in the prognosis of CRC.MethodsThe clinicopathological characteristics of 108 CRC patients who were diagnosed by pathology and underwent surgical resection in Wujin hospital affiliated to Jiangsu University from January 2013 to December 2015 were analyzed. Fbg and PLR thresholds were determined by ROC curve, and the patients were grouped according to Fbg combined with PLR (F-PLR) to analyze the relationship between different F-PLR scores and the clinicopathological characteristics and prognosis of CRC patients.ResultsPLR showed statistically significant difference in the left and right half of the tumor (P<0.05), while Fbg and f-plr showed no significant difference in age, tumor site and lymph node metastasis (P>0.05). Multivariate analysis using COX regression showed that F-PLR score, TNM stage and lymph node metastasis were independent risk factors for the prognosis of CRC patients (P<0.05).ConclusionThe F-PLR score (Fbg and PLR combined) can be used as a predictor of treatment effect and prognosis of CRC patients to guide clinical treatment decisions and prognoses.
7.Expression of COX-1 and COX-2 in the Platelet of Iron Deficiency Anemia Women at Childbearing Age and Its Clinical Significance.

OBJECTIVETo detect the expression level of cyclooxygenase-1(COX-1) and cyclooxygenase-2(COX-2) in the platelet of iron deficiency anemia(IDA)women at childbearing age and to explore its correlation with the different indexes of anemia and platelets.
METHODSForty female IDA patients at childbearing age and 35 healthy controls were enrolled in this study. The Flow cytometry was used to detect the expression of platelet COX-1 and COX-2,the platelet aggregation function as examined by turbidimetric method,and the levels of serum ferritin were analyzed by electrochemical luminescence method,the leval of serum iron was determined by ELISA,and the correlation of different indexes was analyzed.
RESULTSCompared with healthy controls,the levels of platelet COX-1 and COX-2 were significantly lower in female IDA patients at Childbearing age(P<0.05),but platelet count(Plt),mean platelet volume(MPV) and platelet aggregation rate(PAgT)were not statistically different between the 2 groups(P > 0.05). The expression level of platelet COX-1 positively correlated with those of Hb(r =0.623,P<0.01),serum iron(r =0.321,P<0.05) and HCT(r=0.305,P<0.05). but the platelet COX-2 expression did not corelate with these indexs.
CONCLUSIONThe expression of platelet COX-1 and COX-2 in female IDA patients at Childbearing age markedly decrease,and the expression level of platelet COX-1 closely relates with the severity of anemia,that possesses reference value for clinical diagnosis of female IDA patients at Childbearing age..

8.Implementation effect of mixed teaching mode + formative evaluation in the course of normal human morphology

The nursing students of Grade 2009 to Grade 2016 were selected.Their skills,midterm,usual and final performances as well as comprehensive scores of normal human morphology course were analyzed by one-way ANOVA in combination with different stages of curriculum integration in order to compare the teaching effect between traditional teaching mode + evaluation system and mixed teaching mode + formative evaluation in the course of normal human morphology.Statistical analysis by SPSS 17.0 showed that the average score of formation stage (traditional teaching mode) was 67.55 ± 12.72,the average score of integration stage (mixed teaching mode) was 72.79 ± 10.93 and the average score of development stage (formative evaluation +mixed teaching mode) was 83.94 ± 9.58.After making comparisons,the P values of ANOVA of the course of normal human body morphology in all stages were all less than 0.001,having statistical significance.The results showed that the overall development of the curriculum is becoming mature and the new teaching model can help students who usually do not like to review.
9.Combined Influence of Insulin Resistance and Inflammatory Biomarkers on Type 2 Diabetes: A Population-based Prospective Cohort Study of Inner Mongolians in China.

This prospective study was designed to examine the combined influence of insulin resistance (IR) and inflammatory biomarker levels on type 2 diabetes mellitus (T2DM) among 1,903 Inner Mongolians. During follow-up, 205 (10.77%) participants developed T2DM, and the incidence of T2DM was higher among subjects with IR, elevated C-reactive protein (CRP), elevated sICAM-1, elevated sE-selectin, or the coexistences of IR with elevated CRP, elevated sICAM-1, elevated sE-selectin, and elevated angiotensin II (all P < 0.05) compared with patients without IR or any elevated biomarkers. In multivariate analysis, the odd ratios [OR, (95% confidence intervals)] for these conditions were 1.944 (1.405-2.691), 2.003 (1.449-2.767), 1.706 (1.232-2.362), 1.560 (1.123-2.165), 2.708 (1.809-4.054), 1.885 (1.155-3.078), 2.101 (1.340-3.295), and 2.260 (1.426-3.582), respectively. Our findings demonstrated that IR and elevated inflammatory biomarkers were associated with T2DM, and that the coexistence of IR and elevated inflammatory biomarkers increased the risk of T2DM.
