1.Analysis of sagittal anatomic structure of upper airway in patients with ankylosing spondylitis: computed tomography-based three-dimensional reconstruction

Objective To investigate the characteristics of sagittal anatomic structure of the upper airway in patients with ankylosing spondylitis using three-dimensional reconstruction based on computed tomography (CT).Methods Thirty-one male patients with ankylosing spondylitis,aged 20-60 yr (AS group),and 41 common patients (male) without difficult airways,aged 20-60 yr (control group),who underwent spiral CT scan of the head and neck using Helical CT from January 2007 to February 2011 in our hospital,were enrolled in the study.Reconstructed images of the upper airway were obtained using AW4.4 workstation and six distances (D1-D6) and four angles (α-δ) were recorded and analyzed:(1)D1,the arc distance between the upper central incisor and root of epiglottis; D2,the distance between the upper central incisor and root of epiglottis; D3 and D4,the lengths of maxilla and mandible ; D5,the distance between the root of epiglottis and midpoint of glottis; D6,the distance between the end of mandible and midpoint of glottis; (2) angle α,the angle of line D2 and D5; angle β,the angle of line D2 and the lower edge of the upper central incisor to the midpoint of glottis; angle γ,the angle of line D4 and D6; angle δ,the angle of the point of the lower edge of the upper central incisor to the trailing edge of the hard palate and then to the root of epiglottis.Results Compared with control group,no significant change was found in D1,D2,D3,D4 and D5 (P > 0.05),and D6,angle α and angle δ were significantly increased,whereas angle β and angle γ were decreased in AS group (P < 0.05).Conclusion The anatomic structure of the upper airway has the characteristics of specific changes and a laryngoscope blade with a large degree of curvature may be helpful for successful tracheal intubation in patients with ankylosing spondylitis.
2.MicroRNA target predicition based on SVM and the optimized feature set.

MicroRNA (miRNA) is a family of endogenous single-stranded RNA about 22 nucleotides in length. Through targeting 3' UTR of message RNA (mRNA), they play important roles in post-transcriptional regulatory functions. For further research of miRNA function, the identification of more miRNA positive targets is needed urgently. Aiming at the high-dimensional small sample data sets in miRNA target prediction, an algorithm of eliminating redundant features is proposed based on v-SVM in this paper, and classification and features selection are also fused. The algorithm of eliminating redundant features optimizes the combination of features, and then constructs the best features combination which can represent miRNA and targets interaction model. The prior parameter v (0 < u < or = 1) controls the compression proportion of data set and selects more distinguishing support vectors. Finally, the classifier model of miRNA target prediction is built. The unbiased assessment of the classifier is achieved with a completely independent test dataset. Experiment results indicated that in both classification recognition and generalization performance of miRNA targets predicition, this model was superior to the present machine learning algorithms such as miTarget, NBmiRTar and TargetMiner, etc.

3.One-stage posterior lumbar debridement, interbody fusion, and posterior instrumentation in treating lumbar spinal tuberculosis

Objective To analyze retrospectively clinical study efficacy and feasibility of one-stage posterior lumbar debridement,interbody fusion,and posterior instrumentation in treating lumbar spinal tuberculosis.Methods A total of 21 patients (14 males and 7 females) with lumbar tuberculosis collected from January 2009 to May 2012,underwent one-stage posterior lumbar debridement,interbody fusion,and posterior instrumentation.The age ranged 19 to 47 years (mean,34.8 years).All patients presented with presented with back pain,7 patients with constitutional symptoms including weakness,malaise,night sweats,fever and weight loss,2 with limbs numb and 1 with intermittence creep.Every patient underwent lumbar spine X -ray,CT scan and MRI examination of pathologic vertebra before surgery.All patients received at least a standard preoperative 2-4 week anti-tuberculosis treatment.Results All patients were confirmed by pathology or microbiology and were followed up for 12-48 months (mean,18 months).Average operation time was 3.1 h (range,2.5 to 4.3 h).Lumbar tuberculosis was completely cured and the grafted bones were fused 10 months after operation in all patients.There was no persistence or recurrence of infection and no nerve,blood vessel injury.After the treatment,the erythrocyte sedimentation rate (ESR) was decreased to normal level in 5.8 months.Conclusion With effective and standard anti-tuberculosis chemotherapy,the pedicle screw was placed due to pathologic vertebral body.One-stage posterior lumbar debridement,interbody fusion,and posterior instrumentation for lumbar tuberculosis could effectively relieve pain symptoms,and reconstruct the spinal stability.
4.Determination of ginsenoside Rg1 in intracerebral dialysate by LC-MS/MS and comparison of in vivo and in vitro recovery of microdialysis probe

Aim To establish a LC-MS/MS method to measure the concentration of ginsenoside Rg1 in intrac-erebral dialysate and compare the probe recovery in vitro and in vivo. Methods The assay was conducted with a ACQUITY UPLC BEH C18(2. 1 mm × 50 mm, 1. 7 μm) . The mobile phase consisted of methanol and ultrapure water and it was detected by gradient elution. The flow rate was 0. 4 mL·min-1 . Specificity, linear range, precision and accuracy, stability were evaluated to investigate the reliability of the method. The recov-ery of ginsenoside Rg1 in probe in vitro and in vivo was compared. Results The retention time of ginsenoside Rg1 was 1. 91 min, the linear range was 0. 1 ~50 μg · L-1 , intra-day and inter-day precisions were less than 15%. The recovery of ginsenoside Rg1 was (4. 05 ± 0. 28)% in vitro and(26. 96 ± 4. 45)% in vi-vo. Conclusion The LC-MS/MS method is accurate, sensitive, and reproducible for quantitative determina-tion of ginsenoside Rg1 in microdialysate. The probe recovery of ginsenoside Rg1 in vivo is higher than in vitro, and both are stable in different concentrations.
5.Risk factors of early rebleeding after endovascular embolization of ruptured intracranial aneurysms

Objective To discuss the clinical characteristics and risk factors related to the early rebleeding after endovascular embolization of ruptured intracranial aneurysms, to reduce its occurrence and to provide the theoretical basis for the clinical selection of therapeutic methods. Methods During the period of July 2002-Oct. 2007 in the Department of Neurosurgery of Changhai Hospital, patients with DSA-proved ruptured intracranial aneurysms were treated with pereutaneous endovascular embolization. The clinical data and imaging findings of the patients who had occurred early rebleeding after interventional therapy (study group) were retrospectively analyzed. The patients who had not occurred early rebleeding after interventional therapy during the same period were randomly selected and served as the control group. The number of patients in control group was 7.5 times of that in study group. The suspected risk factors were statistically analyzed by using univariate and multivariate methods, the results were compared between two groups. Results Of 881 patients with raptured intracranial aneurysms treated by endovascular interventional embolization, 17 (1.93%) occurred rebleeding in the early time, among them 12 (1.36%) died. The reality or falsity of the aneurysms, the ruptured times of the aneurysms before treatment, the obvious intracranial vascular spasms seen on DSA during the procedure and the degree of embolization were the four main independent risk factors related to the early rebleeding. The probability equation of the early rebleeding of ruptured intracranial aneurysms afterendovascular embolization was calculated.Conclusion Although the incidence of early rebleeding in patients after endovascular embolization of ruptured intracranial aneurysms is low, the outcome, if it occurs, is poor with high mortality. Preoperative prevention measures directed against the risk factors, embolizing the ruptured intracranial aneurysm as completely as possible and prompt postoperative cerebral angiography are all very important for reducing the incidence of the early rebleeding. And active and effective dealing with the rebleeding can definitely improve the prognosis.
6.Clinical practice and discovery of selective coronary venous bypass grafting in the treatment of diffuse coronary artery disease

Objective To summarize the clinical features of patients with diffuse coronary artey diseases,and evaluate the clinical efficacy of off-pump coronary artery bypass grafting(OPCABG) combined with selective coronary venous bypass grafting (SCVBG).Methods Retrospectively analyzed the clinical data of 61 patients with diffuse right coronary stenosis undergoing operation of OPCABG + SCVBG from January 2007 to December 2013,and couducted the comparative study of the patients who underwent OPCABG during the same period based on propensity score.Patients were divided into SCVBG group(61 cases,underwent OPCABG + SCVBG) and control group(60 cases,matched by propensity score and underwent OPCABG without SCVBG).Results Compared with control group,the rate of myocardial infarction in SCVBG group was higher (67.2% vs.46.7c%,P <0.05),the heart rate was faster[(69.92 ± 15.82) bpm vs.(64.48 ± 13.72) bpm,P < 0.05],the low density lipoprotcin and triglyceride were higher[(2.67 ± 0.78) mmol/L vs.(2.37 ± 0.78) mmol/L (1.84 ± 0.79) mmol/L vs.(1.36 ± 0.60) mmol/L,both P < 0.05] and the troponin I was higher in the first postoperative day [0.85 (0.29,3.15)μg/L vs.5.09 (2.02,13.03)μg/L,P < 0.05].The perioperative(postoperative) mortality(1.6% vs.0) and the long-term survival curve difference had no statistically significance(P >0.05).Conclusion Patients with coronary artery disease should pay more attention to the control of heart rate and blood lipids,poorly controlled heart rate and high blood lipids are the important factors for the development of coronary heart disease.The exact efficacv of selective coronary vein arterialization for diffuse coronary artery disease is confirmed through the small sample comparative study.
7.Inhibition of islet allograft rejection by Qa-1/PD-L1 artificial liposome

Objective To explore the effects of Qa-1 and PD-L1 loaded artificial liposomal treatment in allograft rejection and its outcomes .Methods The extracellular domains of Qa-1 and PD-L1 were loaded on liposome surface by streptavidin-biotin system . Mixed lymphocyte reaction (MLR) was performed for measuring Qa-1/PD-L1 liposome biological function .Then liposome was co-transplanted with allo-islets via portal vein .The levels of blood glucose and C-peptide were detected daily after transplantation .Also hepatic lymphocytes after transplantation were isolated for determining the proportion of activated cells and signaling pathway changes .Results Artificial liposome could be easily loaded with biotinylated peptide and its diameter was between 50 to 500 nm . Qa-1/PD-L1 liposome could significantly suppress lymphocyte proliferation , activation and secretion of IFN-γ in MLR by an activation of SHP1/2 and an inhibition of Syk pathway .Qa-1/ PD-L1 liposomes could suppress the activation of hepatic lymphocytes in vivo by activating SHP1/2 ,protecting islet allografts and maintaining a normal level of blood glucose in recipients .Conclusions Qa-1/PD-L1 loaded liposome can effectively suppress allograft rejection and improve the outcomes of islet transplantation .
8.Phenotype and genotyping of Yersinia pestis in China

Objective:To study the phenotype and genotype distribution of Yersinia pestis ( Y. pestis) in different natural foci of plague in China, so as to provide scientific basis for plague prevention and control. Methods:A total of 2 184 strains of Y. pestis isolated from different time periods, regions, hosts and vectors in 11 plague natural foci of China since 1943 were selected for biochemical type identification, glycolysis test, virulence factor test [capsule antigen (F1), pesticin Ⅰ (Pst Ⅰ), virulence antigen factor (VWa), pigmentation factor (Pgm)], different region (DFR) typing and clustered regularly interspaced short palindromic repeats (CRISPR) typing. Results:There were 16 biochemical types of Y. pestis in the natural foci of plague in China, and each biochemical type showed obvious regional distribution in each foci. Most strains were positive for ass hide glue glycolysis (89.79%, 1 961/2 184), maltose (80.13%, 1 750/2 184), glycerol (94.23%, 2 058/2 184), and denitrification (82.78%, 1 808/2 184), and negative for rhamnose (88.78%, 1 939/2 184) and melibiose (85.62%, 1 870/2 184). Virulence factor test results showed that 99.95% (2 183/2 184) of Y. pestis were F1 positive; 99.73% (2 178/2 184) of Y. pestis can produce Pst Ⅰ; 73.31% (1 601/2 184) of Y. pestis were VWa positive and 26.69% (583/2 184) were VWa negative; Pgm positive strains accounted for 72.62% (1 586/2 184), Pgm negative strains accounted for 21.52% (470/2 184), and Pgm mixed type strains accounted for 5.86% (128/2 184). According to DFR typing results, there were 52 genotypes in 2 184 strains of Y. pestis, of which 19 were major genotypes and 33 were minor genotypes. CRISPR typing revealed 16 major genotypes, of which 7 were newly discovered. Conclusion:The phenotypes and genotypes of Y. pestis in various natural foci of plague in China are diverse and have geographical distribution characteristics.
