1.The application of extended supraglottic horizontal partial laryngectomy in advanced laryngeal carcinoma and vallecula carcinoma.

OBJECTIVE:
To discuss the function recovery and treatment effect of extended supraglottic orizontal-partial laryngectomy in the treatment of advanced laryngeal carcinoma and vallecula carcinoma.
METHOD:
Forty-four patients who received extended supraglottic orizontal partial laryngectomy were followed up on the survival, breath function, pronunciation and swallowing function.
RESULT:
The 2-year, 3-year survival rates of this group were 72.7% and 71.4%, respectively. There were no recurrence cases in this group. All cases of death were due to cervical lymph node metastasis or recurrent cervical lymph node metastasis after cervical lymph node dissection. The extractive rate of tracheostomy tube was 97.7%. Forty-two patients own good pronunciations. Two patients who received partial arytenoid cartilage resection own weaker pronunciations. After practising, all the patients can eat without the nasal feeding tube.
CONCLUSION
Extended supraglottic orizontalpartial laryngectomy give complete resection of the primary lesion. Patients who received extended supraglottic orizontalpartial laryngectomy have good breath, swallowing and pronunciation function.

Sirtuin 1 (SIRT1),the Ⅲ class deacetylation enzyme,is a kind of NAD + dependent histone deacetylation enzyme.The role of SIRT1 in tumor has the duality.It can inhibit inflammation,tumor angiogenesis and interact with tumor related gene to inhibit tumor development.However,it can also regulate tumor related genes,and epithelial-mesenchymal transition,promote tumor cell proliferation and tolerance of radiotherapy and chemotherapy,and maintain the stemness of cancer stem cells to promote tumor proliferation,invasion and metastasis.
3.Differential expressions of ciaH, eno and pykF genes of fluoride-resistant Streptococcus mutans cultivated in fluoride environment and its significance

Objective:To investigate the relationship between ciaH, eno, pykF genes and fluoride resistance through determining the differential expressions of ciaH, eno and pykF genes of fluoride-resistant Streptococcus mutans cultivated in fluoride environment. Methods:The cultured Streptococcus mutans and their fluoride-resistant strains were divided into UA (Streptococcus mutans subcultured in BHI without NaF), FR (fluoride-resistant Streptococcus mutans subcultured in BHI without NaF) and FFR (fluoride-resistant Streptococcus mutans subcultured in BHI containing 1 g·L-1 NaF) groups.After 11 h (logarithmic phase) and 20 h (platform stage) cultivation, the expression levels of ciaH, eno and pykF mRNA were detected by RT-PCR method.Results:Compared with FR group, the expression levels of ciaH, eno and pykF mRNA in FFR group were increased both in the logarithmic phase and the platform stage(P<0.05 or P<0.01).Compared with UA group, the expression levels of eno and pykF mRNA in FR group were decreased both in the logarithmic phase and the platform stage(P<0.01), whereas the expression level of ciaH mRNA had no significant difference in the logarithmic phase (P>0.05), but it was increased in the platform stage (P<0.01).Conclusion:Fluoride can increase the expression levels of ciaH, eno and pykF genes in fluoride-resistant Streptococcus mutans, indicating that these genes are related to the production of fluoride resistance.
4.Unstable atlas fractures treated by minitype titanium plate fixation through transoral approach

Objective To investigate the effect of minitype titanium plate fixation through transoral approach in the treatment of unstable atlas fractures.Methods A retrospective case series study was made on 21 patients with unstable atlas fractures treated by minitype titanium plate fixation through transoral approach from June 2008 to June 2014.There were 15 males and 6 females,at age of (40.9 ± 10.6)years (range,21 to 57 years).Anterior 1/2 Jefferson fractures were seen in 12 patients and 1/2 ring Jefferson fractures in 9 patients.Preoperative visual analogue score (VAS) was 4-9 points [(7.6 ± 1.3) points].Before operation,degree of mobility of the cervical vertebra was (15.4 ± 3.9) °in bending,(10.8 ± 2.5) °in extending,(18.3 ± 3.1) ° in left-bending,(18.9 ± 2.7) ° in right-bending,(21.8 ± 5.8) °in left-rotation and (22.4 ± 4.6) ° in right-rotation.Operation time,intraoperative blood loss,VAS,cervical mobility and bone healing were detected after operation.Results Operation time was (86.3 ±25.3)m in,and intraoperative blood loss was (120.5 ± 33.3)ml.VAS was improved to 0-2 points [(1.6 ± 0.4) points] at postoperative 3 days (P < 0.05).All patients were followed up for 12 to 48 months[(23.7 ±5.9) months].VAS was improved to 0-2 points[(0.6 ± 0.1) points] at postoperative 3 months (P < 0.05).Degree of mobility of the cervical vertebra was improved significantly at postoperative 3 months,with the bending of(38.6 ± 4.5) °,extending of (39.3 ± 4.0) °,left-bending of (39.2 ± 4.0) °,right-bending of (39.2 ± 2.9) °,left-rotation of (66.8 ± 8.8) ° and right-rotation of (66.3 ± 9.2) ° (P < 0.05).Postoperatively,there were no surgical wound incision infections and vertebral artery or spinal injuries,Bone union was found in all patients,without the occurrence of implant loosening or breakage and the dysfunction of the cervical vertebra.Conclusion Minitype titanium plate fixation through transoral approach is associated with less trauma,high healing rate and preservation of the activity of cervical vertebra in the treatment of unstable atlas fractures.
5.Association between let-7 gene rs10877887 polymorphism and cancer risk in the Chinese population:A meta-analysis

Objective To evaluate the relationship between the let-7 rs10877887 polymorphism and cancer risk in the Chinese population.Methods We searched all relevant studies published on association between the let-7 rs10877887 polymorphism and cancer risk in PubMed,Embase and Wanfang databases up to December 31,2016 were searched.A total of 4 case-control studies comprising 2 754 cases and 3 481 controls were included in this meta-analysis.The pooled odds ratio (OR) and 95% confidence interval (95%CI) were calculated to examine the strength of the association.Sensitivity analyses were performed to assess the stability of the results,publication bias was also assessed.Results The pooled results showed that there was a significant association between the let-7 rs10877887 polymorphism and overall cancer risk under the dominant model (CC+CT vs.TT:OR=0.90,95%CI=0.82-1.00,P=0.048).Conclusion Base on present studies,the results of this meta-analysis indicated that there is a significant association between the let-7 rs10877887 polymorphism and overall cancer risk in the Chinese population,the let-7 rs10877887 polymorphism could decrease the risk of cancer.
6.Effect of valsartan on the long-term prognosis of patients with coronary atherosclerotic heart disease following successful intervention therapy: Multicenter, double blind, randomized and controlled evaluation

BACKGROUND: Valsartan is an antagonist of angiotensin Ⅱ (Ang Ⅱ ) receptor. Many researches have proved that it can protect heart tissue. Val-PREST suggests that valsartan with a long-term administration can decrease restenosis rate in stent; however, effect of valsartan on restenosis rate of Chinese population is still unclear presently.OBJECTIVE: To evaluate the effect of oral valsartan for 6 months on patients with coronary heart disease (CHD) who undertook successful intervention therapy.DESIGN: Multicenter, double blind, randomized, and controlled evaluation and prospective design.SETTING: Beijing Friendship Hospital Affiliated to Capital Medical University; Beijing Anzhen Hospital Affiliated to Capital Medical University; Peking Union Hospital; People's Hospital of Peking University; Beijing Tongren Hospital Affiliated to Capital Medical University; Beijing Shijingshan Hospital; Beijing Fuxing Hospital Affiliated to Capital Medical University;Beijing Chuiyangliu Hospital.PARTICIPANTS: Eight three-grade A hospitals in Beijing participated in the study. Since December 2002 to October 2003, a total of 200 patients who underwent bare mental stent implantation were consented, but 196 patients were recruited in the end. All 196 patients were randomized into valsartan group (100 cases) and control group (96 cases).METHODS: Basic medicines in the two group included aspirin, clopidogrel, nitrides, statins, β-receptor antagonists, calcium channel antagonists, etc. Additionally, Patients in valsartan group were also given valsartan (Beijing Nuohua Pharmaceutical Co. Ltd., batch number: SD 34004) in a dosage of 80 mg a day. Both groups were followed-up once a month for total 6 months.MAIN OUTCOME MEASURES: ① Major adverse cardiac events within 6 months on clinics (death, non-fatal myocardial infarction, hospitalisation once more due to recurrent myocardial ischemia, and target vessel revascularization); ② Results of duplicated coronary angiography or exercise treadmill test (ETT) of partial patients within 6 months.RESULTS: ① Two patients (2%) in valsartan group were excluded in this study because of intolerance, so 194 patients were involved in the final analysis. ② No significant differences of baseline characteristics in terms of lesion type, the number of diseased vessels and the cardiac function were found between the two groups (P < 0.05). ③ During the period of 6-month follow-up, one case died in control group. One acute myocardial infarction occurred in each group, whilst one case undertook target vessel revascularization in valsartan group. It was found that the proportion of recurrent cardiac events was lower in valsartan group than that in control group (11.2% vs. 15.6%). However, this difference did not reach the statistic significance. ④ During the period of 6-month duplicated contrast examination, one case had restenosis of in-stent in valsartan group. ⑤ The positive rate of exercise treadmill test (ETT) was lower in valsartan group (25.7%) than that in control group (36.4%), but there was no statistic difference.CONCLUSION: Six-month oral valsartan on patients with coronary heart disease who undertook successful intervention therapy can decrease the trend of recurrent cardiac events and positive rate of ETT.
7.Evaluation of application effect of mental intervention on patients with postsurgical gastroparesis syndrome

Objective To study the effect of mental intervention on patients with postsurgical gastroparesis syndrome (PGS). Methods 70 patients with gastroptroparesis syndrome after abdominal operation were divided into the control group and the experimental group with 35 patients in each group according to chronological order.The patients in the control group accepted routine care,and the experimental group was given mental intervention based on routine care.Center for Epidemiological Survey Depression Scale (CESD) was used to evaluated the emotional state before and after intervention.Besides,the recovery of gastrointestinal function was appraised. Results The improvement level of anxiety in the experimental group was higher than the control group.The extinction time of symptoms,indwelling time of gastric tube,recovery time of food intake were shorter than the control group.Drainage time when gastric juice > 800ml/d was shorter,recovery time of PGS and hospitalization time were shorter,the treatment cost was reduced,compared with the control group. Conclusions Mental intervention can alleviate the negative emotion and shorten the recovery time of patients with postsurgical gastroparesis syndrome.
8.Effects of 5-Aza-2'-deoxycitydine and trichostatin A on expression and methylation of CHFR in human laryngreal carcinoma cell line.

OBJECTIVE:
To explore the effects of 5-Aza-2'-deoxycitydine(5-Aza-dC) and trichostatin A (TSA) on the expression and methylation of CHFR in human laryngeal carcinoma cell line.
METHOD:
The mRNA expression and promoter hypermethylation and were detected by Realtime fluro-genetic quantitative PCR and methylation specific PCR in Hep-2 cell line, which were cultured in vitro and then treated with different concentrations of 5-Aza-dC and TSA.
RESULT:
Compared with the control team, 5-Aza-dC alone reactivated expression of the CHFR in Hep-2 cell line (1.75 +/- 0.21). TSA had no effect on gene expression (1.05 +/- 0.13). The combined treatment with 5-Aza-dC and TSA increased gene expression (2.15 +/- 0.18). The cell lines showed a characteristic DNA methylation status. 5-Aza-dC and combined 5-Aza-dC and TSA resulted in demethylation of CHFR. In contrast, TSA alone did not affect the DNA methylation status of CHFR.
CONCLUSION
Hypermethylation of CHFR gene promoter is a common event in the occurrence and development of laryngeal carcinoma. The promoter aberrant methylation of CHFR is a main cause for down-expression of CHFR. After either treatment with 5-Aza-dC alone or in combination with TSA, the expression of CHFR is up-regulated duo to the reversal methylation. It can be a new idea to the therapy of laryngeal carcinoma.

9.Correlation between histone H3-K9 methylation, DNA methylation and expression of gene MGMT in Hep-2 cell line.

OBJECTIVE:
To explore the correlation between histone H3-K9 methylation, DNA methylation and expression of carcinoma suppressor gene MGMT in laryngeal carcinoma Hep-2 cell line.
METHOD:
5-Aza-dC was used to deal with Hep-2 cell cultured in vitro. ChIP, MSP and Realtime-PCR were used to detect H3-K9 methylation, DNA methylation, of MGMT gene promoter region and MGMT gene expression before and after treatment with drugs.
RESULT:
(1) In Hep-2 cell line, gene MGMT was characterized by DNA methylation and histone H3-K9 hypermethylation. (2) 5-Aza-dC was able to reduce H3-K9 methylation of MGMT gene histone in Hep-2 cell line, 5-Aza-dC was able to reverse DNA methylation of MGMT gene histone in Hep-2 cell line, 5-Aza-dC was able to upregulate the down-regulated gene expression of tumor suppressor genes MGMT.
CONCLUSION
Promoter methylation of cancer suppressor gene MGMT may induce the gene inactivity. DNA methylation may increase H3-K9 methylation. 5-Aza-dC can reduce H3-K9 methylation of tumor suppressor gene MGMT histone by reversing DNA methylation of tumor suppressor gene MGMT, and then the expression of tumor suppressor genes is increased and tumor development is inhibited.

10.Study of mRNA expression level and hypermethylation of CHFR promoter in the laryngeal squamous cell carcinoma tissue.

OBJECTIVE:
To explore the relationship between the level of expression and hypermethylation of the CHFR gene and the occurrence and development of laryngeal squamous cell carcinoma (LSCC).
METHOD:
The mRNA expression and promoter hypermethylation were detected by Realtime fluro-genetic quantitative PCR and methylation specific PCR in 50 LSCCs (LSCC group) and 15 normal laryngeal tissue (control group).
RESULT:
1) CHFR mRNA was shown in the control group, while the mRNA was loss expression in the 2 LSCC (4%), and the level of mRNA expression was significantly lower in the LSCC group. The relative ratio was 0.50 +/- 0.12, which is 0.30 +/- 0.04 at the early stage of the LSCC and 0.70 +/- 0.21 at the advanced stage, respectively. The discrepancy had statistical significance (P<0.01). 2) The methylation rate of CHFR was 22% (11/50) in the LSCC tissues, which was not found in the normal tissues. The aberrant methylation of CHFR was observed in 10 of the patients at the stage I and stage II of LSCC , in 1 of the patients at the stage III, and was absent at the stage IV. There was significant difference between the aberrant methylation of CHFR and the stage of carcinoma (P<0.01). 3) The mRNA expression level of the aberrant methylation patients was 0.11 +/- 0.05, which was significantly lower than that of the unmethylation patients 0.75 +/- 0.13. Gene inactivation was observed in 2 of the 11 patients with the aberrant promoter methylation. The methylation was associated with the expression of mRNA, with the correlation coefficient 0.387 (P<0.05).
CONCLUSION
Hypermethylation of CHFR gene promoter is associated with loss or lower expression of CHFR mRNA in the LSCCs, and it may contribute to the occurrence and development of LSCC. The promoter aberrant methylation of CHFR may be one of the early diagnostic and therapeutic marker genes.
