1.Coverage of the thumb soft tissue defect by five kinds of flaps

Objective To explore the curative effect and indication in coverage of the thumb soft tissue defect by five kinds of minute flaps. Methods From January 1998 to March 2003, 52 cases with skin and soft tissue defect of the thumb were treated by five different kinds of flaps: 5 underwent the wrapping flap with nail from the big toe, 15 with the reversible thumb dorso-ulnar artery island flap, 10 with the thenar flap, 8 with the lateral digital island flap, 14 with the index finger dorsal island flap. There were 40 males and 12 females, aged from 17 to 35 years (average, 25 years). The maximum area of the repair flap was 5.0 cm?2.5 cm, and the minimum area was 2.0 cm?1.5 cm. Results All flaps survived. After 6 to 36 months postoperative follow-up, with an average of 20 months. The blood supply and texture of the flaps were excellent, their outer aspect and function were satisfactory. Among 46 cases of flaps with their proper or connecting nerves, the average distance of the two-point discrimination was 5.8 mm in the wrapping flap with nail from the big toe, 8.5 mm in the reversible thumb dorso-ulnar artery island flap, 7.0 mm in the thenar flap, 6.2 mm in the lateral digital island flap, 8.8 mm in the index finger dorsal island flap. Of the 6 cases that the nerves were not connected, the healed flaps had only the pain and touch sensation, the two-point discrimination was poorly recovered. Conclusion The five kinds of flaps are safe and reliable for repairing thumb soft tissue defect, among which the wrapping flap with nail from the big toe and the lateral digital island flap have the advantage of the best sensation recovery, the reversible thumb dorso-ulnar artery island flap and the thenar flap are easy to perform with minimum damage. Better curative effect can be obtained by choosing different flaps according to the site of tissue defect.
2.The diagnostic value of positive 99Tcm-MIBI tumor imaging for thyroid cancer

Objective To evaluate the value of positive 99Tcm-MIBI tumor imaging for thyroid cancer diagnosis. Methods Fifty four suspected thyroid cancer patients underwent 99Tcm-O4 and 99Tcm-MIBI combined imaging procedure. The imaging data were confirmed by pathological findings. Results All the 54 cases had single throid nodules, and 25 of which were pathologically malignant. Fifty two cases of nodules were detected by the 99Tcm O4 thyroid static imaging, including 2 hot nodules,4 warm nodules, 10 cool nodules and 36 cold nodules;2 cases were negative by the imaging. Of the 25 malignant thyroid nodules, 16 nodules were visible by 99Tcm-MIBI uptake and were cold nodules;29 exhibited benign thyroid nodules,of which 15 could be seen by 99Tcm-MIBI uptake,including 1 warm nodules,2 cool nodules and 12 cold nodules. The sensitivity, specificity of the combined imaging of 99Tcm O4 and 99Tcm-MIBI were 64. 00% ( 16/25 ) and 48. 28% (14/29). No significant difference was found for the positivity between benign nodules and malignancy nodules by 99Tcm-MIBI tumor imaging ( χ2 = 0. 83, P > 0. 05 ). Conclusion 99Tcm-MIBI tumor imaging is not specific for the diagnosis of thyroid malignancy.
3.Experimental study of hypothyroidism effects on blood lipid levels

Objective To observe the effects of short-term withdrawal of thyroxine on lipid level and the protective effect of atorvastatin.Methods The study included 60 rabbits which were randomly divided into treatment experiment group 1 (near-total thyroidectomy + 131 I ablative therapy),treatment experiment group 2 (near-total thyroidectomy + 131I ablative therapy + Atorvastatin intervention) and control group(sham thyroidectomy).Compared the thyroid functions and lipid levels among different groups at the points of before surgery.three weeks after surgery and five weeks after surgery.Results (1) Compared to before surgery,the thyroid function at the points of three weeks and five weeks after surgery were obvious reduced both in treatment group.(2) The leves of TG,TC,LDL-C in EG1 were increased gradually with the extension of time after surgery.Compared with the point of before surgery,the level of HDL-C at the point of five weeks after surgery was significantly declined in EG1.Compared with the point of before surgery,the level of TG at the ponts of three weeks and five weeks after surgery was significantly declined in EG2.Compared with the point of before surgery,the level of LDL-C at the point of three weeks after surgery was significantly declined in EG2.Conclusions (1) Short-term hypothyroidism can increase the levels of TG,TC,LDL-C in plasma.(2) Atorvastatin can maintain the stability of blood lipids during Short-term hypothyroidism.
4.Expression of EphB4 and VEGF in esophageal cancer tissues and their relationship with microvessel density and the curative effect under thoracoscope

Objective To explore the expression of EphB4 and VEGF in esophageal cancer tissues and their relationship with microvessel density (MVD ) ,and analysis the curative effect of postoperative esophageal cancer radical under thoracoscope . Methods Theexpression of EphB4 and VEGF was detected by immunohistochemistry in tumor specimens from 76 cases of esopha-geal squamous cell carcinoma and paratumor normal specimens ,used CD34 as marker to count MVD .According to the situation of expression of EphB4 and VEGF ,we analysis their relationship with lymph node metastasis rate ,recurrence and 5-year survival rate . Results The positive expression rate of EphB4 and VEGF in cancerous tissue (57 .89% and 61 .84% ) ,were significantly higher than that in tissue adjacent to carcinoma(0 and 7 .89% )(P<0 .05) .The positive expression rate ofEphB4 and VEGF in high MVD values of patients (67 .44% and 76 .19% ) ,were significantly higher than thatin low MVD values of patients (45 .45% and 44 .11% )(P<0 .05) .The positive expression rate ofEphB4 and VEGF in the patientswith lymph node metastasis group and associ-ated with recurrence ,were significantly higher than that of group without lymph node metastasis and group without recurrence (P<0 .05) .The positive expression rate of EphB4 and VEGF in patients of greater than or equal to 5 years of survival rate(45 .00% and 45 .45% ) ,were significantly lower than in patientsof Less than 5 years of survival rate (80 .36% and 85 .19% )(P<0 .05) .Conclu-sion EphB4 and VEGF are highly expressed in esophageal cancer tissue ,which may be closely associated withmicrovessel density , and lymph node metastasis ,recurrence and 5 years survival rate ;the curative effect of positive expression rate of EphB 4 and VEGF is poor .
5.Chemical constituents and bioactivity of Kalimeris indica.

OBJECTIVETo study the constituents of Kalimeris indica and its bioactivity.
METHODThe constituents were isolated by various chromatographic techniques and their structures were elucidated by their physicochemical properties and the spectral data analysis. Use the disk diffusion method, see whether a sample can produce inhibition zone and its round size, semi-quantitatively evaluate the samples antimicrobial activity.
RESULTEight compounds were isolated and identified as succinic acid (1), glycerolmonopalmitate (2), protocatechuic acid (3), uracil (4), syringic acid (5), protocatechuic acid methyl ester (6), esculetin (7), medicagenic acid (8). The result of activity screening indicate that succinic and syringic acid have bacteriostasis to bacillus subtilis.
CONCLUSIONAll Compounds were isolated from the genus Kalimeris for the first time, succinic acid (1) and syringic acid (5) have bacteriostasis to bacillus subtilis.

6.Expression of CD133 and Notch1 in non-small cell lung cancer and the clinicopathological significance.

OBJECTIVETo investigate the presence of cancer stem cells (CSCs) exist in non-small cell lung cancer (NSCLC) and explore the relationship among the expressions of CD133, Notch1, and vascular endothelial growth factor (VEGF) and their relations with the clinicopathological parameters of the patients.
METHODSA total of 305 specimens of NSCLC and 80 normal lung tissue specimens were analyzed for CD133, Notch1, and VEGF protein expressions by immunohistochemical staining.
RESULTSIn NSCLC specimens, the positivity rates of CD133, Notch1, and VEGF were 48.9%, 43.9%, and 45.6%, respectively, significantly higher than those in normal lung tissues (10.0%, 15.0%, and 0%, respectively, P<0.01). The expression levels of CD133, Notch1, and VEGF proteins were significantly correlated with the tumor grades, lymph node metastasis, TNM stages, and postoperative survival time of the patients (P<0.01). A positive correlation was found among the expression levels of CD133, Notch1, and VEGF proteins. Kaplan-Meier survival analysis showed a significantly lower overall mean survival time of the patients positive for CD133, Notch1, and VEGF than that of the negative patients (P<0.001). Cox regression analysis suggested that positive expressions of CD133 and Notch1 were independent prognostic factors of NSCLC (P<0.05).
CONCLUSIONSCD133, Notch1, and VEGF may play important roles in the occurrence, progression, invasion, and metastasis of NSCLC. CD133 and Notch1 have important values for predicting the prognosis and evaluating disease progression of the patients.

7.Triterpenes from Kalimeris indica.

RESULTEleven compounds were isolated and identified as dammaradienyl acetate(1), friedelin(2), friedelinol(3), beta-amyrin(4), alpha-amyrin(5), lupenyl acetate(6), beta-sitosterol(7), alpha-spinasterol(8), alpha-spinasterone(9), stigmasterol(10), daucosterol(11).
CONCLUSIONCompounds 4-6, 8-11 were isolated from the genus Kalimeris for the first time.

8.Expressions of Snail, Slug and KAI1 proteins in cervical carcinoma and their clinicopathological significance.

OBJECTIVETo explore the expression of Snail and Slug in primary cervical squamous cell carcinoma (CSCC) and their relationship with KAI1 expression.
METHODSThe expressions of Snail, Slug, and KAI1 proteins were examined by immunohistochemistry in 154 specimens of CSCC tissues, 50 specimens of cervical intraepithelial neoplasm (CIN), and 40 specimens of normal cervical tissues.
RESULTSThe positivity rates of Snail, Slug, and KAI1 expression were 0%, 2.5%, and 95.0% in normal cervical tissues, 32.0%, 34.0% and 64.0% in CIN tissues, and 66.2%, 66.9%, and 43.5% in CSCC tissues, respectively, showing significant differences in the rates among the 3 groups (P<0.05). The expressions of Snail, Slug, and KAI1 were significantly correlated with the histological grades of the tumor, depth of invasion, lymph node metastasis, International Federation of Gynecology and Obstetrics (FIGO) stages, and postoperative survival time (P<0.05). The expressions of Snail and Slug were positively correlated (r=0.752, P<0.001), and both of them were negatively correlated with the expression of KAI1 (P<0.001). Kaplan-Meier analysis showed that patients positive for Snail and Slug had significantly lower survival rates than the negative patients (P<0.001), while a positive expression of KAI1 was associated with a higher survival rate of the patients. Cox regression analysis identified Snail, KAI1, and FIGO stage as independent factors that affected the outcomes of CSCC (P<0.05).
CONCLUSIONThe expressions of Snail, Slug, and KAI1 are related to the tumor grade, FIGO stage, invasive depth, lymph node metastasis, and prognosis of CSCC, and their combined detection can help estimate the outcomes of the patients.

9.Prevalence and homology analysis on human and animals severe fever with thrombocytopenia syndrome virus infection in Yantai of Shandong province

Objective To learn the prevalence of infection of human and animals severe fever with thrombocytopenia syndrome bunyavirus(SFTSV)in Yantai,Shandong province,and to analyze the pathogenic features of SFTSV as well as its relationship between human and animal hosts. Methods From April to November in 2011,3 576 serum samples were collected from domesticated animals,including sheep,cattle,pigs,dogs,chickens,in Laizhou and Penglai areas where fever with thrombocytopenia syndrome frequently occured among local residents. Total SFTSV antibodies and virus-specific nucleic acids of the serum were tested by ELISA and Real time RT-PCR,respectively. SFTSV infection on each animal was observed in different months. 2 590 human serum samples were also collected in Laizhou and Penglai areas,with IgG antibodies tested by ELISA. Virus was isolated with Vero cells from the serum which SFTSV viral nucleic acids were positive. S fragments were amplified by RT-PCR and sequenced,with homology analysis conducted on these sequences. Results The overall positive rate of serum samples from animals on the total SFTSV antibodies was 40.24%(1 439/3 576)while the positive rate for specific nucleic acids was 4.56%(163/3 576). The positive rates for SFTSV antibodies were 62.78%,52.97%,45.56%,28.73%,1.45%and the positive rates for specific nucleic acids were 5.72%,4.63%,3.02%,5.25%and 3.73%,in sheep,cattle,chickens,dogs, pigs,respectively. The antigens/antibodies for SFTSV in animals changed seasonally. The overall positive rate for SFTSV IgG antibody from 2 590 human samples was 5.41%. Thirteen virus strains were isolated from these serum samples(10 strains from human and 3 strains from animals). The nucleotide homology of 13S fragments’sequences ranged from 95.23%to 100.00%and the nucleotide homology with the isolates from other provinces were between 94.72%and 99.13%. The homology was considered to be high. Conclusion High prevalence of SFTSV infections occured both in human and domestic animals in Yantai city. The nucleotide sequences of SFTSV were highly homologous among human and domestic animals. The findings suggested that domesticated animals might serve as SFTSV proliferation and the hosts for transmission thus should be attached great importance.
10.Feasibility of Erxian Decoction and Wenshen Prescription in Treatment of Depression Based on Network Pharmacology and Experimental Verification

ObjectiveTo predict the molecular mechanism of Erxian decoction and Wenshen prescription (modified Erxian decoction) in the treatment of depression based on network pharmacology and explore the feasibility of Wenshen prescription in the treatment of depression by comparing the efficacy and mechanism of the two decoctions based on a depression model induced by maternal separation combined with chronic restraint stress. MethodActive components and targets of Erxian decoction and Wenshen prescription were collected through Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and Bioinformatics Analysis Tool for Molecular mechanism of Traditional Chinese Medicine (BATMAN-TCM). Targets related to depression were screened out from databases such as GeneCards, Online Mendelian Inheritance in Man database (OMIM), and DrugBank. Common targets of drugs and disease were obtained and imported to Cytoscape 3.8.2 to plot the drug-active component-target-disease network. STRING platform was used to construct a protein-protein interaction (PPI) network and core targets and related core components were screened out. Gene Ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) functional enrichment analysis were performed on common targets through Metascape platform. The depression model was induced in mice by maternal separation combined with chronic restraint stress. From the 21st day of maternal separation (PD21) to the 111th day of restraint stress completion (PD111), mice were fed with the diet mixed with Erxian decoction or Wenshen prescription for intervention. The depressive state of mice was evaluated according to the sucrose preference test, tail suspension test, open field test, and elevated O-maze test. The expression of ionized calcium-binding adapter molecule 1 (Iba1) in the microglia was observed by immunohistochemistry (IHC). Western blot and Real-time fluorescence-based quantitative polymerase chain reaction (Real-time PCR) were used to detect the expression levels of protein kinase B1(Akt1), brain-derived neurotrophic factor (BDNF), postsynaptic density-95 (PSD95), and synaptophysin (Syn). ResultA total of 126 and 118 targets of Erxian decoction and Wenshen prescription in the treatment of depression were screened out, with only eight more targets of Erxian decoction than Wenshen prescription. The two decoctions shared the same core targets, mainly including Akt1, interleukin-6 (IL-6), interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α). KEGG pathway enrichment analysis predicted that Erxian decoction and Wenshen prescription mainly treated depression through the phosphatidylinositol-3 kinase (PI3K)/Akt signaling pathway, mitogen-activated protein kinase (MAPK) signaling pathway, and neuroactive ligand-receptor interaction pathway. Animal experiments showed that compared with the results in the model group, Erxian decoction and Wenshen prescription could up-regulate the sucrose preference index, prolong the time spent in the central zone, increase the number of crossings, prolong the time spent in opened arm, increase the number of crossings in the opened arm, elevate the expression levels of p-Akt1, BDNF, PSD95, and Syn (P<0.05, P<0.01), shorten the immobility time of tail suspension, and reduce the expression level of Iba-1 in the hippocampal microglia (P<0.05, P<0.01). No significant difference between the two decoctions was found. ConclusionUnder the pathogenesis and syndrome law of depression dominated by kidney yang deficiency, Wenshen prescription modified from Erxian decoction is feasible in the treatment of depression. The mechanism may be attributed to the fact that both decoctions can improve neuroinflammation and synaptic plasticity in the hippocampus by affecting Akt1, IL-1β, IL-6, TNF-α, and other core targets and regulating the PI3K/Akt, MAPK, and neuroactive ligand-receptor interaction signaling pathways.
