1.Perospirone versus Risperidone in the Treatment of Schizophrenia: A Systematic Review

OBJECTIVE:To systematically evaluate the therapeutic efficacy and safety of perospirone versus risperidone in the treatment of schizophrenia,and to provide evidence-based reference for clinical treatment.METHODS:Retrieved from Wanfang database,VIP,CJFD,CBM and PubMed,randomized controlled trials (RCTs) of perospirone(trial group) vs.risperidone (control group) in the treatment of schizophrenia were collected.After data extraction and quality evaluation of included studies according to modified Jadad evaluation criteria,Meta-analysis of response indexes was performed by using Rev Man 5.0 statistical software.RESULTS:A total of 12 RCTs were included,involving 1 050 patients.Results of Meta-analysis showed,the cure rate of 2 groups had no statistical significance [OR=0.99,95%CI(075,1.30),P=0.93].The incidences of extrapyramidal reactions [OR=0.63,95%CI(0.43,0.92),P=0.02],the incidences of prolactin levels increasing [OR=0.23,95%CI(0.14,0.38),P<0.001] and the incidences of weight gain [OR=0.23,95%CI(0.13,0.39),P<0.001] in control group were significantly higher than trial group,with statistical significance (P<0.05).CONCLUSIONS:The therapeutic efficacy of perospirone and risperidone is similar in the treatment of schizophrenia,but the safety of perospirone is better than that of risperidone.
2.Evidence-based Pharmacoecomic Evaluation of Venlafaxine and Escitalopram in the Treatment of Depres-sive Disorder

OBJECTIVE:To systematically evaluate the pharnacoeconomic effect of venlafaxine and escitalopram in the treat-ment of depressive disorders. METHODS:Usingvenlafaxineescitalopramdepressiondepressive episodeantidepressantas key words,retrieved from CNKI,VIP and Wanfang database during base creation date to Dec. 2015,related literatures about venlafaxine and escitalopram in the treatment of depressive disorders were selected. The cure rate of two therapy plans for depres-sive disorders and HAMD score were analyzed statistically by cost-effectiveness analysis of pharnacoeconomics. RESULTS:There was no statistical significance in cure rate and HAMD score between 2 groups(P>0.05),so minimum-cost analysis was adopted. The costs of venlafaxine and escitalopram in the treatment of depressive disorders were 772.03 yuan and 935.77 yuan. CONCLU-SIONS:Therapeutic efficacy of venlafaxine is similar to that of escitalopram in the treatment of depressive disorders,but the for-mer scheme is more economical.
3.Systematic review of tandospirone and buspirone in treatment of patients with generalized anxiety disorder

Objective To evaluate the therapeutic effect and safety of tandospirone and buspirone in the treatment of generalized anxiety disorder.Methods The clinical control study oftandospirone and buspirone in the treatment of generalized anxiety disorder was earched by PubMed,CBM,CNKI,VIP,and Wanfang Data,with deadline from January 2000 to June 2016.Meta-analysis was carried out using RevMan5.0 software to each effect index.Results A total of seven RCTs were included involving 615 patients were identified.Meta-analysis showed that the significant efficiency rate[P=0.34,OR=1.19,95%CI (0.83~1.69)] and HAMA[P=0.80,MD=-0.08,95%CI (-0.72~0.56)] of tandospirone and buspirone had no significant difference.Two groups of adverse drug reactions,dizzy,dry,constipine,insomnia,anorexia,and nausea had no significant difference (P>0.05).Conclusion The efficacy and adverse reaction oftandospirone and buspirone in the treatment of generalized anxiety disorder are fairly.
4.Impacts of concentrations of Tacrolimus on NK cells and receptors in peripheral blood of renal transplant recipients

Objective To study the effects of tacrolimus(Tac) concentrations on the number of NK cells and receptor expression in peripheral blood of renal transplantation receptors.Methods A total of 60 first-time kidney transplantation recipients in our institute from Dec.2007 to July 2009 were followed up.Tac maintenance immunosuppressive therapy was given to all recipients.The recipients were divided into low-concentration Tac group (6.84 + 1.72μg/L,n =30) and highconcentration Tac group ( 11.88 + 2.59 μg/L,n =30) according to concentrations of Tac.Twenty healthy volunteers served as controls.Before and 6 months after operation,concentrations of Tac were analyzed by using micro particle immunoassay chemiluminescent method.NK cells and their receptors (CD85j,CD158d,CD94 and NKG2D) were detected by using flow cytometry.The concentrations of soluble HLA-G5 were detected by using ELISA.Results The number of NK cells in lowconcentration Tac group and high-concentration of Tac group preoperatively was significantly reduced as compared with control group (P < 0.05 ). The percentage and number of NK cells in low concentration Tac group and high-concentration Tac group at 6th month after operation were significantly reduced as compared with control group (P<0.05).The number of NK cells in lowconcentration Tac group was significantly greater than in high-concentration Tac group (P< 0.05).There was no significant differende in the expression of CD85j,CD158,CD94 and NKG2D before operation between two groups(P>0.05).The expression of CD85j and CD158d in two groups was increased,but that of CD94 and NKG2D was decreased at 6th month post-transplantation as comapred with that preoperation.In low-concentration Tac group,the expression of CD85j and CD158d was increased as compared with that in high-concentration Tac group (P<0.05 ).Spearman correlation analysis revealed that the CD85j and CD158d expression had a positive correlation with sHLA-G5(P<0.01 ),but the NKG2D had a negative correlation with sHLA-G5(P<0.01 ).Conclusion There was correlation between the concentrations of Tac and NK cells count and NK receptors. Low concentrations of Tac can safely and effectively protect kidney function.The number of NK cells andtheir inhibitor receptors are increased in the recipients with low concentration of Tac.
5.Clinical and genetic analysis of a child with mosaic chromosome 8 trisomy syndrome.

OBJECTIVE:
To explore the genetic etiology of a child featuring recurrent oral ulcer.
METHODS:
Clinical data of the child was collected. Whole exome sequencing was carried out for her. Candidate variant was verified by low-coverage massive parallel copy number variation sequencing (CNV-seq) of the family trio.
RESULTS:
The child, a 6-year-old girl, has featured recurrent fever and ulcers of the oral mucosa, vulvar and perianal regions. No pathogenic variant was found by whole exome sequencing. However, analysis of chromosome copy number variation using the whole exome sequencing data has revealed mosaicism of trisomy 8. CNV-seq assay has verified the variant in the child, with the percentage of mosaicism being 73%. No abnormality was found in neither of her parents.
CONCLUSION
A case of mosaicism trisomy 8 with recurrent oral ulcer as the first symptom was diagnosed, which has enriched the phenotypic data of trisomy 8 syndrome.

6.Clinical application of laser selective melting titanium alloy for removable partial denture frameworks

Objective:
To investigate the effect of denture stomatitis of selective laser melting (SLM) titanium alloy for removable partial denture frameworks.
Methods :
Twenty patients with dentition defects in our hospital were divided into two groups according to the different methods of creating a removable partial denture framework: the SLM group and casting group. The success rate, placement rate, masticatory efficiency and incidence of denture stomatitis were compared. Experimental data were analyzed with SPSS20.0.
Results :
The success rate of the SLM framework group was 100.00%, which was higher than that of the casting group (90.00%) (P < 0.01). The rate of framework placement in the SLM group was slightly lower than that in the casting group (P < 0.05). The masticatory efficiency of the SLM group was higher than that of the casting group (0.783 ± 0.030 vs. 0.699 ± 0.037, P < 0.001). The incidence of denture stomatitis (10.00%) in the SLM group was significantly lower than that in the casting group (30.00%) (P < 0.001).
Conclusion
SLM is superior to the traditional casting method in mastication efficiency and reducing the incidence of denture stomatitis. This method can meet the clinical requirements, but the accuracy of the long-term stent needs to be improved.
7. Dentification and correction of the lead-time bias and length bias in cancer screening studies

Lead-time bias and length bias were common systematic errors in observational screening studies, which might be a common cause of overstating or distorting the true screening effects. One of key concerns in observational screening studies was how to estimate the screening effects based on the consideration of these two biases. This paper illustrated how to identify and correct the lead-time bias using the tumor volume doubling time and the non-homogeneous Poisson process, and how to correct the length bias using a weighted method. The application conditions of each method were also discussed to present several useful toolboxes to correct the lead-time bias and length bias appropriately and evaluate the effectiveness of the cancer screening program accurately.
8.Participation of clinical pharmacists in pharmaceutical care for a patient with Mycobacterium abscess infection after the resection of intervertebral schwannoma

OBJECTIVE To provide ideas for pharmaceutical care of Mycobacterium abscess infection. METHODS The diagnosis and treatment plan ,about a patient with M. abscess infection after resection of intervertebral neurilemmoma participated by clinical pharmacists ,was analyzed . According to the results of bacterial culture and drug sensitivity test of wound secretion ,the clinical pharmacist timely dealed with the adverse drug reactions during the treatment . For the tinnitus caused by the patient taking amikacin(0.4 g,intravenous drip ,q12 h),it was suggested to stop amikacin and replace it with linezolid ;however,the patient suffered from knee pain after taking linezolid . The clinical pharmacist once again suggested stopping linezolid ,continuing to use amikacin,and adjusting the dose to 0.8 g,intravenous drip ,q24 h;at the same time ,the patients were given medication guidance and ordered to have regular follow -up after discharge . RESULTS The physicians adopted the suggestions of clinical pharmacists , and the patient did not suffer from tinnitus ,knee pain and other discomfort ,and the liver and kidney functions were normal . CONCLUSIONS Clinical pharmacists timely assist clinicians to adjust the medication plan ,which improves the effectiveness and safety of patients ’medication.
