3.Exploration and efficacy assessment of the model for epilepsy management in rural China

Objective To evaluate the applicability and effectiveness of the model for managing patients with convulsive epilepsy in rural China.Methods The model is under leadership of the government departments and medical institutions at all levels organized primary physicians for treatment and management.People with convulsive epilepsy in 24 rural areas were screened by local physicians and confirmed by neurologists.Patients diagnosed with convulsive epilepsy and agreed with managing were recruited into study.They were managed at primary care level and provided with phenobarbital monotherapy.Efficacy was assessed from the changes in seizure frequencies from baseline.Results There were 408 town hospitals and 979 trained primary care physicians participating in our study.Five thousand and thirty-eight patients were enrolled in 2 years,and 90% patients with good compliance.Among the people who were treated with phenobarbital monotherapy,3 170 were managed for 12 months,and 2 005 (63.3%) of whom had a 50% or greater reduction in seizure frequency and 1 246 (39.3%)patients remained seizure free;651 patients were managed for 24 months,and 491 (75.4%) of whom had a 50% or greater reduction in seizure frequency and 202(31.0%)patients remained seizure free.Three hundred and forty-three (6.8%) patients withdrew from the study,and only 35 (0.7%) patients discontinued treatment because of side effects.Sixty-nine (1.4%) patients were dead and 8 (11.6%) of them were caused by status epilepticus.Conclusion This study confirms that this model is beneficial to patients with epilepsy and is suitable to the rural areas of China.

5.Effects of selective Infusion of prostaglandin E_1 to ventilated lung on oxygenation and shunt during one-lung ventilation in pigs

Objective To evaluate the effects of prostaglandin E1 (PGE1 ) infused in the pulmonary artery of the ventilated lung on hemodynamics and pulmonary gas exchange during one-lung ventilation (OLV).Methods Experiments were performed on 12 healthy pigs of both sexes (6 males , 6 females) aged 3 months, weighing 14-17 kg. The animals were anesthetized with intramuscular ketamine 15-20 mg?kg-1 , diazepam 1.5-2.0 mg?kg-1 and atropine. The animals were mechanically ventilated after tracheal intubation (FiO2 = 0.7, VT = 12-15 ml?kg-1, RR=15bpm, I:E=l:2). PaCO2 was maintained at 35-45 mm Hg. Swan-Ganz catheter was positioned in the right pulmonary artery via the left femoral vein. The right femoral artery was cannulated for BP monitoring and blood sampling. Anesthesia was maintained with a mixture of ketamine 100 mg and succinylcholine 100 mg in 2% procaine 100 ml infused i.v. at a rate of 2-3 ml?kg-1?h-1. The animals were placed in right lateral position. The left chest was opened and OLV was established by clamping the left main bronchus. During OLV RR was increased from 15 to 20 bpm but the minute ventilation remained unchanged. The animals were randomly divided into 2 groups of six : group A and B. Group A received PGE1 infusion in the right pulmonary artery with an initial dose of 0.01 ?g?kg-1?min-1 which was gradually increased to 0.02, 0.04, 0.06, 0.08 and 0.1 ?g?kg-1?min-1 . Theinfusion of each dose was maintained for 20 min. In group B normal saline was infused instead of PGE1. MAP, HR, CVP, PCWP, CO, PaO2 PvO2 were recorded at each dose and at 20, 40, 60 min after PGE1 infusion was terminated. Qs/Qt, PVR and SVR were calculated.Results MPAP, PVR, SVR, peak inspiratory pressure (PIP) and Qs / Qt significantly increased while CO, Cdyn and PaO2 significantly decreased (P 0.05) and a significant decrease of MPAP and PVR ( P

Objective To study automatic drawing on target therapy area based on MRI and CT imagings in order to get the precision less than 1 mm in HIFU treatment.Methods Based on MRI and CT imagings,automatic image was recognized and the frame was extracted by image processing and image segmentation,etc..Results The system of software we are studying can be effectively used to recognize medical automatic images and get target therapy areas' image drawings.Conclusion This system of software will be used in medical precise treatment systems,such as HIFU 3D Treatment Regional Planning System.
7.The Effects of Community Intervention Measures on Prevention and Control of Hypertension

Objective To look into the effects of community intervention measures on blood pressure and hypertension control of the population.Methods Two geographic separated urban communities with the population about 50 000 each were selected as either intervention or control communities in Beijing in 1996.In the intervention community,the primary and secondary prevention on high blood pressure were carried out.And the morbidity and mortality of stroke in both intervention and control populations were monitored.A baseline survey and evaluation re-survey were respectively conducted in 1997 and 2000 covering both intervention and control communities with a cluster random sample of about 800 persons each.Results In the baseline survey,there were no difference between intervention and control communities in six of seven variables including systolic blood pressure(SBP),diastolic blood pressure(DBP),prevalence of hypertension,rate of awareness,therapy,control with SBP 140 and DBP

Objective To evaluate effects of pulmonary surfactant (PS) containing ketamine on respiratory failure induced with lung-lavage in rats Methods Twenty Wistar rats were anesthetized with intraperitoneal pentobarbital sodium and were ventilated with the peak inspiratory pressure(PIP) being 1 47 kPa and E:I at 1:1 following intubation through tracheotomy In all rats whole double lung lavage were performed with normal saline (37℃, 40 ml?kg -1 ) 8 10 times after PIP and PEEP were adjusted to 1 96 kPa and 0 49 kPa respectively The rats were randomly divided into A and B groups after PaO 2 decreased to less than 12 kPa In group A (n=10): PS (25mg, 0 5ml) was injected intratracheally to lungs; in group B (n=10): PS (25mg, 0 5ml) containing ketamine 2 5 mg was injected At 120th min after PS injection, PIP and PEEP were gradually adjusted to 1 47 kPa and 0 kpa (ZPEEP), respectively Results PaO 2 in both groups decreased significantly from 59 3 kPa to 10 3 kPa after lung lavage(P0 05 vs pre-lavage) All rats in group B and 4 of 10 rats in group A survived at the end of experiment Conclusions Intratracheal administration with PS containing ketamine not only reverses hypoxemia , but also maintains the respiratory function under low airway pressure without PEEP after respiratory failure induced with lung lavage,
9.Advances of GOLPH2 in tumors

As a novel Golgi protein,GOLPH2 expression is associated with maintaining the structural integrity of the Golgi apparatus.As an oncoprotein,GOLPH2 expression is associated with malignant biological behavior of cancer cells.Important roles of GOLPH2 in the tumorigenesis and metastasis make it become a new target for tumor diagnosis and treatment.
10.Characteristics of hysteromyoma MRI images

Objective To study the common imaging characters of the human tissues such as tumors in MRI based on hysteromyoma in hope of getting the results that the same tissue has the same MR characters and identifying the common,comparatively steady MR figs sectional area which can provide related references for the computer-based automatic recognition orgnization and basis for the question such as whether the same MR figs has the same hyperacoustic impedance characters.Methods We analysed all the images with hysteromyoma on MRI T2 and got the rules on MR figs change with the application of the MRI image analysing software which specially designed by Microsoft Visual C++ 6.0 for the research.Results(1)No significant difference was found in MR figs sectional area in the different periods of the same patient under the same MRI test condition.(2)No significant difference was found in MR figs sectional area in different patients under the same MRI test condition.(3)Significant difference was found in MR figs sectional area after the patient received the HIFU treatment under the same MRI test condition.Conclusion(1)Human tissues have some common MRI imaging characters in the same MRI test equipment under the same MRI test condition.(2)Imaging characters of the human body tissues in MRI can be altered after HIFU treatment.
