1.The study of tumor biomarkers associated with lung cancer in diagnosis

Tumor biomarkers are bioactive substance and antigen produced by lung cancer tissue and cell because of oncogene abnormal expression including protien, enzyme and hormone et al. In recent years, tumor biomarker has been one of the most active areas in basic research and clinical investigation of oncology. A variety of tumor biomarkers have been shown to have potential applications in accessory diagnosis, clinical typing, prognosis, and therapeutic response amd target therapy of lung cancer. In this review, we will discuss about a dozen of tumor biomarkers associated with lung cancer, including CEA, NSE, CYFRA21-1, PCNA, VEGF, EGFR and LTA.
2.Medial epicanthoplasty by using palpebral margin incision

Objective To introduce a new method for correction of epicanthal folds, which could avoid obvious scar in medial canthus. Methods First, through upper and lower palpebral margin incisions, the connective and orbicularis oculi muscle dense fibers underlying epicanthal folds as well as the superficial head of medial canthal ligament were thoroughly released and excised. After repositioning the medial canthus, the redundant skin tissue was removed and then the incisions closed. Results 122 epicanthoplasties had been performed. Postsurgical scars were invisible in all cases. All patients were satisfied with operation results. No recurrence of epicanthal fold was observed. Conclusions The introduced method has advantages in avoiding scar formation and is an especially suitable approach for epicanthoplasty in Asian patients.
3.Determination of Ethambutol Hydrochloride in Ethambutol Hydrochloride-Pyrazinamide-Rifampicin-Isoniazid TabletsⅡ

OBJECTIVE:To establish an HPLC method for the determination of the content of ethambutol hydrochloride in ethambutol hydrochloride-pyrazinamide-rifampicin-isoniazid tabletsⅡ.METHODS:The sample was separated on Alltima CN column.The mobile phase was acetonitrile-triethylamine(50:50)water solution at a flow rate of 1.0 mL?min~(-1).The diluent agent was Na_2HPO_4 solution.The detective wavelength was set at 200 nm;the column temperature was maintained at room temperature and the sample size was 20?L.RESULTS:The linear range of ethambutol hydrochloride was 100.2~601.2?g?mL~(-1)(r=0.999 98)and its average recovery was 99.8%(RSD=0.374%).CONCLUSIONS:The method is simple,accurate and reproducible,and it can be used for the content determination of ethambutol hydrochloride in this tablet.
4.Helicobacter pylori iceA and babA2 genotype and its clinical association in Shanghai area

Objective To determine the distribution of Helicobacter pylori (H.pylori) iceA, babA2 in patients in Shanghai and explore the association of H. pylori strain genotype with its clinical outcome after infection. Methods A total of 141 H. pylori strains was isolated from gastric biopsy samples of 43 patients with chronic gastritis, 47 patients with duodenal ulcer (DU), 30 patients with gastric ulcer(GU) and 21 patients with non cardia gastric carcinoma. The iceA, vacA, cagA, and babA2 genotypes were determined by polymerase chain reaction (PCR). Results iceA1, iceA2 and babA2 were detected in 74.5% (105/141) , 15.6% (22/141) and 63.8% (90/141) of the 141 H.pylori strains, respectively, while 2 of isolated H. pylori strain (1.4%) were positive for both iceA alleles and 16(11.3%) were negative for both iceA alleles. The prevalence of babA2 and the combined genotype of babA2 and cagA in H. pylori isolated from DU patients were significantly higher than that in GU patients (74.5% vs. 50.0% for babA2, P =0.028; 70.2% vs. 46.7% for babA2 and cagA, P =0.039). There was no significant difference in prevalence of babA2 among other disease groups. No association of different clinical diseases with iceA genotype was detected. Conclusions The most common genotype of H.pylori strains isolated from patients in Shanghai is iceA1 +/babA2 +. babA2 may play different role in the pathogenesis of duodenal ulcer and gastric ulcer. No association between iceA status and clinical outcome of H.pylori infection was confirmed in our study.
5.Percutaneous internal fixation of hollow screw Ti-nails in treatment of femoral neck fracture: Curative effect and host response in a follow-up analysis of 57 cases

AIM: To investigate the clinic effect of the femoral neck fracture (FNF) with percutaneous internal fixation of hollow screw Ti-nails. METHODS: A total of 57 patients with closed FNF were recruited from Department of Orthopaedics in the First Mine Hospital of Pingdingshan Coal Group from February 2001 to December 2005. They were treated with 3 hollow screw Ti-nails (produce by Shanghai Qunli Medical Devices Co., Ltd) in 5-mm diameter under C-arm X-ray monitoring after close reduction by hand, punctured with 4-6 bone round nails in 2.5-mm diameter through skin for prefixation. The patients were encouraged to stretch and flex muscles early and perform articular physiological movement. The criteria of outcomes: Excellent: the patients were completely cured in six months after surgery, no pain was found in diseased coxa, the articular movement was normal, and they could walk freely half one year after surgey. Good: The fracture was cured in 9 months lately or no healing, there was a little ache in diseased coxa, the articular movement was limited, and they could walk normally in 10-12 months after surgery and recover slight work. Bad: The fractures were not cured, there were absorption, shifting, and aching in fracture joint early, and they need walk with walking stick. RESULTS: There were 55 cases followed up for 9 months, 51 cases for 18 months, and 57 cases for 42 months.①The curative effect of FNF by internal fixation was excellent in 27 cases, good in 20 cases and bad in 10 cases. ②Host response: 6 patients suffered from fracture shift due to screw-nail displacement, 4 patients got bone un-union, 6 patients experienced femoral head necrosis, and 2 patients got ache at bone fracture and were limited for articulation movement. No infection and rejection happened among 57 patients. CONCLUSION: The treatment of AO cannula Ti-screws internal fixation under closed reduction is effective for FNF, and no abnormal biocompatibility between materials and host appears in the clinical follow-up.
6.Comparison between SPECT and MRI in detecting skull-base invasion in nasopharyngeal carcinoma

Objective To investigate the ability of single photon emission computed tomography (SPECT) and MRI in detecting skull-base invasion in nasopharyngeal carcinoma. Methods Sixty-one patients with nasopharyngeal carcinoma received whole body and skull-base tomography SPECT, and nasopharynx and skull-base MRI before radiotherapy. The results were double-blind compared and evaluated. Results The overall positive rates of skull-base invasion detected by SPECT and MRI were 51% and 46% (P=0.508). In paitents with headache, cranial nerve palsy or both, the rates were 83% and 86% (P=1.000) ,80% and 80% (P=1.000), 88% and 94% (P=1.000), respectively. In patients with T1+T2 and T3+T4lesions,the rates were 22% and 0(P=0.031) ,74% and 82% (P=0.250) ,repectively. In patients with N0+N1and N2+N3lesions,they were 50% and 48% (P=1.000) ,53% and 40% (P=0.500) ,respectively. The conformation rate between SPECT and MRI was 85%. Binary Logistic regression analysis showed that T stage was a risk factor for positive SPECT(χ2=4.23,P=0.040, OR=3.04). Headache tended to be a risk factor for both positive SPECT and positive MRI (χ2=3.13, P=0.077, OR=4.54;χ2=3.64,P=0.056,OR=12.00). Conclusions The detection sensitivity of SPECT in skull-base invasion in nasopharyngeal carcinoma is equivalent to that of MRI. The consistency between SPECT and MRI is good. Moreover, there is a good correlation between SPECT and symptoms, signs and stage. SPECT of skullbase tomography is necessary for patients with severe headache, negative CT and those who can not receive MRI. When SPECT result is positive,skull-base should be considered to be invaded and should be defined as gross tumor volume in radiotherapy planning.
7.Influence of emergency PCI in different time windows to cardiac scar and cardiac function after myocardial infarction

Objective To observe the influence of emergency PCI in different time windows to cardiac scar and cardiac function after myocardial infarction. Methods One hundred and twenty-seven patients with acute myocardial infarction, who were treated in different time windows by emergency PCI, were enrolled into this study. The cases were divided into two groups according to the time when they took PCI after onset;group 3 hPCI included 66 patients had PCI at 3 h after onset and group 6 hPCI included 61 patients had PCI at 6 h. At 4th, 8th and 12th weeks, the fragmented QRS (fQRS) were observed by twelve synchronization cardiogram; ventricle wall movement strain rate were determined by cardiac speckle tracking imaging;left ventricular ejection fraction (LVEF)were measured by cardiac ultrasound to evaluate cardiac events and cardiac function after myocardial infarction. Results At 4th,8th and 12th weeks,the fQRS positive leads were significantly more in 6 hPCI group than 3 hPCI group ( P < 0. 05 respectively ) . The ventricle wall movements strain rate and LVEF improved significantly in 3 hPCI group compared with 6 hPCI group (P < 0. 05 ). Conclusions The earlier emergency PCI treatment was applied and the earlier the"criminal"blood vessels were opened ,the more ischemic myocardial cells were restored. Combination use of fQRS、 STI and LVEF examination could improve the diagnosis of cardiac scar formation and direct protective therapy.
8.Clinical significance expression of MMP-7 in patients with primary non-small cell lung cancer

Objective To study the relationship between expression of matrix metalloproteinases-7 (MMP-7) and clinicopathological characteristics in patients with primary non-smaU cell lung cancer(NSCLC). Methods MMP-7 in 20 normal people and 60 advanced NSCLC patiens were detected with reverse-transcription-polymerase-chain-reaction. Gelatum image analysator analyzed the result. Results The amount of MMP-7 was less in normal people (30.000) than in NSCLC patients(41.231) significantly(P<0.05); the level of MMP-7 was no correlated with gender, age, pathology pattern, tumor size, was inverse correlation with differentiation, and was positive correlation with clinical stages(P <0.05). Conclusion The level of MMP-7 is closely correlated with tissue differentiation and clinical stages of NSCLC, which may serve as a parameter for determining tumor invasion and metastatic.
9.Clinical efficacy of large-spot and low-energy Q switched Nd:YAG laser in the treatment of melasma

ObjectiveTo evaluate the effect of large-spot and low-energy Q switched Nd:YAG laser on melasma,and to observe the changes of melasma lesions with confocal laser scanning microscopy (CLSM) before and after the laser treatment.MethodsTotally,45 patients aged from 24 to 48 years and diagnosed with facial melasma were included in this study,and treated with large-spot and low-energy Q switched Nd:YAG once a week for 10 or more sessions.CLSM was used to estimate the melanin content in melasma lesions before each irradiation and after the last irradiation.ResultsAmong the 45 patients,8 ( 17.78% ) were nearly cured,25 (55.56%) markedly improved,11 (24.44%) improved,and only 1 (2.22%) unimproved after the laser irradiation.The total response rate was 73.33%.As CLSM showed,there was an increment in melanin granules in melasma lesions compared with the normal skin surrounding melasma lesions,but a reduction in melanin granules was induced by the laser treatment in melasma lesions.ConclusionsLarge-spot and lowenergy Q switched Nd∶YAG laser is substantially effective and highly safe for the treatment of melasma,and CLSM can be used to evaluate the therapeutic effect of laser on melasma.
10.Effect of contracted learning on rehabilitation training to patients with stroke

Objective To explore the effect of contract learning on rehabilitation training to patients with stroke . Methods According to the random number table, 120 stroke patients were randomly divided in equal number into control group and observation group. The patients in the control group were treated with conventional nursing during hospitalization and health guidance during follow-up. The patients in the observation group were given rehabilitation training according to contracted learning during the period of hospitalization and follow-up. The time was 6 months. The quality of life was compared between the two groups of patients before and after the intervention, 1 months after discharge, 6 months after discharge. Results After six months of intervention, the 7 dimensions of quality of life, including daily life ability, hand function, mobility, strength, emotion, communication and participation in the intervention group were significantly better than those of the control group (P<0.001). The 6 dimensions of quality of life, including daily life ability, hand function, mobility, strength, emotion, and communication of the control group were significantly better than before the intervention (P<0.001). Conclusion For stroke patients, the rehabilitation training under the learning contract can improve the effect of rehabilitation training so as to improve the quality of life and the prognosis.
