Asimplified European procedure now allows the registration of traditional herbal medicines as medicinal products even without the support of clinical data. This procedure entails the requirement that those products comply with European Good Manufacturing Practice for medicinal products, which in turn implies that the raw herbal materials comply with the European Guidelines for Good Agricultural and Collection Practice. On the basis of a comparison between European Good Agricultural and Collection Practice and China Good Agricultural Practice,as well as direct observation made at sites in China,we issue some recommendations to facilitate good communication between the Chinese producer and European pharmaceutical customer, with a view to ensure full compliance with European expectations.

Objective To discuss the effect of intra-operative and post-operative hyperthermic intraperitoneal lavage chemotherapy for Ⅲb and IV gastric cancer on post-operative peritoneal metastasis and survival rate. Methods The clinical data of 31 cases of gastric cancer(control group)who underwent operation only and 50 cases(chemotherapy group)who receved operation combined with intra-operative and post-operative hyperthermic intra-peritoneal lavage chemotherapy were comparatively studied. Results The survival rate of control group after 2 years and 3 years was lower than that of the chemotherapy group, and the intraperitoneal recurrence raue of cootrol group after 2 years and 3 years was higher than that of chemotherapy group. Conclusions Intra-operative and post-operative hyperthermic intraperitoneal lavage chemotherapy for gastric cancer has important significance in prevention and treatment of peritoneal metastasis and can improve survival rate.

A 37-year-old female was admitted to the hospital for an itching and painful subcutaneous nodule with ulceration on the extensor aspect of her left forearm for more than 6 months. The pain was severe, continuous and localized.Systemic and local treatment with antibiotics resulted in no obvious improvement. The lesion had gradually increased in size over the past 6 months and the ulcer had enlarged for 1 month. On examination,a hard infiltrative plaque measuring about 5.5 cm × 4.0 cm with a well-defined margin was seen on the extensor aspect of her left forearm, along with ulceration and some dirty discharge on the surface. The diagnosis of fibrosarcoma, grade Ⅱ was eventually made by a biopsy of the lesion, which revealed increased pigmentation in the basal layer, and tumor tissue was tightly adherent to the epidermis.Dermis and subcutaneous fat layer were infiltrated with various sizes of spindle cells with fine collagen fiber bundles between the cells.Obvious atypia and mitotic figures were easily observed in some of the cells.Immunohistochemical analysis showed moderately positive staining for fibronectin, but negative staining for human melanoma black-45 (HMB45),S100,smooth muscle actin (SMA),Melan-a,high molecular weight cytokeratin (HCK),CD34,CD68 or cytokeratin. Some diseases should be differentiated from this case, including dermatofibrosarcoma protuberans, cutaneous spindle cell squamous carcinoma, atypical fibroxanthoma, malignant fibrous histiocytoma, and so on.

Non-alcoholic fatty liver disease is a common chronic liver disease in clinical practice. In recent years, with increasing social attention to the health of women and the elderly, the prevention and treatment of non-alcoholic fatty liver disease after menopause has increasingly become a research hotspot in metabolic diseases. This study explores the pathogenesis and treatment method of non-alcoholic fatty liver disease in postmenopausal women based on the theory of "deficient qi and stagnation" and combined with the physiological and pathological characteristics of postmenopausal women and the Western medicine understanding of non-alcoholic fatty liver disease. We believe that the pathogenesis of non-alcoholic fatty liver disease in postmenopausal women is rooted in the "deficient qi" caused by depletion of liver and kidney essence and blood. The imbalance between the physical and functional aspects of the liver due to this "deficient qi" is the primary factor, while the "stagnation" of phlegm and blood stasis is the manifestation. Furthermore, the "deficient qi" and "stagnation" reinforce each other, with the deficiency leading to stagnation and stagnation exacerbating the deficiency, thus accelerating the progression of the disease. The treatment approach should be one that combines nourishing deficiency and resolving stagnation, addressing both root cause and maifestations. Given the female characteristic of "the liver as the innate organ" and the post-menopausal physiological state of "gradual decline of kidney essence", it is important to focus on nourishing the liver and kidneys, nurturing the liver′s physical body while maintaining its function, and also promoting the circulation of qi, resolving phlegm, and invigorating blood circulation to remove blood stasis. This approach aims to reduce the accumulation of lipids in the liver, offering a new perspective and approach for the treatment of non-alcoholic fatty liver disease in post-menopausal women with traditional Chinese medicine.
