1. Analysis of the diagnosis maneuver of lateral semicircular canal benign positional paroxysmal vertigo and observation of the otolith movement using physical engine

Objective To analysis the diagnosis maneuvers of lateral semicircular canal benign positional paroxysmal vertigo (BPPV) and to explore the diagnostic mechanism. Methods The three-dimensional BPPV labyrinth model was established based on the physical engine. The otolith in different positions of the lateral semicircular canal was set up. The otolith movement in different positions during the horizontal roll test was analyzed, and the induced nystagmus could be inferred. Results Horizontal roll test nystagmus manifestations were diverse, including bilateral ageotropic nystagmus, which could be judged as otolith in ampullary and crista, and weak side of nystagmus was the affected side; bilateral geotropic nystagmus could be judged as otolith in the long arm of the lateral semicircular canal, and strong side of nystagmuss was the affected side; Ageotropic nystagmus in one side and geotropic nystagmus in another side, considering otolith in ampullary of long arm. The horizontal roll test may restore otolith to utricle and reduce the sensitivity of the diagnostic tests. The result of 10 repetitions were consistent. Conclusion Sixty degree horizontal roll test effectively corrects the defects of 90 degree horizontal roll test. The analysis of diagnostic tests based on the physical engine is of great significance for us to understand the diagnostic mechanism of BPPV in the lateral semicircular canal and to improve and innovate diagnostic maneuver.
2.A comparative analysis of health promotion demand in practitioners between urban and rural private enterprises.

OBJECTIVETo assess the differences in the health promotion demand of practitioners between urban and rural private enterprises by a comparative analysis, and to probe into the more scientific and targeted health promotion measures.
METHODSStratified cluster random sampling and self-designed questionnaire were adopted to survey 852 practitioners in urban and rural private enterprises of a Chinese city.
RESULTSThere were significant differences in practitioners between the two sorts of enterprises in terms of age, length of service, educational level, and forms of employment (P < 0.05 or P < 0.01). The basic knowledge and skills of practitioners in rural private enterprises were worse than those in urban private enterprises(P < 0.05 or P < 0.01). Practitioners in rural private enterprises were significantly less inclined to gain basic health promotion knowledge through enterprise training and network(P < 0.01). The demand of practitioners for health examination and hazard notification was significantly lower in rural private enterprises than in urban private enterprises (P < 0.05).
CONCLUSIONFocused and targeted health promotion should be carried out based on different demand characteristics of practitioners in rural and urban private enterprises.

3.Otimization of auxiliary lines for proximal femoral intramedullary nail in the treatment of simple femoral intertrochanteric fractures.

OBJECTIVETo investigate the clinical outcomes of C-arm X-ray fluoroscopy before incision to make assistant lines and insert the guide wire of PFNA and PFNA blade.
METHODSFrom January 1st 2012 to January 1st 2015, 132 intertrochanteric fracture patients of type 31A1 and 31A2 according to AO-classification, were retrospectively analyzed. Among them, 62 patients(14 males and 48 females) aged from 52 to 95 years with a mean age of(69.58±8.55) years in traditional group were operated by traditional procedure, while 70 patients in the skin marking group included 15 males and 55 females aged from 61 to 88 years with a mean age of(71.94±7.64) years, on the basis of the traditional operation method, the assistant line of the body surface and the C-arm X-ray was increased, and the guide pin positioning of the proximal femoral nail and the spiral blade was guided by the auxiliary line in the operation. Operative time, frequency of C-arm fluoroscopy, Harris hip score of the third months after surgery and the complications in both groups were queried for statistical analysis.
RESULTSIn addition to skin making group 1 patients had superficial wound infection complications, all patients were stage I wound healing. All patients were followed up for 3 to 18 months with an average of (6.81±3.07) months. The operative time was significantly reduced in skin marking group (56.16±6.36) minutes compared to traditional group (59.06±9.19) minutes (>=0.035). And the frequency of C-arm fluoroscopy of skin marking group was(25.89±5.81) times which was also significantly reduced compared to traditional group(31.32±9.81) times (<0.001). There was no statistical difference in Harris hip score at 3 months after operation and the complication rate between the two groups(>0.05).
CONCLUSIONSIn this study, a simple and easy method of assistant line marking can shorten the operation time and reduce the number of times of operation.
4.Cervical single open door laminoplasty with lateral mass screw fixation for the treatment of cervical spondylotic myelopathy with cervical instability.

OBJECTIVETo evaluate the outcomes of single open door laminoplasty with lateral mass screw fixation in treating cervical spondylotic myelopathy (CSM) with cervical instability.
METHODSFrom March 2010 to October 2012, 25 patients with spondylotic myelopathy and cervical instability underwent single open door laminoplasty with lateral mass screw fixation. There were 18 males and 7 females, aged from 57 to 68 years with the mean of 57 years. Japanese Orthopaedic Association (JOA) scores were used to evaluate clinical effects before operation and final follow up. Radiographical measures were made in change of Cobb angle by sagittal plane from C₂ to C₇, and cervical range of motion.
RESULTSAll the patients were followed up from 18 to 36 months with an average of 25.6 months. Cerebrospinal fluid leakage occurred in 1 case, incision fat necrosis in 1 case, C₅ nerve root palsy in 4 cases. JOA scores was improved from preoperative 5.2±2.1 to 11.3±2.4 final follow up. Cobb angle was changed from preoperative (6.5±3.4)° to (13.2±4.9)° final follow up. Cervical range of motion was changed from preoperative (30.4±9.2)° to (26.5±8.4)° final follow up.
CONCLUSIONSAs an effective treatment to CSM with cervical instability, single open door laminoplasty with lateral mass screw fixation has the advantage of extensive application scope, safety and steady, but the incidence rate of complication must be reduced.
5.Pressure side and tension side comminution of femoral neck cortex are independent risk factors for aseptic necrosis after femoral neck fracture surgery.

OBJECTIVE:
To investigate the related factors of aseptic necrosis of femoral head after closed reduction and internal fixation of femoral neck fracture.
METHODS:
From January 2009 to January 2016, 236 patients with femoral neck fracture were treated with closed reduction and internal fixation with 3 hollow lag screws, including 111 males and 125 females, aged from 19 to 89 (50.17±12.88) years. According to the follow-up results, the correlation of aseptic necrosis of femoral head was analyzed. Univariate analysis of age, gender, injured side, body weight, injury mechanism, preoperative waiting time, Garden classification and whether there was comminution of femoral neck cortex was conducted to obtain the independent variables with significant difference. Then binary logistic regression analysis was conducted to explore the independent risk factors of avascular necrosis of femoral head.
RESULTS:
The average follow-up period of 236 cases was 4.58 years. There were significant differences in the range of injury (24.69% vs. 5.16%,
CONCLUSION
High energy injury, preoperative waiting time (>48 h) and comminution of femoral neck cortex were independent risk factors for aseptic necrosis of femoral head. In addition, cortical comminution on the pressure side and tension side of the femoral neck is a strong prognostic risk factor for aseptic necrosis of the femoral head, because it indicates a more serious and complex injury mechanism.

6.Effect of Shouwu Shudi Yin on dopaminegic neurons in MPTP induced Parkinson's disease mouse model.

In order to investigate the effect of Shouwu Shudi Yin on dopaminegic neurons in MPTP induced Parkinson's disease mouse model and the possible mechamism, the experimental mice were randomly divided into 4 groups: control, Shouwu Shudi Yin, MPTP and the treatment (MPTP+Shouwu Shudi Yin) groups. The number of tyrosine hydroxylase (TH) positive cells in the substantia nigra was measured by immunohistochemistry, and mRNA expression of TH and glutathione peroxidase (GPX) were detected by PCR. The results showed that the number of TH positive cells and mRNA expression of TH were significantly reduced in MPTP group compared with the control (P<0.05), and pretreated with Shouwu Shudi Yin didn't show protective effect. Compared to MPTP group, the mRNA expression of four subtypes of GPX were increased in various degrees in the treatment group pretreated with Shouwu Shudi Yin, although the difference was not statistically significant. These indicated that the preventive medication of Shouwu Shudi Yin don't have protective effect on the mice with Parkinson' s disease induced by MPTP, but it may enhance the antioxidant capacity through increasing the expression of GPX.
7.Risk factors analysis and intervention of blood-borne occupational exposure in medical staff.

Objective: To analyze the risk factors of blood-borne occupational exposure among medical staff and explore the relevant intervention measures. Methods: In June 2020, the data of blood-borne occupational exposure and related factors reported by medical staff in a grade Ⅲ, Grade A general hospital from 2011 to 2019 were analyzed by retrospective investigation. Results: Among 431 cases of blood-borne occupational exposure, 69.37% were nurses. It mainly occurred in medical staff with 0-4 years of service, accounting for 63.57%; The main place of occupational exposure was in the ward 47.56%; Sharp instrument injury was the main occupational exposure route 91.65%. Occupational exposure department was mainly surgery department 17.87%; The main source of exposure was hepatitis B virus (HBV) 37.12%, followed by treponema pallidum 20.19%. Statistical analysis results show that: Exposure sites (χ(2)=43.585, P<0.01) , exposure sources (χ(2)=22.693, P<0.01) , treatment methods after exposure (χ(2)=18.866, P<0.01) , Flushing (χ(2)=31.963, P<0.01) and disinfection (χ(2)=14.216, P<0.01) were significantly different. Conclusion: The effective measures to reduce blood-borne occupational exposure are to strengthen occupational protection training of medical staff, standardize operation procedures, strengthen supervision of key groups and departments, improve reporting, monitoring and follow-up systems to realize informatization, and do a good job in risk control.

8.Study of BPPV precise manipulation treatment

Objective:To explore the effect of the BPPV precision manipulation treatment. Method:With 3D SLICER software we segment structure such as semicircular canal, eye ball, orbital and ear rod from MRI or CT volume data of patients which underwent MRI or CT scaning of inner ear, for measuring the spatial direction of semicircular canals and building semicircular canal modules with standard space coordinate system and embedding into 3D PDF files. Result:With the slice that divide the semicircular canal equally as the semicircular canal plane and the eyeball as reference object to determine whether it is symmetric, it is not only intuitive but also reliable for measuring the angle between the posterior semicircular canal. 3D PDF is intuitive, rotation angle can be adjusted according to the individual differences in the process of manipulation treatment, to observe and demonstrate the theoretical reduction effect before the actual operation. Conclusion:By reconstruction the three-dimensional semicircular canal structure from the inner ear image data of patients and measurement of the semicircular canal space direction, it is of a certain significance for BPPV precision manipulation treatment.
9.Erythromycin effects on apoptosis of epithelial cell in nasal polyps.

OBJECTIVE:
To study the effect of erythromycin on apoptosis in epithelial cell and investigate the significance of epithelial cell apoptosis in nasal polyps forming.
METHOD:
Epithelial cell collected from thirty nasal polyps and six inferior turbinates were cultured in Dulbecco Eagle and Ham F12 (1:1) and divided into two groups, one cultured with Erythromycin(Erythromycin group), another cultured without Erythromycin (control group). Apoptosis was detected by terminal deoxynucleotidyl transferase mediated dUTP nick end labeling.
RESULT:
The AI (apoptosis index) of epithelial cell in nasal polyps after cultured for 1,3,5 days with erythromycin were respectively (33.23 +/- 6.50)%, (38.21 +/- 7.22)% and (52.63 +/- 7.86)%. The AI of epithelial cell in inferior turbinates were respectively (31.02 +/- 5.60)%, (32.13 +/- 7.15)% and (39.64 +/- 7.48)%. There were significant difference between two groups at 5 day after culture (P < 0.05).
CONCLUSION
Erythromycin promoted apoptosis of epithelial cell in nasal polyps.

10.Association between polymorphisms of HIF-1alpha, eNOS genes and individual variation in response to hypoxia training.

AIMTo investigated association between polymorphisms of HIF-1alpha, eNOS genes and individual variation in response to intermittent hypoxia training.
METHODS41 health subjects, completed 4 weeks hypoxia training program (hypoxia exposure 10 h each day and 3 times hypoxia training each week), to observed the relationship between C1772T polymorphism in exon 12 of HIF-1alpha gene and 27-base pair(bp) variable number of tandem repeat polymorphism (VNTR), 4b/a, in intron 4 of eNOS gene and physiological parameters, such as VO2(max), Hb, RBC and SpaO2.
RESULTSThe results showed that the change of VO2(max) was higher in carriers with CT genotype than those of CC genotype, and the changes of Hb and RBC indicated higher tendency in the subjects carrying the CT genotype. Same results were observed in the changes of SpaO2 during period of exercising under hypoxic condition.
CONCLUSIONThese findings suggested the possible associations between SNP/C1772T, VNTR/4ba and the individual variation in response to hypoxia training.
