2.Gelam Honey: A Review of Its Antioxidant, Anti-inflammatory, Anticancer and Wound Healing Aspects

In recent years, there is a growing enthusiasm in honey which has potential health benefits, disease prevention and serves as substitution of modern medicine. Honey is a natural sweet product which is produced by honey bees from floral nectar and has been traditionally used to treat several diseases such as cardiovascular disease, diabetes mellitus, cancer and Alzheimer’s disease since ancient times. Gelam honey (GH) is one of the Malaysian wild monofloral honey which is widely used as traditional medicine by the local population. GH is enriched with several therapeutic properties namely, antioxidative, antiinflammatory, anticancer and accelerated wound healing activities. However, there is lack of literature focusing on the overall effects of GH and its medicinal value. This review paper summarises the phytochemistry characteristics of GH along with its therapeutic values which lead to a broad spectrum idea on several diseases. A concise clarification on the therapeutic properties of GH is expected to provide new knowledge to thescientific society in these modern globalization eras.

3.The effect of individualised Glycemic intervention on Wound Healing Rate in Diabetic Foot Ulcer (The EIGIFU Study)

Objective:
To evaluate the association of glycated haemoglobin (HbA1c) reduction and wound healing in patients with diabetic foot ulcer (DFU).
Methodology:
A 12-week prospective, non-controlled, interventional study in suboptimal-controlled T2DM patients with DFU was conducted. Antidiabetic medications were adjusted with the aim of at least 1% in relation to patient’s individualised HbA1c target. The wound area was determined by using specific wound tracing. The daily wound area healing rate in cm2 per day was calculated as the difference between wound area at first visit and the subsequent visit divided by the number of days between the two visits.
Results:
19 patients were included in the study. There was a significant HbA1c reduction from 10.33 %+1.83% to 6.89%+1.4% (p<0.001) with no severe hypoglycaemia. The median daily wound area healing rate was 0.234 (0.025,0.453) cm2/day. There was a strong positive correlation between these two variables (r=0.752,p=0.01). After dividing the patients into four quartiles based on final HbA1c and comparing the first quartile vs fourth quartile, there was a significant difference in daily wound area healing rates (0.597 vs 0.044 cm2/day,p=0.012).
Conclusion
There was a positive correlation between HbA1c reduction and wound healing rate in patients with DFU. Although this is an association study, the study postulated the benefits of achieving lower HbA1c on wound healing rate in DFU which require evidence from future randomised controlled studies.

7.Design of a Special Shaped Foam Dressing Based on Anatomy.

As the dressings currently used in clinic settings unflat shape in general, they can't be fitted completely on occiput, heel, elbow, knee and other body parts unflat. This paper developed one kind of foam dressing of special shape based on local anatomy. The foam dressing is waterproof and air permeable, it can cover the wound closely enough to prevent bacteria from invasion and infection. With a saturated absorption ratio of 1: 8 or higher, it can keep the wound clean and moisture by absorbing large amounts of wound inflammatory secretions and is almost completely permeable to oxygen and carbon dioxide. Assuring safety and effect meanwhile, it has better outcomes than common dressings in the same application settings.

9.Research advances of real world study in the field of chronic wound.

In recent years, as a powerful supplement to the randomized controlled trial (RCT), real world study (RWS) has received more and more attention. However, in the field of chronic wounds, most of the patients have complex condition, and the strict inclusion and exclusion criteria of RCT were lack of practical application values. The RWS provides data closer to the real medical environment in clinical medical practice, drug and medical device supervision, and health technology evaluation. However, RWS has some problems that need to be resolved, such as inconsistent diagnostic criteria and unclear research endpoints. In addition, RWS in chronic wound research in China seems to have not really started yet, and institutions at all levels need to work together to promote the development of RWS.

10.Antimicrobial and wound healing activity of Ixora coccinea leaf extract in hydrogel formulation.

Objectives. The purpose of this study was to evaluate the antimicrobial and wound healing property of the preformulated hydrogel containing the methanolic leaf extract of Ixora coccinea, as well as its acute dermal irritation using New Zealand rabbits.
Methods. Mature dried leaves of I. coccinea was subjected to extraction using maceration and was concentrated in vacuo. Sodium carboxymethylcellulose and gelatin were used to create hydrogel in which the crude extract was incorporated. Physicochemical properties of the extract and preformulated hydrogel were characterized, while its antimicrobial activities against Pseudomonas aeruginosa, Staphylococcus aureus, and Staphylococcus epidermidis were determined using the agar well method and compared to the standard drug 2% w/v mupirocin ointment. A wound excision model in rats was used to determine the wound healing property of the preformulated hydrogel against povidone-iodine ointment. Lastly, animal testing was performed following the OECD Guidelines and upon approval of the IACUC Committee.
Result. The preformulated hydrogel was effective against S. aureus (p-value ? 0.001) but resistant to P. aeruginosa and S. epidermidis. Furthermore, the wound contraction rate in groups treated with preformulated hydrogel (p-value = 0.006) is significantly higher than in groups treated with hydrogel base and povidone-iodine. Moreover, no dermal erythema and edema were observed with albino rabbits.
Conclusion. The preformulated hydrogel with I. coccinea methanolic leaf extract is non-irritating, effective against staphylococcal infections commonly found in wounds. Hence, it is a good substitute for povidone-iodine in wound treatment.
