1.THE EFFECT OF THE COMBINATION OF MEXILETINE AND PROPRANOLOL ON VENTRICLE EXTRASYSTOLE
Chinese Pharmacological Bulletin 1987;0(01):-
A combination of Mexiletine and propranolol was administered in 40'patients suffering from ventricle extrasystole. The results suggest that the effect of the combination is superior to that of mexiletine administered alone

Objective To investigate the relationship between serum neutrophil extracellular trapps(NETs)levels and carotid intima-media thickness(CIMT)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 112 newly diagnosed T2DM patients were selected from September 2022 to May 2023.Patients were divided into the T2DM group(T2DM,n=35),T2DM+the intima-media thickening group(CIMT,n=39)and T2DM+the atheromatous plaque group(PLA,n=38)according to the CIMT.And 50 cases of healthy people were selected as the normal control(NC)group in the same period. Results Compared with the NC group, the BMI,SBP,FPG,2 hPG,TC,and NETs increased in the T2DM group(P<0.05);The BMI,SBP,HbA1c,2 hPG,TG,CIMT,HOMA-IR,and NETs were higher in the CIMT group than in the T2DM group(P<0.05),while FPG,TC,and LDL-C were higher in the CIMT group than in the NC group(P<0.05).The SBP,HbA1c,2 hPG,LDL-C,CIMT,and NETs were higher in the PLA group than in the CIMT group(P<0.05),while BMI,FPG,TG,and HOMA-IR were higher in the PLA group than in the T2DM group(P<0.05).Spearman correlation analysis showed that CIMT in T2DM patients was positively correlated with NETs,smoking,BMI,SBP,FPG,2 hPG,HbA1c,LDL-C,TC,TG,UACR and HOMA-IR(P<0.05).Multiple linear regression analysis showed that NETs,HbA1c,LDL-C and TG were independent risk factors for CIMT in the T2DM patients. The analysis of the working characteristic curve of the subjects showed that the area under the curve of serum NETs for diagnosing T2DM combined with AS was 0.960,with sensitivity of 97.4%and specificity of 84.7%.Conclusion Serum NETs are associated with atherosclerosis in T2DM patients.

Objective:To analyze the clinical characteristics and prognosis of pregnant women with hemorrhagic fever with renal syndrome (HFRS).Methods:A total of 11 pregnant women with HFRS admitted to The Second Affiliated Hospital of Xi′an Jiaotong University (four cases), The Second Affiliated Hospital of Air Force Medical University (four cases), The First Affiliated Hospital of Xi′an Jiaotong University (one case) and Central Hospital of Xianyang City (two cases) between November 2009 and February 2019 were included as the study group, and 24 age-matched non-pregnant women with HFRS were selected as the control group. The age, complications, clinical classification and laboratory indexes of the two groups were analyzed retrospectively, and the clinical outcomes of pregnant women and their fetuses in the study group were followed up. The data between two groups were compared using Mann-Whitney U test or chi-square test. Results:Patients in the study and control groups were 29 (22, 33) and 32 (24, 37) years old, respectively. Seven of 11 patients in study group were severe and critical cases, which was significantly higher than that in the control group (16.7%(4/24), χ2=7.722, P=0.015). In the study group, 10 patients had hypervolemic syndrome, 10 patients had pulmonary edema and six patients had overlapping hypotension shock phase and oliguria phase, which were all higher than those in the control group ((2/24, 8.3%), (2/24, 8.3%) and (2/24, 8.3%), respectively; χ2=22.828, 22.828 and 9.135, respectively, all P<0.01). Compared with the control group, the pregnant patients in study group had a higher urea nitrogen maximum and serum creatinine maximum, and the differences were both statistically significant ( Z=-2.453 and -2.336, respectively, both P<0.05), while they had a lower serum albumin minimum, hemoglobin maximum and hemoglobin minimum, and the differences were all statistically significant ( Z=-3.742, -3.350 and -4.034, respectively, all P<0.01). All pregnant women with HFRS recovered. Nine pregnant women gave birth to nine healthy infants. All of them received breastfeeding and the feeding duration were more than six months. No abnormal growth and development were found during an average follow-up of three years. Conclusions:Pregnancy can aggravate the severity of HFRS, and pregnant women have higher risk of the multiple stages overlap and the complications such as hypervolemic syndrome and acute pulmonary edema. After recovery from HFRS, mother may carry to full-term pregnancy.

BACKGROUNDSeveral previous studies have shown that diffusion-weighted imaging (DWI) can provide additional information for focal pancreatic lesions by demonstrating more restricted diffusion in solid malignant tumors than in chronic pancreatitis, which can be indicated by a decreased apparent diffusion coefficient (ADC). However, these studies have a modest sample size and convey inconclusive results. The aim of this study was to determine, in a meta-analysis, the diagnostic performance of quantitative diffusion-weighted magnetic resonance imaging in distinguishing pancreatic carcinoma from mass-forming chronic pancreatitis.
METHODSWe determined the sensitivities and specificities across studies. A summary receiver operator characteristic (sROC) curve was constructed to calculate the area under the curve (AUC).
RESULTSThe pooled sensitivity of DWI was 0.86 (95% CI: 0.80-0.91) and the pooled specificity was 0.82 (95% CI: 0.72-0.89). The AUC of the sROC was 0.91 (95% CI: 0.88-0.93).
CONCLUSIONSDWI may be a potentially technically feasible tool for differentiating pancreatic carcinoma from mass-forming chronic pancreatitis. However, large-scale randomized control trials are necessary to assess its clinical value.
