Objective To apply James Sheehy myringoplasty for treatment of large perforation without residual rim and to observe the efficacy. Methods Between January 2001 and June 2003, 60 patients with large perforation without residual rim had underwent James Sheehy myringoplasty, in which 36 were males and 24 were females, aged from 16 to 64 years and with an average of 45.23 years. Preoperative air-bone gap was 5 to 45 dB, averaged 28.5 dB. Fellow-up was done more than 6 months on 54 patients. The key technic: (1) a vascular skin flap was made on the posterior canal wall and the anterior canal wall skin was excised. (2) canaloplasty was performed until the whole tympanic sulcus can be seen under one microscope view. (3) if necessary, the ossicular chain was explored. (4) overlay technic was used to repair perforation with temporal fascia. (5) the free anterior skin and the posterior canal vascular flap were replaced. Results 52 patients were healed in one stage. In 2 patient, two needle tip-like perforations appeared in the center of the grafting tympanic membrane 3 months after surgery, which were closed after gelatin sponge patching. The shape of the tympanic membrane looked as normal. Symptoms of temporomandabular joint appeared in 6 patients and disappeared 3 days later. Lateral displacement of the graft happened in 2 patients. Conclusion James Sheehy myringoplasty is a reliable technique for treatment of large perforation without residual rim, with high success rate, less postoperative response and good shape of new tympanic membrane.

Chinese Journal of Radiation Oncology 2005;0(05):-
Objective To study the dose rate distribution on cultured cell plane and establish a reference dose rate table of in vitro cell line ~(125)I seed irradiator. Methods Thermoluminescence dosimetry(TLD) was used to measure the irradiation dose rate of a single 6711 model(~(125) I) seed with apparent activity of 10.323 MBq in water at point P.Meanwhile, the theoretic value of the irradiation dose rate at point P was calculated with theoretic formula. The difference between the calculated and observed values within 10% was set as standard to analyze the accuracy of the measurement. The irradiation dose rate of a single 6711 model(~(125) I) seed was measured in 1mm-thick polystyrene + water medium at point P.The value was applied to differential or non-differential proof along with the value from water medium to study the effect of 1 mm thick polystyrene on distribution of irradiation dose in water. Finally,by simulating the(~(125) I) seed plane irradiator with nine(~(125) I) seeds, the distribution table of irradiation dose rate on the cultured cell plane was calculated with theoretical formula. Results The observed value(n= 10) of irradiation dose rate with one(~(125) I) seed in water at point P was(0.359?0.023)cGy/h and the calculated value was 0.347cGy/h,the difference was within 10%.The observed value(n=10) of irradiation dose of one(~(125) I) seed in 1mm-thick polystyrene + water medium at point P was (0.350?0.027)cGy/h,which showed no statistical difference from the observed value in water under differential and non-differential proof. The reference table on dose rate distribution for cells exposed to(~(125) I) seed irradiation in vitro was developed. Conclusions 1mm-thick polystyrene gives no significant effect on irradiation dose rate distribution from(~(125) I) seeds in water.A reference table on the dose rate distribution for cells exposed to(~(125) I) seed irradiation in vitro has been developed, which can be used to determine an optimal irradiating strategy for future work.

Chinese Journal of Preventive Medicine 2002;36(6):386-389
OBJECTIVETo assess the exposure level of total N-nitroso compounds (TNOCs) in the residents of high- and low-risk areas for esophageal cancer in southern China.
METHODSSamples of duplicate plate diets and 12 hr overnight urine were collected from 120 male adults in each of the 2 areas, a high-risk area (Nanao county) and a low-risk area (Lufeng county) for esophageal cancer. The 240 male healthy subjects (35 - 64 years old) were selected by a 3-stage random cluster sampling procedure. Levels of TNOC, N-nitrosamino acids (NAAs) and volatile N-nitroso compounds (VNOC) in the samples were measured by Thermo Energy Analyzer.
RESULTSThe detectable rate (95%) of diet TNOC, daily dietary TNOC intake (4.25 +/- 0.84) micromol/day, 12-hr urinary TNOC excretion levels (1.76 +/- 0.23 ng/12 h) and daily dietary intake of VNOC (266 +/- 31.2 microg/day) in the high-risk area were all significantly higher than those of the low-risk area. Oesophageal cancer mortality rates were positively and significantly associated with daily dietary TNOC intake and 12-hr urinary TNOC excretion. Urinary NAAs excretion levels were not different in the two areas.
CONCLUSIONThe results suggest that TNOCs may be implicated in the etiology of esophageal cancer in southern China.
