1.Multicenter trial of therapeutic efficacy and safety of domestic latanoprost on primary open angle glaucoma and ocular hypertension

Background Prostaglandin eye drops is front-line medicines of lowing intraocular pressure for primary open angle glaucoma (POAG),but its price is higher for long-term application.Domestic latanoprost drops has been applied in clinic.However,its trial of lowing intraocular pressure effect and safety from sizable sample and multi-center is lack.Objective This study was to observe the efficacy and safety of domestic latanoprost for POAG and ocular hypertension.Methods This prospective,multicenter trial was conducted at 25 hospitals in 5 territories of China,and 837 eyes of 576 patients with POAG and ocular hypertension were recruited in the hospitals based on the uniform criteria.Domestic latanoprost drops was topically used once per day at night for 3 months.Intraocular pressure (IOP) was measured 1 month and 2 months,3 months to assess the efficacy of drug,and vision acuity,blood pressure,pulse,ocular symptoms and signs were observed to evaluate the safety of the drug.This study approved by the Ethics Committee of Wuhan General Hospital of Guangzhou Military Command,and all patients were informed consent and signed informed consent.Results IOP was (28.36±7.86),(19.66±5.46),(19.59±7.86) and (19.35 ± 5.03) mmHg in pretreatment and posttreatment 1 month,2 months and 3 months,respectively,showing a significant difference among various time points (F =748.325,P =0.000),and the IOPs were markedly lower in posttreatment than those in pretreatment (all at P<0.001).The falling range of IOP was (30.43 ±7.18)%,(30.19 ± 8.72)%,(30.71±9.90) % in posttreatment 1 month,2 months and 3 months,respectively.The efficiency was 95.70%,91.76% and 88.05% in 1 month,2 months and 3 months.Conjunctival hyperemia was observed in 13% eyes at the end of following-up.No drug-related systemic and other ocular adverse reactions were found.Conclusions Domestic latanoprost drops can low IOP effectively without serious adverse reaction in POAG and ocular hypertensive patients.
2.Effects of NaokangⅡon cognitive function and neurogenesis in rats with cerebral ischemia WANG

Objective ToobservetheeffectsofNaokangⅡ,atraditionalChinesemedicineon spatial learning and memory abilities and hippocampal neurogenesis in rats with cerebral ischemia. Methods Atotalof54SDratsweredividedinto6groupsaccordingtothedigitalrandommethod:a control,a sham operation,a model,a low-dose,a medium-dose,and a high-dose NaokangⅡ group (the low-,medium-,and high-dose groups were 5,10,and 20 times of adults (n=9 in each group). A rat model was induced by ligate the bilateral common carotid arteries permanently. The bilateral common carotid arteries were only isolated but not ligated in rats of the sham operation group. The rats in the control group were not given any treatment. After modeling,the rats in the treatment groups were treated with NaokangⅡ. The rats in the control,sham operation,and model groups were given equal volume of isotonic saline (2 ml). Morris water maze test and space search experiment were used to evaluate the learning and memory abilities in rats 9 weeks after modeling. Immunofluorescence staining and immunohistochemical methods were used to observe proliferation and differentiation of neural stem cells (NSCs). Western blot was used to detect the expression of autophagy related protein Beclin-1 and LC-3Ⅱ. Results (1 )The escape latencies of the model group from 1 to 5 days were 100 ± 13,98 ± 13,82 ± 14,72 ± 15,and 46 ± 7 s,respectively. They were significantly longer than those of the sham operation group and the control group (all P<0. 05). The search distances were 1239 ± 127,1011 ± 122,959 ± 123,553 ± 66,and 407 ± 29 cm,respectively. They were significantly increased as compared with those of the sham operation group and control group (all P<0. 01). Both the escape latencies and search distances of the rats in the Naokang Ⅱ low-,medium-,and high-dose groups were significantly shorter than those of the model group (P<0. 05). The escape latencies on the fifth day were 20 ± 6,19 ± 8,and 15 ± 6 s,respectively,and the search distances were 234 ± 38, 297 ± 23,and 99 ± 22 cm,respectively. Nine weeks after modeling,the quadrant search time on the original platform in the rats of the model group was 30 ± 9 s,it was significantly shorter than that of the sham operation group (P<0. 01). The quadrant search time on the original platform of the Naokang Ⅱ low-, medium-,and high-dose groups were 34 ± 6,38 ± 8,and 40 ± 10 s,respectively. They were significantly longer than those of the model group (P <0. 01). (2)Nine weeks after modeling,the results of immunofluorescence and immunohistochemistry showed that the number of BrdU-positive cells and the Nestin positive cells in hippocampal dentate gyrus of the model group were higher than those of the control group and the sham operation group (P<0. 05). The number of BrdU-positive cells and Nestin positive cells of Naokang Ⅱ each dose group were significantly higher than those of the model group (P<0. 05). The expression of LC-3Ⅱand Beclin-1 in the model group was significantly higher than those in the control and sham operation groups (P<0. 01). The expression of LC-3Ⅱ in the medium- and high-dose groups was 143 ± 9 and 129 ± 9 respectively,and the expression of Beclin-1 was 139 ± 10 and 124 ± 7 respectively, andtheyweresignificantlylowerthanthemodelgroup(P<0.01).Conclusion NaokangⅡcanimprove the cognitive function in rats with cerebral ischemia,promote neurogenesis in the dentate gyrus,and inhibit autophagic cell death.
3.Social Psychological Analysis and Implications of Internet Public Opinion on Hospital Violence

Objective:To investigate the public attitudes towards violence incidents from the perspective of in-ternet users, by the means of collecting the hospital violence incidents on the internet. Methods:Selected 7 cases of typical hospital violence and divided those comments into three categories, namely,support the medical side,support the patient side and other. The proportions of all kinds of attitudes were analyzed. Summarize the in-centives of hospital violence and analyze the attitudes of internet users from the perspective of social psychology. Results:Internet users had different attitudes to these 7 cases of hospital violence incidents. The proportion of support the medical side was from 0 . 66% to 95 . 82%, and the proportion ofsupport the patient side was from 0. 18% to 81. 25%. This was mainly due to the in-group favoritism, stereotypes, moral judgment and emotional tendencies of internet public opinion and media-induced and other factors. In internet users' view, the low profes-sional ethical quality is considered the most important incentives, and the proportion was 43. 05%. Conclusions:The tendency of the media coverage has an important influence on the attitude of internet users. Policy makers should correctly treat with the two sides of network public opinions, the hospitals should improve the ability to re-spond to public opinion, the media should regulate coverage and publicity.

Objective To analyse the relationship of blood deficiency syndrome’s principal components of chronic fatigue syndrome (CFS) and mental state and quality of life. Methods 23 cases of CFS blood deficiency syndrome were collected to pick up principal components, and combined with mental state (SCL-90) and quality of life (SF-36). Then, the relationship was analyzed. Results There was significant relationship between CFS blood syndrome principal component 1 and GH, VT and BP in SF-36. And there was significant relationship between CFS blood syndrome principal component 2 and patients’ somatization, obsessive compulsive, interpersonal sensitive, depression, anxiety, hostility and paranoid in SCL-90, and between principal component 1 and somatization. Conclusion There were two principal components in CFS blood deficiency syndrome, one influenced patients’ quality of life, the other influenced their mental state.
7.Correlate Analysis of Qi and Blood Deficiency Syndrome's Principal Components of Chronic Fatigue Syndrome and TCM Syndrome Differentiation

Objective To analyze the relationship of Qi and blood deficiency syndrome's principal components of chronic fatigue syndrome (CFS) and TCM syndrome differentiation. Methods Syndrome principal components of 25 CFS Qi deficiency patients and 23 CFS blood deficiency patients were researched and combined with TCM syndrome differentiation, then their relationship were analyzed. Results There was significantly positive relationship between CFS Qi deficiency syndrome principal component 1 and CFS Qi deficiency syndrome. And there were significantly positive relationships between CFS blood deficiency syndrome principal component l, 2 and CFS blood deficiency syndrome. Conclusion The CFS Qi deficiency syndrome and blood deficiency syndrome's principal components were accord with TCM syndrome differentiation. We could improve the TCM syndrome differentiation's impersonality and nicety, and predigest TCM diagnosis program if we combined two analysis ways.
8.Clinical Characteristics of Comorbid Anxiety and Depression after Cerebral Infarction

Objective To determine the clinical characteristics of comorbid anxiety and depression after cerebral infarction. Methods All the patients enrolled were divided into post-stroke comorbid anxiety and depression (PSCAD) group (84 cases) and the control group (121 cases). Their condition of smoke, alcohol intake, education background, carotid plaque and brain blood vessels were investigated, and they were evaluated with Hamilton Anxiety Scale (HAMA), Hamilton Depressive Scale (HAMD), Barthel index, and Mini-Mental State Examination (MMSE). Results There was not significant difference in condition of smoke, alcohol intake, education background, carotid plaque and brain blood vessels between these groups (P>0.05), but were in total scores and all the factors scores of HAMA and HAMD (P<0.05). PSCAD group showed more obvious anxious mood, depressive mood, tension, insomnia, cognitive impairment and autonomic nervous symptoms. Psycho-anxiety, soma-anxiety, despair, guiltiness, decreased interest were also more significant in the PSCAD group. The scores of MMSE and Barthel index were also impaired. Conclusion The cognitive function and daily living ability are worse in the patients with PSCAD. The prominent symptoms includes: anxious mood, depressive mood, tension, insomnia, cognitive impairment, autonomic nervous symptoms, soma-anxiety, despair, guiltiness, decreased interest.
9.Effect of Tenuigenin on Differentiation of Neural Stem Cells from Hippocampus of Newborn Rats

Objective To observe the effect of tenuigenin on differentiation of hippocampal neural stem cells (NSCs). Methods NSCs isolated from newborn (within 24 h) Wistar rats hippocampus were cultivated in vitro with serum free and clone culturing technology. Tenuigenin of different doses (0, 1, 2, 4 μg/ml) were added in the medium, and the proliferation and differentiation of the cells were identified with immunofluorescence staining. Results The neural spheres obtained from the hippocampi of newborn rats were positive for Nestin expression, with the potential for further cloning and differentiation into neurons or glial cells. The incidence of neuron specific enolase and glial fibrillary acidic protein positive cells increased in all the tenuigenin groups compared to the control (P<0.05), while the Nestin positive cells decreased (P<0.05). Conclusion Tenuigenin may promote the differentiation of neural stem cell into nerve cell.
10.Relationship of Depression and Sleep Quality of Patients with Cerebral Ischemia and High-grade Atherosclerotic Internal Carotid Artery Stenosis

Objective To explore the relationship of depression and sleep quality of patients with cerebral infarction and high-grade atherosclerotic internal carotid artery stenosis in acute seizure phase. Methods The patients targeted to the inclusion criteria were divided into non-depressive group (n=82) and depressive group (n=92) and the depressive group were divided into mild-depressive group (n=36) and moderate-depressive group (n=56) according to the Hamilton Rating Scale for Depressive (HAMD) score. Sleep quality was evaluated with Pittsburgh Sleep Quality Index (PSQI). The correlation was investigated between PSQI and HAMD score. Results The scores in the dimensions of subjective sleep quality, sleep latency, sleep duration, sleep efficiency, sleep disturbance, use of sleep medication and day-time dysfunction as well as the PSQI overall score were higher in the depressive group than in the non-depressive group (P<0.01). The scores of subjective sleep quality, sleep latency, sleep disturbance, day-time dysfunction and PSQI overall score were higher in the moderate-depressive group than in the mild-depressive group (P<0.05). In PSQI, the sleep disturbance and day-time dysfunction were correlated with all the dimensions in HAMD (P<0.01), the sleep latency and PSQI overall score were correlated with all the dimensions expect weight in HAMD (P<0.05). In HAMD, anxiety/somatization, cognitive disturbance and sleep disorder were correlated with all the dimensions in PSQI and PSQI overall score (P<0.05). Conclusion The depression condition is closely related to the sleep quality of patients with cerebral infarction and high-grade atherosclerotic internal carotid artery stenosis. The worse the depression, the worse the sleep quality.
