1.Progress of research in community-acquired methicillin resistant Staphylococcus aureus infections in children

Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) is one of the most important pathogen and can cause infection ranging from superficial skin infections to life-threatening disease. The prevalence of CA-MRSA is rising in children in these years. Therefore, the review will cover the research progress in the CA-MRSA prevalence, risk factors, clinical manifestation, treatment and preventions in children. The risk factors and disease patterns and antimicrobial resistant rates in different parts of China in the recent decade will be also discussed.
2. Brief introduction of the pediatric residency and neonatal-perinatal medicine fellowship standardized training system in the United States

Medical education after graduation is the necessary step for each medical graduate to become an independent practitioner. The present medical education system or standardized residency training system in China is similar to that of the United States and Canada. This system is designed to ensure that all trained physicians are competent in practicing medicine in their trained medical specialties. The article briefly summarizes the current status of the pediatric residency and neonatal-perinatal medicine fellowship training system in the United States. The critical roles of the American Board of Pediatrics and the Accreditation Council for Graduate Medical Education are also presented.
3. History evolution on processing technology and modern research of Polygonatum cyrtonema in Jiuhua Mountain

Polygonatum cyrtonema, which is growing in Jiuhua Mountain, is genuine medicinal materials. The main processing methods of it are cutting, single-steamed, re-steamed, nine times steamed and nine times shined, wine-steamed, wine-stewed, copying and so on. Its effective components are polysaccharides and steroidal saponins, and they also have pharmacological activities such as anti-oxidation, regulating immunity, lowering blood sugar, antitumor, antibiosis, and antiviral. The development history and modern research progress of processing technology of P. cyrtonema in Jiuhua Mountain were summarized, which provides reference and basis for its processing research and lays the foundation for its standard operation rules of GMP processing.
4. Research progress on Polygonatum cyrtonema processed by nine times steaming and nine times shining

The nine times steaming and nine times shining processing method of Polygonatum cyrtonema was originated in ancient China and developed in modern times. The historical evolution of its processing technology includes raw-used, single-steamed, re-steamed, nine times steaming and nine times shining. It can eliminate toxicity, enhance curative effect, change meridians, facilitate storage and eliminate bacteria and so on. After nine times steaming and nine times shining, the chemical composition and drug efficacy of P. cyrtonema changed significantly, and it is widely used in medicine, health care products, common food, cosmetics and other fields. The historical evolution, processing mechanism, composition change, and clinical application of P. cyrtonema are reviewed in this paper, which provide a scientific basis for optimizing the nine times steaming and nine times shining technology of P. cyrtonema, lay the foundation for establishing the quality control standard control system, and provide a reference for the study of observing the ancient processing.
5.Effect of crucumin on vascular endothelial function in atherosclerotic rabbits.

OBJECTIVETo observe the effect of curcumin on nitric oxide (NO) in plasma of atherosclerotic rabbits, activity of constitutive nitric oxide synthase (cNOS) and asymmetric dimethylarginine (ADMA), and discuss curcumin's effect against AS and its correlation with ADMA.
METHODThirty-eight male Japanese white rabbits were randomly divided into four groups: the control group (eight rabbits fed with standard diets), the model group (ten rabbits fed with high-fat diets), the low dose curcumin group (ten rabbits fed with high-fat diets and 100 mg . kg-1 d -1 ) and the high dose curcumin group (ten rabbits fed with high-fat diets and 200 mg kg-1 d-1 curcumin). At the end of the 12th week, their plasmas were tested for TC, LDL-C, NO, endothelin (ET) , ADMA and activity of aortic cNOS. Aortic tissues were collected for histological examinations.
RESULTThe three groups fed with high-fat diets showed higher plasma ADMA and ET than the control group (P <0. 01) , but with decrease in plasma NO concentration and arterial cNOS activity (P <0. 01). Compared with the model group (P <0. 05) , the curcumin groups showed lower plasma ADMA and ET (P <0. 05), but higher plasma NO concentration and arterial cNOS activity than the model group (P <0. 01). There was no significant difference between the two curcumin groups.
CONCLUSIONCurcumin may play an important protective role in AS process by reducing plasma ADMA level. [Key words] atherosclerosis; asymmetric dimethylarginine; crucumin; nitric oxide; nitric oxide synthase

6.A new cinnamic acid ester derivative from Liquidambaris Resina.

Twelve compounds were isolated from Liquidambaris Resina by silica gel column chromatography and thin layer chromatography. Their structures were identified on the basis of spectral data, electron capture detector data, and physicochemical properties as(2'R, 3'R)-2',3'-dihydroxy-hydrocinnamyl-(E)-cinnamate(1),(E)-cinnamyl-(E)-cinnamate(2), cinnamic acid(3), 28-norlup-20(29)-en-3-one-17β-hydroperoxide(4), erythrodiol(5), 13β,28-epoxy-30-hydroxyolean-1-en-3-one(6),(3β)-olean-12-ene-3,23-diol(7), 2α,3α-dihydroxy-olean-12-en-28-oic acid(8), 28-hydroxyolean-12-en-3-one(9), 3-epi-oleanolic acid(10), 3-oxo-oleanolic acid(11), and hederagenin(12). Compound 1 was a new cinnamic acid ester derivative and compounds 2-4,6-8, and 12 were isolated from Liquidambaris Resina for the first time. Compounds 4, 5, 10, and 12 exerted inhibitory effects on the proliferation of human umbilical vein endothelial cells(HUVEC) with the IC_(50) values of(17.43±2.17),(35.32±0.61),(27.50±0.80), and(46.30±0.30) μmol·L~(-1), respectively.

7.Dynamic changes of reproductive hormone levels and related factors in old and middleaged men in health examination.

Objective:
To investigate the dynamic changes of serum reproductive hormone levels in old and middleaged males in health examination and their correlation with age and lipid profile.
METHODS:
This study included 4 333 men in health examination from January 2011 to December 2014. The men were aged from 40 to 85 years old and divided into seven fiveyearspan age groups. We determined the levels of serum testosterone (T), luteinizing hormone (LH), folliclestimulating hormone (FSH), estradiol (E2), progesterone (P), prolactin (PRL), total cholesterol (TC), triglyceride (TG), lowdensity lipoprotein cholesterol (LDLC), highdensity lipoprotein cholesterol (HDLC), and the testosterone secretion index (TSI = T/LH). We analyzed the obtained data using SPSS Pram, KruskalWallis H test, MannWhitney U test, exponential regression, and Spearman correlation analysis.
RESULTS:
Statistically significant differences were found in LH, FSH, E2 and TSI among the seven age groups (P< 0.05). The levels of serum LH, FSH and E2 were significantly higher (P< 0.05) while TSI remarkably lower (P< 0.05) in the ≥70 yr group than in the other six groups. The serum T and E2 levels and TSI were markedly lower in the 40-44, 45-49 and 50-54 yr groups in 2014 than in the other three years (P< 0.05), and so were the levels of serum T and TSI in the 55-59 yr group (P< 0.05). The levels of serum LH, FSH and E2 were correlated positively while those of P, PRL and TSI negatively with age. The serum T level was correlated positively with HDLC but negatively with TC, TG and LDLC. The levels of serum LH, FSH and E2 showed a yearly average increase of 1.9%, 2.7% and 0.5%, respectively, while TSI an annual mean decline of 2.0% in the 40-85 yr group.
CONCLUSIONS
LH, FSH and E2 were increased while TSI decreased with age in the >40 years old males. T and TSI were reduced in the 40-59 years old men from 2011 to 2014, and so was E2 in the 40-54 yr group. Lowlevel testosterone is closely related to dyslipidemia.

8.Polymorphism of STR loci D12S391/D18S865 in Wenzhou Han population.

OBJECTIVE:
To investigate the polymorphism and forensic efficiency values of STR loci D12S391/D18S865
METHODS:
Population studies on D12S391 and D18S865 were carried out in a sample of 454 unrelated Wenzhou Han individuals using PCR followed by PAGE and silver stain detection.
RESULTS:
11/7 alleles and 50/21 genotypes of D12S391/D18S865 were respectively observed among the 454 unrelated individuals. The genotype frequency matched the Hardy-Weinberg equilibrium, The heterozygotes (H) in D12S391/D18S865 were 0.7974/0.7379, Power of Exclusion were 0.9566/0.9077, polymorphism information content (PIC) were 0.8260/0.7279 respectively.
CONCLUSION
D12S391 and D18S865 were high polymorphic loci and the frequencies were in accordance with Hardy-Weinberg equilibrium (P>0.5), so the two loci can be used in forensic medicine and in other genetic research areas.

9.A study of genetic polymorphism of the STR locus D7S2846, D19S400 and D18S535 in Han population in Wenzhou.

OBJECTIVE:
To study genetic polymorphism of D7S2846, D19S400 and D18S535 in Han population in Wenzhou.
METHODS:
DNA was extracted with chelex-100 method from EDTA-blood samples of 194 unrelated individuals in Wenzhou and amplified with PCR technique. The PCR products were analyzed by PAG vertical electrophoresis and silver-stain.
RESULTS:
6 alleles and 15 genotypes of D7S2846, 10 alleles and 36 genotypes of D19S400, 8 alleles and 26 genotypes of D18S535 was observed. The heterozygosities of the three loci are 0.644, 0.724 and 0.772; The power of discriminating are 0.854, 0.940 and 0.938.
CONCLUSION
The heterozygosities of the three loci are high and the frequencies was in accordance with Hardy-Weinberg equilibrium (P>0.05), so the three loci can be used in forensic medicine and in other genetic researches.

10.Investigation and analysis of nosocomial infection in neonates.

OBJECTIVETo investigate the incidence of nosocomial infections of newborn infants in neonates and to explore the risk factors and strategies of infection control.
METHODSThere were 433 confirmed cases of nosocomial infection in the neonatal ward of the authors' hospital from January 2007 to December 2009. Their data of epidemiological and clinical characteristics, results of etiological examinations and antibiotic resistance were retrospectively analyzed.
RESULTSDuring the study, the number of hospitalizations were 6437. Nosocomial infection occurred in 433 patients 513 times. The overall nosocomial infection rate was 6.82%. The overall hospitalization days were 73 663 and nosocomial infection patient-day rates were 6.96‰. The VAP infection rate was 28.7‰. The CRBSI rate was 3.5‰. Gestational age (OR = 1.049), mechanical ventilation (OR = 1.810), umbilical vein catheter (OR = 1.106), hospitalization days (OR = 1.081), premature rupture of membrane (OR = 1.433) were the risk factors for the development of nosocomial infection. There were 197 (38.4%) cases of pneumonia, which was the most common nosocomial infection in Neonatal Ward. There were 129 cases of ventilator-associated pneumonia (VAP), which accounts for 65.5% of pneumonia and 24.4% of cases treated with ventilator. The next was sepsis, 124 cases (24.2%) and 64 cases of diarrheal disease (12.7%). One hundred and eighty two (54.4%) strains of isolates were Gram-negative bacteria, which accounted for the highest proportion. The predominant pathogens of Gram-negative bacteria were Klebsiella pneumoniae (19.6%), followed by Acinetobacter baumannii (8.1%), Pseudomonas aeruginosa (7.2%), Stenotrophomonas maltophilia (4.8%) and Escherichia coli (4.8%). The isolation rates of Klebsiella pneumoniae and Escherichia coli with positive extended-spectrum beta-lactamases (ESBLs) were 91.4% and 75%, respectively. Those two bacteria were universally resistant to cephalosporins. The rate of resistance to imipenem of Klebsiella pneumoniae, Acinetobacter baumannii and Pseudomonas aeruginosa were 1.5%, 11.1% and 41.7%. The isolation rates of methicillin-resistant Staphylococcus aureus and methicillin-resistant coagulase-negative Staphylococcus were 28.6% and 95.5%.
CONCLUSIONIt is important to identify the high risk factors for nosocomial infections in newborn infants. To shorten time for mechanical ventilation and hospitalization days, removal of the central venous catheter as early as possible would be conducive to reducing the morbidity of nosocomial infection. The main pathogens were Gram-negative bacteria. The multidrug resistance of Enterobacteriaceae and Non-fermenters is serious.
