1.Use of the Nickel-Titanium patella concentrator in surgical treatment of patellar fracture

Objective To discuss the application value of the Nickel-Titanium patella concentrator(NT-PC) in surgical treatment of patellar fracture.Methods A retrospective analysis was made on clinical data of 32 cases of patellar fracture treated with the NT-PC in this hospital from January 2001 to December 2003.Results Re-examination of radiography at 12 weeks after surgery showed bone union was achieved in all the cases,with a smooth patellofemoral articular surface.The patella was preserved completely in all the comminuted fracture cases.According to the Lu's criteria,excellent and good outcomes were achieved in 26 and 6 cases,respectively,the rate of excellent or good results being 100%.The NT-PC was removed at 4 months ~ 2 years(mean,14 months) postoperatively without complications.Conclusions Application of NT-PC in the treatment of patellar fracture can preserve the comminuted patella completely,facilitate the reduction and fixation of the patellofemoral articular surface,and make early postoperative motions available.It is worthy of being recommended.
2.Comparison and Analysis of Irinotecan Alone or Combined with Xiaoaiping in the Treatment of Elderly Pa-tients with Advanced Gastric Cancer

OBJECTIVE:To evaluate therapeutic efficacy and safety of chemotherapy combined with Xiaoaiping injection in the treatment of elderly patients with advanced gastric cancer. METHODS:60 elderly patients with advanced gastric cancer were collected from our department during 2012-2014,and then divided into union group and single group with 30 cases in each group. Single group was given irinotecan(CPT-11)150 mg/m2 added into 0.9%Sodium Chloride injection,and union group was addition-ally given Xiaoaiping injection 80 ml,qd,on the basis of single group. A treatment course lasted for 14 days,and both groups re-ceived 4 courses of treatment. The tumor markers,disease single rate,progression-free survival (PFS) and the incidence of ADR were compared between 2 groups. RESULTS:After treatment,the levels of CEA,CA199 and CA242 in 2 groups were significant-ly lower than before;the union group was significantly lower than the single group,with statistical significance(P<0.05). The dis-ease single rate of union group was 70.00%,which was significantly higher than that of single group (33.33%). PFS of union group was(11.23±2.17)months,which was significantly longer that of single group [(9.36±0.84)months],with statistical signif-icance(P<0.05). The incidence of leucopenia,alanine aminotransferase elevation,diarrhea,stomatitis and hand-foot syndrome in union group was significantly lower than in single group,with statistical significance(P<0.05). CONCLUSIONS:Xiaoaiping in-jection combined with irinotecan chemotherapy for advanced gastric cancer has obvious effect with a low incidence of ADR.
4.Growth factors-mediated effects on the differentiation of human adipose-derived stem cells into chondrocytes.

In recent years, there has been a growing emphasis on use of human adipose-derived stem cells (hADSCs) for cartilage tissue engineering owing to their ability to differentiate into chondrocytes, which is mainly induced by growth factors (GFs). In general, GFs for chondrogenic induction come from the transforming growth factor beta (TGF-beta) superfamily. To date, the most commonly used GFs for chondrogenes is TGF-beta1/3. However, the response of hADSCs to GFs may differ significantly from that of human bone marrow stem cells (hBMSCs). It has been reported that bone morphogenetic protein-6 (BMP-6) treatment induced TGF-beta receptor-I expression of hADSCs. It seems that these two cell populations varied strongly in their potency to undergo chondrogenesis in the same medium conditions. Here, we provide a concise review on various GFs used in chondrogenic differentiation of hADSCs in vitro.

6.Effect of APS on recoverable ability from cryopreservation damage of UCB hematopoietic cells

Objective To study the effect of angelica polysaccharides (APS) on recoverable ability from cryopreservation damage of umbilical cord blood (UCB) hematopoietic cells and APS with hematopoietic growth factors (HGFs) on in vitro expansion of UCB mononuclear cells (MNC) after thawing. Methods Thawed UCB MNC were cultured 24 h or 14 d with various concentration of APS (0,50,100,200,and 400 ?g/mL) or with APS and HGFs. 7.Isolation,culture and identification of rat bone marrow mesenchymal stem cells and endothelial progenitor cells

BACKGROUND:There are no unified methods to isolate,culture and amplify bone marrow mesenchymal stem cells(BMSCs) . Endothelial progenitor cells(EPCs) can be harvested by inducing peripheral blood or bone marrow mononuclear cells. However,the method of collecting bone marrow mononuclear cells is various,and the isolation efficiency is different. OBJECTIVE:To isolate and identify BMSCs and EPCs in vitro. DESIGN,TIME AND SETTING:The cytological in vitro study was performed at the Central Laboratory,First Affiliated Hospital,Anhui Medical University from October 2008 to February 2009. MATERIALS:Two clean healthy male Sprague Dawley rats were supplied by the Anhui Animal Center. METHODS:Rat bone marrow was extracted to harvest mononuclear cells by density gradient centrifugation. BMSCs were isolated by the differential adhesion. Following 48 hours of primary culture of BMSCs,non-adhered cells were collected and induced by EGM-2 complete medium supplemented with fetal bovine serum,vascular endothelial growth factor,fibroblast growth factor,insulin-like growth factor-1 and recombinant human epidermal growth factor. There were three groups:early adherence + common culture EPCs,twice adherence + induction culture EPCs,and rat mature aorta EPCs. Nitric oxide content was directly measured in the medium using nitrate reductase method. MAIN OUTCOME MEASURES:The following parameters were measured:in vitro culture of BMSCs and EPCs,and nitric oxide content in the medium. RESULTS:Primary culture of BMSCs was in a spindle-shape,within 24 hours the majority of cells were adherent,9-10 days up to 90% cells were confluent. Purified BMSCs were amplified and uniform,long spindle,with the passage cycle for 8 days. Flow cytometry detection showed that cells were CD34-,CD45-negative and CD105-positive. Second adherent of EPCs induced by 3 days culture showed colony formation following six days,appeared cord-like structure and microvessel-like growth at 8-10 days. At 2 weeks,the majority of cells were polygonal,colony-forming unit was interconnected,showing a typical "cobblestone"-like. At 7-10 days,cells were double stained DiI-acLDL and FITC-UEA-1. 8.Image findings and clinical characteristics of multislice helical CT in intrahepatic mass-forming cholangiocarcinomas

Objective To study the image findings and clinical characteristics ofmultislice helical CT (MSCT) in intrahepatic mass-forming cholangiocarcinomas.Methods Fifteen patients of pathologically confirmed intrahepatic mass-forming cholangiocarcinomas were studied retrospectively.The image findings of MSCT and clinical manifestations,laboratory tests of each case were recorded respectively.Results All patients image findings of MSCT:density was uneven,moderately low congestion densities was 14 patients.High and low congestion densities was 1 patient,pathologically confirmed bleeding.Obscure boundary and atrophy of liver lobe was 10 patients,adjacent liver capsule retraction was 11 patients,combined with ductal dilatation was 11 patients,intrabepatic calculi was 5 patients,ductal wall thickening with or without narrowed bile duct was 9 patients,satellite nodules was 10 patients.Enhancement scanning showed:peripheral enhancement was 11 patients,arterial phase border enhancement was 11 patients,all had delayed enhancement,honeycomb-like enhancement was 3 patients,obliteration or invasion of portal vein was 11 patients.Conclusion There are some clinical characteristics of MSCT findings in intrahepatic mass-forming cholangiocarcinomas.
9.Construction of alienation scale in adolescent students and assessment of its theory

BACKGROUND: Alienation is derived from dissimilation in philosophy.It is practically defined as the subjective state of individuals who develop negative feelings such as social isolation, uncontrollable and meaningless sense, oppression, and self-alienation because they are alien to others, the.society, nature and themselves, and even feel controlled.OBJECTIVE: To investigate alienation from psychological perspective,aiming to construct and verify the theoretical hypothesis of alienation,thereby working out alienation scale for adolescent students.DESIGN: Cross-sectional investigation.SETTING:National Key Research Center of Basic Psychology and National Key Liberal Arts of Southwest Normal University.PARTICIPANTS: The investigation was carried out from March to June 2000. The method of cohort stratified random sampling was adopted and the questionnaire was administered to adolescent students in Chongqing City and Lanzhou City. Inclusion criteria: volunteer students of grade one in junior high school to grade four in colleges. Exclusion criteria: Those who had incomplete data or had differences of more than 6 points in the total scores for three matched lie-testing questions in the questionnaire. A total of 1502 pieces of questionnaire were collected (including 1 066 questionnaires used for exploratory factor analysis and 436 questionnaires for verifying factor analysis); the age of the students covered the whole range of adolescence, and the boy-to-girl ratio in each grade was matched.METHODS:We combined literature analysis, investigation research and interview. The details are as follows: First, theoretical hypothesis was put forward based on literature analysis; then related variances were collected through interviews and open questionnaires, which were compared with the items in scales used home and abroad at present so as to work out predicting questions for adolescence alienation questionnaire. Finally, 52 items were screened out and used to construct the preliminary questionnaire. The formal alienation scale was made after standardized designation of behavioral variances, and verified for alienation analysis in adolescent students.MAIN OUTCOME MEASURES: Behavioral apparent variances were collected based on interviews and open questionnaires, and used as psychological potential variances after statistical processing.RESULTS:Data of 1502 pieces of questionnaire were analyzed.① The correlation coefficient of each item in the scale was over 0.35. ② The correlation coefficient of each item with the dimension was over 0.30, with statistical significance(P < 0.01). ③ The reliability coefficient of each item was over 0.39 with great significance(P < 0.01). ④ Alienation was displayed in three dimensions: social alienation, interpersonal alienation and surrounding alienation, which were associated with the correspondingdimensions of related scales (P < 0.05-0.01). ⑤Alienation could be divided into 9 levels: sense of meaninglessness, sense of self alienation, isolation, nature alienation, parent alienation, environmental alienation, sense of uncontrollability and social alienation. Parameters that accorded with the theoretical hypothesis and used for verifying analysis were proved rational.Based on the analysis of the 9 first-rank factors, second-rank structural model of 3 unrelated factors were obtained using main component analysis and the maximum variance cross rotation with the optimal coefficients of simulated parameters of: X2/df=4.87;GFI=0.94;A GFI=0.90;RMR=0.0S;NFI=0.93;NNFI=0.92;CFI=0.95;IFI=0.95.CONCLUSION: Alienation scale has been proved to be reliable and valid. It can be used to assess alienation in adolescent students and has potential value of application in healthy psychological development and psychological education.
10.Brain-derived neurotrophic factor in the development and metabolism of bone and tooth:promoting or inhibiting proliferation and differentiation?

BACKGROUND:Brain-derived neurotrophic factor has been detected in bone and tooth, and its role in development and metabolism of bone and tooth tissue as wel as its clinical application has become a hot spot. OBJECTIVE: To summarize and analyze the effect and mechanism of brain-derived neurotrophic factor in development and metabolism of bone and tooth tissues. METHODS: Papers addressing the effect of brain-derived neurotrophic factor in bone and tooth tissue were retrieved by computer in Wanfang and PubMed databases with the key words of “brain-derived neurotrophic factor, TrkB, p75NTR, signaling, bone, tooth, osteoblasts, osteoclasts” in Chinese and English, respectively. A total of 53 papers were included for review. RESULTS AND CONCLUSION:Brain-derived neurotrophic factor can be detected in various tissuesin vivo, and can regulate cel survival and apoptosis through binding its two receptors. Brain-derived neurotrophic factor in bone and tooth tissue can bind to target cels, induce or inhibit cel proliferation and differentiation, indicating that brain-derived neurotrophic factor is closely linked to bone and tooth tissue, and may play a role in growth and reconstruction of bone and tooth. Its mechanism of action is mainly through binding to TrkB receptor, to activate downstream pathways and affect differentiation and proliferation of mesenchymal stem cels, osteoblasts, chondrocytes, and periodontal ligament cels. Interaction between p75NTR receptor and TrkB receptor may be one of the factors affecting cel differentiation or proliferation.
