1.Corneal distribution of stromal cell-derived factor-1 and chemokine receptor-4 as homing factors

BACKGROUND:Stromal cel-derived factor-1 (SDF-1) and chemokine receptor-4 (CXCR4) play an important role in stem cel homing during bone marrow, organ and tissue transplantation. But there are few reports on limbal stem cel transplantation and homing as wel as the distribution and content of SDF-1/CXCR4 on the cornea. OBJECTIVE:To detect the expression levels of SDF-1/CXCR4 mRNA in rat corneal tissue by fluorescence quantitative PCR. METHODS: The normal rat cornea was taken completely, and subdivided into central cornea, peripheral corneal and limbal area. Fluorescence quantitative PCR method was employed to detect SDF-1/CXCR4 mRNA levels at corresponding positions. RESULTS AND CONCLUSION: The expression levels of SDF-1 and CXCR4 mRNA were 0.213±0.015 and 0.189±0.02 in the central cornea, 0.529±0.077 and 0.285±0.015 in the peripheral cornea, 0.666±0.069 and 0.258±0.067 in the limbal area, respectively. These data show that there are significant differences in the expression levels of SDF-1 and CXCR4 mRNA in the central cornea, peripheral corneal and limbal area, and these differences exert an important role in cel homing and migration during limbal stem cel transplantation for treatment of limbal stem cel deficiency.
2.Variation of stromal cell-derived factor-1 level in a limbal stem cell deficiency model

BACKGROUND:How to promote homing of scarce limbal stem cel s is the key to improving transplantation efficiency. And stromal cel-derived factor-1 plays an important role in the repair of retinal pigment epithelial cel s injury. OBJECTIVE:To observe changes of stromal cel-derived factor-1 levels in rat and mouse limbal stem cel deficiency models in order to confirm whether these changes wil induce peripheral blood stem cel s homing into the cornea. METHODS:Eye models of limbal stem cel deficiency were prepared in rats and mice by cutting method, then peripheral blood samples were continuously drawn to detect the mass concentration of stromal cel-derived factor-1 by enzyme-linked immunosorbent assay within 1 month after modeling. RESULTS AND CONCLUSION:In the rat peripheral blood, the mass concentration of stromal cel-derived factor-1 started to increase at 24 hours after modeling and firstly peaked at the 3rd day;then it tended to decrease and reached a second peak lower than the first one at the 25th day;subsequently, it returned to the normal level. In the meanwhile, the mass concentration of stromal cel-derived factor-1 in the mouse peripheral blood, increased at 12 hours after modeling, peaked at the 3rd day, subsequently decreased until another peak lower than the former one presented at the 20th day, and final y it returned to the normal level. In conclusion, the level of stromal cel-derived factor-1 in the peripheral blood is significantly associated with limbal stem cel s deficiency, which may play an important role in corneal injury repair.
3.Effect of heme oxygenase-1 on brain edema induced by asphyxial cardiac arrest and resuscitation in rats

Objective To investigate the effect of heme oxygenase-1 ( HO-1) on brain edema in a rat model of asphyxial cardiac arrest and resuscitation. Methods Forty male SD rats weighing 250-300 g were randomly divided into 4 groups ( n = 10 each) : group I sham operation; group II cardiac arrest (CA); group Ⅰ Hemin (HO-1 inducer) and group IV SnPP (HO-1 inhibitor). Asphyxial cardiac arrest and resuscitation were performed in CA, Hemin and SnPP groups (group Ⅰ ,Ⅲ, Ⅳ) . Hemin and SnPP groups received hemin 15 mg/kg intraperitoneally (IP) at 12 h before CA and SnPP K 30 μmol/kg IP at 1 h before CA respectively. The animals were sacrificed at 1 and 6 h after recovery of spontaneous circulation (ROSC) . The water content of the cortex, hippocampus and brain stem and the expression of HO-1 and aquaporin-4 (AQP4) mRNA in cortex and hippocampus ( by RT-PCR) were determined. Results Water content of cortex and hippocampus was significantly higher at 1 h after ROSC in CA and SnPP groups than in sham operation group and was significantly lower in Hemin group than in CA group. There was no significant difference in water content of brain stem at 1 and 6 h after ROSC among all 4 groups. The expression of AQP4 mRNA was significantly higher in cortex and hippocampus at 1 h after ROSC in CA and SnPP groups than in sham operation group and was significantly lower in Hemin group than in CA group.Conclusion HO-1 can reduce brain water content at early stage after cardiac arrest and resuscitation by regulating the expression of AQP4.
4.Reconstruction with fresh deep freezing unicondylar osteoarticular allograft in 12 cases following resection of distal femur tumor

OBJECTIVE:To evaluate the feasibility and initial outcome of reconstructing the defect femur by unicondylar osteoarticular altograft after resection of distal femur tumor.METHODS:Between July 2003 and August 2008,a total of 12 patients with distal femur tumor treated by unicondylar resection and fresh-frozen unicondylar osteoarticular allograft reconstruction of bone defects were retrospectively reviewed at the Department of Orthopedics,General Hospital of Chinese PLA.Of 12 patients,5 medial and 7 lateral femoral condyles were involved.There are 8 patients suffering from giant cell tumor,including 2 patients complicating with pathologic fracture,3patients suffering from osteosamoma and 1 patient suffering from low-grade fibrosarcoma.Before transplantation,MRI and/or CT were used to determine that the tumor was definitely limited in half condyle.Three cycles of chemotherapy preoperatively and six cycles of chemotherapy postoperatively were conducted in patients suffering from osteosarcoma and fibrosarcoma.Patients with giant cell tumor did not receive any intraoperative adjuvant treatment.The surgical margin of resected tumors was evaluated by histological examination.All patients were followed by radiograph or CT.Postoperative function was evaluated by Musculoskeletal Tumor Society (MSTS) scoring system at the time of the latest follow-up.RESULTS:All patients were followed up.The patients were followed up mean 35 months after transplantation.None of the patient died.No local recurrences and metastases were found at the latest follow-up.No evident immune rejection and deep infection were seen.No plate loosening or breakage was observed.All patients had their grafts retained at the time of the latest follow-up.Three patient's allograft showed subchondral bone collapse and articular deterioration,but the function was good.Instability of the knee joint was noted in 3 patients.The mean MSTS functional score (totally 30 points) was 26 points.CONCLUSION:For some patients with giant cell tumor or malignant tumors with clearly defined margins after chemotherapy.When the lesions were limited in half condyle and enough surgical margins can be achieved,reconstruction of bone defect with unicondylar osteoarticular allografts after bone tumors were resected appears to be a reliable alternative.
5.Effect of therapeutic hypercapnia on inflammatory response in rat lung transplantation

Objective To investigate the effect of therapeutic hypercapnia on the inflammatory response in the rat lung transplantation. Methods Male pathogen free Wistar rats weighing 300-400 g were used in this study. The animals were anesthetized with 3% pentobarbital sodium 30 mg/kg, tracheostomized and mechanically ventilated (V_T 10 ml/kg, RR 50 bpm, FiO_2 50%). Carotid artery and femoral vein were cannulated for BP monitoring, blood sampling and fluid and drug administration. Left lung transplantation was performed using modified cuff technique. Forty-eight animals in which lung transplantation was successfully performed were randomized into 2 groups ( n = 24 each) : model group (M) and hypercapnia group (H) . In group H, PaCO_2 was maintained at 80-100 mm Hg by inhalation of CO_2.Arterial blood samples were obtained before lung transplantation (To , baseline) and at 1, 2, 4 h (T_(1-3)) of reperfusion for determination of blood TNF-α, IL-1 and IL-8 concentrations. The animals were then killed and the transplanted lungs were removed for microscopic examination and calculation of wet/dry lung weight ratio. Results The MAP and PaO_2 were significantly higher in group H than in group M. The blood IL-8 and TNF-α concentrations were significantly lower at T_(1-3) in group H than in group M, but there was no significant difference in blood IL-1 concentration between the 2 groups. The elastase content in the lung tissue was significantly lower at T_2 and T_3 in group H than in group M. Microscopic examination showed that the alveolar hemorrhage, the infiltration of the lung by macrophages and neutrophils and lung edema were significantly less in group H than in group M. Conclusion Therapeutic hypercapnia can obviously inhibit the inflammatory response in the rat lung transplantation.
6.Diagnostic value of breast mass biopsy in different locating methods

Objective To explore the value of preoperative diagnosis for breast cancer patients in different locating ways. Methods The tissues were gotten by core needle biopsy from 146 breast cancer patients through different locating ways,and were taken for histopathological examinations and were compared with postoperative pathologic results. Results Seventy-four cases were biopsied with ultrasound-guide (ultrasound-guide group), and 72 cases with free-hand (free-hand group), without serious complications.Compared with postoperative pathologic results,the false negative rate in ultrasound-guide group was 5.41%(4/74);the false negative rate in free-hand group was 18.06% (13/72). The accuracy of the ultrasound-guided biopsy was higher than that in free-hand (U = 13.63,P < 0.01 ). Conclusions Preoperative ultrasoundguided biopsy and postoperative pathologic examination has no significant difference, with high consistency.The study provides a good basis for selection for clinical work,so as to the more effective guide for the comprehensive treatment of breast cancer patients.
8.Expressions of heat shock protein 70,P27 and Bcl-2 protein in mucoepidermoid carcinoma of salivary glands and clinical significances

Objective To study the expressions of Hsp70,P27,Bcl-2 in mucoepidermoid carcinoma of salivary glands and clinical significances.Methods Forty cases of mucoepidermoid carcinoma of salivary glands were chosen and the expressions of Hsp70,P27,Bcl-2 were detected by the method of immunohistochemistry,10 cases of polymorphic adenoma were chosen as control group.Results Both Hsp70 and P27 were stained with aliomycin in nucleus and cytoplasm,the positive rates of them were 75% and 27.5%,respectively,there were significant differences compared with control group(P0.05).The expressions of Hsp70 and P27 had significant differences in different clinical stages,different histological grades and lymph node metastasis(P0.05).Conclusion The expressions of Hsp70 and P27 can be used to estimate prognosis of patients with mucoepidermoid carcinoma.Hsp70 and Bcl-2 can inhibit apoptosis by different ways.
9.Comparison of two remedy for treatment of simple bone cyst in adult

Objective To approach the effective treatment for simple bone cyst in adult. Methods Forty-eight cases were divided into two groups by visiting order, 20 cases (steroid group) were treated by prednisolone acetate and 28 cases (open resection and allograft group) were treated by open resection and allograft. The curative effect were contrasted between two groups. Results The follow-up time was 5-46(26.58 ± 10.81) months. Aecording to the Chigira's healing criteria for simple bone cyst, grade Ⅰ , Ⅱ , Ⅲ,V were 1, 2, 10 and 7 cases respectively in steroid group, contrasting to 0, 3, 5 and 20 cases respectively in open resection and allograft group. The recovery rate was 85.0%(17/20) in steroid group and 89.3%(25/28) in open resection and allograft group, which was no statistically significant difference between two groups (P>0.05). Conclusions Although the two remedies are no obvious difference in curative effect,simple bone cyst at lower extremity of weight-bearing with obvious osteolysis in adult is recommended to open resection and allograft, otherwise or at upper extremity to steroid injection.
10.Diagnostic value of nuron specific enolase and myelin basic protein on patients of early brain contusion and laceration

Objective To investigate the diagnostic value of nuron specific enolase( NSE),S100βprotein,glial fibrillary acidic protein( GFAP)and myelin basic protein( MBP)in patients with early brain contusion and laceration. Methods One hundred and twelve cases with brain contusion and laceration diagnosed by CT or MRI were selected as our subjects who hospitalized Harrison international peace hospital from Apr. 2012 to Jul. 2013. Of them,68 cases with mild head injury were served as mild group and 44 cases of severe traumatic brain injury were served as severe group. And 83 healthy people without lung disease and nervous system diseases were served as control group. Electro chemiluminescence assay and ELISA methods were used to measure the level of NSE,S100β,GFAP,MBP. Results the level of serum NSE,S100β protein,GFAP and MBP in mild group were(18. 14 ± 6. 83),(0. 92 ± 0. 45),(0. 78 ± 0. 37))(4. 37 ± 1. 84)μg/ L,respectively, and(32. 11 ± 12. 48),(1. 58 ± 0. 94),(4. 26 ± 1. 96),(14. 72 ± 6. 77)μg/ L,respectively in severe group, and(8. 94 ± 3. 49),(0. 12 ± 0. 08),(0. 13 ± 0. 09),(1. 98 ± 0. 89)μg/ L,respectively in control group. There were significant differences among three groups( F = 137. 520,120. 083,283. 727,205. 569 respectively;P< 0. 01). All indexes were different between control and mild groups( q = 10. 599,13. 296,5. 881,6. 018;P< 0. 01),as well as between the mild and severe groups(q = 13. 600,9. 249,26. 639,22. 029;P < 0. 01),and between control and severe group(q = 23. 408,21. 258,32. 797,28. 134;P < 0. 01). Conclusion The level of serum NSE,S100β,GFAP,MBP can be used as early indicators of brain injury secondary diagnosis and secondary index for evaluating damage degree.
