1.Clinical applications of different doses of leuprorelin acetate in in vitro fertilization-embryo transfer

Objective To evaluate the clinical effect of different doses of leuprorelin acetate in in vitro fertilization-embryo transfer(IVF-ET).Methods From January 2011 to December 2011,the data of 268 patients undergoing IVF and (or) intracytoplasmic sperm injection (ICSI) in Reproductive Medical Center,Clinical College of PLA,Anhui Medical University were studied retrospectively.All the patients were divided into three groups based on with long protocol and controlled ovarian stimulation (COH) including 83cycles with 1.25 mg of leuprorelin in low dose group,68 cycles with 1.88 mg of leuprorelin in high dose group,117 cycles with 1.25 mg of diphereline in control group.The serum follicle stimulating hormone (FSH),luteinizing hormone (LH),estradiol (E2) and progesterone (P) before gonadotropin (Gn)administration on the days 3-5 of the menstrual cycle and on the day of hCG administration were detected,the dose and duration of Gn,number of oocytes retrieved,number of mature oocytes,the rates of fertilization,embryo cleaved,good-quality embryos clinical pregnancy and early miscarriage were compared among three groups.Results There were no significant differences in age,the level of LH and P on the day of hCG administration among three groups (P > 0.05).The level of FSH was (3.8 ± 1.6) U/L in low dose leuprorelin group,(3.1 ± 1.4) U/L in high dose of leuprorelin group and (2.4 ± 1.3) U/L in diphereline group before Gn administration,which reached statistical difference (P < 0.05).The mean length of Gn stimulation were (9.8 ± 1.7) days in low dose leuprorelin group,(10.5 ± 1.8) days in high dose of leuprorelin group and (11.1 ± 1.4) davs in diphereline group,which reached statistical difference (P <0.05).The mean dose of Gn was (24 ± 7) in low dose of leuprorelin group,which was significantly higher than (27 ± 9) in high dose of leuprorelin group and (28 ± 7) in diphereline group (P < 0.05).The level of LH was (2.7 ± 1.6) U/L in low dose of leuprorelin group and (2.2 ± 1.0) U/L in diphereline group before Gn administration,which reached statistical difference(P < 0.05).The cancel cycles were 5 in low dose of leuprorelin group,4 in high dose of leuprorelin group and 7 in diphereline group.The number of ovum was (14 ±7) low dose of leuprorelin group,(13 ±6) in high dose of leuprorelin group,(14 ±6) in diphereline group.The rates of fertilization was 66.26% (758/1144)in low dose of leuprorelin group,67.01% (589/879) in high dose of leuprorelin group and 68.54% (1111/1621) in diphereline group,the rates of goodquality embryos was 64.22% (472/735) in low dose of leuprorelin group,60.50% (340/562) in high dose of leuprorelin group and 59.59% (640/1074) in diphereline group,clinical pregnancy was 49% (38/78) in low dose of leuprorelin group,42% (27/64) in high dose of leuprorelin group and 50% (55/110) in diphereline group,early miscarriage was 18% (7/38) in low dose of leuprorelin group,15% (4/27) in high dose of leuprorelin group and 15% (8/55) in diphereline group,which did not show significant differences (P >0.05).Conclusions Both 1.25 mg and 1.88 mg leuprorelin acetate could obtain good downregulation effect and clinical outcomes.1.25 mg leuprorelin acetate could decrease patient's costs by reducing Gn dose and duration.
2.Clinical significance of subclavian artery angiography in arterial embolization for hemoptysis

Objective To study the subclavian artery angiography for hemoptysis and its clinical value.Methods Thirty-eight cases of hemoptysis undertook subclavian artery angiography after embolization of bronchial arteries and branches of thoracic aorta. Group A: 4 recurrent cases underwent subclavian artery angiography (unilateral: n =2, bilateral: n =2). Group B: 16 cases underwent subclavian artery angiography according to the manifestation on X-ray and CT (right: n =10, left: n =6). Group C: 18 cases underwent bilateral subclavian artery angiography.Results of subclavian artery angiography were divided into three classes: normal(-),chaotic and hyperplasia of small branchi vessels(+),obviously bleeding (++).Some of the obviously bleeding cases were embolized and analysed for clinical efficacy and complications. Results Bleeding cases accounted for 63.2%(24/38)and bleeding subclavian arteries accounted for 50%(29/58), including(++):37.9%(22/58) and(+): 12.1%(7/58). Positive rate of chronic fibro-cavitary pulmonary tuberculosis was the highest. Bleeding sites were coincided with lung lesions. Twelve cases were embolized with the immediate cessation rate of hemoptysis reaching 100%. Eight cases with long-term follow-up showed 5 cured, 1 with significant effect and 2 recurrent. The complications occurred with fever, vomiting, chest pain, hiccup and dyspnoea.Conclusions Subclavian artery angiography has important clinical value in artery embolization for hemoptysis, especially for lesions in the upper lobes of lungs and with more fruitful result for chronic fibro-cavitary pulmonary tuberculosis.
3.Study on morphologic quantitative characteristics of Oncomelania hupensis Ⅲ. Morphologic variations and their spatial correlation among snail populations

Objective To explore morphologic variations and their spatial correlation among Oncomelania hupensis populations from the mainland of China. Methods Twenty-seven snail populations from ten provinces were collected. The morphologic indices of shells and geographic distances between the snail spots were measured respectively, and the morphologic variations among snail populations and the correlations between morphologic variations and geographical distances were analyzed. Results The median of Euclidean distances was 3.24 (95% CI: 2.88-3.81) among 20 snail populations of O.h.hupensis including a snail population from Guangxi Province, and was 3.15 (95% CI: 2.86-3.76) excluding the Guangxi snail population; the median was 1.69 (95% CI: 1.33-2.27) among 6 populations of O.h.robertsoni. Whether including a snail population from Guangxi Province or not, the median among O.h.robertsoni was less than that among O.h.hupensis (P0.05). The correlation coefficient was 0.2655 for O.h.hupensis including a snail population from Guangxi Province, 0.2567 not including it; and was 0.3121 for ribbed-shell population. Conclusions Morphologic variation among O.h.hupensis is bigger than that among O.h.robertsoni, and morphologic variation among ribbed-shell populations is bigger than that among smooth-shell populations. There is significant spatial structure among ribbed-shell snail populations of Oncomelania hupensis distributed in the mainland of China.
4.Dynamic analysis of distribution of Oncomelania hupensis in mountainous regions

Objective To study the distribution of Oncomelania hupensis snails in mountainous regions and the dynamic charaeteristics of the distribution.Methods An environment calledLanbaoclosed to Puge County.Sichuan Provinee WaS selected as the study field.Random sampling was designed to determine the investigation sites.The snails were collected and the hying snaila were identified by the method of dissection in the laboratory.The distribution of snails was analyzed by some statistical indices,such as mean,variance and so on.Then the negative binomial distribution.log-normal distribution and exponential distribution were fitted to the snail data by the method of maximum likelihood estimation to explore the snail distribution in different time.Results The negative binomial distribution was fitted well to the snail data in April,May,July,August,September,November in 2008,and no distribution was fined to the snail data in June,October.December in 2008 and February in 2009.Conclusions The distribution of Oncomelania hupensis in mountainous regions is not simple negative binomial distribution,but pwbably a dynamic process and an uncertain distribution.
5.Inhibitory effects of LDL-ACM complex on subcutaneous implanted tumors in nude mice

AIM: In order to evaluate the applicable value of LDL as a targeted vehicle for chemotherapeutic agents, we investigated and compared the inhibitory effects of LDL-ACM complex and free ACM on nude mice's subcutaneous implanted tumors derived from gastric cancer cell lines, SGC-7901 and NKM-45. METHODS: LDL-ACM complex was prepared and the tumor model of nude mice was established by subcutaneous implantation of SGC-7901 and NKM-45. Then, the groups of nude mice developed subcutaneous implanted tumors were received either LDL-ACM complex or free ACM. Subsequently, the tumor size, weight and leukemia cell counts were measured and the rates of tumor-inhibition and the survival were compared among the groups. RESULTS: The inhibitory effects of LDL-ACM complex on the tumors, especially on SGC-7901 implanted tumors were much more obvious than that of free ACM. It was also indicated that the action of LDL-ACM complex was mediated by LDL receptor. CONCLUSION: These results showed that LDL-ACM complex had significant inhibitory effects on the implanted tumors and the effect might be mediated by LDL receptor.
6.Angiographic manifestation and transcatheter arterial embolization of proper esophageal artery in hemoptysis

Objective To investigate the angiographic manifestation of the proper esophageal artery (PEA),the hish risk factom for the presence of the anomalous PEA in hemoptysis and to evaluate the safety of transcatheter aaefial embolization(TAE) of the PEA using gelatin sponge(GS).Methods Selective esophageal arteriography WSS performed in forty-three patients with hemoptysis,including 15 cases of pulmonary tuberculosis,18 cases of bmnchiectasis,7 cases of posttuberculous bronchiectasis and three cases of lung cancer. One case experienced failure of bronchial arterial embolization. The angiographic manifestation of the PEAs Was studied.The complications of the procedure and clinical results were observed in the patients who underwent TAE using GS.Results Thirty-nine PEAs were catheterized selectively in 37 patients(86.0%).Eighteen anomalous PEAs(46.2%)were catheterized selectively in 17 patients (45.9%).The anomalous PEAs showed tortuosity,dilatation,hyperplasia,shunting with pulmonary artery and anastomosis with the bronchial artery.All lesions involved basal segment of inferior pulmonary lobar. Bronchiectasis Was the most frequent disease for PEA abnormality. No complications occurred and satisfactory curative effect Was achieved with TAE of the anomalous PEAs.Conclusions It is necessary to perform selective proper esophageal arteriography when the lesion involves basal segment of inferior pulmonary lobar in hemoptysis.Supplemental TAE of the anomalous PEA using GS is safe and valuable in the management of hemoptysis.

Using engineering fluid dynamics (EFD) softwares,we conducted a study on the simulation of flow field in vessel which was partially blocked by a clamp. The flow field of the partial block vessel was studied once more in real vessel when methylene blue as a marker was injected into the vessel. The change in the characteristics of blood flow was observed by way of computer imitation and methylene blue show. It was found that some defect might appear in the conventional method which partially blocked the vein.

10.The application of preoperative neoadjuvant chemotherapy in patients with stageⅢA non-small cell lung cancer

Objective To analyze the clinical efficacy of neoadjuvant chemotherapy in patients with stageⅢA non-small cell lung cancer (NSCLC). Methods From March 2008 to October 2015, there were 92 cases of stageⅢA NSCLC patients received 2 cycles of neoadjuvant chemotherapy and underwent radical surgery for lung cancer 3-4 weeks late ( observation group) , and another group of 65 cases of stage ⅢA NSCLC patients ( control group) underwent surgery for lung cancer without preoperative chemotherapy. The clinical data as well as early and meddle term surgical outcome of both groups were analyzed retrospectively. Results The neoadjuvant chemotherapy effective rate was 73. 9% in the observation group. All surgeries for lung cancer patients were undertaken either with video-assisted thoracotomy or traditional thoracotomy. No operative mortality and the postoperative pathology findings were in accordance with NSCLC. The bleed-ing amount and operation time of two groups were similar; the resection rate of the observation group was greater than that of the control group. All patients were followed up at least 3 years, and the 2 years and 3 years survival rate of the observation group was slightly higher than that of the control group. Conclusions Preoperative neoadjuvant chemotherapy in patients with NSCLC can improve the resection rate and prolong the survival time, which is worthy of clinical application.
