Guinea pigs were exposed to 1 shot of cannon, with a peak pressure at 178dB(SPL). The Avidin-Biotin peroxidase complex immumostaining in combination with glucose oxidase-DAB-nichel enhancement technique was used to investigate the changes of CGRP immunoreactivity(CGRP-IR)in inner and outer hair cell regions of auditory cochlea. The optical densities of the CGRP-IR in the cochlea were quantified with the image amalysing technique and the ABR threshold was measured 24h、72h、1、2 week and 1 month after the exposure. The relationship of ABR threshold shift with the middle ear damage and changes of CGRP-IR in the inner and outer hair cell regions were studied.The results sugessted that ABR threshold shifts of the ears with middle-ear damage tended to be less than that with normal middle-ear after one exposure. The CGRP-eontaining fibers in the outer hair cell regions are more sensitive to the blast injusy than that in the inner hair cell regions. This is well in accordance with the rule of blast injusy on hearing organ. CGRP in the olivocochlear bundle may partly protect hearing function from the injusy due to blast damage. The decline of GCRP may increase the susceptalility of hearing organ to the blast injusy.

Early detection and treatment of hepatocellular carcinoma (HCC) is one of the most direct and effective ways to improve the long-term outcome of patients with HCC. The comparative study on clinicopathological characteristics of surgically resected micro HCC (≤1 cm), small HCC(≤3 cm), middle HCC (3-5 cm), and large HCC (>5 cm) showed that ≤3 cm small HCC had the features of DNA diploidy, well differentiation, relatively slow growth, encapsulation, rare long-distance metastatic foci and thrombosis, easy to be radically resected and long-term postoperative survival. It is suggested that HCC growing to about ≤3 cm is an important growth phase when the change of pathobiological characteristics of HCC from a relatively benign to a more malignant behavior would occur. This is also the important time to carry out radical treatment to get better results. Therefore,further research on the early diagnosis and treatment as well as the pathobiological features of small HCC ≤3 cm should be undertaken as one of the key clinical and pathological studies in the future.

Based on reviewing briefly the development of the general education both in China and United States,selecting respectively a university as the example,the article has analyzed comparatively curriculum programs of general education between the two schools.Aimed at existing problem,from curriculum content carding,structure optimizing,teaching operating and evaluation of the effect,the article has proposed new thinking of reform on the general education of higher institutions.

Surgical-oncological pathology is one of the most important supporting discipline for hepatobiliary surgery.With rapid development in modern hepatobiliary surgery,the traditional model of morphological diagnosis can no longer meet the needs of oncological surgery.To improve the level of pathological diagnosis, better understand clinical concerns,update the mode of pathological diagnosis,introduce new technologies,expand the connotations of pathological diagnosis,provide a new indexing system for clinical treatment programs,and focus on interdisciplinary collaborative research are the most important issues in the development of surgical pathology for hepatobiliary tumors.

Objective To identify the characteristics of Candida utilis uricase and that of Bacillus fastidious intra-cellular uricase.Methods The new strains were cultivated to prepare the uricase.Then the uricase was purified by ammonium sulfate fractionation precipitation,DEAE-cellulose 52 chromatography and preparative PAGE.The activity,characterization and subunit constitution were determined with the purified uricase.Results The two uricas-es were the iso-tetramer.The single peptide chain molecular weight and total molecular weight of Candida utilis uricase were 33.0 ku and 134.0 ku individually,the optimum pH was close to 8.8 and more than 50% activity was reserved at pH 7.4,the Michaelis-Menten constant was (32.8 ± 3.1) μmol/L (n = 10) and the inhibition constant of xanthine was (4.8 ± 0.2) μmol/L(n = 3) for this uricase.For Bacillus fastidious intracellular uricase,its single peptide chain molecular weight and total molecular weight were 35.7 ku and 151.0 ku,the optimum pH was over 9.0 and less than 30% activity left at pH 7.4,the Michaelis-Menten constant and the inhibition constant of xanthine were (204 ± 14) μmol/L(n = 8) and (41 ± 7) μmol/L(n = 5) individually for it.Conclusion Through the research,a significant theoretical basis was funded for constructing hybrid medicinal uricase to treat diseases as-sociated with hyperuricemia.

Duodenal varices are an uncommon disease which occurs along the entire gastrointestinal tract outside the esophagus or stomach.Among ectopic varices,duodenal varices are the most common one.Cirrhosis,hepatic portal hypertension,extrahepatic portal vein occlusion and vasculitis contribute to the pathogenesis of ectopic variceal formation.The unusual pathophysiology,location,diversity and uncertain curative effect of DV are associated with its low detection rate and high mortality of patients.Therefore,the diagnosis and treatment of DV is one of the tough problems in clinical practice.This article reviews the current research advances in pathophysiology,diagnosis and treatment of duodenal varices to provide reference for clinical practice.

Gastrointestinal endoscopy plays a major diagnostic and therapeutic role in most gastrointestinal disorders.However,limited data of clinical safety is available in pregnant patients.Large studies,preferably prospective,with follow-up of fetal outcome are needed to determine fetal safety of endoscopy.This article briefly analyzed the literature on the research progress of gastrointestinal endoscopy during pregnancy,which offered an evident base for diagnosis and treatment in digestive diseases.

To study the immunopathological characteristics and differential diagnosis of hepatic angiomyolipoma(AML).Methods:Thirty-six surgically resected hepatic AML were investigated clinicopathologically and immunohistochemically with 10 antibodies.Results:Hepatic AML occurred in 21 females and 15 males,with the mean age of 41.6 years(26-60 years old).The patients with AML often had no special symptoms even had large space-occupying lesions in the liver.The diameter of AML was 2.5 cm to 14 cm(mean 6.8 cm).Histologically,AML was composed of varying heterogeneous mixture of 3 tissue components:blood vessels,smooth muscle and adipose cells.Extramedullary hemopoiesis sometimes existed.According to tissue components,AML was subcategorized into mixed type(19.4%,n=7),lipomatous type(11.1%,n=4),myomatous type(66.7%,n=24),and angiomatous type(2.8%,n=1).The epithelioid smooth muscle cells were sensitive to HMB-45(100%),SMA(100%),and CD117(66.7%) staining.Conclusion:Hepatic AML often contains smooth muscle elements,which have varied morphological features and should be carefully differentiated from hepatocellular carcinoma,mesenchymal hamartoma，and tumors with rich fat or blood vessels.Immunohistochemical staining with HMB-45 and SMA are the best available markers for the diagnosis of hepatic AML.

By analysing the new requirements of higher medical education curriculum in the new era,making curriculum changes of Nanjing Medical University as an analysed object,the arti-cle analyses the existing problems and shortcomings of China's current medical education course system,puts forward optimizing the course system further by running clear thinking,construction of composite course system,innovating teaching methods,strengthening the building of teaching mate-rials,the establishment of curriculum assessment mechanisms.
