1.The effect between i-gel mask and SLIPA mask in the airway management of general anesthesia

Objective To compare the effect between i-gel mask and SHPA mask in the airway management of general anesthesia.Methods Sixty patients of general anesthesia with ASA I or Ⅱ were randomly divided into two groups:i-gel mask group (30 cases) and SLIPA group(30 cases).After intubation and general anesthesia,fiberoptic bronchoscopy was used to detect and evaluate.The peak airway pressure,mean airway pressure,condition of the laryngeal mask intubation,time of intubation,time of extubation,time of recovery and the side effect occurrence of extubation were recorded.Results The time of anesthesia,time of operation,time of extubation,time of recovery,achievement ratio of first time,achievement ratio of second time,time of intubation,peak airway pressure and mean airway pressure had no significant difference between two groups (P > 0.05).The airway seal pressure in i-gel mask group was significantly higher than that in SL1PA group [(29 + 6) cm H2O (1 c m H2O =0.098 kPa) vs.(23 ± 4) cm H2O] (P < 0.05).For the grade of fiberoptic bronchoscopy in i-gel mask group,there was 27 patients in I grade,3 patients in Ⅱ grade.But in SL1PA group,there was 3 patients in I grade,4 patients in Ⅱ grade,13 patients in Ⅲ grade,10 patients in ⅣVgrade.There was significant difference between two groups (P < 0.05).There were none of regurgitation in two groups.Conclusions The i-gel mask and SLIPA mask can provide the same effective ventilation.The i-gel mask has higher airway seal pressure,less side effect during the intubation and extubation,and have better airway management of general anesthesia.
2.Effects of early enteral nutrition on short-term quality of life in patients with esophageal carcinoma after operation

A total of 62 patients with radical operation of esophageal carcinoma were divided into all-in-one nursing intervention group (research,n =32) and routine nursing group (control,n =30).With regards to postoperative flatus time,removal of chest tube drainage time,postoperative hospital stay and hospital costs,the research group was lower than the control group (P < 0.05).After 2 weeks,from the postoperative quality of life questionnaire with the European Organization for Research and Treatment of Cancer,the scores of overall health of enteral nutrition group were higher than those of parenteral nutrition group while the symptom scores of fatigue,nausea & vomiting,insomnia,decreased appetite,constipation and economic difficulties were less than parenteral nutrition group (P < 0.05).Early postoperative enteral nutrition can improve the quality of life in patients with esophageal cancer.
3.Effect of all-in-one nursing intervention on patients with anxiety and depression of esophageal carcinoma negative emotion and immune function

Objective To investigate the effect of all-in-one nursing intervention on esophageal carcinoma patients with bad emotion and immune function. Methods From the October, 2012 to the October, 2013, 102 patients with radical operation of esophageal carcinoma were divided into all-in-one nursing intervention group (research group, 52cases) and routine nursing group (control group, 50 cases) according to the random number table method.Two groups of patients were evaluated before and after operation of anxiety and depression emotion scores, with the Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) .At the same time, two groups of patients were detected the stress and immune index before and after the operation. Results Before operation, two groups of patients with anxiety, depression, stress reaction and immune index were not significantly different (P>0.05).After operation, the study group patients with anxiety and depression scores were significantly lower than those in the control group [(34.6+6.7) points vs. (44.8+7.5) points, (33.7+6.9) points vs. (40.6+7.3) points], the difference was statistically significant (t values were 7.234, 4.902, P<0.05). Eighth days after operation, patients in the control group FBG, CRP and Cor levels were significantly higher in the study group [(6.98±1.12)mmol/L vs.(6.09±0.85)mmol/L,(18.11±5.93) mg/L vs.(9.24±3.22)mg/L,(293.98±69.09)nmol/L vs.(221.97±67.94)nmol/L],the difference was statistically significant (t values were 4.532, 9.336, 5.307, P<0.05). Eighth days after operation, two groups of patients with IgA, IgM were higher than first days after operation [(2.41±0.46)g/L vs.(1.89±0.56)g/L,(2.51±0.58) g/L vs.(1.35±0.67)g/L and (2.36±0.53)g/L vs.(1.82±0.46)g/L, (2.34±0.62)g/L vs. (1.37±0.64) g/L], the difference was statistically significant (t values were 5.174, 9.439, 5.549, 7.850, P<0.05).The total number of patients blood lymphocytes (LYM) and cytokines (CD3, CD4, CD8, CD4/CD8) were significantly higher than the control group [(2.87±0.65)×109/L vs.(1.98±0.58)×109/L, 0.62±0.10 vs. 0.57±0.07, 0.41±0.08 vs. 0.35 ±0.08, 0.24 ±0.04 vs. 0.26 ±0.05, 2.06 ±0.44 vs. 1.65 ±0.46], the difference was statistically significant (t values were 7.286, 2.915, 6.942, 2.225, 4.601, P<0.05). Conclusion All-in-one nursing intervention can significantly improve the anxiety and depression in patients with esophageal cancer in bad mood, enhance the immune function of patients, conducive to the rehabilitation of patients.
4.Effect of all-in-one nursing intervention on the stress response and postoperative quality of life in patients following esophageal cancer surgery

Objective To investigate the effect of all-in-one nursing intervention on the stress response and postoperative quality of life in patients following esophageal cancer surgery .Methods From the October 2012 to March 2013 , 62 patients with radical operation of esophageal carcinoma were divided into all-in-one nursing intervention group (32 cases) and the routine nursing group (30 cases).The blood samples of the two groups of patients were collected before operation (surgery day) (T0) and on the fifth day after operation (T1) to measure stress response indicators , including fasting blood glucose ( FBG ) , C-reactive protein ( CRP ) , interleukin -6 (IL-6), plasma cortisol (Cor).And using European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-30 ( QOL ) evaluated the early quality of life , before operation and the second week after opreation .Results The stress reaction index before operation had no significant difference between two group (P>0.05).On the fifth day after operation, the FBG, CRP and Cor levels of two groups were significantly higher than the preoperative level (t=3.724,7.203,4.137, respectively;P<0.05), but there was no significantly difference in the IL-6 level (t =0.452,P>0.05).The FBG, CRP, IL-6 and Cor levels of two groups on the fifth day after operation were significantly higher than the preoperative level ( t =3.031,5.361,2.864,3.534,6.990,10.755,2.415,8.166, respectively;P<0.05).Scores of each function item and symptom before operation had no significant difference between the two groups (P>0.05).And two weeks after the operation , the functions of the research group were higher than those in the control group ( P<0.05 ) .Symptoms scores of study group was lower than those in the control group , and there were statistically significant differences in seven aspects ( P<0.05), but the other two aspects had no significant differences (P>0.05).Conclusions All-in-one nursing intervention can significantly reduce stress reaction and improve the quality of life in postoperative patients with esophageal cancer .
5.Influences of integrated nursing care on bad emotion and immune function of postoperative patients with esophageal cancer

Objective To explore the influences of integrated nursing care on bad emotion and immune function of postoperative patients with esophageal cancer. Methods A total of 102 postoperative patients selected from October 2012 to October 2013 were randomly divided into research group ( 52 cases, integrated nursing care) and control group(50 cases, routine nursing). Self rating anxiety scale (SAS) and self rating depression scale ( SDS) were used to evaluate the anxiety and depression of patients before and after operations. We detected the contents of immune index such as immunoglobulin ( IgA, IgG, IgM ) , peripheral blood lymphocyte count ( LYM ) , cytokine and etc. before operation, the first and eighth days after operation. Results Patients in the research and the control group all had anxiety and depression symptoms before operation. After operation, the scores of SAS (34. 6 ± 6. 7) and SDS (33. 7 ± 6. 9) of patients in the research group were lower than that in the control group (P<0. 05). Eighth days after operation, the contents of IgA and IgM rose again compared with that in the first postoperative day (t=5. 174,9. 439;P<0. 05). The contents of LYM and cytokine CD4/CD8 of patients in the research group were higher than that in the control group ( P<0. 05), and all of them were higher than that in the first postoperative day. Conclusions Integrated nursing care can relieve anxiety and depression of patients with esophageal cancer, enhance patients′immune function, and benefit patients′recovery.
