1.Bacteria Monitoring of Mattress in Common Ward of General Hospital

OBJECTIVE To detect bacteria number of the mattress in the common ward in order to reduce the mattress transmitted hospital infection. METHODS Secretion was collected for bacteria test from mattresses of the five common wards at the d0,dd1-6,dd7-13,dd14-20,and d21 and more after admission of patients. RESULTS The longer the patients stayed,the heavier the pollution of mattress was,its bacterial amount was much higher than the criterion made by Ministry of Health(10 CFU/cm~2). CONCLUSIONS Long-term hospitalization is the main factor that makes mattress bacterial contaminant in the common ward,and it should be cleaning and sterilizing regularly.
2.Antibiotic Resistance of Commonly Encountered Pathogens in Neurosurgery Hospital Infection

OBJECTIVE To find out the main commonly encountered pathogen and antibiotic resistance in hospital infection in Department of Neurosurgery. METHODS Clinic samples from Jun 2005 to Jun 2007 were collected and drug sensitive test was taken.RESULTS The most commonly encountered pathogens of infection were Gram-negative bacillI(83.1%),including Klebsiella pneumoniae(25.0%),Escherichia coli(19.1%),andAcinetobacter baumannii(10.3%). The three kinds of pathogens had heavy resistance to at least 8 kinds of antibiotics. The resistance rates to imipenem and ofloxacin were the lowest (6.8% and 32.4%). CONCLUSIONS The pathogens isolated from Departmentof Neurosurgery are Gram-negative bacilli which have multiple antibiotic resistance. The key prevention measures of infection are to control prophylatcic usage of the third generation cephalosporins,strengthen environmental microbial monitoring,hand sterilization and cleaning among the medical staff.
3.Ventilatory catheter-assisted airway stenting under local anesthesia for patients with malignant airway stenosis

Objective To discuss the ventilatory catheter-assisted airway stenting under local anesthesia for airway stenosis.Methods From May 2008 to January 2013,25 consecutive patients with airway stenosis underwent the treatment of ventilatory catheter-assisted airway stenting under local anesthesia.The dyspnea score was grade Ⅱ in 3 patients,grade Ⅲ in 18 patients and grade Ⅳ in 4 patients.During the treatment,a 4 F VER catheter was placed into one of the main bronchi as a ventilatory catheter for oxygen supply in advance,the oxygen flow was set at 2 to 4 L/rmin according to the patients' condition,then the airway stenting was performed,after release of the stent,the ventilatory catheter was removed smoothly.Data on technical success,clinical outcome and follow-up were collected and analyzed respectively.The paired t test was performed to compare variables before and after tracheal stenting.Results Ventilatory catheter-assisted airway stenting under local anesthesia was technically successful in all patients.A total of 25 stents were placed in 25 patients.The stents included I-shaped stents (n=19) and Y-shaped stents (n=6).After the oxygen supply,the patients' SaO2 was increased to 94% to 97% rapidly.When the stent introducer sheaths were passing through the stenotic site,the SaO2 could be maintained at 92% to 97%.The improvement of the respiratory difficulty was achieved in 100% of our patients.The hypoxia symptom was relieved immediately after stents placement,the dyspnea score decreased to grade 0 in 2 patients,to grade Ⅰ in 19 patients and to grade Ⅱ in 4 patients.Respiratory rate and SaO2 improved from (30.0±2.1) times/min and (6.0±3.8)% before stenting to (19.7±1.6) times/min (t=23.33,P<0.01) and (93.0± 1.7)% (t=23.50,P<0.01) after stenting,respectively.Through the follow-up,3 patients suffered the stent-related complications after discharge,the complications included re-stenosis (n=2) and fracture(n=1).Conclusion Ventilatory catheter-assisted airway stenting under local anesthesia can be an effective,simple and safe method for airway stenosis.
4.Anterior superior malleolus flap and second toe transplantation for thumb reconstruction with soft tissue and first web defect

Objective To explore the clinical efficacy of anterior superior malleolus flap and second toe transplantation for thumb reconstruction and soft tissue defect repair or first web reconstruction.Methods A total of 13 cases were underwent anterior superior malleolus flap and second toe transplantation for thumb reconstruction and soft tissue defect repair from June,2008 to January,2016.There were 9 males and 4 females.The average age was 25 years old,ranging from 18 to 52.There were 3 cases of Ⅲ-degree thumb defect with soft tissue defect of hand,6 cases of Ⅳ-degree thumb defect with soft tissue defect of hand,3 cases of single Ⅴ-degree thumb defect and 1 Ⅴ-degree thumb defect with soft tissue defect.There were 5 patients accompanied with injury of other fingers of the affected hand.And the thenar muscles were also damaged to a varied degree.There were 10 patients with soft tissue defect before surgery,with the wound surface ranging from 3 cm × 5 cm-6 cm × 13 cm.And the wound of other 3 cases was closed.After the surgery,follow-up visit was conducted on the appearance,sensation,function,daily life and working condition of reconstructed thumb,the appearance and function condition of donor site.Results The reconstructed thumbs and anterior superior malleolus flaps in 13 cases survived and healed primarily.The average post-operative followed-up time was 5.5 years,ranging from 2.5 to 10.0 years.The appearance of reconstructed thumb was satisfactory,and the temperature sensation,pain sensation and touch sensation were recovered.The two-points discrimination was 8-18 mm.Moreover,the flexion and extension function,opposing function and finger-to-thumb function was recovered,which could meet the demand of daily life and work.The appearance of donor site was satisfactory,without claudication,pain or dysfunction.The survival of transplanted skin area in the calf was good,with no ulcer noticed.According to the Trial Standard for Thumb and Finger Reconstitution Functional Assessment of Hand Surgery Society of Chinese Medical Association,there were 10 cases excellent results,2 good and 1 fair.Conclusion Application of anterior superior malleolus flap and second toe transplantation for first-stage thumb reconstruction and soft tissue defect repair or thenar muscles and first web reconstruction is an effective therapeutic approach.
