1.Clinical study on treating deep venous thrombosis of the low extremity by recombinant tissue type plasminogen activator after gynecological surgery

Objective To explore the clinical effect and safety of using recombinant tissue type plasminogen activator (rt-PA) in early lower extremity deep venous thrombosis (DVT) after gynecological surgeries. Methods Twenty-five cases with early DVT after gynecological operation were enrolled and randomly divided into two groups,contorl group (n = 13) and the rt-PA therapy group (n = 12). The patients from contorl group were treated with Low-Molecular-Weight Heparins Calcium 0. 4 ml/d alone, and the rt-PA group accepted rt-PA 100 mg i. v in addition to the Low-Molecular-Weight Heparins Calcium. The recanalization rates of lamb veins,prothrombin time (PT) ,bleeding event and other adverse complications were observed and evaluated at day seven. Results The recanalization rates of lamb veins were 91. 7% (11/12) and 53. 8 % (7/13) in rtPA and the control group respectively,which showed significant difference between each other (x2 = 15. 68, P <0. 01). One case had vaginal stump bleeding in each group but no other hemorrhage in both group,the difference of bleeding rates between two groups didn't reach statistically significance (x2 = 1. 48 ,P> 0. 05). Two patients with vaginal stump bleeding were healed by local hemostasis. Conclusion The usage of rt-PA could improve the recanalization rate of DVT after gynecological surgery. The rates of adverse complications, including vaginal stump bleeding,were extremly low.
2.Solitary bronchial papilloma: a clinicopathological study of four cases and review of literature

Purpose To explore the clinicopathological features, differential diagnosis of solitary bronchial papilloma, and its relation with and human papilloma virus infection.Methods Four cases of SBP were studied by routine histologic,immunohistochemical staining and in situ hybridization, together with review of the literature.Results One of four lesions was squamous cell papilloma, with focal malignant change of squamous cell carcinoma with microinvasion. The case was an old woman and the cancer located in central bronchus. Others were mixed squamous cell and glandular papilloma, and two cases with features of moderate cytologic atypia. The age ranged from 25 to 73 years (average 54), and tumors were located in the bronchi and segmental bronchi. Papillary arborizing connective tissue stalks were lined by both squamous and glandular epithelium. Four papillomas were exophytic, with one case inverted partly. Four cases were examined for HPV DNA and all were negative.Conclusions SBP in adults is a rare lung neoplasm. Based on uncommon cases association with malignant change, all endobronchial papillomas should be completely excised.
3.Effect of H2O2 stress on glutathione production by Candida utilis.

Glutathione (GSH) plays an important role in the responses of microorganisms to the environmental stimulation and stress. The effect of H2O2 stress under different fermentation time and H2O2 concentration as well as continuous stress on GSH fermentation of Candida utilis were investigated in this paper. It was found that low concentration of H202 accelerated GSH production. When treated by low concentration of H2O2 (36 mmol/L), the final concentration of GSH reached 922 mg/L and the intracellular GSH content reached 1.64%, which increased by 7% and 35% than the controls, respectively.

4.Diffuse large B-cell lymphoma with aberrant expression of CD56: a clinicopathologic and immunohistochemical study.

OBJECTIVETo study the clinicopathologic features and significance of aberrant CD56 expression in diffuse large B-cell lymphoma (DLBCL).
METHODSThe clinical and pathologic profiles of 10 cases of DLBCL with aberrant expression of CD56 were investigated. Immunohistochemical staining, in-situ hybridization for Epstein-Barr virus encoded RNA and gene rearrangement for IgH and Igκ were carried out.
RESULTSThere were 6 male and 4 female patients. The medium age of patients was 46 years. All of them presented with extranodal lymphoma involvement, with gastrointestinal tract being the commonest site (5/10). Histologic examination showed that most of the atypical lymphoid cells were centroblast-like and demonstrated a diffuse growth pattern. Apoptosis and necrosis were identified in some cases. Immunohistochemical study showed that the tumor cells were positive for CD20 or CD79α and aberrantly expressed CD56. Five cases had the GCB phenotype while the remaining cases had the non-GCB phenotype, according to Hans classification. Bcl-6 was positive in most cases (9/10). All cases showed a high proliferation index by Ki-67. The tumor cells were negative for CD3ε, CD138 and granzyme B. In-situ hybridization for Epstein-Barr virus encoded RNA was performed in 7 cases and none of them showed positive signals. IgH gene rearranged bands were detected in 4 cases (4/6) and Igκ was detected in 3 cases (3/6). Follow-up data were available in 8 patients. Two patients died of disease progression within 5 to 13 months after diagnosis and the other 6 patients were alive 8 to 60 months after therapy.
CONCLUSIONSDLBCL with aberrant expression of CD56 is rare. Most of them present with extranodal involvement, show high frequency of bcl-6 expression and high proliferation index. The patients often have good response to chemotherapy.

6.The effect of ultrasound monitoring of inferior vena cava collapse index guiding fluid replacement on circulation in elderly patients during induction of general anesthesia

Objective:To investigate the effect of ultrasound monitoring of inferior vena cava collapse index (IVC-CI) guiding fluid replacement on circulation in elderly patients during induction of general anesthesia.Methods:A total of 71 elderly patients who underwent elective surgery under general anesthesia and tracheal intubation at Hunan Provincial People′s Hospital from April 2021 to September 2022 were randomly divided into control group (35 cases) and observation group (36 cases) using a random number table method. Before anesthesia, both groups of patients underwent IVC ultrasound examination and calculated the IVC-CI value. For patients with IVC-CI≥40%, the observation group was given 8 ml/kg of crystal solution before anesthesia induction, while the control group was not treated. The incidence of hypotension, the use of vasoactive drugs, and the total infusion volume from anesthesia induction to skin incision were recorded in two groups. Mean arterial blood pressure (MBP), heart rate (HR), oxygen saturation (SpO 2), cardiac index (CI), and cardiac volume variability (SVV) before anesthesia (T 0), 5 min after induction (T 1), 1 min after tracheal intubation (T 2), 5 min after tracheal intubation (T 3), 10 min after tracheal intubation (T 4), and 1 min before skin incision (T 5) were recorded and compared between the two groups. Results:The incidence of hypotension (27.8% vs 60.0%) and utilization rate of vasoactive drugs (25.0% vs 48.6%) in the observation group were lower than those in the control group, and the total infusion volume during anesthesia induction was higher than that in the control group, with statistical significance (all P<0.05). SVV, CI and MBP at T 1, T 3, T 4 and T 5 were significantly different from those at T 0 in the control group ( F=3.85, 14.66, 3.96, all P<0.05). SVV, CI and MBP at T 1, T 3, T 4 and T 5 in the observation group were significantly different from those at T 0 ( F=3.51, 13.20, 4.35, all P<0.05). There was no significant difference in SVV, CI, MBP, HR and SpO 2 between 2 groups (all P>0.05). Conclusions:For the elderly patients with preoperative IVC-CI≥40%, pre-filling with 8ml/kg crystal solution before anesthesia induction can significantly reduce the incidence of hypotension and the utilization rate of vasoactive drugs in the elderly patients during anesthesia induction.
