Objective To summarize the experience of diagnosis and management of regional portal hypertension. Methods The clinical manifestations, diagnostic methods and therapeutic modalities of 16 cases of regional portal hypertension were analyzed retrospectively. Results Among 16 patients with ~regional portal hypertension, 12 cases resulted from pancreatic diseases, and 4 cases were complicated with non-pancreatic diseases. The main clinical findings were splenomegaly in 16(100%), abdominal pain ~in 10(63%) , gastrointestinal bleeding in 7(44%) and abdominal masses in 3(19%). All had normal liver function test. The main diagnostic methods were ultrasonography(US), computerized tomography (CT) and endoscopy. Splenic vein thrombosis could be detected by color Doppler ultrasonography (7/7). ~Enhanced CT could demonstrate enlarged and tortuous short gastric veins, gastroepiploic veins, and ~coll- ~ateral vessels around splenic hilum(16/16). Isolated gastric varices (4/5) were revealed mainly by ~gastro- ~scopy . Splenectomy was effective for controlling gastrointestinal bleeding, and its complication of portal vein thrombosis occurred in 1 case. Conclusions It is not difficult to diagnose regional portal ~hyper- ~tension based on findings of US and CT, together with special clinical characteristics. Therapeutic options should be ~individualized according to underlying diseases, however, splenectomy should be performed in the ~patients with gastrointestinal bleeding.

China Oncology 2006;0(09):-
Background and purpose:Hereditary nonpolyposis colorectal cancer(HNPCC) which related to inheritance is an autosomal dominant inheritance. There are many screening standards differing presently from country to country. The Amsterdam standard is internationally recognized. These screening standards are just based on family history and clinical manifestations which can not reflect the essence of the disease. At present, the golden standard for screening HNPCC is mismatch repair genes detection that is in the experimental stage. The purpose of this study was to investigate the expression of mismatch repair genes hMSH2 and hMLH1 in HNPCC and their clinicopathological signif icance, and to fi nd a simple, fast and economic method for screening HNPCC.Methods:Immunohistochemical staining was used to detect the expression of hMSH2 and hMLH1 in 20 cases with HNPCC(Group A),20 cases with ordinary hereditary colorectal cancer(Group B),20 cases with sporadic colorectal cancer(Group C),and 20 cases with colorectal polyps(Group D).Results:The expression rates of hMSH2 and hMLH1 in Group A,B,C and D were 35%(7/20),70%(14/20),95%(19/20) and 100%(20/20) respectively, which illustrated that the probability of HNPCC and the negative expression of hMSH2 and hMLH1 was reduced from Group A to Group D.There were signifi cant differences among these groups(P0.05).Conclusion:The negative expression of hMSH2 and hMLH1 was signifi cantly correlated with HNPCC and location and differentiation of colon carcinoma.Immunohistochemical staining may be useful for screening HNPCC.

BACKGROUND: At present, bone remodeling biological model study usually applys finite element method combing with computer technique to simulate and predict bone quantity or bone structure. In this study the author integrate inversion method with animal experiment to establish a quantification bone remodeling biological model of in vivo bone tissue in real stress environment.OBJECTIVE: To set up a quantification biological model of bone growth and remodeling adaptation, which integrates animal experiments, parameter inversion identification of mathematical functions, and technique of computer simulation. DESIGN, TIME AND SETTING: The experiment was accomplished in Animal Experiment Center of Dalian Medical University in October 2002.MATERIALS: 60 female Sprague Dawley mice of 6-week old were used in this study. Challenger double-energy X ray bone density device was provided from DMS Company, France. Sensation l6 CT machine was provided from Germany Siemens Company.METHODS: 60 mice were randomly divided into two groups: i 5 animals were in normal control groups. 45 in experiment groups. By designing a new animal experiment, we investigate the effects of stress environments on bone growth and remodeling of rapid growing rats and gather the bone mineral density (BMD) of proximal femur in the same interval for the unknown parameters (B and K) inversion of bone growth and remodeling equation to create the femur three-dimension geometrical model based on CT images. MAIN OUTCOMING MEASURES: Body weight of animal, bone density and CT imagine of proximal femur. RESULTS: Body mass in the experiment group and control group was increased with the rat growing; BMD in the control group and overloading group was also increased with the rat growing; but BMD in the unloading group was decreased in the fifth week. Inversion and experimental data showed that parameter B was rapidly decreased as compared to time, and it was closed to zero in the 10th week. Parameter K was rapidly increased as compared to time. and it was gradually increased from the 5th to the 10th weeks, moreover, it was closed to the horizontal line after 10 weeks. This predicted that rapid growth was over, and bone reconstitution and absorption came into another balance cycle. CONCLUSION: The thought and method used in the model creating in.this paper provide clue and reference to establish human model of bone growth and remodeling.
