1.Comparative study of perihepatic lymph node in the healthy individuals and patients with hepatitis C virus infection on ultrasonography

Objective To determine the relation between enlarged perihepatic lymph node (PLN) and viraemia,and to find out whether there is a difference in PLN size between the healthy individuals and patients with hepatitis C virus (HCV) infection. Methods Seventy-four patients with HCV infection and 283 healthy individuals were examined by two-dimensional ultrasound. The length and thickness of lymph node were measured. The lymph-node area index(LN area index) was calculated. The LN area index was evaluated by the ROC curve and compared with serum HCV core antigen (HCV-Ag) levels by correlation analysis. Results Detection rate, the length and thickness, LN area index of group with HCV infection were significantly different by compared with the normal control group ( P < 0. 001 ). The ROC curve showed that the LN area index as a diagnostic indicator had high sensitivity and specificity. The LN area index showed a significant correlation with HCV-Ag level (r = 0.436, P <0.05), but no significant differences were found between LN area index and HCV-Ag grade. Conclusions Perihepatic lymphadenopathy indicates viraemia,and LN area index of 100 mm2 is a reference index in estimating whether patients have HCV infection.
2.Effects of turicamycin on proliferation of high glucose-induced glomerular mesangial cells and its mechanism study

Objective To observe the effects of turicamycin-inhibitor of N-glycosylation of proteins on expression of (?1 integrin,FAK and cyclin D1 in high glucose-induced glomerular mesangial cells (CMC), and to explore the mechanism concerned. Methods Cultured HBZY-1 rat mesangial cells were divided into 5 groups:control group;high glucose group;mannitol group;high glucose plus TM group; TM group.The expression of ?1 integrin was measured by flow cytometry, expression of FAK and cyclin D1 was measured by immunohistochemistry,and proliferation of GMCs was measured by MTT. Results There was a little expression of ?1 integrin, FAK and cyclin D1 on normal mesangial cells. High glucose induced the proliferation and increased the expression of ?1 integrin and cyclin D1.There was no significant difference in mannitol group as compared to control group.The expression of (?1 integrin FAK and cyclin D1 decreased notably by TM.TM could also decrease proliferative abilitiy of cells. All the effects of TM were dose-dependent. Conclusion Through blocking glycosylation of glycoprotein, TM can suppress cellular proliferation and expression of pi integrin induced by high glucose in a dose-dependent manners,then pi integrin affects the expression of FAK and cyclin D1.
3.Clinical studies of cerebrospinal lfuid replacement combined with Vancomycin and dexamethasone intrathecal injection on intracranial infection

Objective To investigate the effect of cerebrospinal lfuid replacement combined with vancomycin and dexamethasone intrathecal therapy on biochemical indicators of postoperative intracranial infection, in order to improve the clinical diagnosis and treatment. Methods 70 cases with intracranial infection collected in Third Hospital of Beijing Armed Police Corps from February 2010 to April 2013 were as subject, and randomly divided into two groups. Control group(n=35) were given cerebrospinal lfuid replacement and ceftriaxone intravenously, observation group(n=35) were given cerebrospinal lfuid replacement combined with vancomycin and dexamethasone intrathecal injection. The clinical effects and biochemical indicators were observed after treatment in two groups. Results In control group, the cure rate was 22.86%and total efifciency was 77.14%. In observation group, the cure rate was 37.14% and total efficiency was 91.43%. The differences between two groups were statistically significant (P<0.05). The differences of leukocytes, glucose, protein, intracranial pressure in two groups after treatment were also statistically signiifcant(P<0.05). Conclusion Cerebrospinal lfuid replacement combined with vancomycin and dexamethasone intrathecal injection therapy can increase intracranial infection.
4.Preliminary application research of Ultrafast Doppler for renal artery ultrasonography

Objective To analyze the accuracy,repeatability and feasibility of Ultrafast Doppler for renal artery ultrasonography.Methods One hundred and one cases were selected that were suspected to have renal artery disease and were successful of conventional and Ultrafast Doppler for renal artery ultrasonography,and 101 cases were grouped by age (≤40 years old,41-60 years old,≥61 years old),body mass index (BMI) (normal 18.5-23.9 kg/m2,overweight 24.0-27.9 kg/m2,obesity ≥28.0kg/m2) and whether there were the presence of renal artery stenosis (no significant renal artery stenosis and renal artery stenosis >60%).Each case was respectively examined by conventional and Ultrafast Doppler for renal artery ultrasonography in a random order.The consistency of Doppler parameters was tested.The duration of each Doppler study was compared and the feasibility of Ultrafast Doppler for renal artery ultrasonography was explored.The Doppler parameters included:renal artery peak systolic velocity (PSV),resistance index (RI),renal segmental artery acceleration time (T) and time consuming (△T).The concordence and △T of two Doppler method were compared.Results ① Ultrafast Doppler had good reproducibility,intraclass correlation coefficient (ICC) values were > 0.6.② For renal artery ultrasonography,the successful number of cases examined by Ultrafast Doppler were more than those examined by conventional Doppler,but the difference was not statistically significant (P >0.05).③The Doppler parameters from all subjects and different groups showed a strong positive correlation between the two Doppler studies (P <0.05).④ UltraFast Doppler required a shorter time than conventional Doppler (P <0.05).⑤The △T of cases with different ages and with or without renal artery stenosis showed no statistically significant (P >0.05),however,their △T were increased with body mass index increasing (P<0.05).Conclusions Ultrafast Doppler for renal artery ultrasonography has a high success rate,a good repeatability and consistency,and a shorter time consuming and simple operation than conventional Doppler.
5.Effects of angelica solution on chronic hypoxic pulmonary hypertension in rats

Objective To investigate the effects of angelica solution on chronic hypoxic pulmonary hypertension in rats.Methods Thirty SD rats were randomized into normoxic group,hypoxic group and angelica solution-protected group.The model of rat chronic hypoxic pulmonary hypertension was made by method of isobaric hypoxia.Angelica solution were injected before hypoxia,while the other two groups were injected normal saline.After 28d of hypoxia,pulmonary artery pressure were measured.Expressions of proliferating cell nuclear antigen (PCNA) and inducible nitric oxide synthase (iNOS) in pulmonary artery were detected by immunohistochemical staining.The index of wall thickness of rat pulmonary arteriole-percentage of the wall area in the total vascular area(wA%) were measured by a computerized image analyzer.Results The mean pulmonary artery pressure (mPAP) of normoxic group,hypoxic group and angelica solution-protected group were10.50±1.90,35.36±9.11,18.32±2.30 (mm Hg);wA% of the three groups were 52.71±5.16,82.38±8.43,64.58±9.54 (%),mPAP and wA% were significantly higher in the hypoxic group than those in the normoxic group (P<0.01) and angelica solution-protected group (P<0.01).PCNA expression of normoxic group,hypoxic group and angelica solution-protected group were 3.15±1.10,24.50±5.72,12.67±3.46 (%).The PCNA expression in the pulmonary artery was significantly higher in the hypoxic group than those in the normoxic group (P<0.01) and in the angelica solution-protected group (P<0.01).iNOS expression of normoxic group,hypoxic group and angelica solution-protected group were 2.13±1.01,17.33±3.53,37.50±7.04 (%).iNOS expression in the pulmonary artery was higher in the hypoxic group than those in normoxic group (P<0.01),and angelica significantly increased iNOS expression in comparison with the normoxic and hypoxic groups (P<0.01).Conclusion Angelica solution alleviates chronically hypoxia induced pulmonary hypertension in rats by inhibiting the espression of PCNA in pulmonary artery and up-regulating the expression of iNOS.
6.Role of endometrial and subendometrial blood flows measured by three-dimensional power Doppler ultrasound in the prediction of endometrial receptivity during IVF-ET treatment

Objective To investigate the role of endometrial and subendometrial blood flows measured by transvaginal three-dimensional(3D) power Doppler ultrasound in the prediction of endometrial receptivity during in vitro fertilization-embryo transfer (IVF-ET) cycles. Methods One hundred and twenty patients undergoing IVF-ET cycles were enrolled in this study. Transvaginal 3D power Doppler ultrasound examination was performed on the day of human chorionic gonadotropin injection to determine endometrial thickness,endometrial volume, vascularization index(VI), flow index(FI) and vascularization flow index (VFI) of endometrial and subendometrial regions by experienced sonograpbers. The patients were divided into pregnant group and non-pregnant group according to pregnant outcome. The data between the two groups were compared and analysed. Results There were no significant differences( P >0.05) between the pregnant group and non-pregnant group in endometrial thickness, endometrial volume. Endometrial and subendometrial VI,FI, VFI were higher in the pregnant group than those in the non-pregnant group, and statistical comparison had significant difference( P <0.05). Conclusions Endometrial and subendometrial blood flows measured by transvaginal 3D power Doppler ultrasound can be as a reliable indicator to assess endometrial receptivity and to predict the pregnancy outcome.
7.Investigation of stroke-preventing knowledge and influential factors in hypertensive patients from the communities of Guangzhou

Objective To investigate the current situations in stroke-prevention knowledge and influential factors in hypertensive patients from the communities of Guangzhou.Methods 94 hypertensive patients visiting the community health service centers in Guangzhou participated in the survey using stroke-preventing knowledge questionnaire.Results The total score on the stroke-preventing knowledge was(62.70±18.39).By stepwise regression analysis,those female and well-educated scored higher. The score on monitoring the blood pressure was the lowest(43.09±25.77).There were mistakes in medication,physical exercises and living habits.Conclusions The hypertensive patients in the communities in Guangzhou lack of stroke-preventing knowledge.In the stroke-preventing education,those male patients as well as those with less education deserves more attention.
8.An etiological analysis of 106 cases with ambiguous genitalia

Objective To explore the causes of ambiguous genitalia.Methods Clinical data of 106 cases with ambiguous genitalia from Ruijin Hospital of Shanghai Jiaotong University School of Medicine were retrospectively analyzed.DNA fragments of related genes from parts of patients were amplified by means of polymerase chain reaction (PCR) and were directly sequenced to detect gene mutations.Results (1)The 106 ambiguous genitalia patients presented a variety of clinical phenotypes.Karyotype of 42 cases(39.6%)were 46,XX,while 62 cases(58.5%)were 46,XY and 2 cases(1.9%)were abnormal.(2)Forty(95.2%)patients with 46,XX were diagnosed with congenital adrenal hyperplasia(CAH) ;one case(2.4%) was adrenal cortical tumor and one case (2.4%) was 46,XX [sex determining region of Y choromosome (SRY) positive] male syndrome.(3) Fifty-three cases (85.5 %) out of 46,XY karyotype were directly sequenced with steroid-5-alpha-reductase,alpha polypeptide 2 gene (SRD5A2),androgen receptor gene (AR) and steroidogenic factor-1 gene(SF-1).Sequencing analysis of SRD5A2 revealed 8 patients with compound heterozygous or homozygous mutations.A patient carried a novel missense mutation of SF-1 and another patient had a mutation of AR.(4) One abnormal karyotype was 46,XX/46,XY and the other was 46,XX/46,XY/46,X.+ may.ish (DYZ3 +) (DXZ1-).Conclusions (1) CAH is the most common cause of genital ambiguity in 46,XX patients but some rare causes such as adrenal cortical tumors or SRY positive should not be ignored.(2) To find the causes of 46,XY genital ambiguity,direct DNA sequencing analysis of candidate genes would be the better choice because of the complicate pathogenesis.(3)Abnormal karyotype also can lead to ambiguous genitalia.
9.An outbreak of brucellosis in Chongqing municipality: data analysis of epidemiological survey results

Objective To master the epidemic prevalence and the risk factors of brucellosis in animals and human,and to provide a basis for making prevention strategies and measures to brucellosis in Chongqing Municipality.Methods Qijiang and Wansheng district of Chongqing Munipality were selected as investigation points.Feeding status of goats in feedlots was investigated,and blood samples of goats were collected for laboratory testing.Epidemiological survey in employees on feedlots,family members closely exposed with goats,and other focus groups such as stockbreeding and veterinary was conducted,and blood samples was collected for laboratory testing according to the principle of informed consent.Clinical features of infested persons were investigated.Blood samples were screened by plate agglutination test (RBPT) and tube agglutination test (SAT).Results A total of 582 households in the two districts were investigated.The number of positive households was 40 households.The positive rate of the households was 6.87%.A total of 20 105 goat blood samples were tested.Of them,989 blood samples tested positive.The positive rate was 4.92%.A total of 337 blood samples of the risk population from 22 towns of the two districts were tested.Of them,45 samples were positive,and the positive rate was 13.35%.Eleven people were active patients of brucellosis.The epidemic sites were distributed in 16 towns,which accounted for 73.73%.The difference of the positive rates between the two districts was not significant (x2 =0.37,P > 0.05).The positive rate of brucellosis among male and female was 11.57% (28/242) and 17.89% (17/95),respectively.The age of brucellosis infection distributed from 2 years old to 83 years old.One preschool child and 3 students were identified positive.The positive rate of brucellosis among feeder was the highest,which accounted for 31.75% (40/125).The difference of positive rate of brucellosis among different professions was significant (x2 =63.40,P < 0.05).Conclusions There is not a brucellosis case among animals or people reported in Chongqing non epidemic areas of brucellosis,but there are lots of infection.Surveillance and prevention of brucellosis should be strengthened in Chongqing.
10.Effect of trichostatin A on the chemotherapy sensibility to 5-Fu of colorectal cancer cell line Lovo and the possible mechanisms

Objective To investigate the effect of histone deacetylase inhibitor trichostatin A (TSA) on the chemotherapy sensibility of 5-fluorouracil (5-Fu) in colorectal cancer cell line Lovo, and to explore the possible mechanisms.Methods According to the treatment methods, the cells were divided into control group, 5-Fu group, TSA group, TSA preconditioning group and combination group (TSA+5-Fu).MTT assay was used to detect cell proliferation at 24 h, 48 h and 72 h after drugs treatment.Transwell assay was used to test cell invasion after 24 h drugs treatment.Flow cytometer was applied to observe the apoptosis after 24 h drugs treatment.The expressions of thymidylate synthase (TS) were detected by Western blot after 24 h drugs treatment.Results Compared with control group, the 5-Fu group, TSA preconditioning group and combination group had a growth inhibition to Lovo cell at 24 h, 48 h and 72 h (P < 0.05), and compared with 5-Fu group, the growth inhibition of TSA preconditioning group and combination group were distinctive at 48 h and 72 h (P < 0.05).However, the inhibition between TSA preconditioning group and combination group were no significant (P > 0.05).Interfered after 24 h, the number of cells penetrating the matrigel in control group, 5-Fu group, TSA group,TSA preconditioning group and combination group were (25.0±4.2), (16.8±2.8), (19.6± 2.5), (8.2±3.2) and (6.5±2.6), respectively (P < 0.05), and the apoptosis rates were (4.26±1.36) %, (11.66± 3.18) %, (8.57±2.69) %, (39.79±8.53) % and (45.18±10.07) %, respectively (P < 0.05).Compared with control group, the number of cells penetrating the matrigel in the experimental groups was significantly decreased, and the apoptosis rate was significantly increased (P < 0.05).Compared with 5-Fu group, the numbers of cells penetrating the matrigel in TSA preconditioning group and combination group were markedly decreased, and the apoptosis rates were markedly increased (P < 0.05), but the number of cells penetrating the matrigel and the apoptosis rate between TSA preconditioning group and combination group were not different (P > 0.05).The difference of TS expression between control group and 5-Fu group was not significant (P > 0.05).Compared with that in control group and 5-Fu group, TS expressions in TSA group, TSA preconditioning group and combination group were markedly decreased (P < 0.05), but TS expressions among the last three groups were not different (P > 0.05).Conclusion TSA can increase the chemotherapy sensibility of 5-Fu in Lovo cells, which may be dependent on reducing the TS expression.
