1.Advances in detection technology for HBV phenotypic resistance

Long-term use of nucleoside ( nucleotide ) analogues for anti-HBV treatment care easily lead to drug resistance. As compared to genotypic resistance detection, the detection technology of phenotypic resistance is a method used to analysis the sensitivity of antiviral drugs in vitro cell model, the result can directly reflect the relationship between drug-resistance mutations and drug susceptibility, thus perform a better prediction of clinical resistance.Among those technologies, some analysis strategies like transient transfection assay, stable cell lines and enzymatic assay, have been developed for early diagnoses as well as dynamic monitoring of drug resistance, and contribute greatly to proper selection of therapeutic regimen, reduction of resistance rate and implementation of individual treatment.

4.Current situation and development of community health management of hypertension

Hypertension is a common chronic disease which can lead to cardiac, cerebrovascular and renal damage; it is seriously harmful to human health. Cardiovascular disease caused by hypertension has become the first cause of death of residents in our country, the burden of disease has become more and more serious, how to manage hypertension has become an important public health issue in our country. In this review, the development and current situation of the community health management of hypertension at home and abroad are described and analyzed, the experience and practice worthy of learning are summarized to provide evidence for the prevention and control of hypertension in community.
5.Effects of guava polysaccharides on blood glucose level and antioxidant activity in diabetic mice

AIM: To study the effects of guava polysaccharides on blood glucose level and antioxidant activity in alloxan-induced diabetic mice. METHODS: The animal model of diabetes was established by injecting alloxan into mice abdominal cavity.The mice were divided into five groups: normal control group,diabetic model group,guava polysaccharides group I,guava polysaccharides group II,glybenclamide group.Blood glucose level was determined with blood glucose monitor 3 d and 10 d after being given polysaccharides.The mice were anaesthetized and killed on the tenth day.Activity of SOD and concentration of MDA in blood serum and liver were determined. RESULTS: Compared with diabetic model group,the blood glucose level of guava polysaccharides group I and guava polysaccharides group II significantly decreased.Activity of T-SOD in blood serum and liver significantly increased,while concentration of MDA in blood serum and liver significantly decreased. CONCLUSION: Guava polysaccharides can significantly decrease blood glucose level and improve the antioxidant activity of diabetic mice.lt is a potential hypoglycemic agent.
6.Repair of trauma soft tissue defects of leg by microsuegery

Objectve To probe the effect of treating the trauma soft tissue defects of the leg by microsurgical technique. Methods Ninety-nine cases of the trauma soft tissue defects of leg were treated by used 12 kinds of microsurgical flaps.Result All cases with soft tissue defects of the leg were repaired successfully.Follow-up 6 months to 10 years,the results were satisfactory and function improvement in 99 cases. Conclusions Accoding to soft tissue defects of size,the section and the technique of the operator to select the different microsurgical flap could be suitable and resolvable problem of soft tissue defects.
7.Inhibition of Respiratory Syncytial Virus by Wuhutang, A Traditional Chinese Prescription Composed of Five Medicinal Herbs

Wuhutang, a well known traditional Chinese. herbal decoction for the treatment of as thma was found to havc an antiviral effect on respiratory syncytial virus (RSV ) both invitro and in vivo. A 50% reduction of plaque. formation was observed op Hep-2 cell monolayer at a concentration of 1:80. At such concentration, the. value of lg TC ID50 of. Viralyield was 2, 1, and 55% RNA synthesis of RSV was inhibited. It was found that among thefive medicinal herbs in prescription Ephedra sinica Stapf is responsible for the antiviralactivity on RSV. IC50 for plaque inhibition was found to be 1.6mg/ml. In animal model,Wuhutang singn ficantly reduced the titre of RSV and accelerated RSV clearance in mice,and alleviated viral pneumonitis by either oral or aerosol route.
8.Expression of Platelet-derived growth factor and Collagen type Ⅰ in hepatic tissue in oral drinking arsenic exposed mice

Objective To study the expression and its significance of Platelet-derived growth factor(PDGF) and Collagen type Ⅰ (Col- Ⅰ) mRNA in hepatic tissue in mice exposed to arsenic in drinking water. Methods Fifty mice were divided into control group, sodium arsenite group(iAs3+ group, 300 mg/L) and sodium arsenate group (iAs5+ group, 300mg/L) at random. The mice were sacrificed after 10 months for liver function test (ALT, AST and GLB examined by automatic biochemical analyzer) and pathologic examinations. Masson staining and immunohistochemical SABC methods were used. Through the result of Masson staining fibrosis semi-quantitative analysis of the fibrosis number was carried out in the per unit area. Using immuonhistochemical SABC methods, the expression of PDGF and Col- Ⅰ was detected. Results ①Serum ALT was statistically significant among control group [(36.67±3.51) U/L], iAs3+ group[(61.46±13.85) U/L] and iAs5+ group[(43.31±4.21) U/L, F= 6.56, P < 0.05], especially between the control and iAs3+ group (P < 0.05) ;Serum AST had a statistical significance among control group [(135.00 ± 20.42)U/L], iA3+ group [(510.86 ± 59.01)U/L] and iAs5+ group[(258.93 ± 22.40) U/L, F= 83.36, P < 0.05] ;Serum GLB had no statistical significance among control group [(20.86 ± 0.61)g/L], iAs3+ group [(26.94± 3.73)g/L] and iAs5+ group[(24.59 ± 5.27)g/L, F = 2.80, P < 0.05]. ②Masson staining revealed notable liver cell necrosis, regeneration, infiltration of inflammatory cells in portal area. Masson staining Semi-quantitative results showed significantly increasing size of fibrosis in model group. Compared with control group(0.069 ± 0.013) and iAs5+ group(0.143 ± 0.122), fiber hyperplasia of iAs3+ group(0.192 ± 0.108) had statistical significance (F = 1.91, P < 0.05). ③Immunohistochemistry results showed higher expression of PDGF in the portal area, sepetal and preisinus cells in model group. The heavy expression of Col- Ⅰ distributed in the surrounding of blood vessels and bile duet and along portal wall extensting to the liver in model group. Semi-quantitative results showed that PDGF among control group(202.79 ± 7.46), iAs3+ group(174.38 ± 7.71) and iAs5+ group(177.64 ± 7.81) had a statistical significance(F= 102.91, P < 0.05);Col-Ⅰ had a statistical significance among control group(200.11 ± 7.46), iAs3+ group (176.47±10.20) and iAs5+ group (177.378 ±7.948, F = 78.51, P < 0.05). Conclusions Exposed in oral drinking arsenic, mice can develop hepatic injury and hepatic fibrosis. The PDGF and Col-Ⅰ play significant effects on the process of hepatic fibrosis caused by oral drinking arsenic exposed mice.
9.The value of quantitative PCR detection in early skin tissue fluid of syphilis

Objective To study the value of fluorescence quantitative polymerase chain reaction (FQ-PCR) detection in early skin tissue fluid of syphilis.Methods A total of 40 patients of suspected syphilis who received therapy from September 2014 to September 2016 in our hospital were selected in this study.Five mL venous blood samples were collected in all the patients,and detected by toluidine red unheated serum test (TRUST) and treponema pallidum antibody enzyme-linked immunosorbent assay (ELISA),and skin tissue fluid were collected and performed FQ-PCR detection,all patients were treated with benzylpenicillin for 3 and 6 months,then detected again,the conversion rates were record.Results There was no significant difference in the detection rate of ELISA and FQ-PCR[97.50%(39/40) vs.95.00%(38/40),P>0.05].The total detection rate of ELISA and FQ-PCR were significantly higher than that of TRUST[97.50%(39/40) vs.67.50%(27/40),95.00%(38/40) vs.67.50%(27/40),P<0.05].After 3 and 6 months of treatment,negative rates of ELISA and FQ-PCR were significantly higher than that of TRUST(P<0.05),there was no significant difference on negative rate between FQ-PCR and ELISA (P>0.05).In the FQ-PCR detection results,the average value of TP-DNA was significantly decreased after treatment,there were significant differences in the phase Ⅰ,phase Ⅱ compared with before treatment(P<0.05).Conclusion FQ-PCR could be used to measure treponema pallidum (TP-DNA) effectively in early stage,it′s conducive to the diagnosis of syphilis,the clinical application value is high.
10.Interferon α plus adefovir dipivoxil therapy for patients with HBeAg positive chronic hepatitis B nonresponding to interferon monotherapy

Objective To evaluate the efficacy and safety of interferon alpha (IFNα) combined with adefovir dipivoxil (ADV) for patients with chronic hepatitis B (CHB) non-responding to 24-week IFNα monotherapy.Methods Sixty CHB patients admitted to the First Affiliated Hospital of Xiamen University during 2009 and 2012 were enrolled in the study.All patients recieved IFNα monotherapy for 24 weeks and had no response.The patients were randomly divided into 3 groups by number table with 20 cases in each group.The experimental group was treated with IFNα combined with ADV,the control group 1 continued IFNα monotherapy,and the control group 2 shifted to ADV monotherapy.Virological,serological and biochemical responses were compared,and adverse reactions were observed.SPSS 19.0 software was used for data analysis.Results After 24 weeks of treatment,there was no statistical difference in HBV DNA loads,ALT levels and titers of HBeAg and HBsAg among three groups (F =0.985,0.717,0.985 and 0.717,P > 0.05).And no HBeAg seroconversion was observed.After 48 weeks of treatment,the experimental group had higher HBV DNA negative conversion rate,ALT normalization rate and HBeAg conversion rate than control group 1 (x2 =10.00,3.956 and 4.800,P < 0.05),but no statistically significant difference was found in HBeAg negative conversion rate (x2 =0.693,P > 0.05).There was no significant difference in HBV DNA and HBeAg negative conversion rates between experimental group and the control group 2 (x2 =1.026,1.905 and 0.156,P >0.05),but the HBeAg conversion rate in experimental group was significantly higher than that in control group 2 (x2 =4.800,P < 0.05).No HBsAg negative or serological conversion was observed,and there was no significant difference in titers of HBsAg among three groups (F =1.935,P > 0.05).No adverse reaction was observed.Conclusion For patients nonresponding to IFNα monotherapy,combination of IFNα and ADV can achieve higher ALT normalization rate,HBeAg conversion rate and HBV DNA negative conversion rate,and improve the overall efficacy of CHB antiviral therapy.
