1.Bushen-Huoxue decoction combined with oral calcium and vitamin D for senile osteoporosis

Objective To evaluate the therapeutic effect ofBushen-Huoxue decoction combined with oral calcium and vitamin D for senile osteoporosis.Methods A total of 120 patients with OP were enrolled and randomly divided into aBushen-Huoxue decoction group (60 patients) and a elcatonin group (60 patients). Routine use of calcium and vitamin D was adopted. On this basis, theBushen-Huoxue decoction group was treated byBushen-Huoxue decoction combined with oral calcium and vitamin D, while the elcatonin group was treated by intramuscular injection of elcatonin with oral calcium and vitamin D. VAS of low back pain and bone joint pain was evaluated, the changes of BMD in the lumbar spine and femoral neck was observed, serum calcium and phosphorus, ALP concentration changes were also observed.Results After the treatment, the low back pain in the Bushen-Huoxue decoction group (3.30 ± 0.38vs.4.32 ± 0.51,t=11.148), bone joint pain (3.15 ± 0.22vs.3.87 ± 0.26,t=11.880) VAS were significantly lower than those in the elcatonin group.Bushen-Huoxue decoction group L2-4is BMD (L2: 0.74 ± 0.15 g/cm2vs.0.63 ± 0.14 g/cm2,t=4.153; L3: 0.75 ± 0.13 g/cm2vs. 0.70 ± 0.10 g/cm2,t=2.361; L4: 0.76 ± 0.06 g/cm2vs.0.71 ± 0.10 g/cm2,t=3.321) were significantly higher than those in the elcatonin group(P<0.05).Bushen-Huoxue decoction group serum calcium (2.36 ± 0.45 mmol/Lvs. 2.19 ± 0.17mmol/L,t=2.737), serum phosphorus (1.28 ± 0.12 mmol/Lvs.1.22 ± 0.13 mmol/L,t=2.627) were significantly higher than those in the elcatonin group (P<0.05), ALP (89.20 ± 9.41 U/Lvs.114.00 ± 11.80 U/L, t=12.733) was significantly lower than that in the elcatonin group (P<0.01).ConclusionBushen-Huoxue decoction can effectively improve the clinical symptoms of senile osteoporosis, and elevate BMD content. Its efficiency was better than intramuscular injection of elcatonin.

The effect of gelatin on anaero-cultivation of Bifidobacterium adolescent is and stabilization of its living cell in dried preparation was investigared. It was shown that the well-distributed mixture of gelatin and medium could have a cushioning effect on pH reduction resulting from acid-production while incubating, and the dried mixture comprising living cell of bifidobacterium adolescentis was prepared, in which the stability and viability of cell come from cultures of gelatin mixture was increased compared with that without gelatin mixture. It was shown that oxygen toxicity to anaerobic cell was well reduced because of cell immobilization by the gel mixure after drying with saccharide substance , and then fermentable dynamics in 50L fermentor was tested as well as the relationship of growth kinetics, pH, dissolved oxygen rate, and content of soluble solids.
3.Watchful questions on diagnosis and treatment of traumatic diaphragmatic rupture and diaphragmatic hernia

Objective To explore the clinical characteristics and the treatments on traumatic diaphragmatic rupture and diaphragmatic hernia.Method A retrospective study was carried out in 49 cases of traumatic diaphragmatic rupture and diaphragmatic hernia.Results There were 32 cases with left diaphragmatic hernia,13 cases with right diaphragmatic hernia and 4 cases with bilateral diaphragmatic hernia.One case with closed combined thoraco-abdominal wound and shock,died of multiple organ failure after the surgery,3 cases were diagnosed more than 24 h after injury,the others closed injury patients were diagnosed within 24 h and cured.In patients with open diaphragmatic rupture and diaphragmatic hernia,16 cases underwent laparotomy surgery,5 cases of misdiagnosis experienced re-thoracotomy,7 cases thoracotomy,2 cases changed the thoraco-abdominal surgery.Three cases underwent thoraco-abdominal surgery.Twenty-five cases were cured; 1 patient with suppurative costal chondritis,duration up to 13months.Conclusion Traumatic diaphragmatic rupture and diaphragmatic hernia is easily missed and misdiagnosed,the establish of rational examination preoperatively can reduce complications,misdiagnosis and mortality.
4.Simultaneous determination of nine phenolic acids in Salvia miltiorrhiza by RRLC-UV.

OBJECTIVETo establish a RRLC-UV method for simultaneous determination of nine phenolic acids in Salvia miltiorrhiza.
METHOD3,4-Dihydroxyphenylacetic acid was used as internal standard, and analysis was carried out on an Agilent Zorbax Eclipse Plus C18 (2.1 mm x 50 mm, 1.8 microm) column with acetonitrile-0.1% formic acid aqueous solution as mobile phases in gradient elution. The flow rate was 0.3 mL x min(-1), and the UV detector was monitored at 286 nm.
RESULTAll calibration curves showed good linear regression within test ranges (r > or = 0.999 5); and the overall recoveries were in the range of 98.2%-102.6%, with RSD less than 3.1% (n = 3). The overall RSD of precision test were less than 2.4%.
CONCLUSIONThe developed method was simple, rapid, accurate and reproducible, and can be used for the quality control of S. miltiorrhiza.

5.Comparative Study on Electroacupuncture Versus Warm Needling for Low Back Pain

Objective To observe and compare the clinical efficacies between electroacupuncture and warm needling in treating low back pain.Method Seventy-eight eligible low back pain patients were randomized into group A of 28 cases, group B of 26 cases, and group C of 24 cases. Group A was intervened by electroacupuncture, group B was by warm needling, and group C was by medication. The short-form McGill Pain Questionnaire, Japanese Orthopaedic Association Scores (JOA), and Oswestry Disability Index were observed before and after treatment, and the therapeutic efficacies were compared.Result In group A, the McGill item scores [Sensory Pain Rating Index (S-PRI), Affective Pain Rating Index (A-PRI)] respectively after 1-week and 2-week treatment as well as in the 1-month and 3-month follow-up were significantly different from that before treatment (P<0.01,P<0.05). In group B and C, the McGill item scores after 2-week treatment and in the 1-month and 3-month follow-up were significantly different from that before treatment in the same group (P<0.01,P<0.05). The JOA and Oswestry scores were significantly changed respectively after 1-week and 2-week treatment and in the 1-month and 3-month follow-up in the three groups compared with that before treatment (P<0.05,P<0.01). After 1-week and 2-week treatment and in the 1-month and 3-month follow-up, the JOA and Oswestry scores in group A were significantly different from that in group C (P<0.05,P<0.01). In the 1-month and 3-month follow-up, the JOA scores in group B were significantly different from that in group C (P<0.05). The total effective rate was 85.7% in group A and 73.1% in group B, both significantly higher than 58.3% in group C (P<0.05). Conclusion Electroacupuncture and warm needling both can produce a significant efficacy in treating low back pain, but warm needling acts comparatively slowly and is less safe.
6.Determination of carnosic acid in rat stomach and intestine by high performance liquid chromatography method.

OBJECTIVETo establish a HPLC method to determine the carnosic acid in the stomach and intestine of rats and study its tissue distribution characteristics.
METHODAfter intragastric administration of carnosic acid (90 mg x kg(-1)), rats for each time-point were sacrificed by decapitation. After removal of the blood, various tissues were rapidly removed and weighted, all tissues were treated with a series of pretreatment before HPLC. Chromatographic separation was achieved on a Kromasil C18 column (4.6 mm x 150 mm, 5 microm) protected by an ODS guard column at 25 degrees C, using acetonitrile-0.1% phosphoric acid solution (55:45) as mobile phase, at a flow rate of 1 mL x min(-1). The wavelength of the UV detector was set at 210 nm for carnosic acid and internal standard.
RESULTGood linearities were obtained in every tissue over a range of 0.3212-160.6 mg x L(-1). The recovery, intra-day and inter-day precision and accuracy of three concentrations of carnosic acid in tissues met the requirements of methodology. And the stability of the tissue samples were also validated. The results of distribution in stomach and intestine showed that the highest concentration was (307.1 +/- 119.2) microg x g(-1) in stomach and (33.32 +/- 17.70) microg x g(-1) in intestine after intragastric administration of carnosic acid.
CONCLUSIONThe HPLC method was established to determine the concentration of carnosic acid in tissues. This method is quick, precise, and reproducible. It is the first time to study the tissue distribution of carnosic acid in rats after intragastric administration.

8.Significance of carbonic anhydrase IX protein in fibrotic foci of mammary invasive ductal carcinomas.

OBJECTIVETo study the relationship between fibrotic focus (FF) and carbonic anhydrase (CA) IX in invasive ductal carcinomas (IDC) of the breast.
METHODSIn 167 cases of IDC, the FF was assessed morphologically, and expression of ER, PR and CA IX was evaluated using MaxVision immunohistochemistry.
RESULTSThe expression of CA IX in IDC with and without FF was 56.3% (45/80) and 28.7% (25/87) respectively, with significant difference (P=0.001). In IDC with FF, the CA IX expression of tumor cells in tumors with CA IX-positive fibroblasts (35/40, 87.5%) was significantly (P<0.001) higher than that in tumors with CA IX-negative fibroblasts (10/40, 25.0%). In IDC with FF, the CA IX expression of fibroblasts of FF in grade 3 IDC (23/33, 69.7%) was significantly (P=0.006) higher than that in grade 1+2 tumors (17/47, 36.2%). The ER and PR expression of tumor cells in tumors containing CA IX-negative fibroblasts was 72.5% (29/40) and 65.0% (26/40) respectively, whereas the ER and PR expression of tumor cells in tumors containing CA IX-positive fibroblasts was 50.0% (20/40) and 42.5% (17/40) respectively; the difference was statistically significant (for both ER and PR, P=0.04). The age of patients with tumors containing CA IX-negative fibroblasts was significantly (P=0.002) older than those containing CA IX-positive fibroblasts. The FF diameter/tumor diameter in tumors containing CA IX-positive fibroblasts was significantly larger than those containing CA IX-negative fibroblasts. (3) For the groups of tumor size≤2 cm and tumor size between 2 cm to 5 cm, the diameter of the fibrotic focus was significantly (P<0.01) smaller than the fibrotic focus size of tumors>5 cm in size.
CONCLUSIONSCA IX expression is correlated with FF, and that in fibroblasts of FF correlated with patients' age, tumor grade, hormone receptors and FF diameter/tumor diameter. CA IX expression in FF might be a marker for poor prognosis in patients with breast cancer.
