1.Predictive value of preoperative frailty combined with nutritional status for prolonged postoperative ileus in patients with gynecologic malignancies

Objective:To evaluate the predictive value of preoperative frailty combined with nutritional status for prolonged postoperative ileus (PPOI) in the patients with gynecological malignancies.Methods:Patients undergoing elective surgery for gynecological malignancies in the Affiliated Hospital of Jiangnan University from April 2022 to February 2023 were selected. The Frail scale was used to evaluate the frailty within 24 h of admission, and the nutritional status was evaluated by the Controlling Nutritional Status score. The general characteristics of patients and occurrence of PPOI were recorded, and the risk factors for PPOI were analyzed by multivariate logistic regression. The ability of frailty, nutritional status and their combination to predict PPOI was assessed by the receiver operating characteristic curve.Results:Two hundred and fourteen patients were finally included, 52 cases developed of PPOI, and 98 cases were frail patients. Preoperative frailty combined with moderate to severe malnutrition was an independent risk factor for PPOI in the patients with gynecological malignancies ( P<0.05), and the area under the curve in predicting the occurrence of PPOI was 0.796 (95% confidence interval 0.736-0.857) in the patients with gynecological malignancies. Conclusions:Preoperative frailty combined with moderate to severe malnutrition has a higher accuracy in predicting PPOI in the patients with gynecological malignancies.
2.Construction and verification of a Nomogram model for fall risk in patients with chemotherapy-induced peripheral neuropathy for gynecological cancer

Objective:To investigate the influencing factors of falls in patients with chemotherapy-induced peripheral neuropathy for gynecological cancer, and establish a Nomogram model based on independent influencing factors.Methods:A total of 216 patients with peripheral neuropathy who received chemotherapy for gynecological cancer in The First Affiliated Hospital of Army Military Medical University of the People′s Liberation Army from February 2021 to April 2023 were retrospectively selected, and were divided into the modeling set and the validation set according to the ratio of 7∶3 using random number table method. According to the occurrence of falls in the modeling set, the patients were divided into the fall group and the non-fall group. Independent influencing factors were obtained through univariate analysis and multivariate Logistic regression analysis, and a prediction model was constructed based on the regression analysis. A Nomogram map was drawn using R language software. Receiver operating characteristic curve (ROC) and calibration curve were used to test the prediction efficiency, and the predictive benefits were evaluated by clinical decision curve.Results:Among the 216 patients, 151 were included in the modeling set and 65 in the validation set. Among the patients in the modeling set, 53 cases were in the fall group, aged (62.89 ± 8.58) years old, and 98 cases were in the non-fall group, aged (59.48 ± 8.12) years old. Chemotherapy cycle, Neuropathy Impairment Score-Lower Limbs, Barthel index, Home Falls and Accidents Screening Tool, regular exercise habits, diabetes and depression were all independent influencing factors for falls in patients with chemotherapy-induced peripheral neuropathy of gynecologic cancer ( OR values were 0.312-3.727, all P<0.05). The area under the curve (AUC) of the established Nomogram model was 0.873, with good discriminating ability. The mean absolute error (MAE) of calibration curve was 0.048, which fitted the ideal curve, indicating that the model has good calibration performance. The AUC of the validation set was 0.873 and the MAE of calibration curve was 0.073, indicating that the model has good external prediction efficiency. Conclusions:This Nomogram model can predict falls in patients with chemotherapy-induced peripheral neuropathy of gynecological cancer and provide evidence for clinical prevention of falls.
3.Immune infiltration mechanism of cuproptosis genes in multiple sclerosis and prediction of potential Chinese medicine treatment

Objective:To analyze the correlation between cuproptosis genes and immune infiltration during the occurrence and development of multiple sclerosis(MS),and to predict the traditional Chinese medicine,to provide theoretical basis for the mecha-nism study of cuproptosis in MS immune infiltration and Chinese medicine to intervene in immune regulation.Methods:The gout chip of MS was downloaded from the GEO database and standardized;based on the processed data,immune cells and functions were ex-tracted and quantified,and the correlation and differences of immune cells and functions were analyzed;at the same time,cuproptosis genes related to MS were screened out,a risk model was constructed,and enrichment analysis was carried out;the prediction of cu-proptosis genes and immune-related biological processes were carried out.Results:① Among immune cells,T follicular helper cell and B cell showed the strongest positive correlation;among the immune functions,parainflammation and typeⅠIFN reponse showed the strongest positive correlation;②B cell,T helper cell and human leukocyte antigen were lowly expressed in MS patients,while major histocompatibility complex class Ⅰ,parainflammation and typeⅠIFN response were significantly expressed;③ The cupropto-sis genes associated with MS were SLC31A1,PDHA1,NLRP3,MTF1,GLS and DBT,of which DBT was the most likely risk factor for MS;④The occurrence and development of MS involves biological processes such as IL-4 production,T-helper cell differentiation,and acute inflammatory response,which were related to pathways such as arginine biosynthesis,citrate cycle,and propanoate metabo-lism;⑤Banxia,Danshen,Jianghuang and other traditional Chinese medicines may be used as potential molecular drug sources.Con-clusion:The expression of cuproptosis gene is closely related to MS-related immune cells and functions,which can provide support for the basic research of MS.
4.Relationship between the SUV max of primary lesion on 18F-FAPI-42 PET/CT imaging and the clinicopathological characteristics of patients with gastric cancer

Objective:To explore the relationship between 18F-fibroblast activation protein inhibitor (FAPI)-42 SUV max of primary gastric cancer and clinicopathological factors of patients. Methods:Fifty-one patients (31males, 20 females, age: 51(47, 65) years) with gastric cancer who underwent 18F-FAPI-42 PET/CT before surgical resection in Nanfang Hospital, Southern Medical University from February 2022 to January 2023 were analyzed retrospectively. The clinicopathological factors that might affect tumor SUV max (including gender, age, tumor location, pathological type, histological grade, Lauren classification, vascular and(or) neural invasion, programmed cell death-ligand 1 (PD-L1) expression, pathologic(p)T stage, pN stage and pTNM stage) were evaluated by the univariate analysis (Mann-Whitney U test or Kruskal-Wallis rank sum test) and multivariate analysis (multiple linear regression analysis). Results:The sensitivity of 18F-FAPI-42 PET/CT in the diagnosis of patients with primary gastric cancer was 82.35% (42/51). The diagnostic sensitivities for early gastric cancer (T1) and locally advanced gastric cancer (T2-T4) were 59.09%(13/22) and 100%(29/29), respectively. The SUV max of primary lesion was 4.90(1.71, 12.51). The univariate analysis showed that SUV max of primary gastric cancer was related to tumor location ( z=-2.00, P=0.046), pT stage ( H=36.94, P<0.001), pN stage ( z=-3.89, P<0.001), pTNM stage ( H=31.49, P<0.001) and vascular and(or) nerve invasion ( z=-5.22, P<0.001), but not related to pathological type, histological grade, Lauren typing, and PD-L1 expression ( z values: from -1.78 to -0.09, all P>0.05). pT stage was found to be a significant independent factor for SUV max in primary gastric lesion by multivariate analysis ( t=2.52, P=0.015). Conclusions:The 18F-FAPI-42 SUV max of primary tumor was related to tumor location, pT stage, pN stage, pTNM stage, and vascular and(or) nerve invasion; pT stage is an independent factor affecting tumor SUV max. The ability of 18F-FAPI-42 PET/CT to detect gastric cancer is mainly affected by pT stage.
5.Evaluation on the quality of 236 National Demonstration Areas for comprehensive prevention and control of chronic diseases betweem 2017 and 2019

Objective:To evaluate the quality of the National Demonstration Area for Comprehensive Prevention and Control of NCDs (referred to as "the Demonstration Area").Methods:Based on the evaluation scores of the Demonstration Area field survey from 2017 to 2019, we counted the scores of each indicator, comparing the scores among indicators and regions. x± s was used to describe the scores. The 95% CI of the score was used to test the statistical difference among regions. Each score was converted into a hundred-mark system to compare the scores among indicators. Results:Of 236 Demonstration Areas, the total score was 83.5. The scores of the first-level indicator listed from high to low appeared as Integrating System of NCD Prevention and Control (92.8), Policy Perfection (90.3), Building Supportive Environment for NCD Prevention and Control (88.4), Implementation of Health Education and Health Promotion (87.4), Whole-course Management of NCDs (78.1), Innovation and Guidance (76.5), Surveillance and Evaluation (75.1). Total scores were higher in the east (259.2±18.8) comparing to the middle (243.2±15.2) or the west (245.4±19.7) regions.Conclusions:Substantial variations on the quality in the Demonstration Area existed across different regions in China. These disparities are important to the government when developing health policies and allocating resources. Whole-course Management of NCDs, Surveillance and Evaluation, and Innovation and Guidance in the Demonstration Area also needs to be improved.
6.Epitranscriptomic 5-Methylcytosine Profile in PM2.5-induced Mouse Pulmonary Fibrosis

Exposure of airborne particulate matter (PM) with an aerodynamic diameter less than 2.5 lm (PM2.5) is epidemiologically associated with lung dysfunction and respiratory symptoms, including pulmonary fibrosis. However, whether epigenetic mechanisms are involved in PM2.5-induced pulmonary fibrosis is currently poorly understood. Herein, using a PM2.5-induced pulmonary fibrosis mouse model, we found that PM2.5 exposure leads to aberrant mRNA 5-methylcytosine (m5C) gain and loss in fibrotic lung tissues. Moreover, we showed the m5C-mediated regulatory map of gene functions in pulmonary fibrosis after PM2.5 exposure. Several genes act as m5C gain-upregulated factors, probably critical for the development of PM2.5-induced fibrosis in mouse lungs. These genes, including Lcn2, Mmp9, Chi3l1, Adipoq, Atp5j2, Atp5l, Atpif1, Ndufb6, Fgr, Slc11a1, and Tyrobp, are highly related to oxidative stress response, inflammatory responses, and immune system processes. Our study illustrates the first epitranscrip-tomic RNA m5C profile in PM2.5-induced pulmonary fibrosis and will be valuable in identifying biomarkers for PM2.5 exposure-related lung pathogenesis with translational potential.
7.Preparation and Quality Evaluation of Gastric Floating Tablets of Schisandra chinensis Total Lignans

OBJECTIVE:To study the prepar ation technology of gastric floating tablets of Schisandra chinensis total lignans (SCTL),and evaluate the quality of prepared tablets. METHODS :Based on single factor test ,the orthogonal experiment was conducted to optimize the formulation of SCTL gastric floating tablets with the contents of hydroxypropylmethylcellulose (HPMC) K15M,NaHCO3 and microcrystalline cellulose as the factors ,using starting time ,holding time and cumulative release rate of gastric floating tablets as evaluation indexes. The properties ,weight difference ,floatability and accumulative release rate of the prepared SCTL gastric floating tablets were determined. The gastric floating tablets were qualitatively identified by TLC ,and the contents of schisandrin A and total lignans were determined by HPLC and UV spectrophotometry. RESULTS :The optimal formulation of SCTL gastric floating tablets was made up of 23% SCTL extract ,20% HPMC K 15M,40% microcrystalline cellulose,15% sodium bicarbonate ,1% octadecyl alcohol and 1% polyvinylpyrrolidone. The results of detection of this preparation were in line with the related provisions of “0101 tablet”stated in 2015 edition of Chinese Pharmacopoeia (part Ⅳ). TLC indicated that the chromatogram of the test sample showed the main spots of same color as the corresponding positions of the chromatogram of schizandrol A control ,Schisandra chinensis reference substance and raw material ,while the negative control has no interference. Content determination results shows that the average content of schizandrol A and total lignans in SCTL gastric floating tablets is 3.187,19.617 mg. It was preliminarily formulated that the content limitation of schizandrol A in one tablet should not be less than 2.50 mg,and the content of total lignans (calculated by schizandrol A )should not be less than 15.50 mg. CONCLUSIONS:The preparation technology of SCTL gastric floating tablets is stable ,feasible and controllable in quality.
8.Application of dentin in bone tissue engineering

Defects in oral hard tissue caused by various factors have a negative impact on the functional and aesthetic results of prosthetic treatment. In recent years, the usage of bone tissue engineering for bone reconstruction has drawn widespread attention. Bone tissue engineering exhibits significant advantages, including the abundance of building materials and few side effects. In this paper, the composition and structure of dentin and its application in bone tissue engineering are reviewed, providing a new way to further optimize its performance. The results of a literature review show that the structure of dentin is very similar to that of autogenous bone. The inorganic component is mainly hydroxyapatite (HA), while the organic component is mainly collagen I, noncollagenous proteins (NCPs) and growth factors. Because of its unique composition, dentin can act as a scaffold and/or growth factor source through different processing methods. The deproteinization process removes most of the organic substances and creates a HA-based scaffold material with high porosity, which allows for vascularization and cellular infiltration. Demineralization increases dentin porosity by reducing the crystallinity of the mineralized components, so that part of HA, collagen fibers and growth factors are preserved. Demineralized dentin possesses various regulation functions ranging from differentiation, adhesion and proliferation of primitive cells and bone forming cell lineage. Extracted NCPs, as bioactive molecules, have been proved to play important roles that control cell differentiation, crystal nucleation and mineralization in bone formation. NCPs could be combined with variety of scaffold materials and modify their properties.
9. Inspiration of the Astana Declaration to the prevention and control of non-communicable diseases in China

2018 witnessed the 40th anniversary of the Alma-Ata Declaration. On October 25, 2018, the World Health Organization issued a new Astana Declaration, which reiterates and further develops the concept and core elements of primary health care. It is also proposes that the implementation of the primary health care concept will facilitate to cope with the increasing burden of non-communicable diseases in different countries. Based on the analysis on the policies and practices of the prevention and control of non-communicable diseases in China, this paper points out that the "government-leading, multi-sectoral collaboration, social mobilization and participation by all people" which we have always emphasized is just the application of this primary health care concept, and the Astana Declaration also brings a new and important inspiration to the prevention and control of non-communicable diseases in China.
10.Correlation between the immunophenotypes, treatment strategies and prognosis of gastrointestinal diffuse large B-cell lymphoma

Objective To explore the differences in the prognosis of patients with different immunophenotypes gastrointestinal diffuse large B-cell lymphoma (GI-DLBCL) who received different treatment strategies.Methods From March 2006 to January 2016,at Nanfang Hospital,Southern Medical University in Guangzhou,the clinical data of 99 patients with pathologically confirmed GI-DLBCL were retrospectively analyzed.According to treatment strategies,patients were divided into chemotherapy alone group and combination of surgery and chemotherapy group.According to immunophenotypes,patients were divided into germinal center B-cell-like (GCB) type and non-GCB type.The two-year progression-free survival (PFS) rate and overall survival (OS) rate were evaluated.Kaplan-Meier analysis,log-rank test and Cox regression were performed for statistical analysis.Results Among the 99 patients with GI-DLBCL,51 patients were treated with chemotherapy alone,and 48 patients were treated with combination of surgery and chemotherapy.Forty-one cases were GCB phenotype and 40 cases were non-GCB phenotype.The median follow-up time was 25 months.The two-year PFS and OS rates were 70.9% and 89.5%,respectively.The two-year PFS and OS rates of chemotherapy alone group were 63.6% and 85.0%,respectively;both were lower than those of combination of surgery and chemotherapy group (79.4% and 94.7%),and the differences were statistically significant (x2 =4.232,P =0.040 and x2 =4.260,P =0.039).The two-year PFS and OS rates of GCB group were 68.8% and 93.9%,respectively.And the two-year PFS and OS rates of non-GCB group were 73.2% and 85.6%,respectively.There were no statistically significant differences between these two groups (both P > 0.05).Among 41 patients with GCB type,25 were treated with combination of surgery and chemotherapy and 16 were treated with chemotherapy alone.The two-year PFS rate of patients treated with combination of surgery and chemotherapy (83.1%) was higher than that of patients treated with chemotherapy alone (49.2%),and the difference was statistically significant (x2 =5.627,P =0.018).The results of multivariate analysis indicated that treatment strategy was not an independent prognostic factor for all the enrolled patients and in patients with GCB type (all P > 0.05).Conclusions Immunophenotypes may lack evaluation value of prognosis in patients with GI-DLBCL.Although among all the enrolled patients and patients with GCB type,the prognosis of patients treated with combination of surgery and chemotherapy is better than that of patients treated with chemotherapy alone,treatment strategy is not an independent prognostic factor.Multi-factors should be evaluated before selection of treatment strategy.
