Atlanto-axial complex is the most important functional unit of the cervical spine.Atlanto-axial instability or dislocation,one of the most serious pathologies in spinal surgery,is often managed by posterior atlantoaxial fixation and fusion.Several posterior fixation techniques have been used to stabilize the atlanto-axial complex,including wire/cable-bone fixation,Halifax clamps,transarticular screw fixation,screw-plate fixation and screw-rod fixation.For atlanto-axial dislocation that cannot be reduced with skull traction before operation,intraoperative reduction is often needed.Current intraoperative reduction techniques can be classified into transoropharyngeal surgical release followed by posterior reduction and fixation,transoropharyngeal reduction and fixation,and posterior reduction and fixation using screw-rod system according to approaches.Furthermore,posterior reduction can be performed using elevating-pull,leverages,and cantilever techniques.This study reviews the intraoperative reduction and fixation techniques mentioned above.

OBJECTIVE:To establish an LC-MS method for the determination of the ritodrine content in human plasma.METHODS:The determination was performed on Agilent Eclipse Plus C18 column with the column temperature kept at 40 ℃.The mobile phase consisted of methanol-water(95∶5,containing 0.02 mol?L-1 ammonium formate)with a flow rate of 0.5 mL?min-1.The extraction agent was acetic ether-dichloromethane(4∶1).A triple quad mass spectrometer system equipped with an electro spray ionization ion-trap source was used as the detector and operated in the positive ion model.Selected reaction monitoring(SRM)was employed to detect ritodrine(m/z 288.2→121.1)and the internal standard dobutamine(m/z 302.1→107.0).RESULTS:The linear range of ritodrine was 0.25～100 ng?mL-1(r=0.999 1),with the lower limit of quantification at 0.25 ng?mL-1.Both the intra-day RSD and inter-day RSD were less than 15%.The methodological recovery was 98.00%～103.44%.CONCLUSION:The method is sensitive,accurate,reproducible,specific and suitable for the pharmaceutical study of ritodrine.

The C3 cleavages of human blood kept at different temperatures in vitro were studied. In 14 autopsy cases, the postmortem intervals(PMI) were estimated based on the C3 cleavage rates and the rectal temperatures. The 95% confidence of predicting PMI was?11 hours. The phenotyping of C3 could be achieved while the C3 cleavage rate was measured. It is suggested that it is useful for the personal identification of unknown corpses.

Objective: To develop a novel method using high-performance capillary electrophoresis coupled with DAD (HPCE-DAD) for the simultaneous determination of 10 phenolic compounds (luteolin-7-O-glucoside, rutin, quercetin-3-glucoside, quercetin-3- rhamnoside, salicylic acid, luteolin, kaemferol, quercetin, gallic acid, and protocatechuic acid) in Bidens bipinnata. The extraction efficiency of above mentioned compounds was compared using six different ethanol/water solutions. Methods: The effects of pH value and concentration of buffer, applied voltage, and temperature on the separation were researched. The 10 compounds were baseline separated within 16 min in a 60 cm length capillary at a separation voltage of 25 kV with a running buffer consisting of 25 mmol/L borate (pH 9.5) at wavelength of 214 nm. Results: Excellent linearities of luteolin-7-O-glucoside, rutin, quercetin-3-glucoside, quercetin-3-rhamnoside, salicylic acid, luteolin, kaemferol, quercetin, gallic acid, and protocatechuic acid were observed at 5.0-120, 5.0-120, 5.0-120, 5.0-120, 1.0-120, 2.5-120, 5.0-120, 2.5-120, 2.5-120, and 2.5-120 mg/L, respectively. The detection limits were at 2.5, 2.5, 2.5, 2.5, 0.5, 1.0, 2.5, 1.0, 1.0, and 1.0 mg/L, respectively. The relative standard deviations (RSD) of precision were below 5.17% (n = 6). The mean recoveries for 10 phenolic compounds in B. bipinnata ranged from 94.4% to 105.8%, with RSD values of 0.32%-4.33%. Conclusion: The contents of luteolin-7-O-glucoside, rutin, quercetin-3- glucoside, quercetin-3-rhamnoside, salicylic acid, luteolin, kaemferol, quercetin, gallic acid, and protocatechuic acid in B. bipinnata are 95.04-105.84, 78.03-110.45, 96.45-177.96, 178.78-300.00, 62.06-66.54, 71.08-72.85, 77.39-95.30, 68.27-77.43, 68.47- 88.47, and 107.24-142.43 μg/g, respectively.

Objective To develop an auxiliary recovering device for prone position nursing after the retina vitrectomy in order to improve comfort and treatment compliance.Methods The device was made of stainless steel,and consisted of a base,pulleys,supporting rods and a placing case.Totally 40 patients receiving retinal detachment operation were divided into an experimental group and a control group.The patients in the experimental group applied the auxiliary device and the ones in the control group underwent conventional nursing,and then a 2-week observation was executed on the prone time,overall satisfaction and adverse response after the operation.Results The device behaved well in prone time,patient comfort and satisfaction,and the experimental group gained advantages over the control group in prone time,relieving muscle pains,arthralgia,poor breath,anxiety and insomnia.Chi-square test proved the experimental group had the patient satisfaction significantly enhanced when compared with the control group (P＜0.05).Conclusion The device can be used for auxiliary nursing after retinal detachment operation with simple structure,easy operation and high comfort,and thus is worthy promoting practically.

Objective To investigate the changes of vitamin D level and its relationship with bone alkaline phosphatase(BAP)and parathyroid hormone(PTH)in infants fed with breast milk and vitamin D supplementation for 0-8 months in People`s Hospital of Longhua New District in Shenzhen city.MethodsTotally 67 newborns in the Department of Obstetrics and Gynecology of People`s Hospital of Longhua New District were selected.These newborns were breastfed and began to supplement vitamin D 400 U/d in the 15th day after birth.The serum 25-(OH)D3 and BAP and PTH levels were measured in newborns and 4-/8-month-old infants,and the correlations of serum 25-(OH)D3 with BAP and PTH were analyzed by Pearson correlation analysis.ResultsCompared with the newborns,the serum 25-(OH)D3 [(78.47±36.41)nmol/L,(75.26±34.73)nmol/L],BAP[(231.43±78.81)μg/L,(205.97±71.64)μg/L],PTH[(31.76±11.54)pg/ml,and(32.94±13.82)pg/ml] levels of 4-and 8-month-old infants were significantly higher,and the difference were statistically significant(t4 months=7.823,12.568,11.852,t8 months=7.364,9.473,11.947;all P=0.000).There was a negative correlation between serum 25-(OH)D3 and BAP in newborns and 4-/8-monthold infants(r=-0.372,-0.463,-0.309;P=0.008,0.016,0.021),the serum BAP was positively correlated with PTH in infants aged 4 months and 8 months(r=0.487,0.338;P=0.005,0.032),and the serum 25-(OH)D3 was negatively correlated with PTH in infants aged 4 months(r=-0.297,P=0.041).Conclusions Infants at birth are prone to vitamin D deficiency in Shenzhen City.After 4 months of breastfeeding and vitamin D400U/d supplementation,the vitamin D levels in most infants become normal.25-(OH)D3 is correlated with BAP and PTH in different months.

With the development of drug discovery, more and more candidate compounds need to be studied. Methods that can screen compound rapidly received significant attention. Parallel artificial membrane permeability assay (PAMPA) as a powerful tool has been applied to drug studies. It uses an artificial lipid membrane to mimic the barrier for drug permeability studies. This article introduces the establishment and characteristics of PAMPA, as well as its applications in screening compounds. It can be used as models (e.g. predicting the ability of compound in gastro-intestinal absorption, blood-brain barrier transportation and skin penetration) by changing the component of artificial lipid membrane. PAMPA has advantages in high throughput selection of valuable compound with low cost, versatile, low dose, and good reproducibility.

OBJECTIVE:To provide new ideas for the supervision of TCM decoction for patients. METHODS:The mode of outsourced TCM decoction for patients in Nanjing and Hangzhou area were investigated and analyzed,especially their operation mode. The safety problems of quality of the mode were analyzed from the legitimacy and standardization. RESULTS & CONCLU-SIONS:The present problems include the mismatch of professional and technical personnel of TCM decoction pieces and their qua-lification requirements;the lack of communication of pharmacists and prescribers;no restrictions for outsourced decoction of toxic TCM decoction pieces for patients;the insufficient attention for the quality and safety of packaging materials;being difficult to im-plement the main responsibility of medical institutions,etc. It is suggested to establish the filing system of centralized TCM decoc-tion institution for patients;the stationed system of pharmacy technician in medical institutions;the training and evaluation system of dispensing,decocting and other stations;the regular inspection system of health and pharmaceutical administration department;the quality evaluation system of TCM decoction for patients by following up medical staff and patients. So that it can explore a new idea for monitoring ofclearing the powers of regulatory,full participation in medical institutions,standardization and management of pharmaceutical wholesale enterprises,active supervision by patients.
