1.Effects of propofol on excitatory synaptic transmission in hippocampal CAl neurons in rats

Objective To investigate the effects of propofol on the whole-cell excitatory postsynaptic currents (EPSC) and spontaneous excitatory postsynaptic current (sEPSC) in hippocampal CAl neurons.Methods Wistar rats (13-19 days old) weighing 40-60 g were decapitated and the hippocampi were immediately removed and placed in 0-4℃ artificial CSF aerated with 95% O2 and 5% CO2 . The hippocampi were sliced (400 ?m thick) . EPSCs were recorded in hippocampal CA1 neurons by stimulating the Schaffer collateral / commissural pathway. The 50 slices were divided into 5 groups (n = 10 each): group Ⅰ intralipid-1; group Ⅱ propofol-1; group Ⅲ SR95531 + propofol; group Ⅳ intralipid-2 and group Ⅴ propofol-2. In group Ⅰ, Ⅱ and Ⅲ after EPSCs were recorded for 10 min, 10% intralipid 90 ?l, 1% propofol 90 ?l (final concentration was 100 ?mol?L-1 and SR95531 10 ?mol?L-1 + propofol 100 ?mol?L-1 were added to the perfusate and again EPSCs were recorded for 40min. The changes in amplitude of EPSC were analyzed. In group Ⅳ and Ⅴ after the cell membrane was perforated and being stabilized for 10-15 min 10% intralipid 90 ?l and 1 % propofol (100 ?mol?L-1) was added to the perfusate and sEPSCs were recorded without stimulation. The holding potential was - 70 mV. Results Intralipid didn't affect EPSC but propfol 100 ?mol?L-1 reduced EPSC to 47.7% of the baseline value. SR95531 could reverse the effect of propofol on EPSCs. 2.Application of bone marrow fenestration technique for rotator cuff repair at the foot-print zone of rotator cuff under arthroscopy

Objective To evaluate the efficacy of bone marrow fenestration technique used for ro-tator cuff repair at the foot-print zone of rotator cuff under arthroscopy. Methods A total of 30 patients with full-thickness rotator cuff tear ( from 3 cm to 5 cm) were enrolled for this retrospective study who had been treated at Department of Orthopaedics, The People's Hospital of North Jiangsu from January 2015 to December 2016. Of them, 15 were treated under arthroscopy with simple suture bridge technique ( SB group ) and the other 15 under arthroscopy with bone marrow fenestration and suture bridge techniques ( BMSB group ) . The 2 groups were compared in terms of visual analogue scale ( VAS ) , American Shoulder and Elbow Surgeons ( ASES ) scale, Constant scores,range of motion of the affected shoulder and rate of retear after surgery. Results All the patients received follow-up for 11 to 14 months. Significant improvements were achieved in both groups in VAS scores at 3 months ( 1.9 ± 0.6 and 0.9 ± 0.6 ) , ASES scores ( 90.7 ± 3.6 and 92.3 ± 4.2 ) and Constant scores ( 88.9 ± 2.5 and 87.4 ± 3.4 ) at the last follow-up, compared with the preoperative values ( 4.7 ± 1.1 and 4.3 ± 1.5, 58.2 ± 7.2 and 55.6 ± 4.4, and 57.3 ± 6.7 and 55.9 ± 10.3, respectively) ( P <0.05), but there were no significant differences between the 2 groups in ASES scores, Constant scores or range of motion of the affected shoulder at the last follow-up ( P> 0.05). The VAS scores at 3 months for the BMSB group (0.9 ± 0.6) were significantly lower than that for the SB group (1.9 ± 0.6) ( P <0.05). There was no significant difference either between the 2 groups in the retear rate [ 20.0% ( 3/15 ) for SB group versus 26.7% ( 4/15 ) for BMSB group ] ( P > 0.05 ) . Conclusion Application of bone marrow fenestration technique in addition to suture bridge technique can obviously relieve pain after rotator cuff repair, though it shows no advantage in functional recovery of the shoulder joint.
3.Correlation between nociceptin/orphanin FQ(N/OFQ)and perioperative myocardial injury in elderly patients with coronary heart disease

Objective To evaluate the relationship between perioperative myocardial injury(PMI)and serum N/OFQ levels in elderly patients with coronary heart disease.Methods Totally 120 elderly patients who underwent hip fracture surgery under general anesthesia from January 2022 to May 2023 were included,including 60 patients with coronary heart disease(CHD group)and 60 patients without coronary heart disease(control group).The venous blood of patients was collected 10 minutes before anesthesia induction(T0),12 hours after surgery(T1)and 24 hours after surgery(T2)to detect the content of N/OFQ and high-sensitivity myocardial troponin I(hs-cTnI)in serum.Record perioperative adverse cardiovascular events(PACE)and the use of vasoactive drugs during surgery.Results Compared with the control group,the N/OFQ and hs cTnI levels at T0 and T1 in the CHD group were significantly increased(P<0.05).There was a positive correlation between N/OFQ and hs-cTnI levels at T1 and T2 in CHD and control group(P<0.05).The use of PACE and intraoperative vasoactive drugs in the CHD group was higher than that in the control group(P<0.05).Conclusion There is a correlation between the increased N/OFQ content and PMI in elderly patients with coronary heart disease after surgery,which may become an early predictive indicator of PMI.
