1.The impact of throughout patient-controlled epidural analgesia on labor progress and delivery mode

Objective To explore the impact of throughout patient-controlled epidural analgesia(PCEA) on labor progress and delivery mode. Methods We enrolled 120 nulliparous women with term cephalic singleton preg-nancies. They were divided into two groups ,throughout analgesia group(n=60):when they are in labor,we started an-algesia and continued to the end of second stage of labor. While 60 nulliparous were selected to non-analgesia group as the control group. Data were collected about duration of the first, second stage of labor, oxytocin dosage, postpartum hemorrhage,outcome of delivery, neonatal Apgar scores, umbilical cord vein and artery blood gas analysis and the effect of epidural analgesia, etc. Results There wsa is significant difference in the duration of the first stage delivery, the utilization rates of oxytocin, delivery mode (P<0.05) ;and there was no remarkable differences in the second stage of labor process, postpartum hemorrhage, neonatal Apgar scores, umbilical cord vein and artery blood gas analysis (P>0.05) Conclusion The throughout patient-controlled epidural analgesia can shorten labor progress, cut down uterine-incision delivery and reach satisfying analgesis effect, without commidng any impact on the outcome of delivery.
2.Effects of Different Doses of Budesonide Combined with Ipratropium Bromide on Rational Symptom, Lung Function and Arterial Blood Gas Indexes of AECOPD Patients

OBJECTIVE:To investigate the safety and effects of different doses of budesonide combined with ipratropium bro-mide on rational symptom,lung function and arterial blood gas indexes of patients with acute exacerbation of chronic obstructive pulmonary diseases (AECOPD). METHODS:186 AECOPD inpatients were selected and randomly divided into low-dose,medi-um-dose and high-dose groups,with 61 cases in each group. All groups received routine treatment as ipratropium bromide 500 μg+0.9%Sodium chloride injection(NS)2 ml,oral inhalation with oxygen drive atomization nebulizer,oxygen flow rate of 4-5 L/min, 15-20 min/time,tid. Low-dose,medium-dose and high-dose group were additionally given Budesonide suspension 0.5,1.0,2.0 mg+NS 2 ml respectively,oral inhalation with oxygen drive atomization nebulizer,oxygen flow rate of 4-5 L/min 20 min/time,bid. Pa-tients in 3 groups were treated for 7 days. Pulmonary function,MRC score,arterial blood gas indexes and ADR were compared among 3 groups before treatment,3 and 7 days after treatment. RESULTS:FEV1% of 3 groups were significantly improved after treatment;3 and 7 days after treatment,FEV1% of high-dose group was higher than that of medium-dose and higher than that of low-dose group,with statistical significance(P<0.05). The MRC scales of 3 groups were significantly decreased after treatment;3 days after treatment,MRC scale of high-dose group was lower than those of medium-dose and lower than that of low-dose group,with statistical significance(P<0.05). 7 days after treatment,MRC scales of medium-dose and low-dose groups were low-er than that of low-dose group,with statistical significance (P<0.05),but there was no statistical significance in MRC scale be-tween high-dose group and medium-dose group after treatment 7 days(P>0.05). After treatment,there was statistical significance in PaCO2 and PaO2 among 3 groups (P<0.05),and the improvement of high-dose group was better than those of medium-dose and better than that of low-dose group,with statistical significance(P<0.05). No severe ADR was found in 3 groups. CONCLU-SIONS:Aerosol inhalation of 2.0 mg budesonide combined with ipratropium bromide can effectively improve pulmonary function, rational symptoms and arterial blood gas indexes with good safety.
3.Clinical Observation of 2 Kinds of Glucocorticoid Administration Regimens for Elderly Patients with AE-COPD

OBJECTIVE:To investigate the effects and safety of 2 kinds of glucocorticoid administration regimens including 5 d and 15 d on clinical efficacy,pulmonary function,quality of life in elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). METHODS:A total of 160 elderly patients with AECOPD selected from Haikou Forth People's Hos-pital during May 2013-Dec. 2015 were divided into group A and B according to lottery method,with 80 cases in each group. Both groups received symptomatic treatment as eliminating phlegm and relieving asthma,controlling infection,etc. Based on it,group A was given Methylprednisolone sodium succinate for injection,40 mg/d for 15 d;group B was given Methylprednisolone sodium succinate for injection,40 mg/d for 5 d. Clinical efficacies were compared between 2 groups as well as the levels of pulmonary ven-tilation function indexes,blood gas analysis indexes,Spitzer life quality index scale (QLI) scores before and after treatment. The occurrence of ADR was also compared between 2 groups. RESULTS:Total response rate of group A was 92.50%,and that of group B was 90.00%,without statistical significance(P>0.05). Before treatment,there was no statistical significance in the levels of pulmonary ventilation function indexes,blood gas analysis indexes,QLI scores between 2 groups (P>0.05). After treatment, FVC,FEV1,FEV1% and PaO2 of 2 groups were significantly higher than before treatment,while PaCO2 was significantly lower than before treatment,with statistical significance (P<0.05);there was no statistical significance between 2 groups (P>0.05). QLI scores of 2 groups were significantly higher than before treatment,and the group B was significantly higher than the group A, with statistical significance (P<0.05). The incidence of ADR in group B (6.25%) was significantly lower than group A (22.50%),with statistical significance(P<0.05). CONCLUSIONS:Glucocorticoid administration regimen including 5 d and 15 d in the treatment of elderly patients with AECOPD possess the same effects in improving lung function;glucocorticoid administra-tion regimen for 5 d application is superior in effectively improving the quality of life of patients,reducing the risk of ADR,and has clinical value.
4.The exact estimation of visual acuity by VEP technology: A report of 726 cases of eye injury.

This study explored the accuracy of using visual evoked potentials (VEP) technology for visual acuity estimation. The enrolled 726 patients with post-traumatic unilateral decrease in visual acuity included the injured eyes served as the experimental group, and the healthy eyes as the control group. The least signal visual angle (LSVA), and amplitude and latency of P(100) were chosen as test indexes. The results under different experimental conditions were recorded by PRVEP technology. All data collected were processed and analyzed by SPSS software. The results showed that the coincidence between subjective and VEP visual acuity was 96.7% in control group, but there was very significant difference in experimental group. It was concluded that with the regression formulation for the amplitude of P(100) and vision under LSVA, visual acuity can be estimated more accurately and impartially.
5.Effect of rehabilitation on ADL scores in patients with type 2 diabetes complicated with cerebral stroke

Objective To observe the effect of rehabilitation on ADL scores in patients with type 2 diabetes complicated with cerebral stroke.Methods According to the selected order,three hundred and twenty type 2 diabetes patients complicated with cerebral stroke were numbered in Arabic numbers 1,2,3 and so on,the odd number patients were randomly enrolled into the observation group,the even number patients were assigned into the control group.The control group was only given corresponding clinical treatments.In addition to the same clinical treatments,rehabilitation training was given to the observation group at the same time.The ADL scores,change between the two groups were compared after three months.Results After treatment,the ADL score of the observation group was significantly improved,the difference was statistically significant (t =-24.9,P =0.000,P < 0.05).The ADL score of the control group was also improved,and the difference was statistically significant (t =-15.52,P =0.000,P < 0.05).The ADL score between the observation group and the control group had statistically significant difference (t =24.24,P =0.000,P < 0.05).Conclusion Type 2 diabetes patients complicated with cerebral stroke who are early given rehabilitation training actively,car significantly improve the function of hemiplegia limbs,improve the ADL scores and the quality of life.
6.Amyloid-? protein deposition in area of axon degeneration in Goll’s column of mouse

Objective To investigate the relationship between axon degeneration of Goll’s column system and Amyloid ? protein (A?) deposit sludge. Methods A? protein deposition in area of axon degeneration of Goll’s columns was measured by using immunohistochemical method.Results There was found strong positive round like A? deposit in the area of axon degeneration of Goll’s column system of GAD mouse. Spheroid appeared in the area of medullar nucleus of the Goll's column system at an age of 4 weeks. However the A? was negative and 9 weeks later, it became positive in partial spheroid and formed a strong round like material and became more and more standing up with age increase. The material mentioned above appeared consistently progressive in degeneration of axon in sequence.Conclusion It was suggested that the deposition of A? should be the result secondary to degeneration of nervous axon and be closely correlated with axon degeneration.
7.STUDIES ON EFFECTS OF ANISODAMINE TO INTRA-AND EXTRACELLULAR Ca~(2+) IN ISOLATED RABBIT AORTA AND GUINEA PIG ATRIA

In the isolated rabbit aorta, like verapamil ( Ver ) anisodamine ( Ani ) might inhibit the contraction induced by phenylephrine ( PE ) and high K+ with IC50 value of 120 and 86 ? mol/L, respectively. PE/K+ ratio of IC50 was 1.4 which was less than that of Ver. It showed that although PDC may be more sensitive to Ani than ROC, the difference in sensitivity of Ani on 2 calcium chan- nel are less marked than that of some calcium antagonists. Ani had inhibitory effects on 2 contractile components of serotonin (5-HT ) . Ani and Ver could obviously inhibit post-resting potentiatipn and staircase phenomenon in guinea pig atria. Ani also might inhibit the contraction induced by calcium and shift concentration-response nonparallelly to right on both tissues above.The results suggest that Ani possesses nonspecific calcium antagonistic effect and reduces the tension by affecting PDC and ROC to decrease extracellular calcium influx and inhibiting intracellular calcium release.

9.Clinical efficacy of adjuvant therapy with thymosin alpha-1 and effect to celluar immune function and T-like receptor in patients with sIAI

Objective:To study the clinical efficacy of adjuvant therapy with thymosin alpha -1 in patients with severe peritoneal cavity infection(sIAI),and to explore its effect to celluar immune function and T-like receptor.Methods: A total of 60 patients with sIAI,who received treatment in the intensive care unit of our hospital in January 2012-December 2014,were divided into observation group and control group.Cases in control croup received routine treatment ,and cases in observation group received routine treatment besides thymosin alpha-1.T-lymphocyte subsets were determined by flow cytometry and expression of Toll-like receptor (TLR)2 and TLR4 in the peripheral blood mononuclear cells were determined by real time PCR before treatment and in the 2nd week after treatment.The scores of acute physiology and chronic health evaluation ( APACHE)Ⅱscore and gastrointestinal function were recorded before treatment and in the 2nd week after treatment.The rate of systemic inflammatory response syndrome (MODS),the rate of death in hospital stays,intestinal function recovery and hospital stays in both groups were compared .Results: (1) CD3+,CD4+and CD4+/CD8+of cases in observation group significantly increased ( P<0.01 ) , and CD8+significantly decreased ( P<0.01 ) compared with those in control group.(2) The expression levels of TLR2 and TLR4 mRNA in PBMCs of cases in observation group significantly decreased compared with those in control croup (P<0.01).(3)The scores of APACHEⅡ and gastrointestinal function in observation group significantly decreased compared with those in control group ( P<0.01 ) .( 4 ) The recovery of intestinal function , vanishment of SIRS,hospital stays and the rate of MODS in observation group significantly decreased compared with those in control group (P<0.01), the rate of death in hospital stays had the trend of reduction with no statistic significance (P>0.05).Conclusion:Adjuvant therapy with thymosin alpha-1 in patients with sIAI has good clinical efficacy ,and can improve the cellular immune function ,decrease the expression of Toll-like receptor.
10.Enrichment and identification of lung adenocarcinoma initiating cells from A 549

Objective To obtain the lung adenocarcinoma initiating cells from the A 549 cell line based on paclitaxel treatment combination with serum-free cultivation and to validate spared cells can represent tumor initiating cells (TICs) .Methods After dis-sociated by trypsogen ,about 106 /mL cells were suspended in serum-free medium supplemented with 0 .4% bovine serum albumin (BSA) ,insulin ,basic fibroblast growth factor (bFGF) ,human recombinant epidermal growth factor (EGF) and obtained spheroid cells .At the second passage ,paclitaxel was added at a concentration of 100 nmol/L for 48 h and then replaced with completely fresh medium once or twice per week until new spheroids emerged .Results The subpopulation of cells that survived serum-free cultiva-tion and paclitaxel treatment could highly express the cluster of differentiation 133/cluster of differentiation (CD133/CD326) mo-lecular markers and have features of stemness including differentiation ,high expression of cancer stem cells (CSCs)-associated genes and stronger capability of tumorigenesis .Conclusion The survived subpopulation that highly express the CD 133/CD326 molecu-lar markers presenting the characteristics of stemness in vitro and in vivo ,and could be used in future researches of biological functions .
