1.Comparative of paliperidone extended release tablets and risperidone tablets for women's glycolipid metabolism of episode schizophrenia

Objective To compare the sustained-release tablets paliperidone and risperidone tablets starting glycolipid metabolism in female patients with schizophrenia.Methods Eighty-five cases of women treated in our hospital episode schizophrenia patients were randomly divided into observation group (42 cases) and control group (43 cases).were treated with sustained-release tablets paliperidone and risperidone tablets monotherapy two months.Measuring body mass index before and after treatment (BMI),waist circumference (waist),triglyceride (TG),high density lipoprotein (HDL),fasting plasma glucose (FPG),2 h glucose after OGTT (2 h PG),Positive and Negative Syndrome scale (PANSS) for efficacy evaluation.Results Comparison minutes before treatment PANSS total score and factors,the difference was not statistically significant.After treatment,PANSS total score and factor scores,the difference was not statistically significant.Compared with the previous treatment,both groups PANSS total score and factor scores were significantly decreased after treatment,the difference was statistically significant (P < 0.05).Two groups of patients before treatment indexes,the difference was not statistically significant.After treatment in the control group TG,former TC,HDLC,LDLC,BMI,and waist circumference with treatment,the difference was statistically significant (P < 0.05);the observation group BMI and waist circumference compared with before treatment,the difference was statistically significant (P < 0.05).After observation group TG,TC,LDLC,BMI and waist circumference were significantly lower than the control group,HDLC significantly higher,the difference was statistically significant (P < 0.05).Two FPG,2hPG,SBP and DBP,the difference was not statistically significant (P > 0.05).Adverse reactions in patients in the observation group were significantly lower than the control group,the difference was statistically significant (P < 0.05).Conclusion The sustained-release tablets paliperidone and risperidone female first-episode schizophrenia patients have the same effect,but paliperidone extended release tablets in female patients improve blood lipids,BMI and waist circumference is superior to risperidone.

3.The value of dual-source CT perfusion imaging in evaluation of rabbit liver fibrosis

Objective To investigate the value of dual-source CT perfusion imaging in evaluation of liver fibrosis stage.Methods Liver fibrosis models were established with 35 rabbits (experimental group)which were intraperitoneally injected with carbon tetra-chloride.Meanwhile,other 5 rabbits were regarded as control group which were intraperitoneally inj eced with physiological saline.7 rabbits of experimental group and 1 of control group were undergone dual-source CT perfusion scan at 4,6,8,10,12 weeks respec-tively.Arterial liver perfusion (ALP),portal venous perfusion (PVP),total hepatic perfusion (THP),hepatic perfusion index (HPI)were recorded,and staging of liver fibrosis were assessed by pathological method.Results With the increase of liver fibrosis degree,HPI showed an upward trend,whereas PVP was opposite.HPIs of S2,S3 and S4 stage were significantly different com-pared to that of control group respectively.The differences of HPI between S3,S4 stage to S0 and S1 stage were significant.PVP between S2,S3,S4 stage to S0 stage and S1 stage were also significant different.Conclusion HPI and PVP can reflect the perfu-sion changes of liver fibrosis stages.Combination of HPI and PVP may help the diagnosis of S2,S3 or S4 stage of liver fibrosis.
4.Investigation of the status quo of on-the-job training of head nurses in Shandong Province

Objective To investigate the status quo and existing problems in 2A or higher level general hospitals in Shandong Province,in order to provide information for establishing and perfecting the head nurse training system of Shandong Province.Methods 666 head nurses from 129 hospitals were investigated by a self-designed questionnaire,namely The questionnaire of status quo of head nurse training in 2A or higher level general hospitals in Shandong Province.Results The head nurses participating in the survey were average (40.1±5.8) years old,66.7% of whom had intermediate titles,78.8% with vocational school as basic education,86.8% with bachelor as the highest degree.The head nurse training contents were mainly level hospital evaluation (73.9%),quality of nursing management training (65.8%) and nursing quality control (64.2%); training forms were mainly seminar (63.7%),conference (51.4%)and short-term training courses (40.5%); cumulative training time was within two weeks; 73.8% of the teachers derived from clinical nursing specialist.79.3% of the head nurses did not attend personalized evaluation before training,while 54.7% received different forms of evaluation after training.57.2% of the head nurses considered there had been problems in previous training,such as less practical knowledge (64.1%),a single mode of training (50.7%),lack of pertinence (34.6%),training teachers level was uneven (32.2%).Conclusions The systematic,hierarchical,normal,required,flexible-form,effective-evaluation and talents head nurse training system should be established to improve the nursing management level of the head nurse fundamentally.
5.Application of MR PROSET Technique in Diagnosing Spinal Nerve Roots Diseases

Purpose To investigate the clinical value of MR PROSET technique in showing and diagnosing in lumbarsacral nerve root diseases.Materials and Methods Conventional MRI sequence and PROSET sequence coronal images were performed in lumbosacral nerve roots of 15 healthy volunteers and 47 low back pain patients,each case performed 3D maximum intensity projection (3D MIP) post-processed.Spinal nerveroots were observed.Results The PROSET could show dura,dura sheath,spinal nerveroot,ganlias,segments of the nerves posterior to the ganglions well and could be observed from multiple aspects by the 3D MIP.Among the 47 cases of luber nerve root disease,PROSET could show the level of the invasion and compression to the ganglias.Conclusion According to the PROSET technique and conventional MRI picture,can provide the satisfied proof to the diagnosis of lumbosacral nerve root diseases.
6.Fragmentation behaviors of taxanes drugs using electrospray ionization with quadrupole time-of-flight mass spectrometry.

The fragmentation pathways of two taxanes drugs have been studied in positive ion mode by Q-TOF with the advantages of high mass accuracy and high resolution analysis. The [M+H] + ions were observed by ESI-MS, from which the molecular weights were obtained. Using the protonated pseudo-molecular ions [M+H]+ as internal reference compounds, the accurate mass and element composition of the fragment ions were determined. The collision induced dissociation (CID) data of the [M+H] ions provided fragmentation pathways of related compounds. Results showed that the major cleavage pathways of paclitaxel and docetaxel were the same that the cleavage of C-O bond between the side chain and taxol skeleton easily occurred, then stripping of the functional groups on the parent ring. Some common fragments were formed, such as m/z 105.033 7, 291.137 3, 309.148 5, 327.159 7, 387.181 2 and 509.217 4, which would provide a basis for future qualitative and quantitative analysis of taxanes in vitro and in vivo.
7.Analysis of colonoscopy surveillance in 5-year follow-up after polypectomy of non-advanced colorectal adenoma

Objective To assess the results of colonoscopy surveillance in 5 years after polypectomy of non-advanced colorectal adenoma and to identify its risk factors. Methods Patients undergoing colonosco-py and followed up with colonoscopy within 5 years between January 2003 and December 2013 were retro-spectively analyzed.No substance or only small quantity of clear water left in the intestinal tract and colono-scopes accessing ileocecus were regarded as complete examination. The initial colonoscopy was regarded as the baseline colonoscopy. Patients with non-advanced adenomas were assigned to the case group and those without were to the control group. Data of clinical characteristics and colorectal findings were estimated and risk factors were identified. Results A total of 828 patients were included,374 patients in the case group and 454 in the control group on baseline colonoscopy.The case group had a low incidence of advanced adeno-mas at a 1 to 5 years interval when compared with the control group,both with adequate baseline examination [1. 5%(5/ 326)VS 2. 2%(9/ 408),P = 0. 51]. The detection rates of advanced adenomas on follow-up colonoscopy at 1 to 3 years and 3 to 5 years in case group were 1. 7%(3/ 178)and 1. 4%(2/ 148),respec-tively(P>0. 05).Regression analysis showed age≥50 years old and being male were the independent risk factors for advanced adenomas recurrence within 5 years follow-up. No colon cancer was found in 828 patients during the follow-up. Conclusion Surveillance colonoscopy intervals within 5 years is of little benefit to pa-tients who had adequate polypectomy. Too early reexaminations due to concerns about advanced adenomas recurrence can be avoided.
8.Clinical and pathological features of upper gastrointestinal serrated lesions

Objective To analyze the clinical and pathological features of upper gastrointestinal serrated lesions.Methods A total of 21 patients with upper gastrointestinal serrated lesions in Tianjin Medical University General Hospital between January 2011 and December 2015 were retrospective analyzed.Data of the patients including demographics, clinical and pathological features were collected.Among the 21 patients, 18 patients, who underwent colonoscopy simultaneously or within six months, were selected as the study group, each patient was compared to 4 randomly selected controls without serrated lesions, who also underwent colonoscopy within the same time period.Differences of colorectal neoplasia detection were analyzed between the cases and controls.Results The mean age of 21 patients was 55.3±17.2 years, and 11 cases were male.Involving the locations of serrated lesions, 17 cases were found in the stomach (including 3 in the cardia, 9 in the corpus, and 5 in the antrum), and followed by 3 in the duodenum and 1 in lower esophagus.The mucosa pathological morphology showed that 6 cases were serrated hyperplasia, 8 cases were hyperplastic polyps, 6 cases were serrated adenomas with low grade dysplasia and 1 case was in the duodenum intramucosal carcinoma.Seven (38.9%) colorectal adenomas were found in the study group, including 3 (16.7%) non-advanced colorectal adenomas and 4 (22.2%) advanced colorectal adenomas.Eight (11.1%) colorectal adenomas were found in the control group, including 5 (6.9%) non-advanced colorectal adenomas and 3 (4.2%) advanced colorectal adenomas.The rate of colorectal adenoma detection in the study group was significantly higher than that in the control group (38.9% VS 11.1%, P=0.010, OR=5.091, 95%CI:1.534-16.890).Conclusion Upper gastrointestinal serrated lesions can be found in various mucosal lesions with different pathological morphologies.Moreover colonoscopy is likely to be recommended to detect concurrent colorectal adenoma for these patients.
9.Effects of Drug-containing Serum of Ficus Hirta on Oxidative Damage of Spleen Lymphocyte in Aged Mice

Objective To study the effects of drug-containing serum of Ficus Hirta on oxidative damage of spleen lymphocyte due to aging in aged mice; To discuss its mechanism of action.Methods Forty aged mice were randomly divided into control group and high-, medium- and low-dose Ficus Hirta groups. Control group was given 0.9% sodium chloride solution for gavage, while high-, medium- and low-dose Ficus Hirta groups were given 6.6, 4.4, and 2.2 g/kg aqueous extract of Ficus Hirta for gavage. The spleen index was observed for optimum dose in aged mice. The optimum time and dilution of drug-containing serum of Ficus Hirta were confirmed by MTT method in lymphocyte proliferation test. The positive rate of senescent cells, the activity of T-SOD and the contents of MDA and ROS were determined in cellular antioxidant experiment after treated by optimal drug-containing serum for 48 h. Results Compared with the control group, the spleen index was significantly improved in high-, medium- and low-dose Ficus Hirta groups (P<0.05,P<0.01). 20% drug-containing serum of Ficus Hirta cultivated for 48 h had the best effects on lymphocyte proliferation in aged mice. 20% drug-containing serum of Ficus Hirta could significantly decrease the positive rate of senescent cells (P<0.01), improve T-SOD activity and decrease the contents of MDA and ROS (P<0.05,P<0.01).Conclusion The drug-containing serum of Ficus Hirta can improve the proliferative activity of spleen lymphocyte in aged mice and the mechanism of action may be involved in decreasing the positive rate of senescent cells and increasing antioxidant ability of lymphocyte.
10.The primary study on multi-b-value DWI in diagnosis of aypical infectious diseases in liver

Objective To discuss the value of the multi-b-value diffusion weighted imaging (DWI)in diagnosis of liver tuberculosis,abscess and atypical hydatid disease.Methods 23 patients with different liver lesions including abscess in 1 1,tuberculosis in 3 and alveolar hydatid in 9 were scanned with conventional MRI and multi-b-value DWI,and the imaging results were retrospectively analyzed.The ADC1 0b maps with 10 b values,three ADClow ones with low b values of 0 s/mm2 ,50 s/mm2 and 100 s/mm2 ,three ADChigh ones with high b values of 500 s/mm2 ,750 s/mm2 and 1 000 s/mm2 were gotten respectively.The ADCperf value was defined as a difference between ADClow and ADChigh values.Two regions of interest (ROI)with the same size were drawn in the liquefied region and mar-ginal area of the mass.The ADC values of ADClow ,ADChigh ,ADCperf and ADC1 0b maps were calculated and analyzed.Differences in b value and region among the three liver diseases were compared.Results The differences in ADC10b and ADChigh values between atypical liver hydatid and liver abscess in the liquefied region and marginal area were significant (P <0.05);the differences in ADC10b and ADCperf val-ues in the marginal area among liver tuberculosis,abscess and atypical hydatid were significant (P <0.05).The differences in ADCperf value in the liquefied region between thydatid disease and abscess,and in ADChigh value in marginal area between tuberculosis and abscess were not statistically significant (P >0.05).No differences in the ADClow value were found in both areas of three lesions (P >0.05). Conclusion Multi-b-value DWI and ADC maps and ADC values can provide valuable information in diagnosis and differentiation of liver tuberculosis,abscess and atypical hydatid disease.Different ADC values play more important role in the diagnosis of liver diseases.
