1.Curative effect and safety of the adjuvant therapy of bifidobacterium bifidum-sine capsules to helicobacter pylori positive peptic ulcer

Objective To discuss the curative effect and safety of the adjuvant therapy ( AT) of bifidobacteri-um bifidum-sine capsules to helicobacter pylori ( HP) positive peptic ulcer .Methods 76 HP infected peptic ulcer patients were selected and divided into the observation group and control group at random .The patients in control group were given traditional triple therapy of omeprazole ,amoxicillin and carat enzyme for 4 weeks,while the patients in observation group were given bifidobacterium bifidum-sine capsules(420mg) three times daily for 4 weeks.Clinical symptom remission,ulcer healing and HP clearance of patients in two groups were observed and compared .The inci-dence rate of drug adverse reaction ( DAR) during treatment period was observed ,and the recurrence rates of peptic ulcer within half a year and one year were followed up .Results The HP clearance rate of the observation group was significantly higher than control group (χ2 =4.21,P<0.05).The incidence rate of DAR of the observation group was 10.26%,which was significantly lower than 29.73%of the control group (χ2 =4.55,P<0.05).34 cases in the ob-servation group and 30 cases in the control group were healed ,these patients were followed up for half a year and one year,and the recurrence rate of peptic ulcer in observation group was significantly lower than that in control group (χ2 =4.51,5.23,all P<0.05).Conclusion The bifidobacterium bifidum-sine capsules combined with PPI triple therapy,can increase the eradication rate of HP ,reduce DAR and the recurrence rate of ulcer .

3.Reverse-transcription polymerase chain reaction of cytokeratin 19(CK19) in detection of circulating tumor cells in breast cancer

Objective:To evaluate the value of CK19 mRNA detection in diagnosis of breast cancer. Methods: Patients were categorized into 3 groups:Group 1 consisting of 77 stageⅠ/Ⅱbreast cancer patients, group 2 consisting of 77 stage Ⅲ/Ⅳ breast cancer patients, and control group consisting of 40 non-cancer volunteers. Total RNA was extracted from peripheral blood cells with the RNA Kit as described by the manufacturer, which was followed by one step RT-PCR of RNA. Agarose gel electrophoresis and sequencing were carried out to confirm the amplification of expected sequence (314 bp). ?~2 test was used to compare CK19 mRNA positive rates in different stages of breast cancer. Results: 15/77 (19.48%) of stage Ⅰ/Ⅱ breast cancers patients, 31/77 (40.26%) of stage Ⅲ/Ⅳ breast cancers patients and 2/40 (5.00%) of non-cancer volunteers demonstrated positive RT-PCR results. The positive rate of CK19 mRNA in peripheral blood of stage Ⅰ/Ⅱ or stage Ⅲ/Ⅳ breast cancer patients was significantly higher than that of non-cancer volunteers(P=0.037, P=0.001). The positive rate of CK19 mRNA in peripheral blood of stage Ⅲ/Ⅳbreast cancer patients was significantly higher than that of stageⅠ/Ⅱbreast cancer patients(P=0.005). Conclusion: CK19 RT-PCR is a promising method for detecting circulating tumor cells in patients with breast cancer, which may provide valuable information for tumor staging and treatment.
4.Therapy progress of patent ductus arteriosus in low-birth-weight premature infants

Patent ductus arteriosus( PDA) has high morbidity in new-borns. This disease tends to occur in premature,especially in the low-birth-weight infants. It can lead to severe complications including intracranial hemorrhage,respiratory distress syndrome,and chronic lung disease without treatment. Treatment for PDA usual-ly involves medications,operations,interventions and symptomatic treatment. This paper summarizes the progres-ses of the therapy of PDA in low-birth-weight premature infants.
5.Treatment of congenital heart disease after cardiopulmonary bypass in infants

Objective To study the treatment and outcome in infants with congenital heart disease after cardiopulmonary bypass. Methods The clinical data of 217 infants with congenital heart disease after cardiopulmonary bypass in Shengjing Hospital from Jan 2005 to Nov 2010 were retrospectively reviewed. Results Eleven infants died and the mortality was 5. 1% ( 11/217 ). Four infants died of severe low cardiac output syndrome,2 died of pulmonary hypertension crisis,2 died of low cardiac output syndrome complicated with acute renal insufficiency, 1 died of severe infection, 1 died of severe hypoxemia,and 1 died of chronic pericardial tamponade. The postoperation length of hospital stay was 7 ~ 52 d, 14 d in average. Conclusion With the improvement of surgical techniques,cardiac surgery with cardiopulmonary bypass on infants can result in good clinical effect. Active and effective treatment can reduce the perioperative mortality after cardiopulmonary bypass.

7.Microanatomy and virtual anatomy of retrosigmoid approach

Objective To explore the related structures of retrosigmoid approach by microanatomy and virtual anatomy and provide a reliable approach with more morphologic data. Methods Twenty sides of 10 adult cadaveric heads were dissected to simulate retrosigmoid approach under the microscope. The neurovascular structures of pontocerebellar trigone were observed, and the related structures were simultaneously measured based on the junction of superior petrosal sinus and sigmoid sinus and internal acoustic pore. The internal auditory canal was opened by drilling the margin of the internal auditory meatus and its structures were watched. With the help of Dexotroscope system, the data of five patients' CT and MRI were applied to reconstruct and anatomize the structure of retrosigmoid approach. Results It was found that the distance from the junction of superior petrosal sinus and sigmoid sinus to the trigeminal nerve was (38.50±2.64)mm, to the acoustic-facial bundle (27.80±2.25)mm, to the glossopharyngeal nerve (32.70±2.11)mm, to the hypoglossal nerve (44.30±2.05)mm, and the distance from internal acoustic pore to the trigeminal nerve was (5.68±1.55)mm, to the abducent nerve (13.80±1.81)mm, to the tentorium of cerebellum (5.00±0.66)mm, to the glossopharyngeal nerve (6.34±1.24)mm. The pontocerebellar trigone was divided into the anterior compartment, the middle compartment, the posterior compartment built on the acoustic-facial bundle and the glossopharyngeal nerve. Their structures were displayed after drilling the margin of the internal auditory meatus. Dexotroscope system clearly displayed asterion, the angle of transverse and sigmoid sinus, jugular foramen, internal acoustic pore, basilar artery and its branches, and theirs spatial relationship.Conclusion The three compartments of the pontocerebellar trigone are helpful to understand the feature of the neurovascular layer, the measurement is favorable to quantize the relation of the related structures and to judge the space of each compartment. Recognizing the anatomical marker of internal acoustic pore can support preservation of the inner structures. Virtual anatomy of Dexotroscope system can display local anatomical structure respectively. Both microanatomy and virtual anatomy have their advantages and disadvantages respectively and integration can be beneficial to understand neurovascular structures in the pontocerebellar trigone.
8.Transurethral urethrotomy with the contact Nd:YNG laser for urethral stricture and obliteration.

Objective To evaluate the short ferm effect of transurethral urethrotomy with the contact Nd:YNG laser for urethral stricture and obliteration. Methods 26 patients with urethral stricture or obliteration were treated with transurethral urehrotomy from August 1993 to October 1999.All patients underwent transurethral urethrotomy with the contact Nd:YNG laser delivered with the 600?m bare fiber at 30 to 50W.The urethrotomy was guided only by a metal sound introduced through the suprapubic tract. Results Satisfactory voiding was achieved after catheter removal in all patients.26 patients were followed up for 6~24 months.Normal micturation was observed in 24 patients(92.3%).Recurrent stricture occurred in 2 patients (7.7%).Of the 2 patients one needed periodical urethral dilatation and the other required repeat internal urethrotomy.The only complication was epididymitis in 2 patients.Urethroscopy was performed in 18 patients 4~12 weeks after catheter removal.Urethrotomy revealed a widely patent uretha with complete epithelialization at the site of the previous urethrotomy. Conclusions Transurethral urethrotomy with the contact Nd:YNG laser is a safe and effective treatment for urethral stricture and obliteration.The rate of stricture recurrence is relatively low.Complications not frequeutly seen and satisfectory short-term effect can be obtained.
9.Action machanism of SIRT1 involves TNF-α-induced intestinal epithelial barrier destroy

Objective To observe the influence of silent information regulator factor 1(SIRT1)on TNF-αinduced intestinal epi-thelial Caco-2 cell barrier function destroy and to investigate its molecular machenism.Methods Caco-2 cells were randomly divided into three groups:normal control group (control),TNF-αgroup (TNF-α,100 ng/mL for 24 h)and 100 ng/mL plus 40μm resvera-trol group (TNF-α+Res).Transepithelial electrical resistance (TER)was determined.SIRT1 and the protein expressions of ZO-1 , occludin were examined by using Western blot.Results The relative expression amounts of SIRT1 protein were 0.81 ± 0.02, 0.43±0.04 and 0.60±0.03 respectively.TER of three groups were (154.00±5.00),(97.00±4.00)and(128.00±6.00)Ohm/cm2 respectively.Compared with the control group,the expression of SIRT1 protein was reduced by 47% and TER was decreased by 37.00% in the TNF-αgroup.After resveratrol precondition,TER was increased by 32.00% compared with the TNF-αgroup. The relative expression amounts of ZO-1 and occludin protein in the control group,TNF-αgroup and TNF-α+Res group were (0.62±0.06,0.57±0.03),(0.23±0.05,0.33±0.04)and(0.41±0.03,0.50±0.02)respectively.After TNF-αtreatment,the ex-pressions of ZO-1 and occludin protein were significantly deceased(P<0.05),but the resveratrol precondition could attenuate this phenomenon,compared with TNF-αgroup,the protein expression was increased by 78.00% and 51.00% respectively (P<0.05). Conclusion Under the condition of TNF-αtreatment,the SIRT1 level is decreased,but increasing SIRT1 level could increase the in-testinal tight j unction protein ZO-1 and occludin protein expression,thus alleviate the damage of TNF-αon the epithelial barrier function constituted by Caco-2 cells.
10.99mTc-EHIDA hepatobiliary scintingraphy and treatment of obstructive jaundice in infants

Objective To discuss the diagnosis,differential diagnosis and treatment of obstructive jaundice in infants.Methods Infants with obstructive jaundice were examined by ECT to obtain dynamic images of the liver,biliary tract and intestine to find out the position and nature of the obstruction.Results Eight patients with delayed nuclide imagining were diagnosed as obstructive jaundice of cholestasis.Five of which with aggravated jaundice were cured by biliary irrigation and 3 by medication.For 16 patients with congenital biliary atresia,10 were exterior hepatic types and 6 interior hepatic types.They had the nuclide image of liver and biliary tract.Ten cases of the exterior hepatic types and four cases of the interior types received the Kasai operation.And bile was drained after the surgery.Seven patients survived and 4 of interior heaptic types died postoperatively.Due to bile fistula,3 patients died after the hepato-porto-jejunostomy for internal drainage.Two patients of interior hepatic types who had no operation died of the hepatic failure.Conclusion The nature and position of obstructive jaundice could be identified by nuclide imaging.Young patients with jaundice of cholestasis could be treated by antiinflamation,liver function protection,Hymecromone and Dexamethasone for two weeks.Once the serum bilirubin rises,operation would be indicated.In the case of congenital biliary atresia,operation should be done earlier on the basis of medication.
