1.Otimization of auxiliary lines for proximal femoral intramedullary nail in the treatment of simple femoral intertrochanteric fractures.

OBJECTIVETo investigate the clinical outcomes of C-arm X-ray fluoroscopy before incision to make assistant lines and insert the guide wire of PFNA and PFNA blade.
METHODSFrom January 1st 2012 to January 1st 2015, 132 intertrochanteric fracture patients of type 31A1 and 31A2 according to AO-classification, were retrospectively analyzed. Among them, 62 patients(14 males and 48 females) aged from 52 to 95 years with a mean age of(69.58±8.55) years in traditional group were operated by traditional procedure, while 70 patients in the skin marking group included 15 males and 55 females aged from 61 to 88 years with a mean age of(71.94±7.64) years, on the basis of the traditional operation method, the assistant line of the body surface and the C-arm X-ray was increased, and the guide pin positioning of the proximal femoral nail and the spiral blade was guided by the auxiliary line in the operation. Operative time, frequency of C-arm fluoroscopy, Harris hip score of the third months after surgery and the complications in both groups were queried for statistical analysis.
RESULTSIn addition to skin making group 1 patients had superficial wound infection complications, all patients were stage I wound healing. All patients were followed up for 3 to 18 months with an average of (6.81±3.07) months. The operative time was significantly reduced in skin marking group (56.16±6.36) minutes compared to traditional group (59.06±9.19) minutes (>=0.035). And the frequency of C-arm fluoroscopy of skin marking group was(25.89±5.81) times which was also significantly reduced compared to traditional group(31.32±9.81) times (<0.001). There was no statistical difference in Harris hip score at 3 months after operation and the complication rate between the two groups(>0.05).
CONCLUSIONSIn this study, a simple and easy method of assistant line marking can shorten the operation time and reduce the number of times of operation.
2.Changes of apoE protein expression in lung of mice with hypoxic pulmonary arterial hypertension.

OBJECTIVE:
To observe the changes of apolipoprotein E (apoE) protein expression of pulmonary tissue in mice with pulmonary hypertension induced by hypoxia.
METHODS:
The animal model of hypoxic pulmonary hypertension was established by exposing the mice to isobaric hypoxic chamber for 3 weeks (23 h/d, regular chow feed).Twenty male wild type (WT) C57BL/6 mice and twenty apoE gene knockout (apoE-KO) mice were randomly divided into normoxia group and hypoxia group. The plasma concentrations of low density lipoprotein (LDL), high density lipoprotein (HDL) and total cholesterol were detected by ELISA method. The protein expression of apoE in lung and liver, and peroxisome proliferators-activated receptor gamma (PPARγ) in lung were measured by Western blot.
RESULTS:
①In WT mice, the right ventricular systolic pressure (RVSP) and the weight ratio of right ventricle (RV) to left ventricle plus septum (LV+S) of hypoxia group were significantly higher than those of normoxia group by 68% and 59% (<0.05), respectively. The plasma concentration of HDL and HDL/LDL of hypoxia group were significantly lower than those of normoxia group by 17% and 40% (<0.05), respectively.The protein expression of apoE in lung and in liver of hypoxia group were significantly down-regulated than those of normoxia group by 48% and 52% (<0.05), respectively.The protein expression of PPARγ in lung was significantly down-regulated than that of normoxia group by 37%(<0.05).RVSP were significantly negative correlated with the protein levels of apoE and PPARγ in lung (<0.01).② In apoE-KO mice, RVSP and the weight ratio of RV to LV+S of hypoxia group were significantly higher than those of normoxia group by 96% and 86% (<0.05), respectively.RVSP and RV to (LV+S) of hypoxia group in apoE-KO mice were significantly higher than those of hypoxia group in WT mice by 29% and 24% (<0.05), respectively.
CONCLUSIONS
Down-regulated expression of apoE in lung tissue participates in the pathological proceeding of pulmonary hypertension induced by hypoxia.

3. Prospective application of virtual touch tissue imaging quantification in the diagnosis of initial polymyositis

Objective:
To explore the application value of virtual touch tissue imaging quantification(VTIQ) technology in initial polymyositis(PM).
Methods:
Sixteen PM patients collected from February 2016 to March 2019 in Wenzhou People′s Hospital were selected as PM group, and 33 healthy subjects as healthy control group. After gray and color Doppler ultrasound, the shear wave velocity(SWV) values of target muscle and control muscle in the same side of affected limb were measured by VTIQ technique. Maximum velocity(Vmax) and minimum velocity(Vmin) were measured, average velocity(Vmean) and standard deviation(SD) were calculated, and compared between the two groups. SWV difference and ratio between target muscle and control muscle were calculated, and creatine kinase(CK) was detected in the same day.
Results:
Part of PM patients had abnormal ultrasonographic manifestations of full muscle shape (31.25%), increased or decreased echo of muscle bundles (56.25%), blurred muscle texture (25.00%) and increased blood flow signals (37.50%). 4.Effect of Shouwu Shudi Yin on dopaminegic neurons in MPTP induced Parkinson's disease mouse model.

In order to investigate the effect of Shouwu Shudi Yin on dopaminegic neurons in MPTP induced Parkinson's disease mouse model and the possible mechamism, the experimental mice were randomly divided into 4 groups: control, Shouwu Shudi Yin, MPTP and the treatment (MPTP+Shouwu Shudi Yin) groups. The number of tyrosine hydroxylase (TH) positive cells in the substantia nigra was measured by immunohistochemistry, and mRNA expression of TH and glutathione peroxidase (GPX) were detected by PCR. The results showed that the number of TH positive cells and mRNA expression of TH were significantly reduced in MPTP group compared with the control (P<0.05), and pretreated with Shouwu Shudi Yin didn't show protective effect. Compared to MPTP group, the mRNA expression of four subtypes of GPX were increased in various degrees in the treatment group pretreated with Shouwu Shudi Yin, although the difference was not statistically significant. These indicated that the preventive medication of Shouwu Shudi Yin don't have protective effect on the mice with Parkinson' s disease induced by MPTP, but it may enhance the antioxidant capacity through increasing the expression of GPX.
6.Effects of Mirror Therapy on Motor Function of Lower Extremities and Walking in Subacute Stroke Patients

Objective To observe the effects of mirror therapy (MT) combined with functional electrical stimulation (FES) on motor of lower extremities, walking ability and activities of daily living for subacute stroke patients. Methods From July, 2016 to December, 2017, 38 subacute stroke patients were randomly divided into treatment group (n=20) and control group (n=18). All the patients received routine rehabilitation. The control group accepted FES, and the treatment group accepted FES and MT, for eight weeks. They were assessed with Fugl-Meyer Assessment-Lower Extremities (FMA-LE), Functional Ambulation Categories (FAC) and modified Barthel Index (MBI) before and after treatment. Results Both groups improved in the scores of FMA-LE and MBI, and grade of FAC after treatment (Z>3.002, t>7.985, P<0.01), and the scores of FMA-LE improved more in the treatment group than in the control group (Z=-2.037, P<0.05). There was no difference between two groups in the scores of MBI and grade of FAC (t=-1.044, Z=-1.287, P>0.05). Conclusion The addition of MT on FES may further improve the lower extremities motor function, but not enough to improve their walking and activities of daily living in subacute stroke patients.
7.Association between polymorphisms of HIF-1alpha, eNOS genes and individual variation in response to hypoxia training.

AIMTo investigated association between polymorphisms of HIF-1alpha, eNOS genes and individual variation in response to intermittent hypoxia training.
METHODS41 health subjects, completed 4 weeks hypoxia training program (hypoxia exposure 10 h each day and 3 times hypoxia training each week), to observed the relationship between C1772T polymorphism in exon 12 of HIF-1alpha gene and 27-base pair(bp) variable number of tandem repeat polymorphism (VNTR), 4b/a, in intron 4 of eNOS gene and physiological parameters, such as VO2(max), Hb, RBC and SpaO2.
RESULTSThe results showed that the change of VO2(max) was higher in carriers with CT genotype than those of CC genotype, and the changes of Hb and RBC indicated higher tendency in the subjects carrying the CT genotype. Same results were observed in the changes of SpaO2 during period of exercising under hypoxic condition.
CONCLUSIONThese findings suggested the possible associations between SNP/C1772T, VNTR/4ba and the individual variation in response to hypoxia training.

9.Clinical efficacy of Zisheng pills in treatment of adverse reactions in patients with primary hepatocellular carcinoma after transcatheter arterial chemoembolization

ObjectiveTo investigate the clinical efficacy of Zisheng pills in the treatment of adverse reactions in patients with primary hepatocellular carcinoma (HCC) after transcatheter arterial chemoembolization (TACE). MethodsThis study included 72 patients with liver cancer hospitalized in the Department of Liver Diseases and Department of Oncology, Wenzhou Hospital of Traditional Chinese Medicine, from June 2011 to May 2014. These cases were randomly divided into treatment group (n=37) and control group (n=35). All the patients were treated with TACE, as well as conventional symptomatic and supportive treatment. In addition, the treatment group was given Zisheng pills (one bag) three times a day for at least three consecutive months. Comparison of categorical data was made by chi-square test, while comparison of continuous data (expressed as mean±SD) was made by t test. Survival comparison was made by Kaplan-Meier method and log-rank test. ResultsAfter the first TACE, there were significant differences in poor appetite and abdominal distension between the treatment group and the control group (P<0.05); significant differences were also observed in CD3+ and CD4+ T lymphocytes between the two groups (P<0.05). After the second TACE, there were significant differences in postoperative fatigue, poor appetite, and abdominal distension, as well as postoperative Karnofsky score, between the treatment group and the control group (P<0.05); significant differences were also found in CD3+, CD4+, and CD8+ T lymphocytes and CD4+/CD8+ ratio between the two groups (P<0.05). The median survival time showed no significant differences between the treatment group and the control group (82 vs 74 weeks, P>0.05), but the 2-year overall survival rate was significantly higher in the treatment group than in the control group (43.2% vs 20%, P<0.05). ConclusionZisheng pills have good clinical efficacy in the treatment of adverse reactions in liver cancer patients after TACE, and it can improve the symptoms and quality of life, regulate peripheral blood T lymphocyte subsets, and increase the 2-year overall survival rate.
10. The progress of the anatomical study of perforator flaps

Based on literature review and comparative studies, the most useful anatomical method that were convenient for both medical researchers and surgeons were introduced. Practical examples were used to illustrate how to gather and rectify the information of the perforating vessels and to design a perforator flap. In addition, the survival mechanisms of cross-territory perforator flap and modalities that can be used to promote the flap survival were also reviewed. Previous research showed: ①The position, diameter, pedicle length, axiality, and vascular territory of a specific perforator were important parameters for observation. ②The traditional cadaveric study and the X-ray examination of post-perfusion cadaveric specimens could provide two-dimensional information about the perforating vessels. CT angiography could provide more accurate information of the perforators. Adjacent structures could be clearly revealed when the data of CT angiography was dealt with the MIMICS software. ③The survival of the cross-territory perforator flap could be enhanced by various different maneuvers. How to reconstruct the new blood circulation system in the multi perforator cross zone flap is a research hotspot and challenges in the study of anatomy. It is necessary to work closely with the surgeons.
