1.Laparoscopic vaginoplasty using vascularized ileal flap

Objective To study surgical techniques and curative effects of laparoscopic vaginoplasty with transferring vascularized ileal flap.Methods Clinical data of 62 cases of laparoscopic vaginoplasty using vascularized ileal flap from March 2004 to March 2006 in this hospital were retrospectively analyzed.There were 16 cases of total laparoscopic surgery and 46 cases of laparoscope assisted surgery.Results The operation was successfully completed in all the 62 cases.The intraoperative blood loss was 30~50 ml(40.0?0.6 ml) and the operating time was 90~313 min(165.0?5.6 min).Intestinal obstruction occurred 1 month after total laparoscopic surgery in 1 case,requiring a re-operation of ileectomy with end-to-end anastomosis.Vaginal stenosis occurred in 3 cases because of incorrect use of the vaginal mold.The remaining 58 cases had satisfactory vaginal dilatation outcomes. Conclusions Laparoscopic vaginoplasty with vascularized ileal flap is micro-invasive and accordant to normal physiological conditions,being an ideal method for vaginoplasty.
2.Global Campaign Against Epilepsy: An update on the China Demonstration Project and Crest Study

Global Campaign Against Epilepsy conducted the Demonstration Project to improve epilepsy care in
rural China from 2000 to 2004. The project involved 5 provinces and 2,455 patients with convulsive
epilepsy using phenobarbital monotherapy. Forty percent of the patients were seizure free in one year
and another 30% had their seizures decreased by over half. With the support of the central government,
the project has currently been extended to cover 12 provinces with 11,000 patients. The long-term plan
is to integrate epilepsy management into the existing primary health delivery system of China.
3.Lipids and Stroke

The correlation between lipids and stroke is complex. The levels of serum total cholesterol can increase the risk of stroke no matter it is too high or too low. Low-density lipoprotein is the risk factor of atherosclerotic stroke, while high-density lipoprotein is its protective factor. Triglyceride is closely associated with ischemic stroke. This article reviews the relationship between lipids and stroke, as well as the role of lipid-modulating therapy in the prevention and treatment of stroke.
4.The Effects of Community Intervention Measures on Prevention and Control of Hypertension

Objective To look into the effects of community intervention measures on blood pressure and hypertension control of the population.Methods Two geographic separated urban communities with the population about 50 000 each were selected as either intervention or control communities in Beijing in 1996.In the intervention community,the primary and secondary prevention on high blood pressure were carried out.And the morbidity and mortality of stroke in both intervention and control populations were monitored.A baseline survey and evaluation re-survey were respectively conducted in 1997 and 2000 covering both intervention and control communities with a cluster random sample of about 800 persons each.Results In the baseline survey,there were no difference between intervention and control communities in six of seven variables including systolic blood pressure(SBP),diastolic blood pressure(DBP),prevalence of hypertension,rate of awareness,therapy,control with SBP 140 and DBP
5.Early image alternations in 55 patients with primary liver cancer treated by high intensity focused ultrasound

ObjectiveTo evaluate the early image alterations of primary liver cancer (PLC) treated by high intensity focused ultrasound (HIFU). MethodsA total of 55 PLC patients underwent extracorporeal HIFU ablation. Color Doppler ultrasound, digital substraction angiography (DSA), computed tomography (CT) or magnetic resonance imaging (MRI) were performed to monitor the therapeutic response.ResultsImage examinations obtained 2 weeks after HIFU ablation showed significant response of PLC. MRI was the best for evaluating HIFU efficacy. Unenhanced MRI revealed signal alterations on both T 1 and T 2 weighted images, and enhanced MRI demonstrated the absence of contrast enhancement in the treated HCC and a thin peripheral rim of enhancement surrounding the coagulation necrosis. ConclusionMRI rapidly assesses HIFU induced complete ablation of HCC, residual viable cancer tissure in treated region, and coagulation extent of HIFU treatment.
6.Effects of exogenous pulmonary surfactant on inflammatory response in rats with ventilator-induced lung injury

Objective To investigate the effects of exogenous pulmonary surfactant(PS)on the inflammatory response in rats with ventilator-induced lung injury(VILI).Methods Twenty-eight adult male Wistar rats weighing 310-388 g were randomly divided into 4 groups(n =7 each):normal control group(group C),group VILI,group PS and air control group(group A).VILI was produced by high-pressure ventilation(HPV)with peak inspiratory pressure(PIP)40 cm H2 O,respiratory rate(RR)20 hpm and without positive end-expiratory pressure(PEEP)for 20 min.The rats were sacrificed by exsanguination immediately after anesthesia and after VILI in groups C and VILI,respectively.In groups PS and A,PS 100 mg/kg(50 mg/ml)and the equal volume of air were injected into the trachea via the airway after the tracheal edema fluid was removed respectively,and the rats were mechanically ventilated(Vr 10 ml/kg,RR 45 bpm and PEEP 7.5 cmH2O)for 120 min and then sacrificed by exsanguination.The blood samples were taken from femoral artery for determination of the plasma levels of IL-6,IL-10,MIP-2 and TNF-α by ELISA.The tracheal edema fluid was collected to determine the protein concentration by Bradford method.The lungs were removed for microscopic examination and the number of neutrophils was counted under microscope.Results There was no significant difference in the plasma levels of TNF-α among the four groups(P > 0.05).Compared to group C,the plasma levels of MIP-2,IL-10 and IL-6 were significantly increased and the number of neutrophils was significantly enlarged in group VILI(P < 0.05).The number of neutrophils was significantly smaller in group PS than in group A(P < 0.05).There was no significant difference in the plasma levels of MIP-2,IL-10 and IL-6 between groups PS and A(P > 0.05).The inflammatory damage in lung tissues was observed obviously in groups VILI,A and PS.Conclusion When exogenous PS is used to treat the rats with VILI,PS reduces neutrophil recruitment,but can not inhibit the release of inflammatory cytokines.
7.Effects of different ventilation modes on efficacy of exogenous pulmonary surfactant for treatment of rats with ventilator-induced lung injury

Objectlve To investigate the effects of different ventilation modes on the efficacy of exogenous pulmonary surfactant(PS)for the treatment of rats with ventilator-induced lung injury(VILI).Methods Forty-two male Wistar rats weighing 310-356 g were randomly divided into 6 groups(n=7 each):group CVT6,group SVT6,group CVT10,group SVT10,group CVT14 and group SVT14.The tidal volume(VT)was set at 6,10 and 14 ml/kg respectively and the respiratory rate(RR) was 75,45 and 32 bpm respectively.The animals were anesthetized with intraperitoneal 3% Pentobarbital 50 mg/kg,then tracheostomized and intubated.VILI model was induced by high-pressure ventilation (HPV) with peak inspimtory pressure (PIP) 40 cm H2O and without positive end-expiratory pressure (PEEP).The air was injected into the trachea via the airway at the end ofexpiration before HPV (T0,baseline value) and 15-25 min of HPV,the airway pressure monitored and the lung compliance(C) calculated.When C was decreased to half of the baseline value,PEEP was increased to 7.5 cm H20.After the tracheal edema fluid was removed,the PS 100 mg/kg was immediately injected into the trachea in group SVT6,SVT10 and SVT14.The equal volume of air was injected into the trachea in group CVT6,CVT10 and CVr14 instead of PS.Then the rats in different groups were ventilated with the corresponding ventilation modes.MAP was monitored and blood samples were token from femoral artery for blood gas analysis at T0, 5 min after HPV (T1 ), and 15, 30, 60, 90, 120 min (T2-6) after administration of PS. The tracheal edema fluid was collected at T1 and T6.The rats were killed at T6 and the lung tissues taken for microscopic examination. Results With the same ventilation mode, the VILI was significantly alleviated after administration of PS. With different ventilation modes,the lung injury was significantly reduced in group SVT 10 compared with the other groups. Conclusion The efficacy of PS for the treatment of rats with VILI is good using the ventilation strategy with VT of 10 ml/kg and RR of 45 bpm.
8.Study on service demands towards Institute of Postpartum Care

Objective To investigate the maternal attitude and demands for Institute of Postpartum Care in order to provide targeted services.Methods 285 women were a investigated about maternal attitude and demands towards Institute of Postpartum Care with self-designed questionnaire.Results 97.9% (279/285) women thought sitting the month (puerperal period) was very important,50.2% (143/285)women were interested in Institute of Postpartum Care.They thought the main three advantages were energy saving,medical expertise and technology and avoidance of family conflicts;The three major issues were high price,lack of monitoring mechanism and unfamiliar environment and less family company;33 items of services demands over four points accounted for 78.6%,the three modules with highest scores were cleaning services scored 4.64±0.47,newborn services scored 4.45±0.41,consultation service scored 4.41:±0.53.Conclusions Professional postnatal care services have been a trend and social need,and a large proportion of women have great interest and demand on Institute of Postpartum Care which should make full use of its concentrate resources,and provide specific services according to maternal demands for services.It is urgent to build unified service standards and evaluation indicators to improve service quality.
9.The protection effect and mechanism of aloe polysaccharide on experimental colitis mucosa

Objective To investigate the protective effect of aloe polysaccharide on experimental colitis mucosal. Methods The TNBS-induced experimental rat colitis model was successfully established and were treated by aloe polysaccharide, and enteric-coated tablets with sulfasalazine (SASP). Results DAI score, gross morphology score , histopathologic score and MPO in the model group were significantly higher than that in the normal group (P < 0.01), while those indicators were significantly lower after treatment (P < 0.01), IL-12 and IFN-γ mRNA and protein expressions in the model group were significantly higher than that in the normal group (P < 0.01), and IL-12 and IFN-γ expression levels in treatment group were significantly lower than that in the model group (P < 0.01), IL-4 and IL-13 mRNA and protein expression levels in the model group were significantly lower than that in the normal group (P < 0.01), and IL-4 and IL-13 expression in treatment group was significantly higher than that in the model group (P < 0.01). Conclusion Aloe polysaccharide on experimental colitis in rats has a good protective effect , of which mechanism may be reduced by IFN-γ and IL-12 levels, increased IL-4, IL-13 levels, and correct the imbalance of Thl/Th2.
10.Clinical analysis of psychiatric symptoms after liver transplantation

ObjectiveTo explore causes and prevention experience for neuropsychopathic symptoms after liver transplantation.MethodsThe clinical data of 62 patients who underwent transplantation in our hospital were retrospectively analyzed for the causes of neuropsychopathy symptoms and treatment experience was Summarized.ResultsSixteen patients(25.8% ) had psychiatric symPtoms after liver transplantation in three weeks,four of them more severe.The common clinical manifestation was sleep disorders,mania,anxiety,depression,delirium and change of cognition.The pre-operation encephalopathy history,liver function,blood loss,time in the ICU after surgery,post-operation infection and the concentration of immunosuppressive agents were risk factors for psychiatric symptoms.All patients were cured.ConclusionsPsychiatric symptoms are commonly seen after liver transplantation and have a variety of etiology and clinical manifestations.Psychological counseling should be strengthened.The appropriate therapeutic measures for different etiology can improve patient outcomes.
